PROFESSIONAL NOTES Jonathan Appelbaum, M.D. will serve on the Big Bend Cares 2019-2020 Board of Directors as their Secretary.

Armand Cognetta, M.D. recently spoke at Mayo Clinic’s 4th Annual Cutaneous Oncology Symposium Conference on Hydrochlorthiazide (HCTZ) and skin cancer. Dr. Cognetta’s research and that of others suggest that HCTZ, a common antihypertensive medication which is known to be photosensitizing, has a link to several common types of skin cancer. Dr. Cognetta and coresearchers think HCTZ use may represent a correctable cause in some patients with multiple skin cancers.

Tallahassee Memorial HealthCare’s Radiology Department presented Alan Chen, M.D. with the 2019 Ambassador Award on November 6, 2019 in recognition of his display of diplomacy, enthusiasm, positive attitude, public relations, and enhancing the well-being of the Radiology Department. Following the award presentation, Dr. Chen reflected, “I am truly humbled to receive this award by my colleagues who I would refer to as teammates. Medicine is a team sport, and there is no way I could have accomplished all that I have alone. I would not be here today if I did not have the support of my team at TMH. This award is a reflection of just that.

Daniel R. Schaefer, MPH, (Class of 2022) has been named the FSU College of Medicine-Capital Medical Society Student Liaison. In his new role, he will facilitate M1 and M2 medical student engagement with the Capital Medical Society (CMS). He will share information about upcoming meetings and events and will encourage students to engage in organized medicine by joining the local medical society. As liaison, he will also make the CMS aware of any bills the medical students are proposing for the FMA Annual Meeting. Daniel chose to attend the FSU College of Medicine because of its commitment to minority and underserved populations. He is the current Recruitment Chair for the FSU COM’s AMA/FMA Chapter, which provides medical students opportunities to get involved on campus, in Tallahassee, and on state and national levels.

Capital Regional Medical Center expanded access to emergency care in southeast Leon County with the opening of the Southwood freestanding emergency department on November 5, 2019. In southeast Leon County, the new freestanding ER is located at 2674 Capital Circle SE & Orange Avenue, near Southwood. The 5-acre site includes a 10,800-square-foot, one-story facility.

The Southwood emergency department offers 24/7 ER care and is expected to serve more than 12,000 patients per year. The 13 million-dollar project features 12 emergency room beds and employs approximately 30 full-time healthcare professionals, including board-certified physicians. The freestanding ER will offer a full-range of capabilities - from pediatric to adult care. Features include a full-service lab, CT, trauma room, and x-ray.

Capital Regional Medical Center’s CEO, Alan Keesee has been named by the Virginia Commonwealth University as one of the university’s top 10 graduates of the previous decade. The VCU Office of Alumni Relations’ 10 Under 10 awards celebrate alumni who earned their first VCU degree within the past 10 years and have enjoyed remarkable professional success, made important contributions to the community and/or loyally supported the university.

IS THERE REALLY A SANTA CLAUS?

In 1949, school let out for Christmas and Pedro and Emmett rode the school bus home, talking and thinking about Santa Claus. Emmett said that he had tried to be good and wanted a BB gun. Pedro had gotten old enough and big enough and bright enough to know that if Santa Claus was going to bring him his favorite toys, he needed to pray real loud so Mama, Daddy and Grandmama could hear him. He also knew that it helped to turn down the pages in the Sears & Roebuck catalog in the outhouse. But deep down, Pedro wanted to believe in Santa Claus just like his younger friend, Emmett.

Emmett grinned like a baked opossum and was excited as a bug in a tater patch. Emmett hugged Pedro, and Pedro hugged back. At nine years old, at that moment, Pedro once again learned there really is a Santa Claus.

On the way home on his new bike without his BB gun, Pedro kept thinking, “Please Mama, don’t be mad,” and she wasn’t.

Christmas morning arrived and there it was, the shiny brand new bicycle just like Pedro wanted. Also there was some fruit and a brand new Little Red Ryder BB gun. Pedro couldn’t wait to show Emmett and headed to Emmett’s house. Upon approval, Emmett looked over at Pedro, barely able to speak, and said, “Pedro, Santa Claus didn’t come. Either I’ve been bad, or he ran out of toys.” Pedro could see the hurt in Emmett’s eyes and hear the disappointment in Emmett’s voice. Pedro, without thinking, replied, “Emmett, Santa did come. He thought you were spending the night with me, and he left your BB gun at my house. I was a-bringin’ it to you.”

EXHAUSTED

By Michelle Miller, M.D. I never expected to be so bonetired after finishing residency four years ago. Leaving behind night-float, twenty-four-hour calls, and eighty-hour work weeks, I expected to be riding high at my new job as an outpatient primary care physician where I would work Monday through Friday (with a free half-day each week!). I even took off a full month after graduation to travel and see friends and family members that I had not been able to spend time with during my training. During this “month of fun,” I hiked trails at the Rocky Mountains and saw the Grand Canyon for the first time. This Florida girl even finally learned how to surf, though not well. However, within just a few short weeks, I was exhausted to the point of calling in sick several days in a single week.

