I hope you will find it valuable, whether you want to know more about what support or care is available for yourself, or for a relative or friend. Whether you pay for your care and support services yourself or if they are paid for by the local authority, we will do our best to help you find the support that suits your circumstances.

Somerset County Council is committed to helping people maintain their quality of life, where possible in their own home. Most people want to remain independent for as long as possible. But we know that deciding what care and support is best for you or someone you care for can be difficult.

In this Directory you will find the contact details of many organisations and bodies that can help you make those decisions. We have also taken the opportunity to provide you with details about what is available in your local community. We realise that having local contacts can be crucial in supporting you to remain independent.

One in ten people in the UK cares for friends or family members. We support unpaid carers in various ways, for example by providing training, links to support groups and by paying towards short breaks (respite). Please see page 8 for more information about carer support.

Somerset County Council works closely with colleagues in the health services, registered care

providers and the Care Quality Commission (the independent health and social care regulator) to raise the standards of care and support in Somerset.

What is the difference between care and support?

In this Directory, care is used to describe any service that helps you with your personal care, for example, helping you to get up, washed and dressed in the morning.

Support is anything provided to you that doesn t include personal care, for example, domestic cleaning, shopping and attending community groups.

Of course, some people need a mixture of both of these things; the same person or organisation, or different organisations may provide them. You may arrange them yourself, we can help you to make the arrangements, or we can organise them for you.

The important thing is that you have the right information to make your own choices.

How can this Directory help me?

We know that many people and their carers, perhaps with a little support, can successfully live at home and enjoy their life to the full.

Our key principle is that we want people to remain in control of their lives, to remain independent and to have the opportunity to make their own choices about their care and support arrangements.

Many people, who in the past may have had a package of care to support them, with a little imagination, can often find other, more flexible

and suitable ways to help them. This Directory covers some of these things, called support in the community before moving into the more well known services, such as home care and care homes.

These services too, are developing more ways to be flexible and tailored to what you need.

Although we have laid out the Directory in a way to make it easy to follow, the support you need may be built up from each section, as it can all link up to provide the most suitable support for you.

Where do I start?

The information in this Directory is designed so that you can find solutions for yourself, if you wish to, or you can ask us for help and advice if you need it.

The best way to start to understand what care and support you need is by writing a list of the things you are finding it more difficult to do. From this list you can then start to come up with solutions. These solutions could be things you can do for yourself, things you ask others to help you with, or things you talk to us about.

On page 7 there is a blank support plan that you can complete if you wish, to help you work out what you may need. You can then see if information

about the things you ve written down are in the directory.

Writing your support plan You may find this a useful way to understand what you need, and then how to find solutions. It is very similar to what we would use during an assessment.

1. Things you would like to achieve In this section, list the things you are finding it more difficult to do, or feel you would like more support to do. This could be one thing, or lots of things ‒ try to list all the things that are important to you.

2. Solutions. Using the list you ve written

Obviously, you may prefer to miss this section and just read on.

3. In this section, write down the help and support that you think others could provide for you, for example, a friend or family member, or a local community group.

4. In this section, write down the things that you think we could help you with.

Help for carers According to Carers UK, one in ten people in the UK cares for a friend or a family member. Somerset County Council is committed to supporting carers who carry out a valuable role for the person they care for. It is estimated there are over 50,000 carers in Somerset. Many do not realise that they are a carer, or think of themselves as a carer, and do not know that there is a range of services and

information available to them.

Who is a carer?

You are a carer if you give up your time without pay, to look after a family member, a friend, or a neighbour who is ill, frail or has a disability. You can be a parent carer, a young carer, or care for an older person.

Compass Carers ‒ the new carers support service for Somerset Somerset County Council and NHS Somerset jointly fund a support service with Compass Disability Services, a Somerset based user-led organisation, to deliver a countywide support service for carers.

This service is called Compass Carers . The service is for carers of all ages who care for people with any condition. It doesn t matter how long they spend in their caring role. The aim of the service is to support carers, to offer individual support so that they can maintain their caring role and also to have a life outside of caring.

Compass Carers have experienced staff who deliver the new service, including: • office-based staff (who co-ordinate the team and man the advice / information / support telephone line); • link workers (who liaise with GP surgeries and Carers Champions); • voice workers (who liaise with, and help establish carer support groups); and • Carer Support Workers (who visit carers to help establish and monitor a support plan).

You can contact their workers to find out information and get advice. They can provide you with the emotional and practical support that most carers need to keep them going. Every carer is an individual. This service will listen to you and help you identify the things that are important to you and how to achieve them. Whether it is to do with family, finances, employment, isolation or having a break, we can work with you to help find solutions.

If you are an unpaid carer, then please contact Compass Carers for: • emotional support; • information in various formats to suit your needs; • pointing you in the right direction to get help or information; or • access to local carers support groups, or a visit.

Carers assessments Carers are entitled to an assessment by social care to find out if, through the provision of additional care and support, it will help you in your caring role.

Some support may be paid for by us.

How do I apply for a carer s assessment?

If you would like more information, ask your social care worker, or phone Somerset Direct on 0845 345 9133.

When you contact Somerset Direct they will ask

you for information about you and your needs.

They may suggest you talk with Compass Carers, or they may be able to give you some advice and information straight away over the phone. This may be followed up by a member of the social care team coming to see you and, if you wish, the person you care for, to give you more advice. This is called a needs assessment . If your assessment shows that providing support to either you or the person you care for is needed, then we will help to arrange this.

Carers assessments continued Do I have to be assessed?

You do not have to have a carer s assessment. If you choose not to have one, it will not stop the person you care for from receiving services.

What happens next?

If we can help, we will write a care and support plan for the person you care for, taking into account your needs as a carer. The care and support plan explains what support will be arranged and who will provide it. If your needs are eligible, we can arrange the support for you or we may be able to

provide finance so you can buy the support yourself using the Direct Payments scheme.

What if I am not eligible?

If you are not eligible for services, we can still provide information and advice and put you in touch with other carer support organisations, such as Compass Carers.

Carers information, support and counselling On the carers page of our website, www.somerset.gov.uk/carers, you can find a list of information sheets that may help you. You can view them online, save them or print them. They help to explain who a carer is and some of the things carers might have to deal with in their caring role, as well as giving you advice about what support is available and the services we can provide for carers in Somerset.

Opportunities for carers There are many learning and training opportunities available to carers. These can be designed to help with your caring situation, for example, basic first aid, looking after yourself, and other opportunities that offer personal development. Practical support for carers Carers Assessment Workers are based in community mental health teams. They specialise in assessing the needs of carers who care for someone with mental health problems and can advise carers about: • inpatient services; • outpatient services; • day services; • residential care and short breaks; and • contacting social workers and community psychiatric nurses.

Carers champions A member of staff in most GP surgeries in Somerset has been appointed as a carers champion. They can register you as a carer on the Carers Register.

This will alert a GP that you are a carer when you attend an appointment and may also help you to access preferential appointment times and carer health checks. The carers champion can provide you with information about where to go to find out more about carers services.

Breaks for carers There are different ways you can get a break from caring, anything from a few hours to a few weeks depending on what is right for you and the person you care for.

The Care at Home Service may be able to provide you and the person you care for with support at home to help with personal care and essential tasks. Care at home is provided as part of an agreed care and support plan, after a needs assessment to agree what is needed.

Short breaks can be arranged to help meet your needs as a carer as well as those for the person you care for as part of an agreed care and support plan.

The sitting service can help you take a short break with peace of mind. This service provides fully trained staff who can go into a carer s home or take the person they care for out, and is not part of the Care at Home Service.

Residential short breaks make it possible for the person you care for to spend time away in a care home.

Carers information, support and counselling continued Take a break scheme Following an assessment, instead of having your breaks arranged in advance, there is a scheme where you can contact us to arrange your break when you need it.

