Introduction from Manchester City Council The Directorate for Adults, Health and Wellbeing at Manchester City Council provides a range of services for adults in Manchester, from traditional social care, to homelessness prevention and helping people into employment.

The Directorate focuses on reducing people’s need for care and support through a preventative, enabling approach where people are supported to be as independent as they can be through services such as Reablement and telecare.

The aim of this Directory is to provide Manchester residents with details of services which are locally available to meet their social care needs such as care homes as well as other key services. With so many care services to choose from, it can be difficult sometimes to decide which is the most appropriate for you or a loved one. If you think you need some form of care, your first step should be to contact Manchester City Council’s 24 hours Contact Centre. This telephone-based service will be able to advise if you are eligible for services or support from Manchester City Council.

The Contact Centre provides clear and consistent information and advice about adult social care services across Manchester. This is a single contact number that you can call with all your social care

concerns and, in many cases, our advisors will be able to solve your query with just one phone call.

Assessment When you get in touch, we will ask you some questions about yourself to begin to assess your needs according to the Fair Access to Care Service criteria set out by the Government. Everyone is entitled to a free Community Care Assessment. The assessment determines what care needs you have and your eligibility for social care support.

If you are not eligible for social care support, we

aim to help people find other ways of meeting their needs by redirecting them to other organisations, by helping them to maximise their own resources or by giving advice.

You are also entitled to an assessment if you are a carer. The carer’s assessment will cover many things, including employment, lifelong learning and your wish to take part in leisure activities.

Planning your support Services come in many different shapes and sizes.

Some will suit you more than others - and there may be more on offer than you expect.

Social care services are not simply split between home care and residential care. There are lots of other community services, like extra care housing or sheltered housing. These can help you to stay as independent as possible, with support available when you need it. You may be able to mix and match different types of support, including regular short breaks (respite care) for a relative or friend who supports you.

times that fit in with your life • join a support group, or pay for someone to support you on holiday • sign up for voluntary work or courses to bring your work skills up to date • employ a personal assistant to support you to attend college • buy assistive technology to keep you safe in your home, such as mats to alert when someone is out of bed • take driving lessons to help you get out and about, or travel to work or college.

You might want to: • employ a Personal Assistant, to help you take care of yourself and get out and about • buy a freezer and a microwave, to make sure that you eat and drink enough • buy a computer and broadband for online grocery shopping and banking • agree flexible times with your home care providers so that you can get up and go to bed at

Care at home If you are finding it difficult to manage at home, family and friends may be able to help.

Alternatively, you could consider using the services of a home care provider who employ care assistants to provide help in your own home. They can assist with personal care such as bathing, washing and dressing. They will be trained in personal care and safety procedures, moving and handling and hygiene, and some home care providers employ registered nurses.

All home care providers are registered with the Care Quality Commission (CQC) who inspect them against the national essential standards of quality and safety. Community alarm service The community alarm service provides peace of mind for people who feel at risk in their own homes. It gives you freedom to live your life independently knowing that you can obtain assistance when you need it within your own home.

There are many reasons why people need

support, from ill health to fear of crime. The community alarm service operates 365 days 24 hours a day. Your call is answered within seconds by an experienced Contact Officer who can summon help from the 24 hour Response and Installation team, Emergency Services, your doctor or a nominated relative or contact.

Everyday life at home Most people want to look after themselves as much as possible. Some of us need help and support to live independently. You might get help with some tasks from other people - like friends, family or professional carers. But with the right equipment, you might find you can do more for yourself than you thought.

Some really simple pieces of equipment can make a big difference. Sitting down and getting up again might be easier if you have your armchair raised on special blocks. With a ‘helping hand’ grabber you can pick up small items from the floor without bending down, and a special lap tray will help you keep a plate steady while you eat. If you find it difficult to use electrical plugs, turn handles and knobs, or use keys, you can get equipment to help with those tasks too.

There are pieces of equipment, such as flashing lights or pager systems, which can help if you

are having difficulties hearing the doorbell or the phone. If you are unable to use the phone any more, we can consider either amplified phones or alternative methods such as fax or textphone (minicom).

