Oxfordshire Support Finder tells you about support services across Oxfordshire to help you or a loved one lead the lives you want to. It is designed to give you help and advice so that you can find your own way to meet any needs for support and care.

Oxfordshire Dial-a-Ride This minibus service, for people with mobility problems, will pick you up from your home. You can catch a minibus between 9am and 5pm but you must book in advance.

Blue Badges The national Blue Badge Scheme helps people with severe walking difficulties that travel as drivers or passengers to park close to their destinations.

Parking Oxfordshire County Council may provide a disabled person’s parking place on a public road where there is a need.

Residents can apply for one of these parking places to be provided close to their homes or work. To apply,

Oxfordshire Community Transport Scheme For a free directory of Oxfordshire’s community transport schemes car schemes and community

Oxfordshire Accessible Transport and Access Group The group works with bus and rail companies to make sure transport in Oxfordshire is accessible and affordable. It raises issues affecting disabled people with County and District councillors, works on national disability issues and contributes to a fortnightly disability column in The Oxford Mail.

If you are a disabled passenger and have a complaint, the group will look into it for you and try to get the issue resolved. Railcards A Railcard will give you discounts on rail fares, online or from a staffed rail station. There are several schemes for passengers with different needs, including carers .To buy your card online, find out about ticket prices or get help planning your journey go to: www.nationalrail.co.uk call: 08448 714036 or email: customer.relations@nationalrail.co.uk.

Public toilets - including accessible toilets Your District Councils (contact details on page 27) have a list of all public toilets in your area and can tell you how accessible they are. They’ll also let you know about local shops and businesses which have accessible toilets.

If you are in Oxford and need to change or use a hoist, go to the Shopmobility Unit at the Westgate car park, Norfolk Street, Oxford. You will find it next to the car park office. The toilet is open from 8am to 6pm.

Some businesses in Oxford have agreed to let members of the public use their toilets whether they are buying anything or not. A full list can be found on Oxford City’s website: www.oxford.gov.uk.

Disability, mental health and loss of sight or hearing Mental health Oxford Health NHS Foundation Trust provides specialist mental health services and physical health care to patients in Oxfordshire. Their teams, made up of psychologists, community mental health nurses, approved social workers and occupational therapists, provide services to people over 65 and younger people with dementia.

Dementia support A number of services offer support to people living with dementia, their families and friends. There are Memory Cafes, training courses for carers, and opportunities to socialise.

Community-based dementia advisers and support

workers, employed by the Alzheimer’s Society, Age UK and the County Council, are available across Oxfordshire. They provide tailored information, advice and support to help people live well with dementia.

Shared Lives The Shared Lives service gives vulnerable and disabled adults the opportunity to live as independently as possible. Adults in Oxfordshire who need care and support can be supported to live in their own home or share a home with a Shared Lives carer.

Health, recovery and wellbeing ‘Reading Well - Books on Prescription’ Oxfordshire libraries are part of the national scheme ‘Reading Well - Books on Prescription’ which is an approved list of books on mental health issues such as anger, anxiety, depression and stress. Local health practitioners can ‘prescribe’ a book from the list or you are simply welcome to self-prescribe and borrow the titles that you think will help you.

The Oxfordshire Reablement Service Go to the Oxfordshire Reablement Service to help you get back to living your life as independently as possible.

It is designed to be a short term service of only one or two weeks and can be extended up to a maximum of six weeks.

Delivered by Oxford Health NHS Foundation Trust, this service: • helps people to maintain or regain their independence by learning or re-learning the skills necessary for daily living; • assists in the accurate assessment of people’s needs for on-going social care support;

• is free at the point of delivery up to a maximum of six weeks for Oxfordshire residents over the age of 18, who need and can benefit from the service and meet the eligibility criteria; • is delivered to people in their own homes; and • is available to people living in the community or who have recently been in hospital.

NHS continuing healthcare NHS continuing healthcare is free care outside of hospital that is arranged and funded by the NHS. It is only available for people who need ongoing healthcare and

meet the eligibility criteria. NHS continuing healthcare is sometimes called fully funded NHS care. For more information visit the NHS Choices website: www.nhs.uk.

NHS Health Checks NHS Health Check is a national programme that aims to prevent vascular disease. Once every five years patients between 40 and 74 years old are invited to have a free Health Check at their GP to assess their risk of developing heart and kidney disease, stroke, diabetes and other diseases.

Once assessed, you are given the advice and support to help you reduce or manage that risk and you will receive free personalised advice about what you can do to stay healthy. For more information visit the NHS Choices website: www.nhs.uk.

NHS Direct NHS Direct operates a 24-hour advice and health information service, providing confidential information on what to do if you or your family are feeling ill, particular health conditions; local healthcare services, such as doctors, dentists or late night opening

pharmacies; and self help and support organisations.