There may be a charge for some of the short break services. Your social care worker or carer support worker will be able to give you more information, or you can phone Somerset Direct on 0845 345 9133.

Young carers The Young Carers Project aims to: • identify and support young people (18 years old and younger) with caring responsibilities in their family; • promote recognition and awareness of their needs and achievements; and

• work alongside other agencies to develop schemes and services which support young carers.

The project has Young Carers Project Co-ordinators working all over Somerset.

Information, advice and support are available, including: • answering questions and giving advice; • carrying out an assessment of young carers needs; • speaking up for young carers so that ideas and needs are heard; • introducing young carers to one another; and • organising groups and activities for fun and having a break away from home.

Health and wellbeing In 2013 responsibility for health and wellbeing has transferred from the NHS to Somerset County Council, as it is recognised that social care and health and wellbeing are closely linked.

Health is influenced by many factors which become more complex as we get older, and we recognise how important it is to make sure you can access support for emotional and mental wellbeing as well as physical health issues.

Things that can impact on our health and wellbeing include: • staying warm; • an adequate level of income; • getting out and about;

• having a healthy diet; and • staying safe from falls.

Keeping active is really important, so the Zing website www.zingsomerset.co.uk has lots of information about activities across Somerset.

If you are concerned about your memory, there is lots of information on the Dementia Somerset website www.dementiasomerset.org.uk, which also lists support for carers and various services available.

Staying safe from falls Staying steady on your feet becomes more important as we get older - the benefits of remaining physically active are discussed on page 13.

As we age, our bodies become more likely to develop health conditions that can cause us to fall. Our muscles can become weaker and our eyesight and hearing may decline; we can also lose confidence, which affects how active we are.

We may suffer from chronic illnesses and have to take medication which can all contribute to the risk of falling.

Other factors, such as wearing badly fitting or worn shoes, foot problems and things in your home such as loose rugs, trailing wires and other trip hazards, can all make you less safe from falls.

Staying safe from falls continued sure they identify reasons why you fall and help you to address the issues. If you are unable to get to your local Community Hospital, a physiotherapist may be able to set up an activity plan with you at home.

If you do fall and break a bone, you may find that you won t be able to cope with living in your own

home. So, it makes sense that you reduce your risk now, so that you can remain independent for as long as possible.

Support in the community The following section lists community support that you can go to directly.

There are lots of things going on in your local community that could help you to remain living independently in your own home. These include: • various groups that meet to provide companionship, they may even arrange for someone to collect you to take you to group meetings;

• pubs that provide meals, usually at great value; • befriending schemes, where volunteers can come and visit you at home or accompany you if you would like to go out; and • organisations that can help you with some domestic tasks, such as cleaning, shopping, gardening, or simple repairs to your property.

These are simple, but important things that can help you feel safe and give you peace of mind.

Somerset s You Can Do Service The Somerset You Can Do Service is funded by Somerset County Council. If you would like to find out about what community support is available in your area, you can contact the free information service, which can: • put you in touch with organisations and groups in your community that are there to help you; • provide you with information on local community organisations you might like to join; • help you to get the information you need if you have sensory loss or other disabilities • help you with your skills, development and learning; • put you in touch with Village Agents, who are being recruited by the Community Council for Somerset to provide people in Somerset s rural communities with easier access to information services; • tell you about getting practical help at home;

• give you information about equipment you could hire or buy to help you live independently at home.

As well as the Information Service, Somerset s You Can Do also provides a volunteering service that supports and recruits people who want to volunteer to support others. There is also a community development service. This supports groups and organisations to become established and helps to improve the community support they offer by making it more accessible.

Home Improvement Agencies Ridgeway Care and Repair Somerset, part of Aster Living, is the preferred provider of Home Improvement Agency services for Somerset. They are funded by Somerset County Council, the District Councils and NHS Somerset to help you arrange a

major adaptation to your home if you need it.

They can provide technical services, such as drawing plans and getting quotations for work.

They work with occupational therapists in Social Care Services and the District Councils to make

Home Improvement Agencies continued sure the work progresses smoothly. They keep lists of reliable contractors, who can provide a quality service and make sure that everything is as it should be when the work is finished. They can help you to complete the works privately if you are not eligible for funding under the Disabled Facilities Grant (DFG) means test.

The Handihelp service is also available as a home from hospital discharge service if you need minor repairs and adaptations to your home to make sure that your move back is safe. Some parts of this service may be provided at no cost.

Leaving hospital If you, your carer, family or hospital staff feel that you will need extra support when you leave hospital, you can ask us to carry out an assessment whilst you are in hospital or before you go into hospital if you are waiting for an operation. See page 39 for an explanation of what an assessment is.

A social care worker will agree a discharge or care plan with you, which tells you what help and

support will be arranged for you when you return home. This makes sure that you can leave hospital safely, as soon as you no longer need hospital care. Someone from the local social care team will contact you after you have returned home to check that the care plan is still right for you. The help may be increased or reduced depending on how you are. The aim is to provide the right short-term help to allow you to recover your independence.

Red Cross Home from Hospital service The Home from Hospital Service helps many people each year to go back home after a stay in hospital.

The service can give short-term practical and emotional support to help you get your confidence back and live independently.

Help can be provided by: • taking you home and helping you to settle in; • shopping for you; • collecting prescriptions and pensions; and • sitting with you if your carer has to go out.

Aster home from hospital service Aster also offer a home from hospital service. If possible they will visit you in hospital before you go home. But, they can come to see you at your home if you prefer or if there isn t enough time before you go home. They ll talk to you and find out what your needs are and agree with you how they can help. They can do lots of things to help you. Here are a few examples, they can: • arrange for basic food supplies ready for your return home, or help you get a food parcel if you re short of money;

• provide a visiting service to reassure you; • give advice or carry out checks to see if you are receiving all the benefits or grants you might be entitled to, and they ll help you fill in the forms if needed; • carry out home safety checks; • give advice and help to find things like care, cleaning, gardening, shopping or dog walking; and • provide lots of information about local groups or organisations that offer help in your area.

Working in partnership with voluntary, community and statutory organisations, the Somerset Active Living Network has over 120 community groups, clubs and organisations across Somerset.

Staying well and active Active Living Centres provide an opportunity to take part in local activities, enjoy the company of others, and access all sorts of information about keeping well and active. Centres are a vibrant hub providing a café style environment, and hosting

a variety of activities, as well as being a source of information about preventative services and activities for older people.

Volunteers run groups, giving their time, to support the development of local services and activities for older people.

Activities are often run by older people for older people and provide all kinds of exciting opportunities such as Tai Chi, healthy walks, singing for fun, history groups, and art activities ‒ these are just a few examples.

Active Living and Somerset County Council work together in partnership with many other agencies to provide activities and information for people aged 50 and over. Partner agencies involved with Active Living include: Somerset NHS, Somerset You Can Do , Avon and Somerset Police, District Council Lifestyles Teams, Age UK Somerset and the wider voluntary and community sector.

Day time care and support Day time care and support is a special service, designed to help people who live in their own home but who are not able to get out and about without help.

The service is often run in the community. Care and support is available in a way that encourages and enables people to remain as independent as possible by helping them to maintain and / or develop skills for daily living.

The service may also allow carers to have a break.

If you qualify, your social care worker can book the service for you, or you can book it yourself if you have a Direct Payment (explained on page 45). This service does not include an overnight stay.

During 2013 Somerset County Council is looking at ways of making this service more local, individual and fair.

The Council is designing a service that will: • provide daytime care for adults who need local, specialist and appropriate care and support; • support adults to continue to live at home with an agreed amount of daytime support that could include: a) support to do things that will promote independence and give service users contact with other people, b) support to meet complex care and support needs, c) a break for carers to do the things they want, allowing them to continue their caring role, d) respite for service users, • offer a range of other services that adults can buy; • provide a safe daytime environment; and • offer support that is local and easy to access.

