PREPARING FOR 2021 The Board of Governors met in September to review the Strategic Goals of the medical society over the past year and to develop goals for 2021. One of our major goals is to continue to help better connect the medical community.

BLUE BLOOD

By Pam Irwin, CMS Executive Director According to Webster's dictionary, a blue blood is an aristocrat. “Blue bloods come from privilege, noble families that are wealthy and powerful.” During my last visit to Cumberland Island in search of wild horses and magnificent horseshoe crabs on the beach, I read about an entirely different meaning of “blue blood” and its significance, especially during the COVID-19 pandemic.

Not only was I rewarded with sightings of twelve horses, but also I found several horseshoe crabs along the beach.

Some had washed ashore and missed their return to the ocean. Others had molted as part of the growth cycle. A crab can be up to 25% larger after molting, and Cumberland Island's arthropods tend to be very large.

But, I digress.

Each year, more than 400,000 horseshoe crabs are bled for a substance found in their blood.

Horseshoe crabs are captured, cleaned of their barnacles, and stuck with stainless steel needles to draw their blood. Horseshoe crab blood runs blue and opaque, like antifreeze mixed with milk. Horseshoe crabs' blood, quite literally, saves millions of human lives. Horseshoe crabs use hemocyanin to carry oxygen through their blood. Because of the copper present in hemocyanin, their blood is a beautiful shade of cornflower blue.

More importantly, this crab blood is a resource to the medical field because of its ability to detect bacterial contamination in minute quantities. In the simplest terms, the blood contains a special amebocyte that is separated and then used in FDA testing and other applications. It is used to test for contamination during the manufacture of anything that might go inside the human body: every shot, every IV drip, and every implanted medical device. Did I mention COVID-19 vaccines?

LAL, which stands for limulus amebocyte lysate, is the test used to determine bacterial contamination. Limulus refers to Limulus polyphemus, the species of horseshoe crab native to the Atlantic coast of North America.

Amebocyte refers to cells in the crab's blood. Lysate is the material freed from the cells once they have been 12

“lysed” or broken. This material is the component sensitive to bacterial toxins. The first person to figure this out was a pathologist named Frederik Bang in the 1950s.

Before his discovery, the standard way to test for bacterial toxins was to inject a sample into a rabbit and check the rabbit every 30 minutes for three hours for signs of fever, which would suggest bacterial contamination. Without the ability to check for bacterial toxins, doctors were finding their patients given sterile shots coming down with “injection fever,” or in the worst cases, the patients were exposed to toxins that caused septic shock and even death.

For decades, horseshoe crabs have played a role in vaccine development. Now, researchers are looking at how they can help with the COVID vaccine. Lab testing of the new vaccines is underway using the blood of the horseshoe crab. “Without LAL, you aren't going to be able to produce the billions of inoculations that are going to have to come about to handle this pandemic,” says John Tanacredi, the director of Environmental Research and Coastal Oceans Monitoring lab at Molloy College.

Horseshoe crab blood harvesting will not occur without consequences or controversy.

Although horseshoe crabs are not intentionally killed during the “bleeding,” up to 15% of crabs die during the process. Many don't survive after the draining 30% of their blood and removing them for three days from the ocean. Many of the survivors do not spawn again.

Environmentalists are concerned about risks to the horseshoe population and the threat to migratory birds that depend on the crabs' eggs for food. Dr. Larry Niles, a wildlife biologist, says, “A synthetic alternative to horseshoe crab blood called Recombinant Factor C, or RFC, is already being used by pharmaceutical companies in Europe. For some reason, the U.S. pharmacopeia has placed so much restriction on RFC that it's going to take years to get it approved.”

So, if you see a horseshoe crab marooned on its back the next time you walk the beach, flip it over and leave it alone. Consider just how vital this arthropod is to patient safety. Perhaps, spare a moment to contemplate the magnificence of the blue-blooded Horseshoe Crab.

AMA UPDATES

AMA’S SURVEY RESULTS – IMPACT OF THE COVID-19 PANDEMIC ON PHYSICIAN PRACTICES In July and August, the AMA conducted a nationwide on-line survey of 3,500 physicians to better quantify the impact of the COVID-19 pandemic on physician practices. The results below present summary findings of that effort on issues such as income, use of telemedicine, availability and cost of personal protective equipment (PPE), and usefulness of the various federal financial relief programs available to physicians.

