Introduction I am pleased to introduce this Directory which provides valuable information about care services for people living in Northumberland.

Getting help If you think you need help, you are entitled to ask for an assessment of your needs by a care manager. This will involve discussing with you how you manage your health, personal care, essential household tasks, how you stay free from harm and how you continue with your current education or employment. With your permission we will ask to talk to any relative or friend who looks after you, a carer, your doctor, nurse or consultant and/or other relevant professionals in order to help establish what help or support you need.

If you are supporting someone ill or disabled you are entitled to ask for a carer’s assessment of your own needs to ensure that they are met. If you need an assessment a care manager will visit you and discuss your needs.

Assessments in Northumberland are provided by care managers employed by Northumbria Healthcare NHS Foundation Trust, who provide adult social care on behalf of Northumberland County Council.

Managing your personal budget yourself need not be complicated, even if you use it to employ care workers – there are organisations which can deal with all the administrative aspects of employment on your behalf.

You may have to make a contribution towards the cost of your support – and if you have savings or a substantial pension, you may be asked to pay the full cost. However you will not have to pay anything for an assessment of your needs.

A care manager will also try to offer any advice you need if you intend to arrange help privately, either with personal care or in areas of life where we cannot usually offer to pay for support, such as housework. If you feel you need a care manager’s advice, please do not hesitate to contact Adult Social Care.

As you read through this Directory you will find details of how to contact our Adult Social Care service and more information on how we assess the needs of individuals.

You will also find a range of other useful information relating to the availability and use of care services.

We want you to be fully informed so that you are able to maintain your independence and are able to make choices about the care you receive which best suits you.

Care managers are people who are trained to understand the effects of disability or illness and the ways in which these can be overcome. Many are social workers, nurses or occupational therapists. If their assessment highlights that you need care and support, your care manager will offer you a personal budget to pay for this. You can choose either to manage the personal budget yourself (this is

Care in Northumberland Staying at home People who have had a serious health crisis, accident or have just come out of hospital often need support for a short period in their home while they recover. One of the ways we do this is through our Short Term Support Service (STSS). This team works with people to help them regain their independence, skills and confidence.

This support will usually be provided for up to six weeks but it may only take a single visit, or support for a few days to get you back to your usual routine. Depending on the person’s needs the service may provide: • a rehabilitation programme led by a physiotherapist or occupational therapist • support with daily living and personal care tasks such

as washing, dressing, or meal preparation. The service aims wherever possible to do things with people, rather than doing it for them, to help them recover their independence • arranging pieces of equipment or minor adaptations to the person’s home to help them live safely and independently.

The team includes care workers, therapists, rehabilitation workers and healthcare assistants. There is usually no charge for this service. However, you may have to pay a charge if the service lasts more than six weeks.

The Short Term Support Service contact numbers can be found on page 20, listed under ‘Adult Social Care’.

Crisis support at home Some people urgently need support at home, though they would not benefit as much from a programme of rehabilitation or re-ablement like the Short Term Support Service. If you are in this position, we can arrange crisis support home care. If you only need support for a brief period, this may be all that you require; if you need longer-term support, this service can fill the gap while you choose how you want to be supported later on.

If we think you may need crisis support, we will arrange

for a care manager to see you. They will discuss with you how much support you need. If you are likely to need support for more than a few weeks, they will also start assessing your long-term needs and helping you to make choices about how to meet them.

There is a charge for crisis support home care, though we will only ask you to pay as much of this charge as we think you can afford to pay. See page 12 for information about charges.

Self-directed support and personal budgets If you need longer term support to maintain your safety, independence and dignity at home, Adult Social Care can help with this, based on an assessment of your needs. Whatever your illness or disability, and for adults of any age, we aim to make sure you are in control of the support you receive, with personal budgets providing you with as much opportunity as you wish to choose your own support arrangements.

Self-directed support and personal budgets can be used by people with a range of needs including older people and adults with an illness or disability.

People are offered a personal budget which they are encouraged to manage themselves, or with help from family and friends, though we can also make arrangements on people’s behalf. We set the amount of the personal budget based on an assessment of their needs.