Meals There are organisations in Somerset that will sell you meals. Often this is by providing you with a frozen meal that you can warm up and eat when you choose.

Somerset Community Meals service Somerset Community Meals service is for older people and people with disabilities who Somerset County Council agree need help to make sure they have regular cooked meals. This is provided at a discounted rate from a menu by Wiltshire Farm Foods. Each meal provided as part of the Meals at Home Service costs ÂŁ3.45. There are some more expensive meals that you can buy if you want to.

The Meals at Home service delivers frozen meals to you every two weeks. The frozen meals must be kept in a freezer and can be reheated in a microwave, oven or special steamer. This means that you can choose what you eat and when you

eat it and can have a cooked meal every day. Your dietary, religious or ethnic needs can be catered for.

If you need to borrow any of this equipment from Wiltshire Farm Foods there is no charge for this, as long as you place the minimum order of ten meals every two weeks.

If you need help to get your meal ready or to eat it, this can be arranged after an assessment of your needs. You may have to pay for this; how much will depend on your circumstances.

You can still have frozen meals without being assessed by Somerset County Council. Wiltshire Farm Foods is a private company that provides a service to many parts of the country. You can buy meals from them privately but this won t be at a discounted rate. Equipment There is a wide range of equipment available that can easily be used in the home to make life easier and help you remain independent. These things could be a stair rail, raised toilet seating, equipment to help in the bath or shower, or items to help you prepare and eat food and drink. They allow you to continue: • moving around your home; • getting in and out of a chair or bed; • getting in and out of the bath or shower; • preparing food and drinks; • reaching the front door; or • carrying out parenting tasks.

Equipment for short-term loan is available through the British Red Cross Society loans service and is ideal if you need some equipment temporarily, for example, while a disabled relative is on holiday with you or to try out before you buy. You can phone them on 01823 273746.

People in Somerset have access to a website called AskSARA that you can use to see what equipment is available locally to you and how to obtain it. Please see our website for updates: www.somerset.gov.uk/asksara You can buy some equipment that might help you from local shops that specialise in disability equipment. There are also some local voluntary organisations and private providers that sell equipment. We can tell you more about this if you contact us.

Sometimes, someone from the occupational therapy team may need to come and see you at home to make sure you get the best advice. They will also need to visit you if you need adaptations to your home. Please see page 11: Home Improvement Agencies .

Assistive Technology Would you be surprised to know that you use assistive technology already in everyday life? It comes in the form of devices to alert us when: • it s time to get up - alarm clocks; • there s smoke in the house - smoke detectors; • there s someone at the door- door bell; or • there s a call for you - telephones and mobiles

There are many other products available to support more vulnerable people to live as independently as possible in the community for longer. The aim is to give people more choice and control over how they live their lives and the risks they are able to take.

Telecare In Somerset, there is a range of Telecare services available through the occupational therapy service following an assessment. You can contact the community alarm providers direct if you wish to buy their service privately and don t want a visit from an occupational therapist. The Community Alarm Services employ Telecare advisors who will help you choose the right product to meet your needs.

These are products and services that can prompt, remind and inform people and their carers about things in daily life which need to be done, such as taking their medication on time. They can alert a family member or carer that a person has got out of bed at night and needs assistance. Or they may alert a monitoring centre that something has happened and appropriate action should be taken.

In Somerset, Community Alarm services are already available through District Councils. See page 57 for contact details. They can provide a device that sits between the phone and its socket point, and an alert button that you wear. By pressing the button, an alert is directed to a call centre that will respond instantly. They can speak to you even if you are not near the phone, but this will only happen when the button is pressed.

If we are supporting you to go home following a period in hospital, we will pay for you to use the Community Alarm service for up to six weeks. You will be asked to pay for the service if you want to use it for longer than six weeks if you need it. Advice from an independent occupational therapist If you would like advice from an occupational therapist you can visit one of the Independent Living Centres, where you will be able to try out equipment and get advice about what to buy. Or, you could ask a private occupational therapist, who can come and see you in your own home.

Care and support at home Independent Living Teams (Reablement service) This is a new way we are working, currently only available in Taunton and South Somerset, but soon to be started in Mendip, before being developed across the rest of the county.

Somerset County Council and the NHS have integrated some of their staff into single teams, so that they can share their different skills and provide a co-ordinated approach, to help you regain your independence.

A team includes: a social worker, an occupational therapist, a physiotherapist, care workers and support workers. Other support, for example from a District Nurse, Carers Support or support from staff skilled in mental health issues can also be arranged if you need them.

You may be recovering from an illness or surgery, or just need help to do some of the things you used to be able to do. Your GP or hospital doctor may refer you to this service if they think you require the

support the team can offer. The very experienced staff will talk with you and your carer, if you have one, to understand you and what you need, and then help to design and deliver a support plan. This may be simply explaining how you can get further support, for example housing advice, through to providing you with equipment, physio support or care if they feel it is needed. This is a short-term service, with the aim that you will regain your independence in that time. Some people may need some care and support for longer, the team will talk to you about this and help you to make the arrangements you need.

In areas of the county where this new service is not yet in place, your GP will need to make separate referrals to Health and Social Care. These services are then provided by Somerset Partnership Trust Community Health s rehabilitation teams and social care occupational therapists (OT) or social care workers.

Occupational therapy (OT) assessment If your assessment shows that you need specialist equipment, for example a hoist and sling, or minor works such as stair rails, then, following an OT assessment, they may provide them free of charge.

Having the right equipment can often mean you don t need further care and support or it can reduce the amount of support you need.

The occupational therapist will also be able to give you some advice and information about the various housing options available to you. This may include moving to alternative housing or you may need major alterations to your current home, such as putting in a stairlift or wet-floor shower. Occupational therapy (OT) assessment continued district council to consider you for a Disabled Facilities Grant. The district council will ask you for information about your finances and you may have to pay towards the cost of the work. Adaptations that can be considered for a Disabled Facilities Grant include: • access - widening doorways for wheelchairs; • bathroom adaptations; • fixed overhead hoists; • lifts ‒ stair-lift or through-floor lifts; and

• general alterations including permanent ramps.

If you are not eligible for a grant, the OT will give you information and advice so that you can make you own arrangements to adapt your home.

It is important that you do not start any work before you have received grant approval from your District Council - you will not normally be entitled to any help for work already undertaken.

Telehealth These are devices that can support people with particular long-term conditions, such as chronic obstructive pulmonary disease, heart failure or diabetes. They have the ability to collect vital measurements including weight, blood pressure and heart function. This reduces the need for outpatient appointments and alerts nurses and

doctors to manage your health needs.

These devices can be used in people s homes or in GP surgeries. They rely on a control box collecting information about the person s condition from sensors, such as blood pressure cuffs or scales.

They are only provided following a health care assessment of need.

Coping with a sensory loss We have lots of information if you, or someone you know, has a sensory loss. You can find this on our website www.somerset.gov.uk (search for hearing and sight loss ).

Care at home Before you decide you need care at home you should consider and ask for advice about equipment and adaptations. This may reduce, or even mean you don t need, care at home.

Care at home services provide someone to help people who are struggling to cope in their own homes with daily activities, such as, cleaning and shopping and personal care, including, getting in and out of bed, washing, dressing and using the toilet. For people with special needs, specially trained staff are available. With the right help many people are able to recover their independence after a crisis.

If you are frail, have been ill, or have just come out of hospital, the short-term support of a home carer could be welcome. Also known as domiciliary care, care at home is available for older people or people of any age with a physical or learning disability, or a mental health problem. Agencies

can also help parents and carers of children with disabilities and provide support to your own carer if you have one.

Care at home can be provided by private or voluntary agencies or individuals. If you choose agency care, it will be delivered by a small team of carers. They must be specially trained in areas such as manual handling, first aid and food hygiene.