COVID-19 PHYSICIAN PRACTICE FINANCIAL IMPACT SURVEY RESULTS

The economic impact of COVID-19 on health care continues to reveal itself through reductions in patient volume and revenue and in higher practice costs. As the pandemic stretches on, physician practice viability remains under threat. The following graphics, which represent the results of an AMA nationwide physician survey, illustrate the precarious trends and realities physicians now face.

The vast majority of practice owners said federal financial assistance programs were very or extremely helpful.

It is clear the COVID-19 pandemic continues to have a negative impact on physician practices, both in terms of decreased revenue and increased costs. While initial federal financial relief programs were helpful to practices that applied, it appears the road to recovery, particularly for smaller physician-owned practices, remains difficult and more economic relief is needed.

SURVEY METHODOLOGY

WHAT DID WE ASK

• Forty question web-based survey (administered from mid-July through end of August 2020).

• Sample of 3,500 physicians selected from the ISO-certified M3 Global Research panel.

• Sample drawn to match physician population by age, gender, specialty, and census division.

• Surveyed physicians met following criteria: provided at least 20 hours of patient care per week prior to the pandemic, were working in one of the 50 states or the District of Columbia, were not employed by the federal government, and had completed residency.

• In some cases, percentages shown do not sum to 100% due to rounding.

• The findings presented here focus on the impact of the COVID-19 pandemic on patient visits, and on practice revenue and spending on PPE as well as the helpfulness of federal financial assistance programs to combat those impacts.

• Physicians were asked about total patient visits and telehealth patient visits. The number of total patient visits and telehealth patient visits were captured for a typical week in February (pre-pandemic) and for the week prior to the survey. Physicians were also asked about the number of weekly telehealth visits in the month (February-June) in which telehealth use was at its height. In-person visits are the difference between total and telehealth visits.

• We asked physicians for the percentage change in practice revenue and in spending on PPE between February (pre-pandemic) and the time of the survey.

BOND COMMUNITY HEALTH CENTER ENROLLS FIRST FLORIDA PATIENT IN NATIONAL STUDY TO EVALUATE BLOOD THINNERS AS COVID-19 TREATMENT Bond Community Health Center (CHC) in Tallahassee is the first site in Florida to enroll patients in a national study to see if blood thinners can help prevent life-threatening blood clots from forming in people who have tested positive for COVID-19.

“We are honored to be one of the first clinics in Florida to begin this study,” Dr. Robinson, CEO of Bond CHC says, “COVID-19 continues to impact our community, and Bond was proud to have been a leader in testing for the virus, and even prouder to be a leader in finding treatment options for those impacted.”

According to the National Heart, Lung and Blood Institute (NHLBI), which is overseeing the study, researchers have found that many patients who died from COVID-19 had formed blood clots throughout their bodies. Blood clots that form in the blood vessels can travel through the bloodstream to other parts of the body and trigger heart attacks, strokes, pulmonary embolisms (blood clots in the lungs) and other life-threatening problems.

Blood thinners help prevent blood clots from forming. These drugs have been used for many years to help prevent heart attacks and strokes in older adults who have been diagnosed with heart and blood vessel diseases.

Doctors don’t know yet if blood thinners might be effective in treating patients with COVID-19, according to Lisa Merck, M.D., M.P.H., associate professor of emergency medicine and vice chair of research in emergency medicine in the UF College of Medicine. Merck is the site physician for the ACTIV-4 Outpatient trial in Florida. “We also don’t know when patients should start using blood thinners after they test positive for COVID-19,” she said.

The University of Florida and the OneFlorida Clinical Research Consortium are coordinating recruiting efforts in Florida with several of OneFlorida’s consortium partners, including Bond CHC. The NHLBI’s ACTIV-4 Outpatient study, which is funded by the White House’s Operation Warp Speed initiative, is currently recruiting 7,000 participants at more than 100 sites worldwide to test the safety and effectiveness of blood thinners as a treatment for COVID-19.

MEMORIES OF CHRISTMAS Charles D. Williams, M.D., FACR, FAAP (“Pedro”) Christmas is just around the corner and memories will be made. The joys of this Christmas will be tomorrow’s memories and precious memories will linger with us.

Most of us have a rich treasury of Christmas memories from our own childhood and so often we continue customs from our own heritage.

I have wonderful memories of Christmas in Moultrie and very few have anything to do with money or material things. I remember the feelings of this season and things that money can’t buy.

Christmas for Pedro began on the day school let out.

That was the same day of the Christmas play where Pedro got to play the donkey except for the one time they used a real donkey. That was also the same day the Off-Key Children’s Gospel Group sang “The First Noel”. The program was heavy on scripture, prayers and Christmas carols. That was before the Supreme Court ruling.