Help in the kitchen There are lots of items that can make it easier to prepare meals and drinks safely and to help you if you find it difficult to use ordinary kitchen equipment. There are electric can openers, special bread boards, easy-grip peelers and graters, and safe kettle tippers.

People who are blind or visually impaired can get talking microwaves, scales and measuring jugs or labelling equipment for kitchen appliances and food containers.

Getting out and about Some people find it difficult to walk, and walking sticks and walking frames can provide the extra support that’s needed. These walking aids can help in the home and also make it possible for people to get around outside the house. People with visual impairments might find a white walking stick or cane useful, and we can also train you how to use them.

Wheelchairs and electric scooters can make it possible for people with mobility difficulties to travel around on their own. We don’t provide this type of equipment directly but we can help you find a supplier. When purchasing your own electric

scooter, we strongly advise you to consider how you would access your home to arrange recharging the scooter battery, as you may not be eligible for a ramp. This can add additional cost.

There are adaptations to your home that can make it easier and safer to move about. Simple things like grab rails fixed to the wall to help you steady yourself can make a big difference. A stair lift can remove the risk of a fall and ramps can make things easier for people who use a wheelchair. If you would benefit from a home with specialist adaptations, we can help you find a suitable place.

Extra security at home Even if you are used to living independently at home, you still might need to know that you can call on help in a crisis.

There is technology available right now that makes it easy for you to call for help - day or night if you have a fall, are taken ill or you are frightened by an unwelcome visitor. We even have equipment that can automatically alert our control centre if the bath or sink overflows, or if there is smoke in the house. This sort of equipment - and the emergency response service backing it up - can give you, your friends and family real peace of mind that someone will step in if you have a problem.

There are some small items of equipment that we provide on request, but usually one of our expert staff will visit you at home to carry out an assessment of your needs. This will look in detail at your situation and help work out with you what equipment or adaptations would best help. If there is someone who cares for you, we might look at what could help them too.

‘Just Checking’ We also have a system called ‘Just Checking’, which is usually used for a short time to find out how safe somebody is in their own home and work out how much support they need. ‘Just Checking’ is a set of movement detectors that are installed temporarily in someone’s home - with their permission of course. Information about the person’s routine is then analysed by computer and shows, for example, whether they are getting up and moving about the house at odd times in the night, or perhaps leaving a front or back door open.

In many cases, ‘Just Checking’ can reassure people that they are more able to live independently than they, their relatives or other people thought.

Care homes There are two types of care home: homes offering personal care only, and homes offering ‘care with nursing’. Your care needs assessment carried out by the Directorate for Adults, Health and Wellbeing will help determine the best type of home for you.

Personal care only This type of home will suit you if you are reasonably active but require help with tasks such as bathing, eating, dressing and moving about and cannot remain in your own home without the need for 24 hour support. It may be worth considering what your future needs may be. Will the home be able to meet them in a few years time?

Care home with nursing Traditionally called a ‘nursing home’, this kind of home is suitable for those needing nursing care, where it will be delivered by qualified nursing staff alongside care workers. Nurses are on duty around the clock. Fees are generally higher than personal care only homes because of higher ratios of staff to residents.

Some of the cost of your nursing care will be met by the NHS, who will pay the home directly. At the moment there are two rates for NHS Continuing Healthcare and NHS funded Nursing Care: the standard rate is £108.70 and the higher rate is £149.60 per week. Figures may change during the life of this Directory.

Assessment and quality of care providers The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Since October 2010, all care providers must be registered under a new system, introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety.

CQC hold and publish up to date information

about the compliance of adult social care providers with essential standards of safety and quality.

Following an inspection, each care home, care home with nursing and home care agency is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC website: www.cqc.org.uk.

Inspections are unannounced and focus on the standards of care that people who use the service receive and whether they are happy with their care.

Personalised care and support Manchester City Council’s Directorate for Adults, Health and Wellbeing aims to replace the traditional system of social care, by putting people in control of the support they need to live the life they choose. It recognises that some people can manage their support on their own whilst others need help - from family or friends or people who are paid to help.

Ways of paying for care When we complete an assessment, we look at how much help you need and work out how much we think that support should cost, which gives you a total budget.