Carers will also have had background checks with the Criminal Records Bureau and the Disclosure and Barring scheme.

Not all agencies provide the same services and charges vary. Some agencies offer a night sitting or even a live-in service and some are also registered as Nurses Agencies, meaning they can provide registered nurses if needed.

All home care organisations that provide personal care must register with, and be inspected by, the Care Quality Commission (CQC), which makes

Somerset County Council (SCC) for help to arrange it. If SCC contributes money towards your care, the amount of money you receive will depend on both your needs assessment and a financial assessment (see page 39).

Live-in care Moving into residential care, where they have the support they need and the companionship of others is the best solution for some people. For others, staying in the family home with its lifetime of memories and personal possessions is of greater consideration.

Live-in home care is an alternative to residential care. It allows you to retain control of your life and what is happening around you â&#x20AC;&#x2019; such as when and what to eat and what to watch on TV. If you have any pets, they can continue to live with you, and family and friends can still visit or stay.

There are many advantages of having one carer to look after you too. A live-in carer can get to know your routine and do things the way you like them done. As well as help with personal

care and domestic chores, they can assist with transportation and home administration, and you have the reassurance that there is someone you can call on at any time of day or night. As carers are often matched to your needs and interests you also have the opportunity to establish a oneto-one relationship with them and enjoy their companionship.

Not only is 24-hour live-in care available on an on-going basis for people who need full-time support it can also be provided on a short-term basis, for example, following a stay in hospital or to provide a regular carer with a break.

Home care agency checklist If Somerset County Council arranges your care and support we will use care agencies that we have an existing contract arrangement with. The agencies used will have been accredited to work for us.

People who arrange and pay for their own care, either privately or through a Direct Payment (see page 45), are able to choose their own worker or care agency. The things in the list below may be useful to think about when you are considering using the services of a care at home agency.

Agencies • Care at home agencies providing personal care must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate or check that they are listed on the CQC website www.cqc.org.uk • What experience does the agency have with your particular needs?

• Can they supply references for you to check?

• How long has the agency been operating?

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick? Will you be told in advance that a different carer will be attending?

• How can you contact the agency in an emergency or outside office hours?

• How hard or easy would it be to make a complaint and how are things then put right?

• If this is a private contract, ask for a copy of the agency s contract terms and read them carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this.

Care workers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable care workers for your particular needs are chosen. Can you talk to them before deciding?

• Care workers should be fully trained or be in ongoing training. Ask the agency about their training policies.

• You will have a care and support plan prepared by the agency that the care workers will work to.

Ask how often this plan will be reviewed by the agency.

• Care workers must be checked with the Disclosure and Barring Service (DBS) and have a criminal records disclosure ‒ make sure this is the case.

Housing with support, or care and support FirstStop Advice This is an independent, free service offering advice and information for older people, their families and carers, about housing and care options in later life. It is led by the charity Elderly Accommodation Counsel (EAC) working in partnership with other national and local organisations. The service

spans housing, care, finance and rights, and is delivered through their website, telephone advice line and network of local and specialist partner organisations.

Sheltered and retirement housing Sheltered and retirement housing is suitable for people who can live independently but may need a small level of support or easy access to certain services, either occasionally or more regularly.

There are many different types of sheltered schemes and retirement housing, available both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings. These may be bedsitting rooms, self-contained flats, bungalows or luxury apartments.

Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm. Most schemes have a scheme manager or warden and a community alarm service, and some schemes also have support workers who can visit you at set times during the week. There are often communal facilities, such as a lounge, laundry, guest flat and garden. Meals are not normally provided, but a few schemes include a restaurant and some can arrange hot meals.

In a sheltered (or retirement) housing scheme

you would live independently in a flat or bungalow built especially for older people, within a larger complex. The complex is often built around a garden or communal facility and is linked to a central control centre for security. The scheme manager provides support, advice and a link to services for older people and other vulnerable adults.

Councils and housing associations provide sheltered housing to rent in Somerset. Increasingly, this is also open to younger vulnerable adults and not just older people. It is a practical step if you would like more security and companionship, and have some support needs. Care providers and health workers can also visit you in sheltered housing when you have care needs and your sheltered home is still suitable to meet these needs.

Private developers build retirement housing for older people, who are usually over 55 years, to buy or to rent, or for shared ownership. They set their own criteria for people with care and support needs.

Extra Care housing Extra Care housing is a form of housing, similar to sheltered schemes, with self-contained accommodation together with some communal facilities. In an Extra Care scheme, instead of the low level support traditionally provided in sheltered schemes, higher levels of care and support are provided on site. This is for people with assessed care needs and is provided by a designated team of care workers who can be available 24 hours a day.

Extra Care allows people to remain safely in their own self-contained accommodation and have regular care services delivered.

Housing-related support is now called Pathways Somerset County Council commissions integrated support using two Pathways.

1. The Somerset Pathways to Independence (P2I) is a multi-agency, integrated housing related support service for young people aged 16 to 25.

It includes care leavers who live in or have a local connection with, the Somerset area. The service allows young people with housing related needs to progress along a pathway of personal goals until they can live independently. But first and foremost, it will try to prevent the need for a young person to step onto the pathway at all, through targeted prevention measures.

The P2I Services will prevent youth homelessness and enable the young people who are accommodated to progress from support and supervision to independent living by providing: • targeted prevention and homelessness awareness through education, information and advice; • floating support that includes mediation and reconciliation; • emergency accommodation; • supported accommodation; • move on accommodation; and • resettlement support.

2. The Somerset Pathway for Adults (P4A) provides an integrated range of prevention, accommodation and resettlement services

for vulnerable people who are considered to be socially excluded . These include people with a criminal background, or who have been identified as having poor mental health, or are sleeping rough, single and homeless and who need accommodation or support to prevent homelessness so that they can be independent long-term.

There is evidence that many people can be thought of as temporarily socially excluded at different times in their lives. Most people can deal with these situations through their own resilience and resources.

The overall aim of the P4A is to provide vulnerable people with a targeted support service. The service is tailored to the person and focuses on them getting and keeping somewhere appropriate to live.

It also helps them integrate into the community with support to identify sustainable income routes, reducing a reliance on benefits, with access to education, training and work related activities, including volunteering. It is a priority for these services to support people who do not have the resilience or resources, either through themselves, or their family or community, to overcome problems of exclusion and enable them to find stability.

Care homes Sometimes people decide to move to a care home when they are in a crisis and cannot work out how to get the help they need to stay at home. Often people who are helped through the crisis can keep living at home with the right support.

Before considering moving into a care home, we recommend you talk to us first to make sure that all other options have been exhausted that could help you to remain living independently at home, or in other suitable accommodation. If this is no longer possible, moving to a good quality care home may be the best option. These are the types of care homes:

Residential care homes provide personal care, such as washing, dressing, bathing and assistance with toilet needs, if required. They do not offer nursing care. In some homes some staff may only be on call and not on duty at night. Homes with fifteen residents or more must have at least one member of staff awake on duty at night. Some provide specialist care for people with mental health difficulties or physical disabilities. If you have a health problem that needs a qualified nurse, this care will be provided by the District Nursing Service on the same basis as for people living in their own homes.

Care homes continued Care homes with nursing provide nursing care 24 hours a day. Usually people living in these homes will be very mentally and/or physically frail or immobile, and have health care needs that can only be met by a registered nurse. Some offer general nursing care for older people, while others specialise in mental health or physical disability.

Care homes with nursing are often dual registered to provide both residential personal care and nursing care. If you choose one of these homes you won t need to move if your condition changes.

Also, this type of home helps couples who may have different needs to live together in the same home.

Care homes that specialise in supporting people who have severe physical disabilities, learning difficulties or autism. These care homes do not normally provide nursing care.