Personal budgets can be used flexibly to overcome the obstacles which illness or disability put in the way of living in the way you choose. Depending on your situation this could include personal care and assistance to live in your home or to take part in leisure and educational

activities, support to help carers get a break, and items of equipment designed for people with a disabiity.

If someone decides to employ their own care worker, they will become an employer with all the responsibilities that being an employer brings. The support planning team can advise you on how to go about this.

You will need to agree a support plan with us showing how you will use your personal budget. We will check that this is safe and legal. Our support planners can also help you to draw up your support plans, or if you want you can do that yourself.

For the person who receives the personal budget, the money is not classed as income and does not affect any benefits you may receive.

If your disability or illness does not cause you any problems in critical areas of life, you may not be eligible for a personal budget. However we may still be able to give you advice about services and equipment which might help you, and about other possibilities such as adaptations to your home (which you may be able >> to get financial help for).

Self-directed support and personal budgets continued If your disability or illness is so serious that you cannot realistically stay in your own home, we can help you to move into supported housing or a care home.

Equipment If you have a disability or illness that makes daily life difficult, you may be able to get equipment, such as a walking aid, or home adaptations, such as handrails, to make life at home easier for you and anyone who helps you with care tasks. Northumbria Healthcare provides equipment free to meet both social and health needs through its Joint Equipment Loans Service (JELS).

To get equipment a health or social care professional will assess the needs of the person who needs help and the needs of the carer. You can ask to see an occupational therapist by contacting Adult Social Care. If you choose to buy your own equipment you still have the right to ask for an assessment beforehand so that you can choose equipment with the benefit of skilled professional advice.

Adaptations to your home may be recommended following an assessment by an occupational therapist, who will pass on any recommendation to the Home Improvement Agency or Housing Department who will offer help.

Telecare Telecare is equipment that can detect falls, inactivity, smoke, flooding, gas or extreme temperatures in the home. Sensors are triggered, and an alert is received either by trained operators at a 24 hour call centre, or directly by a relative or carer. Telecare can allow users to remain independent and relatives and carers are reassured, safe in the knowledge that should an incident occur, they will know about it. Some devices can also help monitor particular health conditions and reduce the need for a hospital admission.

Telecare can help restore confidence for people who feel vulnerable in their own homes for a variety of reasons – including living alone, frailty or the need to summon help in an emergency. It could also be as a short-term measure during a period of convalescence.

A number of organisations provide telecare services in

Northumberland, including the council’s own county wide service, Northumberland Telecare (formerly known as Valley Care). If you would like to find out more speak to your care manager or contact Northumberland Telecare on 01670 827100.

Social care and health information points We have five information points based in libraries in Morpeth, Amble, Berwick and Haltwhistle and the Council information point in Cramlington. These are staffed by our support planners, on specific days, who can offer you advice and information about local support groups for people living with a disability or illness. They can also advise on issues that you may have about getting around at home

or going out, activities available nearby, your entitlement to support and organisations who provide care and support.

Shared Lives Northumberland Shared Lives Northumberland provides care and support to adults with a learning or physical disability, a mental health problem or an older person in the Shared Lives carer’s own home. People who come to stay with carers are treated as part of the carer’s own family and either live there long term or for short breaks, for example when their families are away.

The aim of the scheme is to maximise the independence of individuals, provide high quality individualised care and enable people to fully participate

in social and community life.

We envisage when someone comes to stay for a short break with one of our families it will be a more of a holiday experience.

Shared Lives Northumberland is run by Northumbria Healthcare NHS Foundation Trust in partnership with Northumberland County Council. It is regulated and inspected by the Care Quality Commission (CQC) and a copy of the scheme inspection report can be seen on the CQC website.

Short break care You may have a relative or friend living with you, or perhaps living nearby, who helps you on a regular basis, a ‘carer’. Caring can be rewarding, but it can also bring stresses and strains, and many carers welcome the opportunity to have a break from the caring responsibilities. For peace of mind, you may need someone else to be with you so that your carer can have time to themselves and you may be able to make arrangements with other members of your family or friends to spend time with you.

Whether you have a carer or not, you may wish to have a short break as a means of taking a rest from the day to day tasks you need to manage in order to maintain your

own independence, or from the regular care that you receive.