The main attraction in Moultrie was the strings of Christmas lights on the court house square leading up to the sky to the top of the court house. A star towered above the lights reminding us of another star over 2000 years ago.

There were memories of going with Daddy into the woods to select and cut a perfect tree. The perfect tree always seemed crooked. We’d drag it home and decorate it with paper chains and other homemade ornaments. Aunt Pearlie always said that it was the prettiest tree she had ever seen.

The magic of Christmas always carries me back to the unheated bedroom of my childhood. I remember pressing my face against the cold glass of the bedroom window while covered with two or three quilts hoping to catch a glimpse of Old St. Nicholas’ sleigh and reindeer dashing among the stars. Sometimes I still glance up to the heavens on Christmas Eve looking for St. Nick. I’ve never seen him in flight, but I’ve seen the magic he leaves behind.

Mama said that Christmas was not about money. She said that Christmas was about love and caring. That’s what Mama said. It seems to me that we do a lot of unusual things at Christmas to reflect that love. It also seems that we don’t understand how to express it so we go to Walmarts and Stein Marts trying to find the perfect gift which turns out to be somewhat less than perfect.

Some of the most precious memories are those spent with family. My family and I have been so blessed at Christmas but I feel saddened that so many children will not have the opportunity to experience the kind of Christmases that many of us have seen and felt. There are many children who have never known the joy of wishing for something special and then finding it under the tree. There are also children who have never known the joy of giving to someone else.

One of my most favorite Christmas memories came about when I was about eight. There was a family who lived a short ways down our dirt road. The father had been ill and out of work. We wrapped and loaded up some of our old toys for distribution later. Mama had baked some cakes and cookies. She also dressed and fried some chicken and made an ice-box fruitcake.

Daddy had picked up some pecans from the yard and we headed over to their place in our old Ford coupe.

I’ll never forget the excitement of those kids. It’s been over fifty years now and I don’t even remember their names. However, I’ll never forget their faces and the tears in their Mama’s eyes. I wish all people could know the special feeling and memories that Pedro felt during that Christmas at that time.

Still there are families even in Tallahassee who are doing without this year at Christmas. Still there is plenty of time.

I am a second-year medical student at the Florida State University College of Medicine.

Medical student Jacqueline Lowell and I were co-chairs in planning the Florida State University College of Medicine’s annual “Healthcare for All Week.” As a Naples, Florida resident, I was not familiar with access to healthcare issues in Tallahassee, but was thrilled with the opportunity to better understand the needs in this community. In the past, local medical students have fund-raised during “Healthcare for All Week” to help cover healthcare costs for uninsured patients.

This year, we wanted to donate the money we raised throughout the week to the Capital Medical Society Foundation’s We Care Network program. The program’s mission to “provide a safety net to those low-income, uninsured patients most in need in our community through donation of specialty medical care and dental care” aligned perfectly with the week’s focus. “Healthcare for All Week” ran from Sunday, October 18thFriday, October 23rd.

Fundraising for the We Care Network was part of our goal. The week was kicked off with a partnership with local business Painting with a

Twist. They offered to host a virtual “Painting with a Purpose” event on October 18th to raise funds for the We Care Network. Thirtyone attendees enjoyed painting while enabling Painting with a Twist to present a check to the Capital Medical Society Foundation Executive Director Pam Irwin for $449.50. Store Manager Janelle Murraine said they were happy to help with this effort and were thrilled to give back to help the community. An additional $454.00 was raised through Bonfire t-shirt sales.

Two panel discussions were also hosted in conjunction with the focus on access to healthcare. Two We Care Network patients were included in the patient panel. It was a very important and powerful discussion. It gave a voice to patients who experience issues accessing healthcare and how the We Care Network program has been a resource for those with no other options. They were both such incredible additions to the panel. We cannot thank them enough for their bravery in sharing their stories. We have heard nothing but positive feedback from audience members. Personally, hearing the stories of Ms. Richardson and Ms. Ward re-invigorated my passion for legislatively advocating for patients.

Our focus was the humanistic side of the healthcare issue. Ms. Ward’s recording and Ms. Richardson’s presence on the panel were both incredibly enlightening and inspiring. I was impressed by the work We Care Network does before the panel, but after hearing the firsthand accounts of patients, I am so incredibly honored that we were able to partner with the Capital Medical Society Foundation’s We Care Network for Healthcare for All Week.