Specialised residential care homes are residential homes, only available in Somerset, that specialise for people with a high level of dementia but who do not have nursing needs. Somerset Partnership NHS and Social Care Trust employ Specialist Care Development Nurses (SCDNs) who support residents in these specialised care homes and provide advice and support to the care staff. They are involved in discussing the needs of possible new residents to make sure that the home is suitable for them, and also that the overall dependency levels in the home can be managed by the staff who work there. They will agree whether the home is suitable for all new residents.

Checking quality When considering care services, it s always a good idea to check a service s inspection report on the Care Quality Commission s comprehensive website: www.cqc.org.uk The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Since October 2010, all care providers must be registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety.

Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. You can see each service s report on the CQC

website: www.cqc.org.uk, or ask the care provider you are considering for a copy of their latest inspection report.

The focus of an inspection is on the standards of care that people who use the service receive and whether they are happy with their care. Almost all inspections are unannounced.

Choosing a care home To decide which type of home would be right for you, you should ask for a needs assessment. See page 39 for an explanation of what an assessment is.

A social worker will carry out the assessment. Other professionals, such as an occupational therapist, a district nurse or your doctor will also be involved, with your permission. This will help make sure you choose the type of home that would best meet your needs.

If you are moving to a care home with financial help from us you have the right to choose a home anywhere in England or Wales, as long as we agree that it meets the following requirements. â&#x20AC;˘ The home is suitable for your assessed needs.

This means we must be sure that the home can give you the help your assessment shows you need. For example, a residential care home will not be suitable if you require regular nursing care. If we consider a home to be unsuitable we will tell you why, and ask you to choose another. â&#x20AC;˘ There is a place available. We may be able to arrange for you to move to another home while you wait for a place to become available in the home of your choice.

â&#x20AC;˘ The home owner agrees to our usual contract conditions. If they do not, you will be asked to choose again. â&#x20AC;˘ The home does not cost more than we usually pay for the type of care that you need. If you wish to move to a care home that charges more than this someone will have to make up the difference â&#x20AC;&#x2019; this is called a top-up or third party payment.

Choosing a care home continued Things you need to think about Every home should produce a brochure or service users guide to tell people about the service it provides and the facilities offered to its residents.

Choosing the right care home is important. If you are able to do so, visit more than one home before you make up your mind. You may be able to spend the day, have a meal and talk to people who live in the home and meet some of the staff. This will help

you make up your mind. You may be able to have a trial stay before you finally decide.

You should ask to see the Care Quality Commission report before you visit.

Before you visit think about the things that are important to you and what questions you want to ask. When you visit the home take the care homes checklist on page 37 with you.

How can Somerset County Council help you?

Assessments If you are facing a major change in your life because of an illness or disability you can contact us. If we cannot provide the information you need over the phone, we will ask someone to visit you and talk to you, and the people who matter to you, so that we can help you decide what you want to do and how we can help. This process is called a needs assessment and is something everyone is entitled to.

Somerset County Council Social Care and Support for Adults assesses the need for adults of working age and older people who have disabilities, mental health problems, a sensory loss or general frailty, and can provide support to people who are most in need. The aim is to actively promote independence and choice and help people to be self-reliant and live in their own homes for as long as possible. When this is no longer possible we help people to choose good quality supported housing or care homes.

Anyone can ask for a needs assessment, regardless of their financial situation. The assessment and advice is free. Some people may have to make a financial contribution towards their care and support; for people who can afford to pay this may be the full cost.

A needs assessment is a discussion to make sure that we understand you, and are clear about what you need. It includes finding out: • what difficulties you have; • what you can do for yourself; • what things you cannot manage at all; • what things you can t manage now, but with some support may be able to do in the future; • who helps you now;

• what you think you need or would like to achieve; and • the best way of meeting your needs.

With your permission we will also talk to people closely involved with you, for example, your carer, family and friends. Other professionals, such as an occupational therapist, physiotherapist or district nurse may be included as part of your assessment.

This will make sure you are given the right information to make choices about your future.

At the end of your assessment you will help to write your support plan. This will list all the things you need and who will help you to do them. We may provide some of this help, if you are eligible (see page 7).

If you are a carer (you provide regular and substantial care to a relative or friend, and are not paid for it), you are entitled to an assessment, even if the person you look after doesn t have help or funding from Somerset County Council. Services that support you as a carer may be included in the care plan of the person you care for.

All the information you provide will be kept securely. We will share relevant information with professionals like a doctor or nurse unless you ask us not to.

Throughout your assessment you can always have your carer, a relative, an advocate or a friend present to support you.

The assessment will take account of your religious beliefs and cultural practices.

Assessments continued How to get an assessment You, or a friend, relative or a health professional like your GP, can ask for an assessment by phoning Somerset Direct on 0845 345 9133. A social care worker may come to see you or the assessment may be carried out by phone first and then followed up by a visit up to four weeks later, depending how urgent your situation is. If you are in hospital, the nursing staff can arrange for a social worker to contact you, either during your stay in hospital, or on your return home.

What happens after the assessment?

There are many different ways you can receive help and support. We will discuss the best way of doing

this with you. If you are eligible for our help towards paying for your care and support (see below) you may want us: â&#x20AC;˘ to buy services from other organisations for you; or â&#x20AC;˘ to give you (or someone on your behalf) money so that you can arrange the help for yourself - this is called a Direct Payment (see page 45).

If we pay for all or some of your care, we will regularly review the help you get. If your eligible needs have increased, we will consider additional help. Equally, we will consider changing, reducing or stopping help from the Council if you no longer need it or no longer meet the eligibility criteria.

Advocacy The Total Advocacy service, a partnership between A4e, Age UK, and the Advocacy in Somerset provides advocacy support for anyone who is FACS eligible (including carers) (see below), and anyone contesting their eligibility.

Paying for care Will we contribute towards your care costs?

Following your care assessment, we use national guidelines called Fair Access to Care Services (FACS) to work out if you are eligible for financial support from us. We will discuss this with you and take into account your views and those of the people caring for you. Whether we contribute to the costs of your care or not, we will always provide help and advice or direct you to someone else who can help.

To make sure that the way people get our support is as fair as possible we use these principles: â&#x20AC;˘ people with the greatest needs will have priority; â&#x20AC;˘ people will have equal access to services regardless of their age, the type of disability they have, or where in Somerset they live; and â&#x20AC;˘ the help people receive will help them maintain

their choice, independence and inclusion in society.

If we can help you with the cost of care, you may be asked to make a contribution - see page 41. There may be things you want to do that we can t pay for.

If you have care and support at home, we will work out how much it will cost and allocate this money to you. This is your personal budget. You will then have the opportunity to have this money as a Direct Payment (minus your contribution) to spend on meeting agreed objectives, or you can ask us to manage your money for you (see page 45).

Working out your contribution Financial Assessment and Benefits Team The The person who assesses your care and support needs may ask for someone from the Financial Assessment and Benefits (FAB) team to arrange to visit you.

This person will talk to you in confidence about your income and capital. They will complete a benefits check to make sure that you are receiving all the benefits that you are entitled to. They also help people by completing benefit claim forms, if

they would like them to, and follow up claims to check that payments have been made.

If you have any money, care or support provided by us, they will work out how much, if anything, you should contribute towards this.

This service is available to anyone who is receiving a service through Somerset County Council.

Will you have to make a financial contribution towards care at home?

If you have over £23,250 in savings and income (or £46,500 if you are part of a couple) or choose not to give details of your finances, you will need to buy your care privately. We can help you to do this if you need us to.

If you have under £23,250 in savings (or £46,500 if you are part of a couple) a member of the Financial Assessment and Benefit (FAB) team will visit you to work out how much you can afford to

contribute. They will look at your savings, income and expenditure relating to any illness or disability you may have. You may not have to pay anything.

Will you have to pay the costs of a care home?