If your assessment confirms that you or your carer need help to take a break, a care manager may be able to arrange a short break for you. This could be a sitting service at home, somewhere to go during the day or a short stay away from home in an appropriate setting. You can also choose to make your own arrangements using a personal budget.

Even if you do not require financial support from Adult Social Care, a care manager can offer advice about making arrangements for your break independently, if that would be useful.

NHS Continuing Healthcare NHS Continuing Healthcare is a package of on-going care arranged and funded by the NHS, when it has been assessed that the individual’s primary need is a health need. It can be provided in any setting. In a person’s own home, it means that the NHS funds all the care that is required to meet their assessed health needs. In care homes, it means that the NHS also makes a contract with the care home and pays the full fees for the person’s accommodation as well as their care.

A national policy has been drawn up by the Government that sets out the types of conditions that

will meet the eligibility criteria for continuing health care.

Some people may be eligible for shared care, with some of the costs paid by the NHS and some by social services.

You can ask for an assessment by health and social care professionals if you think you may be eligible to have some of your care paid for by the NHS. Care managers can advise you about this.

Regular reviews will be carried out so if your needs change your care and/or funding arrangements will change to meet them. You can pick up a leaflet about NHS Continuing Healthcare from your GP or hospital.

Sheltered (or retirement) housing This is a practical step if you would like more security and companionship but don’t need to move into a care home.

In a sheltered (or retirement) housing scheme you would live independently in a flat or bungalow built especially for older people within a larger complex, sometimes around a garden or communal facility and linked to a central control centre for security.

Some schemes have access to a resident or nonresident manager who provides support, advice and a link to services for older people. After your needs have been

assessed, you will still be able to receive care in the home if required.

Extra care This type of housing offers older people who need more support the chance to continue living independently within their own self-contained properties. Couples and friends can stay together and there is a mix of able and less able people. Extra care schemes offer higher levels of on site care compared to sheltered housing.

Each resident is assessed in the normal way to work out how much care they need and there may also be support on call for all those in the scheme. In addition to personal care, extra care schemes may also offer domestic support, meals and leisure and social activities. Residents can choose to join in social activities or remain private.

In Northumberland, The Manors which is located on the West Wylam estate in Prudhoe provides 46 one and two bedroom flats. There are 34 for rent and 12 for shared ownership. It is owned and managed by Isos Housing in partnership with Northumbria Healthcare NHS Foundation

Trust, and Northumberland County Council. The building also plays a key role in the local community with a restaurant open to the public and internet café available. To make an enquiry, phone Isos Housing on 0191 223 8321.

Northumberland County Council and Northumbria Healthcare are also building 12 additional extra care flats to support older people as part of a new hospital facility being built on the site of the existing Haltwhistle War Memorial on Westgate. This is due to open in 2014.

Supported Living Supported Living is the term given by Local Authorities to offer people their ‘own homes’ via a tenancy or home ownership or shared ownership and have personal and housing related support provided by an outside organisation.

Hatfield Chase provides accommodation with flexible and individual support for people with a learning disability to help them to live independently in their own home. It consists of 10 bungalows in Bedlington, each

with its own bedroom, lounge, kitchen, bathroom and garden. All bungalows are also wheelchair accessible and have special equipment and sensors, called telecare (discussed on page 6), which provides tenants with an emergency on call system.

Staff are on hand seven days a week 24 hours a day to provide as much or as little support as required.

Daytime activities If your disability, illness or age prevents you from leaving your home to socialise and you have been assessed as needing support from Adult Social Care, you can use your personal budget (explained on page 5) to overcome obstacles which prevent you from pursuing your own interests and make use of existing leisure and social facilities which provide day time activities where you live.

You may also be entitled to support to participate in day time activities to give the person who looks after you a break.

Website www.northumberlandlife.org is a local directory of groups and services in and around Northumberland. It is a valuable reference for anyone who wants to know what Northumberland’s communities can offer, from parents looking for activities for young children to older people looking for like-minded groups; from people who want to take up new activities to people who want to make contact with others affected by the same disability or illness.