You will have to make a Support Plan. We may

be able to make a contribution to the cost of your support, and we will be able to let you know exactly how much you will have to pay when we have completed a financial assessment.

We then give you several options: • You can organise your care yourself. We call this a ‘Cash Individual Budget’.

• You can choose someone else to organise care for you, though they may ask you to pay them for this. We call this an ‘Individual Service Fund’.

• You can choose to have the Council organise your care. We call this a ‘Virtual Individual Budget’.

Personalised care and support continued organise your own care. The Council pays you an agreed amount of money. You can then hire the support service of your choice. That way they work for you and not for the Council. Even though the money for all or part of your support might come from an Individual Service Fund or a Virtual Individual Budget, you are the customer. You have a right to be listened to and your opinions should be respected. We are here to help you and will do everything possible to respect your needs.

You will need to do your homework if you want to organise things yourself. In Manchester you can

speak to your Primary Assessment Team member, Social Worker or the Self-Directed Support Team before you make a final decision. They can tell you the different choices and community services available for you.

• Individual Cash Budgets or Individual Service Funds are available to residents and carers in Manchester who have been assessed as being eligible for social care support.

Paying for care Social care, unlike health care, is not free for everyone. Councils only have a limited amount of money and cannot provide services directly for everyone.

If you have been assessed as being eligible for social care support, we will look at what help you need and what it should cost. This gives you an overall annual individual budget for the cost of your care.

If you have been assessed as needing an individual budget to pay for your support, you will need to let us see details of how much money you have. The Council works out if you are able to pay some or all of the charges yourself. This is called a financial assessment.

An officer will contact you with options for completing your financial assessment form. This may be through a visit at a suitable time and place of your choice or via a self assessment form.

You will need to sign a consent form which allows us to speak to the Department for Works and Pensions and other Government services on your behalf. This will help us to work out your charges and make sure you receive the right welfare benefits.

If you do not provide us your financial details or do not want a financial assessment, you will have to pay the full costs of your service.

NHS Continuing Care Funding NHS continuing healthcare is a package of care which is arranged and funded solely by the NHS to meet physical and/or mental health needs that have arisen because of disability, accident or illness.

You can receive continuing healthcare in any setting, including your own home or a care home.

NHS continuing healthcare is free, unlike social care help from the Directorate for Adults, Health and Wellbeing, for which a financial charge may be made depending on your income and savings.

Anyone assessed as having a certain level of care needs may receive NHS continuing care. It is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where that care is provided.

If your overall care needs show that your primary need is a health need, you should be eligible for NHS continuing healthcare. Our Primary Assessment Team will be able to advise you further on this area.

Not everyone with ongoing health needs is likely to be eligible but there are times when it could appear you may have a need for such care: • when you are ready to be discharged from hospital and your long term needs are clear • once a period of intermediate care or rehabilitation or other NHS-funded services, offered at the end of a period of acute hospital treatment, has finished and it is agreed no further improvement in your condition can be expected • your eligibility must be considered before a decision to find a permanent place in a nursing home is made • whenever your health and social care needs are being reviewed as part of a community care assessment • if your physical or mental health deteriorates significantly and your current level of care - at

Paying for care continued home or in a care home - seems inadequate when, as a resident of a nursing home, your nursing care needs are being reviewed. This should be undertaken at least annually if you have a rapidly deteriorating condition with an increasing level of dependency and may be approaching the end of your life.

In these circumstances you should raise the issue of NHS continuing care eligibility and possible application of the checklist tool with discharge staff, staff co-ordinating your intermediate care, your GP or social services.

NHS Nursing Care Contribution NHS-funded nursing care is funding paid directly to a nursing home for care provided by registered nurses employed by the home. Services provided regularly by a registered nurse are likely to involve: • provision of nursing care supervision or monitoring of care provided by a non-registered nurse • planning and reviewing a care plan • monitoring and reviewing medication needs • identifying and addressing potential health problems.