If you have capital or savings of more than £23,250 you will have to pay for any care services you require, but you can still ask us for a needs assessment to make sure the care home is suitable.

We will still provide help and advice.

If you need a nursing home, a specially trained nurse will assess your nursing needs and the NHS will then pay the cost of the nursing care part of

your fees to the home. Currently this is a flat rate of £109.79 per week. The NHS does not pay for personal care.

If you have savings of less than £23,250 you may be entitled to funding from us (see page 43).

If you are moving into a care home, you may be entitled to some of the following financial assistance and support.

Twelve-week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250 and your income is not enough to meet

your care home fees, we will help with the cost during the first twelve weeks of permanent care, providing we agree that care is needed.

Deferred Payments After the twelve-week property disregard period, you can continue to receive financial help from us

by asking for a Deferred Payment.

What is a Deferred Payment?

If your property is taken into account you may enter into a Deferred Payment agreement with us. This means we will continue to pay the part of your care fees you cannot afford after the twelve-week period ends, as a loan, to be repaid when your property is sold. This allows anyone who doesn t want to sell their property immediately, or who can t sell

it quickly enough, to pay their care fees. The FAB officer will discuss this with you.

If you ask for a Deferred Payment at a later date it will start from the date we receive your application and will not be backdated.

What is a Deferred Payment? continued you must pay when you apply for a Deferred Payment agreement. It cannot be added to the loan.

It is very important that you decide whether or not you wish to ask for a Deferred Payment before the end of the twelve-week period. We ask that you let us know by the end of the ninth week so that we have time to set up your Deferred Payment. If we don t hear from you we will stop paying towards your care after this. You will become self-funding and will have to pay the full cost of your care direct to the home.

If you ask for a deferred payment we will place a legal charge on your property and will not charge interest on the loan. If your property remains unsold for longer than 56 days after your death, interest will accrue on the debt at 2% above the bank base rate.

You do not have to sell your home if you don t want to. You may, for example, decide to use rental income to increase the amount that you pay each week. This will reduce the weekly payments made by us and the eventual Deferred Payments debt.

Running out of money If your capital is likely to reduce to ÂŁ23,250, you must let us know well in advance, as we will step in to help with your care fees. We suggest that you let us know when your capital drops to about ÂŁ30,000 to give plenty of time to put arrangements in place. We will complete a care assessment and will normally make a contribution â&#x20AC;&#x2019; this is described on page 39. If the home you have chosen charges more than what we would normally pay for that type of care, you must find someone to help pay the difference â&#x20AC;&#x2019; this is called a top-up payment.

Whoever does this, whether it is your family or a charity, they should realise that they may have to

pay this for some time. If there is no one who can pay your top-up you could talk to the home owner to see if they will reduce your current cost, or you may have to find a cheaper home.

It is important that you understand your rights and obligations before moving into a care home.

There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before making a decision you cannot change.

Attendance Allowance This is a non-means-tested, non-taxable benefit from the Department for Work and Pensions (DWP) paid at the lower rate of ÂŁ53.00 each week for people who need care by day or night, and at the higher rate of ÂŁ79.15 each week for people who

need care both during the day and night. Everyone who needs care should claim this benefit. However, if Somerset County Council is helping to pay the cost of your care, this will affect your claim.

NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care, you may be entitled to a non-means-

tested NHS Funded Nursing Care Contribution (currently ÂŁ109.79 each week) towards the cost of your nursing care. This is paid directly to the home.

NHS Continuing Care Funding If you live in a care home with nursing (usually known as a nursing home ), you may qualify for the full cost of your care home fees, including board and accommodation, under the National Framework for NHS Continuing Healthcare.

If you live in a residential home and need help from a registered nurse, this will be free, covered by the NHS again.

What will you have to pay for your care?

Do you have more than £23,250 in capital and savings?

The value of your home is included when assessing capital unless: • your partner; or • a relative who is over 60 or disabled; or • a child under 16 who you or a former partner maintain, still lives there.

How much?

You will have to pay the full cost of your care, but we will still give you help and advice.

If, apart from your property, your savings are less than £23,250, we may be able to help with your care costs for the first twelve weeks. After this time, any money from us will be regarded as a loan and will need to be repaid once your home is sold.

No Your local authority will contribute financially towards your care costs if you cannot afford them.

Whatever your circumstances: Remember: • If your partner still lives at home, they will not be means-tested. If you have a private pension, only half will be considered when you are assessed.

• Your assessment will be made up of two elements, a care part and a financial one.

• A nursing home will generally be more expensive than a residential home offering personal care only.

How much?

To work out how much they will pay, you must first ask for a care needs assessment. This will make sure the homes you consider are suitable. To do this, phone Somerset Direct on 01823 345 9133.

Your choice of care home will be limited to those that accept Somerset County Council s funding level. More expensive homes will expect you to arrange a third party to top-up the difference. You are not allowed to do this yourself if your capital is below £23,250.

If you have savings of between £14,250 and £23,250 you will be expected to contribute £1 each week for every £250 you have above £14,250, plus an amount from your income.

If you have savings of less than £14,250 you will not have to contribute from your savings but you will have to contribute from your income.

Your social worker will ask a Financial Assessments and Benefits (FAB) Officer to visit you to work out how much you can afford to pay. Nearly everyone has to pay something. Most people will contribute most of their income and will be left with an amount for personal expenses of £23.90 each week.

• You will be eligible for the NHS Nursing Care Contribution ‒ currently £109.79 a week.

Always seek advice: • Independent help is available to guide you through your financial options. There may be a number of solutions to retaining your capital while paying for care.

Fee levels in Somerset from April 2013 The maximum amount we usually pay ranges from £335.76 to £433.88 each week for residential care and £461.73 and £592.50 for nursing care, depending on your needs.

Third party payments If the care home you eventually choose costs more than the rate we usually pay for a person with your needs, someone will have to make up the difference. This extra payment is often referred to as a top-up or third party top-up .

Except in limited circumstances, the law states that you are not allowed to make this additional payment yourself. The responsibility for this often falls to a member of your family or a benevolent sponsor such as a charity. Once this person or organisation has been confirmed, they must sign an agreement formalising the arrangement.

Whoever agrees to do this for you, it is important

they are made aware that the amount could increase in the future and they must be confident that any increases will be met. If payments stop for any reason, you should seek help and advice immediately.

If a top-up stops, we may ask you to move to an alternative home that accepts our fee level.

Because financing care is complicated, we advise you to contact us to ask for specific advice and guidance based on your circumstances and needs.

Personal budgets A personal budget is an amount of money allocated to you to meet your assessed needs and objectives written in your support plan.

If you meet our Fair Access to Care eligibility criteria (see page 40) and have less than ÂŁ23,250 in capital and income (2013/14), there are options for you to consider about how we can support you.

If you have more than ÂŁ23,250 in capital and income, you will need to pay for your care and support and make your own arrangements if you can, but we can provide information and help for you to do this if you would like us to.

Your personal support plan When we visit you we will talk about the things you would like to achieve to help you to be as independent as possible. These are written into your personal support plan.

The plan lists the things you will arrange yourself, or ask someone to do for you. It also lists any money, services or equipment we have agreed to provide for you.

Your personal budget The services we agree to provide in your support plan are calculated into a weekly budget. This is your personal budget and is also shown in your support plan. We currently have two ways that your personal budget can be managed (we hope to introduce more next year):

Option one We can give you a contribution to your personal budget to manage yourself, so that you can be flexible to make your own support arrangements; but it must be used to achieve the things written in your support plan.

Option two You can ask us to hold your personal budget for you and we will organise your support for you. This is called a Local Authority managed budget.

You may also decide to have a mixture of both options.

If you choose option one, the money will be given to you as a Direct Payment. To find out more please read our information sheet B4: Direct Payments guidance .

How is my personal budget calculated?