Day services for older people are also available in

day centres managed by a number of voluntary sector organisations. Northumberland County Council manages specialist day centres for older people with dementia at Essendene Day Service in Bedlington and Tynedale House Day Care Centre in Blyth and there are specialist dementia day services in other areas run by voluntary organisations. Access to any of these services can be arranged for people who need it through Adult Social Care. See page 20 for contact details.

Day services for people with a learning disability are also provided within Northumberland for those people who are assessed as requiring this service. These offer

Daytime activities continued a range of activities depending upon the needs of the individual. They provide care and respite for people who have more complex needs but also work experience and voluntary activities in most Northumberland County

Council day services. There are also opportunities for people to explore alternative activities such as supported work and college places in various community settings across Northumberland. See page 20 for contact details.

Care homes If your care needs are too great to be met in your own home, you may need to consider the alternative of moving into a care home. There are two types of care home: a residential care home and a nursing home. If you think you may need to move into a care home, we recommend contacting Adult Social Care to discuss your options. Even if you will be able to pay for a care home place yourself, you may find it helpful to discuss what other kinds of support might be available.

All care home providers must be registered with the Care Quality Commission. Homes are categorised by the type of care they provide and are listed in this Directory.

All homes are regularly inspected by the CQC, who report on their findings. These inspection reports are available at the home or from the CQC website www.cqc.org.uk Both residential care and nursing homes provide personal care, including help with bathing, feeding, dressing and moving around. If your assessment says that you need to live in a care home, a care manager from Adult Social Care can make arrangements for you.

People supported to live in a care home will usually have to pay at least part of the fees, unless they are eligible for support arranged as NHS Continuing Healthcare. Your care manager can advise you about whether you may be eligible for this. If not there will be a financial assessment to decide how much you can afford to pay, taking account of your income, your savings and the value of any property you own.

Residential care homes (personal care only) Moving to a care home is a major change in a person’s life and needs very careful consideration.

If you feel you can no longer live in your own home

but do not need active nursing care, a care home offering only personal care may be your best option.

Care homes with nursing If you need nursing care, even if you are not entitled to NHS Continuing Health care to pay full fees, you are entitled to an NHS payment to cover the element of the fees of the care home which is specifically for nursing care. This applies whether or not a care manager has arranged your place in the home. If you are making private arrangements to move into a nursing home, you should ask to have your nursing needs assessed before going into the home, to make sure that you know whether you are eligible. The care home manager will be able to help you to arrange this.

If a care manager makes arrangements for you to live in a care home, whether residential or nursing, private, voluntary or County Council run, you have certain rights and choices.

On moving in, you should be given a personal plan, written by your care manager and agreed with the home.

This will define which is your room and the basic service you can expect.

Should you have to leave the home, to go into hospital for example, your place in the home will be kept for you for at least six weeks and you will have to keep up payments.

The home will liaise with your care manager throughout.

Once in, you have a right to leave at any time but you can also stay for as long as you wish, provided your behaviour doesn’t impact on the quality of life of other residents, and you observe house rules.

Should your needs change, you may need to move to a more suitable home - a care manager will help with this.

Specialist care Learning disability Adult Social Care will usually try to arrange support for people with a learning disability to live in ordinary housing, perhaps sharing with people. However, there are also care homes that specialise in support for people with learning disabilities. A good home will provide opportunities to learn and maintain skills, and opportunities to take part in enjoyable and stimulating activities. Social workers and community nurses from Northumbria Healthcare who specialise in learning

disability are able to give information to enable people to make the right choices for themselves.

Dementia For some people with dementia who are no longer able to stay in their own homes, moving to a care home can help to maintain and improve their quality of life. People with dementia should be able to live in a care home >>

Specialist care continued that offers quality personal care and person-centred care.

Some care homes offer specialist units with experienced staff and facilities adapted to the needs of residents with dementia.

People with dementia can benefit from a range of group or individual therapies. Most people benefit from social interaction and mental stimulation. Activity coordinators and occupational therapists can provide people with reality orientation, reminiscence and validation therapy. Music, art and other creative therapies can also be helpful. Behaviour management approaches that promote understanding of the behaviour of the person with dementia can also be helpful and trained staff can offer this service within many establishments.