Assessments You can arrange for an assessment online with Oxfordshire County Council’s Social and Health Care team, either for yourself or for someone else at www.oxfordshire.gov.uk/requestassessment. This is also the case for carers. A carer’s assessment is for people who are informal, unpaid carers who look after

a friend, relative, or neighbour. It may help you find out what support is available to you as a carer, if you are eligible for a carer’s grant and an assessment in your own right from a social worker.

Personal Budgets A Personal Budget is an upfront sum of money calculated to meet your needs after you have been assessed. Using Personal Budgets is a way to help you take control over your life, identify your needs and the things you want to do and, if you wish, to manage your own support.

Direct Payments Direct Payments are available to people who meet Adult Social Care eligibility criteria. A Direct Payment is money from Oxfordshire County Council paid into a person’s bank account - you have to spend it on meeting your care needs.

Support brokers An Independent Support Brokerage Service is offered

by Age UK Oxfordshire.

They provide a free service to people who have received either an assessment from Adult Social Care and who are eligible to receive services at home, or are eligible for continuing healthcare.

Personal Assistants Personal Assistants can help with everyday tasks like going to the shops, as well as personal care such as washing and getting dressed.

Looking after someone Caring can be very stressful but you are not alone. At any one time in Oxfordshire over 60,000 people are providing unpaid care for a family member, friend or neighbour. It is very important that you take time to look after yourself while you are caring for someone else.

You may not see yourself as a ‘carer’ - you see yourself as a son, daughter, partner, friend or neighbour. But if you regularly support and care for someone who could not manage without you then you are a carer and there is help available to support you to carry on with this vital work.

If you provide, or intend to provide, regular unpaid

support to someone who could not manage without your support, the law says you have a right to have your own needs assessed, even if the person you care for has refused support services or an assessment of their own needs.

This assessment, called a ‘Carer’s Assessment’, gives you the chance to say what you need as a carer, and to find out what support might be available to help you.

The assessment can also give you information about services available, and other ideas for supporting you in your caring role. Your carer’s assessment will show if you qualify for support services from Adult Social Care.

Carers Voice Oxfordshire is an independent voice for carers promoting carers’ interests, supporting their needs and ensuring their views are heard. For more information please visit: www.carersvoiceoxfordshire.org.

The South and Vale Carers Centre provides free and confidential face-to-face information, advice and support to unpaid carers of all ages living in South Oxfordshire and Vale of the White Horse.

It gives unpaid carers who are struggling financially,

practically and emotionally access to the support they need so their quality of life improves and so they never feel alone in their caring role.

Carers’ grants A carer’s grant is a single payment that you can use towards the cost of something specific that will support you in your caring role. For example, you could use it for leisure activities, education or training, improving your own health

Respite at home Home-based respite services give carers a break from their caring responsibilities.

A care worker comes in to look after the person you care for, so you can have some time to yourself.

Respite at home is considered to be a service for the

person you care for. So if they have a Personal Budget for their own needs (see page 13) they could use that money to pay for it. A financial assessment of the person being cared for will confirm whether they need to pay towards the cost of this service.

Benefits you may be entitled to as a carer Carer’s allowance If you care for someone for at least 35 hours a week and you are over 16 you may be entitled to Carer’s Allowance. The Carer’s Allowance rate for 2014/15 is £61.35 a week. This figure may change over the life of this Directory. However, you cannot claim it if you are in full-time education or employment and it can affect other benefits or pension you receive.

Carer’s Premium If you are over 65 you may qualify for Carer’s Premium.

This can entitle you to an increased amount of incomerelated benefits (such as Pension Credit, Council Tax Benefit and Housing Benefit).

Carer’s Credit Carer’s Credit is a National Insurance credit which will let carers build up qualifying years for the basic State Pension and additional State Pension. This means that there will be no gaps in your National Insurance record if you have to take on caring responsibilities.

Help in an emergency Emergency Carers’ Support Service is a support service for adult carers of adults in Oxfordshire. Experienced staff look after the cared-for person when the carer

has an emergency and is unable to do so. Care can be provided from one to 48 hours and, in exceptional circumstances, up to 72 hours.

Living at home For many people it is important to stay in their own, familiar surroundings near to friends and family and to

retain their independence. But some of us may need a little extra help.

Adapting your home County Council occupational therapists can make recommendations for adaptations to your home and work with you to arrange these. Adaptations include over-bath showers; level access showers; access ramps; stair lifts and grab rails.

Occupational therapy Oxfordshire County Council’s Social and Health Care team take referrals for occupational therapy, which can help identify if you would benefit from any pieces of equipment, such as a walking aid or a second stair rail, to major adaptations such as walk-in showers.

If you have difficulty with day to day activities such as getting washed and dressed or getting yourself something to eat, a care needs assessment by a social worker can help you find out if you may be entitled to carers to come in and assist you.