When we visit you to talk about your care and support needs we will also be able to roughly work out how much any care and support will cost. This is called your indicative budget . Knowing this will help you to start planning your personal support plan.

We then work out how much you should contribute, if anything, towards your budget.

Someone from our Financial Assessment and Benefits team will arrange to visit you to talk about your income and capital. Personal budgets continued Back at the office, your care manager will work out exactly how much it will cost to buy the things you need each week, using known market place values. They then deduct what you have been assessed to pay to work out our contribution.

Our contribution, added to your assessed contribution, is your final personal budget.

This is a weekly amount. Most people have

occasions when they don t use the full amount, for example, when they are away for a few days, of have family staying who can do these tasks instead. We will agree with you what to do with any unused budget.

If you have been assessed to have short breaks, the costs of these are different to your normal weekly cost. Your support plan will show your personal budget for short breaks separately.

Direct Payments A Direct Payment is one way a person can have a personal budget. It is a cash payment given to people who qualify for our support to purchase what they need to meet their personal preferences to achieve the things in their support plan.

Many people choose a Direct Payment as it gives them a much greater flexibility to purchase the service they want or employ the person they choose to meet their needs.

If someone is not able to consent to a Direct Payment, we can arrange for someone to receive the Direct Payment on their behalf, for example, a friend, family member or volunteer.

Direct Payments can also be used to pay for some of your care while Social Care and Support for Adults arrange the rest of it. This is a good way to see if a Direct Payment is what you want.

If you qualify for help, the social care worker who completes your needs assessment will ask you if you would like to use the Direct Payment Scheme.

You must want to use the scheme and be able to manage your care and support arrangements, and keep information about how the money is used.

You can ask someone to help you manage your Direct Payment, such as a friend, family member or volunteer. You will take on the responsibility of directly employing the people who provide your care and you must obey the law as an employer.

If you use the Direct Payments Scheme you will have money paid into a separate bank account that is only used for your Direct Payments. The money can be used to help you with what you want to achieve.

For example, this could be personal care, support to use daytime activities or respite care. You can buy help from an agency or employ your own carers.

You cannot use Direct Payments • for long-term residential care; • to buy equipment for daily living; or • to pay for food, gas, electricity or other utilities.

You can receive Direct Payments as well as benefits.

Your Direct Payment will not affect the benefits you receive.

It is not the same as Direct Payments from the Department for Work and Pensions.

The Direct Payment Advisory Scheme An independent advisory service is available to support people who have Direct Payments. This is provided by a company called Enham. Everyone new to the scheme should meet, or talk, with one of their experienced advisors. They can: • visit you at home to explain the help they can provide; • discuss your responsibilities as a Direct Payment user; • help you to set up a Direct Payment bank account; • help you recruit and employ staff; • help you to set up a system for dealing with tax and National Insurance for someone you employ; • offer a payroll service, if needed; • help and explain how to keep financial records to show what you have spent; and • be at the end of a phone if you need advice.

The advisor will tell you about the financial and other information you will need to keep. Enham offer a payroll service to help you pay your carers correctly.

When you join the scheme Enham will be able to help you manage the paperwork for a short time until you are confident with it. Your Enham advisor will support you through the whole process, from deciding whether or not to use Direct Payments to setting everything up to get going. The advisor is there to support you once your arrangements are set up and if you have any problems or concerns about employing your staff.

Specialist care Dementia care The term dementia is used to describe a range of progressive organic brain diseases. It describes a collection of symptoms, including a decline in memory, reasoning and communication skills, mood changes and a gradual loss of the skills needed to carry out daily activities.

Alzheimer s disease is the best-known form of dementia and accounts for 62 per cent of dementias in England. However, there are over 100 types in total.

Dementia can affect anyone irrespective of gender, ethnicity or social class. Although dementia is commonly seen as a condition of the elderly it also affects people under 65, and people with learning disabilities are a group at particular risk.

The prevalence of early onset dementia is higher in men, with late onset higher in women. It is

estimated that there are about 700,000 people in the UK with dementia. In Somerset there are estimated to be 8,125 people aged 65 and over and 153 people under 65 with dementia. These figures are projected to increase to 13,088 people aged 65 and over and 175 people aged under 65 by 2025.

Dementia services in Somerset These include services to support people with early onset dementia, and care homes that offer a type of care for people with dementia that is unique to Somerset called Specialised Residential Care .

Somerset also has Dementia Advisors , who can provide information to people throughout their lives with dementia.

The best indication of a good home is that the residents appear happy and responsive.

Do staff compile life story books including photographs and mementoes?

If the person with dementia needs, or is likely to need, equipment or adaptations, you may want to check:

Is there a full assessment at home before a resident moves in?

Mental health When someone is worried about their mental health they will usually visit their GP, who may refer them to Somerset Community RightSteps, or if they require a specialist mental health service, Somerset Partnership NHS Foundation Trust.

Somerset Community RightSteps is a free and confidential service to help people in Somerset access appropriate mental health support quickly and easily. Referrals will normally be made by GPs and other health professionals, but if someone is concerned about their emotional health and wellbeing they can contact the service directly by phoning 01278 727447.

Somerset County Council has a partnership agreement with Somerset Partnership NHS Foundation Trust to provide social care for adults of working age who have mental health problems, to

help them make the most out of their lives through a recovery-based approach. The Trust also provides mental health services for young people through a separate agreement with Somerset County Council (CAMHS service). Staff provide support to help manage everyday living, organise activities, employment and training to build self-confidence, and to give encouragement to become part of the local community and be more independent.

There is increased demand for home care for people with mental health problems, arising from greater use of supported living instead of care home placements. Like other specialist care services, this can be paid for (if the person is assessed as eligible) with Direct Payments, discussed on page 45.

Learning disability The Government says that services for people with learning disabilities should promote the following key principles: â&#x20AC;˘ respect people s rights; â&#x20AC;˘ actively promote people s independence; â&#x20AC;˘ make sure that people have, and are able to make choices, in all areas of their lives; and â&#x20AC;˘ create opportunities for people to be included, rather than separated from society.

In Somerset, the Community Teams for Adults with Learning Disabilities will assess people with a learning disability to help understand what support they may need. This could include, for example, help in the home, access to work, or transitions from children s services. The teams have a range of

specialist health and social care professionals who can offer advice and guidance.

Increasingly in Somerset, adults with a learning disability are supported to live in the community, accessing ordinary housing and having support consistent with their individual need.

Physical disability Somerset County Council s Physical Disability Service aims to help people aged between 18 and 64 with a physical disability or sensory impairment to maintain their independence by living in their own homes and communities.

If you have a physical disability or a long-term

illness, Somerset County Council and other organisations can provide a range of ways to support you and give you advice to help you live as independently as possible in your own home.

Support is also available to help the person who looks after you - your carer.

Essential information How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney. Some can also offer guidance on immediate and long-term care plans, making sure (if applicable) the NHS has made the correct contribution to your fees.

Lasting Powers of Attorney (LPA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/ or your property and financial affairs. An LPA is only valid once it is registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable.

The Court of Protection can issue Orders directing the management of a person s property and

financial affairs if they are incapable of managing their own affairs, if they do not have a LPA. The Court procedure is presently very slow and the fees are quite expensive so preparing a LPA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney.

An advance directive allows you to communicate your wishes about of future medical treatment but it is not legally binding. You may instead wish to make a living will, properly known as an advance decision , setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result.

Any proposed gift out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for

Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHS-funded care and welfare benefits.

If you don t have your own solicitor, ask your family or friends for their recommendations.

Contact several firms, explain your situation and ask for an estimate of cost and an idea of timescales involved. Many firms will make home visits

if necessary and will adapt their communications to meet your needs. It s important to find a solicitor who specialises in this area of the law. The Citizen s Advice Bureau offers an advice service and will be able to recommend solicitors in your area.