Mental health Most people with other mental health needs are able to live in ordinary housing with support, and care managers will

always start by trying to find solutions which enable people to do so. But for a small number of people with complex needs, residential care may be able to help improve the quality of their life, perhaps as a short term arrangement.

The structured environment and activities provided can build up confidence. The care providers should assess the person carefully to make sure the placement is right for the individual. A qualified member of the residential care staff may visit the person at home, talk to them and look at their assessments of health/social care need.

To find out more organise visits to prospective care homes. Ask residents about the care they receive. Are psychiatric nurses available if needed? Do the activities provided assist rehabilitation? What are the attitudes to medication and other therapies? How does the care home approach different types of challenging behaviour?

Do the staff have close links with the Community Mental Health Team?

Out-of-county care Even if you are asking a care manager to arrange your place, you can choose a care home outside Northumberland. You may want to be closer to friends, family members or you may want to relocate to another part of the country.

The home you choose must be suitable for your assessed needs and comply with Northumberland County Council’s contractual terms and conditions. The Council will usually be able to pay at most the level of fees that

the local authority pays. If you choose a more expensive home, you may have to find funds to pay the difference in cost.

If your care manager is arranging your place, they will be able to get information about options in the area you want to move to. You can also get free information on care provision in other regions, by contacting Care Choices on freephone 0800 389 2077 with details of your requirements.

End-of-life care National Minimum Standards outline requirements that homes must meet in relation to staff dealing positively with the death of a resident with dignity and respect.

Important questions to consider when assessing homes on this matter would include:

• What staff training is in place?

• How are families involved with any advanced care planning?

• Are ‘Living Wills’ implemented and followed?

• How are the religious, social and spiritual needs of residents met?

• Can relatives stay at the home during the final stages?

Carers’ support If you support a friend or family member who is ill, disabled, frail in old age, or someone with alcohol or drug related problems, then you are a ‘carer’. The care you provide may be emotional, physical or practical help.

Caring for someone can be very rewarding but it can also be incredibly stressful and exhausting at times and take a toll on your own health and wellbeing. You may need help

yourself. You can discuss your needs with the care manager as part of the assessment for the person you look after, or during the review of their care plan. We will also invite you to contribute to the discussion of the needs of the person you care for (their assessment), though we will usually need to discuss with that person whether there are some issues they wish to talk about privately.

Carers’ support Alternatively you can ask for a separate assessment of your own needs, this is called a carer’s assessment. This can happen even if the person you care for refuses help.

Some of the ways we can help include arranging for someone else to look after the person you care for so that you can have a break; arranging training and advice for you about how to provide help safely and easily; putting you in contact with other carers who can share their experience with you and with other organisations who may be able to help. Your care manager will endeavour to find solutions to best support you in your caring role.

Local carers’ services Carers Northumberland is an independent local charity which exists to support anyone caring for a partner, relative or friend who needs help because of any illness or disability. This includes: • a one-stop information service - on issues affecting carers and signposting to other services • individual support including emotional support • the opportunity to meet other carers locally • help to access short breaks from caring

• support when your caring role changes or ends • a carers emergency card scheme in partnership with British Red Cross • a unified carer voice - to ensure that carers have an impact on decision-making and can influence local policy, planning procedures and outcomes.

Safeguarding vulnerable people from abuse People who are ill or disabled may be particularly at risk of being abused or exploited by others, or being neglected when they need help. Compliments, comments and complaints continued Complaints about care homes and social care services If you have a complaint about a care home, care home with nursing or any other social care service, the first thing you should do is tell the home or service. This applies whether you pay for your care or your local council funds it.

By law, every care home and social care service must have an efficient procedure for dealing with complaints.

Ask for a copy of the service’s complaints procedure so that you understand what you must do and how they will deal with your complaint.

The care service should try to sort out your complaint themselves and put things right to prevent the same thing happening again.

Paying for care There are no charges for NHS services or for seeing a care manager. In addition you will not have to pay the charges for home care, day time activities, meals on wheels or short break (respite) care if: • you are receiving after-care following compulsory treatment under mental health legislation • you are living in a care home • you have Creutzfeldt-Jakob disease (although short break charges may still apply).

The charges quoted below (as of August 2013) are re-considered at least annually.