Improving your home Each District Council in Oxfordshire provides the following assistance to help residents remain safe, secure, independent and warm in their own homes: Disabled Facilities Grants A Disabled Facilities Grant can help eligible disabled people to adapt their homes to make their day to day life easier. This is a means-tested grant - whether you can receive it depends on your other sources of income.

To apply for a Disabled Facilities Grant you must first be assessed by an occupational therapist (see above).

Essential Repair Grants This is a means-tested grant for home owners. An Essential Repairs Grant of up to £5,000 can be made available to help with essential repairs to the home.

Flexible Home Loans This loan scheme for owners and occupiers aged 60 and over is designed to help improve the comfort, safety, security and repairs to your home.

Small Repairs Service Available to owners, occupiers and tenants who are aged 60 and over, disabled and suffering from a long-term health condition. This service covers small plumbing, electrical and basic household repairs.

The labour charge is £15 + VAT plus the cost of any materials used.

Housing options Extra Care Housing Extra Care Housing allows people to have their own home, for rent or purchase, and have flexible care services delivered to them by the on-site team if they need it. This ensures that residents have peace of mind knowing they are able to remain safely in their own home. For many people Extra Care housing is an alternative to a residential care home.

To get Extra Care housing, you normally have an assessment of your care needs from a social worker, and

an assessment of your housing needs by a housing officer.

Checklist for choosing a care home Patient Voice represents members of the public who attend Oxfordshire Acute Hospitals (John Radcliffe, Churchill, Nuffield Orthopaedic and Horton General). Its members have wide experience as patients, ex-staff, university and college tutors.

Patient Voice has drawn up a checklist to help people choose the right care home for their loved ones. This has been developed following comments from members of the public and experiences of members of Patient Voice.

A summary of the checklist is below. The detailed checklist can be found on Oxfordshire Support Finder at: www.supportfinder.oxfordshire.gov.uk.

First steps: • Visit Support Finder, or contact your relevant County Council for information on care home vacancies. A comprehensive list of care homes and care homes with nursing in Oxfordshire starts on page 35.

• Decide the criteria from which you are going to choose a care home such as location and ease of access.

• Take advice about finance.

Once you have narrowed down and visited your choice of care homes, it is important to look at the following areas: • Accommodation.

• Activities.

• Staff.

• Food (and accessibility where necessary).

Of course, this is just an example of how you can better prepare yourself for choosing a care home for a loved one or relative. It is also important to gain personal knowledge of the care home you are looking at. Ask locals if they have any experience of using the care home, or ask visitors about their experience.

Do I have a choice of care home if the Council is paying?

Yes and it can even be in a different county. The home you choose must be suitable for your assessed needs, comply with any terms and conditions set by your local authority and not cost any more than they would

usually pay for someone with your needs.

What if a home costs more than the local authority is prepared to pay?

The local authority will allow a third party to top-up your fees as long as they are able to do so for the long term. You are not allowed to top up the fees yourself from capital below £23,250.

My partner needs care, how does this affect me financially?

The Council will do a joint financial assessment for you and your partner, and will also carry out individual

Care funded by the Council continued assessments on whoever needs care. The contribution you are asked for will be whatever the lower amount

is from these assessments. If you receive joint benefits you will only be offered a joint assessment.

Paying for your own residential care If you are self-funding your care because you are not eligible for local authority funding there are other forms of financial assistance you may be entitled to. The important thing is not to try to do it alone, always seek professional advice.

My Care My Home is Oxfordshire County Council’s approved provider. They offer a free telephone and face to face advice service through home visits aimed at helping people find a suitable home care provider,

independent financial advicer, or care in sheltered or Extra Care schemes and care homes. They are also able to visit people who are currently in hospital and require a package of care before they can be discharged.

Keeping warm in winter You could get between £100 and £300 tax free to help pay your heating bills if you were born on or before 5 January 1952. This is known as a ‘Winter Fuel Payment’.

Protecting vulnerable adults Far too many vulnerable adults suffer abuse and neglect, often at the hands of relatives or people in trusted positions. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited.

Abuse could be physical, sexual, psychological and emotional, or financial. It could also cover the issue of neglect. Institutional abuse affects more than one person within an organisation and is not address by the service’s management.

Domestic abuse Oxfordshire Domestic Abuse services offer emotional and practical support to victims suffering or fleeing domestic abuse.

Having your say How to make a complaint or comment If you are unhappy or worried about Oxfordshire County Council’s Adult Social Care services, it is important to let someone know. Usually talking to someone in the service can put it right. You can talk to the people who provide your care or their managers.

They will have a process in place for managing complaints and listening to concerns that are raised.

Wherever you raise a concern it should be listened to and taken seriously. The good and bad things people raise can help improve services for everyone. You will not be treated unfairly because of anything you raise. It is never wrong to give the service a call.