Keeping vulnerable adults in Somerset safe from abuse Who is a vulnerable adult?

A vulnerable adult is someone aged 18 or over who is, or may be: In need of community care services because of a disability, age or illness and is unable to take care of themselves, or unable to protect themselves against significant harm or exploitation. ( No Secrets 2:3, Department of Health) What is abuse?

Abuse is a violation of an individual s human and civil rights by any other person or people. It can consist of a single act or repeated acts and can take the following forms:

Physical abuse - Including hitting, slapping, pushing, kicking, misuse of medication and inappropriate restraint or control.

Emotional abuse ‒ Including verbal abuse, humiliation, bullying or the use of threats.

Neglect ‒ The repeated denial of help that a vulnerable adult needs which, if withdrawn, will cause them to suffer.

Sexual abuse - Direct or indirect sexual activity where the vulnerable adult cannot or does not give their consent.

Financial abuse ‒ Including theft, fraud, exploitation, pressure in connection with wills, property or inheritance, or financial transactions, or the misuse or misappropriation of property, possessions or benefits.

Discrimination ‒ Including abuse based on a person s ethnic origin, religion, language, age, sexuality, gender or disability.

Institutional abuse - Including poor care standards, misuse of medication, inappropriate restraint, lack of privacy or personal dignity, lack of flexibility, control and choice, lack of appropriate care planning and reviews, restricting access to toilet or bathing facilities and lack of record keeping.

Who commits abuse?

Vulnerable adults may be abused by a wide range of people including: • relatives and family members; • professional staff; • paid care workers; • volunteers; • other service users; • neighbours; • friends and associates; and • people who deliberately exploit vulnerable people and strangers.

Where does abuse happen?

Abuse can happen anywhere, for example: • in the street; • at home; • at a friend s house; • where you work; • at a day centre, residential home or college; or • somewhere you spend your free time.

What are the signs of abuse?

Possible signs of abuse could include: • injuries ‒ bruises, broken bones and burns; • lack of personal care; • bills not being paid; • sudden loss of assets, friends or family threatening a person to gain access to money;

How can I report my suspicions of abuse?

First of all, you should tell someone you can trust.

This could be a: â&#x20AC;˘ family member; â&#x20AC;˘ friend; â&#x20AC;˘ member of staff; â&#x20AC;˘ social worker or nurse; or â&#x20AC;˘ an advocate.

It is everyone s responsibility to know about and report abuse of vulnerable people.

If you, or someone else, are in immediate danger, need medical attention or if a crime has been committed phone the emergency services on 999.

Making a complaint We hope you will be happy with the care and support you choose for yourself or someone who matters to you. However, there may be times when you wish to raise a concern, make a comment or suggestion, or a compliment about the support provided. This may be about anything, from the way you feel you are treated by staff to the food you are served.

You should feel free to make comments and suggestions about possible improvements to your surroundings and the services that have been provided.

All care providers are required to have a complaints procedure and are actively encouraged to record feedback received from service users.

The best feedback about a service comes from the people who use it or have direct experience of it. If you have something you feel should be said, then there are a number of ways you can be heard.

All care providers should have their own easy to use individual complaints policy. If you are concerned about the care that you, or a friend or relative is receiving, it makes sense to speak to the manager of the service about your concerns before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should first contact the registered owners of the service.

If the care is being provided by an independent agency or organisation, ask to speak to the person who handles their complaints and feedback. They have a duty to respond to any complaints made.

The Registered Care Providers Association Ltd Founded in 1981 as a professional association, the Registered Care Providers Association Ltd (RCPA Ltd) represents proprietors, managers and employees of care providers in Somerset and the surrounding area, whether services are private, voluntary, domiciliary or residential. Members work with the elderly, as well as adults with a variety of physical and learning disabilities.

RCPA Ltd provides a valuable gateway, helping the exchange of information and ideas and fostering best practice amongst members. Acting as a voice for care providers, we aim to represent the views of all members, bringing their concerns and queries to the attention of service commissioners as well as government and regulatory bodies.

Amongst their aims is the wish to represent the interests, views and concerns of people involved

with providing care in the Somerset area.

We also aim to provide members easy access to information and advice on any matter relevant to the provision of care and to develop, monitor and evaluate care strategy and policy and to disseminate information to members.

New initiatives Tell Us Once service A free and simple service to help you tell us that someone has died.

When someone has died, there are a lot of people who need to be told, at a time when you probably feel least like doing it. Somerset County Council s Registration Service provides a service which we hope will make things easier for you. This service means that when you tell us about a death, we can then contact other organisations on your behalf.

How the service can help you After someone has died, one of the first things that must be done is to legally register the death with the Council s Registration Service. Once that s done, several other organisations may still need to be contacted and given the same information. To save you time and worry, Somerset s Registration Service

offers a free service which can pass this information directly to a number of other government departments and local council services.

How you can contact us to use the service All Registration Offices in Somerset will automatically offer the Tell Us Once service when you make an appointment to register a death. You can find contact details and opening hours for all of the Registration offices in Somerset at www.somerset.gov.uk/tellusonce. Or, you can phone 0845 345 9144 for information about your local office. The website also tells you more about what information you will need to give us to register a death, and also the services involved in the partnership of Tell Us Once .

NHS 111 Anyone in Somerset can now phone 111 to access urgent healthcare services. The free to call number is available all day, every day to respond to people s healthcare needs when: • they need medical help fast, but its not a 999 emergency;

• they don t know who to call for medical help or they don t have a GP; • they think they need to go to Accident and Emergency or another NHS urgent care service; or • they require health information or reassurance about what to do next.

NHS 111 continued When you phone you will talk with someone from a team of highly trained call advisers who will be supported by experienced nurses. They will use a clinical assessment system and ask questions to assess your needs and determine the most appropriate course of action including: â&#x20AC;˘ callers who are facing an emergency will have an ambulance dispatched without delay; â&#x20AC;˘ callers who can care for themselves will be provided with information, advice and reassurance;

â&#x20AC;˘ callers who require further care or advice will be referred to a service that has the appropriate skills and resources to meet their needs, for example, a pharmacy or their GP; or â&#x20AC;˘ callers who require services outside the scope of NHS 111 will be provided with information about an alternative service.

This new service builds on the expertise of NHS Direct and is gradually being rolled out across the whole country.

Healthwatch Somerset Healthwatch Somerset is the new local consumer champion for people of all ages, taking equal account of both health and social care issues. It actively seeks the experiences of local people to feed them into service planning.

Healthwatch Somerset will provide a range of statutory functions including: â&#x20AC;˘ Providing information and advice to the public about accessing health and social care services â&#x20AC;˘ Making the views and experiences of local people known to Healthwatch England â&#x20AC;˘ Promoting and supporting the involvement of local people in the monitoring, commissioning and provision of local care services â&#x20AC;˘ Obtaining the views of local people and producing reports and recommendations about how services can be improved â&#x20AC;˘ Representing the views of people who use services, carers and the public through a seat on the Health & Wellbeing Board and on the Clinical Commissioning Group.

The Blue Badge Scheme provides parking concessions to people who have severe walking difficulties, are severely sight impaired or receive certain benefits. You can apply for a badge as a driver or a passenger. We have a responsibility to protect vulnerable adults from abuse or neglect. Abuse can be physical, financial, emotional, and sexual, or it can be an act of discrimination or neglect. Somerset Waste Partnership manages waste and recycling services on behalf of all local authorities in Somerset. We offer this free service for people who have mobility difficulties or an impairment that means they cannot carry their recycling or refuse containers to the kerbside when there is no one in the household who can assist them.

Working with many organisations including the Police and other councils, we oversee activities that help make Somerset a safe place to live.

Court House Retirement Home, Cheddar Court House is a very special place to live, the very fact that it is made up of different areas of accommodation adds to its attraction.