Charges for non-residential social care services A financial assessment and benefits officer will visit you to assess your ability to pay, based on your income and

savings. This person will also be able to answer any questions you may have about paying for services.

Home care The standard charge is currently £3.21 per quarter hour of care, up to a maximum of £251.75 per week for people who can afford to pay that much.

Many people do not have to pay as much as this and some people don’t have to pay at all. If you are receiving a personal budget (see page 5) these can be used to purchase home care services directly from the supplier.

Day services Charges for day services vary depending on how much it costs to provide. However you will only have to pay for the service itself if your financial assessment says that you can afford to – and the maximum anyone has to pay for home care and day services is £251.75 per week. These

charges do not include the costs of any meals provided in day services, or transport to day services. If you use these there are charges of £2.50 a day for meals, while transport provided by the day centre, or funded by us to get to the day service generally costs £2 a day.

Short break care charges Charges for this will vary, depending on your financial circumstances. If you depend on social security benefits entirely, you will pay between £47.80 and £181 a week. If you have savings of more than £23,250 you will have to pay £419.58 a week. This will increase if you receive more than 56 nights of care in a year.

Top-up fees If you want to be supported by Adult Social Care in a home which costs more than the rate Northumberland County Council usually pays for someone with your needs, you will have to ask someone else to make an additional payment. This means that they will have to pay the difference between the Council’s rate and the amount the home charges. This additional payment is often referred to as a ‘third party top-up’. You should establish whether a top-up is required before choosing a care home. It is important to note that if there is no top-up required when you take residency the care provider should not introduce one at a later date.

The law says that you are not allowed to make this additional payment, except in limited circumstances, because your own money is taken into account in setting charges. Therefore, the additional payment normally has to be made by someone else, for example, a family member or charity.

Before anyone agrees to make additional payments on your behalf they should be aware that the amount could increase and they need to be sure that they can sustain the payments for as long as they are required. If the additional payments stop being paid for any reason, then you should seek help and advice from your care manager.

Charges for residential and nursing care If you have capital or savings of more than £23,250 you will have to pay for any care services you require but you are still entitled to an assessment of your needs by Adult Social Care. If you have assets of less than £14,250, you are likely to be entitled to funding from the Council, though you will still have to make a contribution from your income, and if you have a substantial pension, you may have to pay the fees in full. Between these two figures, a sliding scale operates whereby you must pay £1 per week for each £250 you have above the lower figure, in addition to the amount you are assessed to pay from your income. Assuming you will be moving into care, you may be entitled to some of the following financial assistance and support.

Twelve-week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250 and your income is not enough to meet your care home fees, the Council will help with the cost during the first twelve weeks of permanent care.

Deferred Payment Agreements After the twelve-week property disregard period, any financial help from the Council will be charged against the value of your home and recovered once your house has been sold. However, the Council may limit how much they will pay and it may affect your entitlement to pension credit if your property is not seen to be

on the market and becomes treated as capital by the Department of Work and Pensions (DWP).

Attendance Allowance This is a non-means tested, non-taxable benefit from the DWP paid at the lower rate of £53.00 per week for those needing care by day or night, and at the higher rate of £79.15 per week for those needing care both during the day and night. All older people who are ill and disabled can, and should claim this benefit. Although you may not receive payments if you are in a care home and part of your fees are being paid by the Council.

NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to a non-means-tested NHS Nursing Care Contribution (currently £109.79 per week) towards the cost of your nursing care. This is paid directly to the home.

NHS Continuing Healthcare Funding In certain circumstances you may be eligible for NHS Continuing Healthcare services. If this is applicable you may qualify for the full cost of your care home fees, including board and accommodation to be met by the NHS. Typically Continuing Healthcare services are provided in a nursing home setting but they may also be provided in your own home or in a residential care home. >>

Charges for residential and nursing care continued Running out of money If your capital is likely to reduce to £23,250, you must let the Council know well in advance as they will step in to help with your care fees. They must conduct an assessment of your situation and will make a contribution - described on page 5.

If the home you have chosen charges more than this, you must find someone to help pay the difference – a ‘top-up’ payment. Whoever does this, whether family or a benevolent charity, they should realise that they may have to pay this for some time. Alternatively, you would

have to find a cheaper home.

Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself.