Eligibility for NHS-funded nursing care in a nursing home should not be considered until it has been agreed that you are not eligible for NHS continuing healthcare and that a place in a nursing home is the best option for meeting your needs. It is particularly pertinent to consider if a nursing home offers the best option, when you are about to be discharged from hospital following an unplanned admission, and up to this point were living in your own home.

Self-funders You may be called a self-funder if you are meeting the costs of your support without any help from the Council. This can include support to live independently at home or in a care home. Some people choose to be self-funders because they do not want to be financially assessed, or they choose to make their own arrangements for support.

Manchester City Council can only make a contribution to the cost of your support if: • we have assessed you as being eligible for support from us, under the Fair Access to Care criteria

• you have had a full financial assessment from us and we have told you that you do not have to pay for the full cost of your care.

Even if you don’t think the Council will be able to make a financial contribution to the costs of your support, and you think you will be funding all of your support from your own resources, it makes sense to have a free assessment. You may find you are entitled to help with the costs after all.

If we can’t help with the costs, as a self-funder you are still entitled to the same free detailed help and advice in working out your needs and finding the right service as everyone else.

If you can’t get financial help from us, there are many other ways we can help you.

• We can provide you with a free assessment of your support needs, which will help you to decide what kind of support will best meet your needs. It doesn’t have to be in a care home; there are many other ways of meeting your needs that can help you to continue to live independently at home.

You can arrange an assessment through the Contact Service.

• We can give you free help and advice about organising your support.

• If you ask us to assess your need for support, we will review your needs on an annual basis or at your request if your needs change.

• Having an assessment of need and an annual review means we will have your details on record. This means we can provide support when your savings drop below the savings threshold of £23,250. Many people are not aware of this and worry what they will do when their savings run out.

• You are entitled to free help from some of our preventative services, such as Reablement, to help you get back up to speed if you need help to stop things getting worse.

• We will provide you with advice and guidance about services and opportunities that are available for you. Paying for care continued Care homes checklist provider. We can give you the details of voluntary organisations that can provide independent advice.

• If you or someone you know is a victim of abuse, you can report it. You can do this in a number of ways, but the most effective way is either to speak to the Contact Service or to the police.

• We can arrange for you to have a welfare benefits check from the Financial Assessment team or an advice agency to make sure you are receiving your full benefit and tax credit entitlement. Even if you are a self-funder, you may be eligible to receive some benefits.

We also operate a deferred payment scheme. If you need residential support and you own your own home, you may be able to get an interest-free loan to help you with the payment of your care home fees. In return for a legal charge on your property, Manchester City Council will pay your care home

costs, which you will have to repay when the house is sold. To qualify you must have an assessment of need.

Leaving hospital When you go into hospital, it may be for a planned treatment or as an emergency.

If you know in advance when you will be going into hospital, it is a good idea to plan ahead, perhaps with the help of a relative, friend or carer.

You may need to think about things such as the care of a pet or pension collection, as well as getting all the things together that you will take with you into hospital.

If you or someone you care for gets social care support at home or you are in a residential home, you need to let the care provider know as soon as possible when you are going into hospital and how long you think your stay will be, because: • we may be able to help make alternative arrangements for people who may rely on you for their care • it can prevent staff from worrying about you if they cannot get an answer when they call at your home • we can make arrangements to keep your care home place available for a period of time. Your benefits and any contributions you make towards care may be affected.

If your hospital stay is an emergency, let one of

the nurses know as soon as you can if any of the situations mentioned above affect you. You may also need help to arrange for the care of pets, to collect your pension or secure your home.

All Manchester hospitals have staff on site from Manchester City Council’s Directorate for Adults, Health and Wellbeing, who work closely with NHS staff and will help you sort out emergency, shortterm or longer-term social care needs.

The Directorate for Adults, Health and Wellbeing can support customers, their families or carers to facilitate safe discharge from hospital. Many people leaving hospital require little help, and it may be that you only need information - but our staff will gladly direct you to the most appropriate source.

Other people leaving hospital may need services such as reablement, meals service, a community alarm and other specialist services when they get home. If needed, we can arrange help from our community teams for people who have mental health needs or learning disabilities.

While many people leaving hospital get help from their family and friends, if you can’t make these arrangements yourself then we will look at ways of arranging services for you.