This transition happened so gradually that it wasn’t overly alarming to me or my husband. For the first few weeks, I just went home at lunch to nap. Then it got to the point where I was setting my alarm for later in the morning and leaving the house for work five minutes after waking. I would come home to nap during mid-day and then again prior to dinner. I was sleeping all the time, but I never felt refreshed. I wasn’t snoring, so I didn’t suspect sleep apnea. I chalked up this tiredness to post-residency exhaustion, burnout, or even undiagnosed depression. Since physician wellness is a hot topic now, I had thought that maybe I just had not taken the time to fully relax or recover from finishing primary care residency, the hardest thing I’d accomplished.

Because I had seen a few patients in clinic with vestibular infections, I thought that I had a case when I woke up one morning with vertigo. I couldn’t stand for longer than a few minutes without having to quickly lie down, and I had to text my supervisor that I would not be coming in to work. Subsequently, my tongue went partially numb. Initially, I was nonchalant about this symptom, but the numbness spread across the whole right side of my tongue, and I had to face the denial I was in. After I saw a neurologist, I was quickly diagnosed with Multiple Sclerosis. When I read the MRI report and saw that there were “numerous 12 CAP SCAN - A CAPITAL MEDICAL SOCIETY PUBLICATION

lesions” in my brain, I felt like I had been sucker punched. How could this be, since I had been a healthy individual prior to this?

Memories of my only patient with multiple sclerosis during residency started haunting me: would I end up like her in a mobility scooter with a suprapubic catheter and unable to move except for one hand? I grieved for my previous health, but I hoped that I would not succumb to this disease process. I prayed that my immune system would calm down and stop attacking my brain without causing optic neuritis or transverse myelitis.

Since this diagnosis and subsequent treatment, I have come to learn to be more patient with myself and with my limitations. I enjoy more of my surroundings, taking in the sun rising from the parking lot when I get to work early (since I am able to focus better on paperwork prior to seeing patients early in the morning), or stopping to watch bees collecting pollen from flowers when I walk my son around the neighborhood (as I need the break, especially in the heat). Now when one of my patients complains of fatigue or paresthesias, I’m more inclined to obtain an MRI of the brain. It’s amazing to me now that MY physician brain would not even consider multiple sclerosis with the constellation of symptoms I presented with and, instead, a viral illness, or depression was higher on my differential.

I have been lucky that this first episode has been my only symptomatic episode. I have since had one active lesion on a MRI for drug surveillance postpartum, but had no symptoms with this. Of note, it was a frontal lobe lesion, so if I say something off the wall, I now blame my multiple sclerosis. MS has allowed me to connect with patients with autoimmune diseases or chronic fatigue in a personal manner.

Luckily, my paresthesias resolved with the standard high dose steroids, but the exhaustion has not. It waxes and wanes, but most days I feel just as tired as when I was a resident coming off of night-float or twenty-four-hour call. So, if you ask me how I am doing, I will likely respond “tired,” but really, I’m exhausted.

CMS PREPARES FOR 2020

PREPARING FOR 2020 The Board of Governors met in September to review the Strategic Goals of the medical society over the past year and to develop goals for 2020. One of our major goals is to continue to help better connect the medical community. If you are interested in serving on one of the following committees or service opportunities, please get in touch with Pam Irwin at pirwin@capmed.org.

EDUCATE - CME Committee - to supervise the selection and review of all CMEs offered, as well as selection of other educational programs that would benefit the membership.

ADVOCATE - Legislative Resource Committee educate and inform CMS physicians, policy makers and the public on issues the public’s health, patient care, and the practice of medicine.

ENGAGE - Membership Committee - to investigate applicants for membership as indicated by Chapter 1 of the CMS Bylaws and make appropriate recommendations to the Society.

SERVE - We Care Network - participate as a donating specialty medical provider in the We Care Network.

- Social Events Committee - to plan events that will help bind the medical community and their families to strengthen relationships and communication for the ultimate benefit of the patient.

LEAD - Physician Leadership Committee promote physician leadership and engagement in a variety of settings. This includes physicians employed by physicians employed by hospitals, health systems, health plans, and government; physicians in private practice and small group setting and physicians; residents, and medical students in academic and medical education programs.

- Scholarship Committee - award scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice medicine upon completion of their residencies.

The Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.” ➢ Access to healthcare is increased through gifts to the We Care Network and the We Care Network Endowment Fund. More than $105.4 million in donated specialty medical and dental care has been provided to low-income, uninsured patients since the program was created in 1992. In fiscal year 2018-19, which ended June 30, 2019, more than $7.4 million in health care was donated. Gifts support the operational expenses of the program including salaries and benefits of the case management team who coordinate the donated care. ➢ To provide incentives to keep medical students trained at the FSU College of Medicine in the Big Bend after completion of their residencies and help meet the growing demand for physicians in Florida, gifts to the Scholarship Fund provide scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice after completion of their residencies. $50,000 was awarded in 2019. ➢ Gifts to the Capital Medical Society Foundation help provide operational resources for the Capital Medical Society Foundation. In addition, modest grants are awarded to area non-profits whose programs improve access to healthcare for the underserved. In 2019, the CMSF awarded a total of $10,000 in grants to Elder Care Services for their In-Home Services Program, the Big Bend Hospice for bereavement services, and the Lighthouse of the Big Bend to provide clients who have vision loss with the aids needed to manage their health and medical care independently.

Gifts to Honor and In Memoriam: You can make a gift to the CMS Foundation in memory of someone who has passed away or in honor of someone, such as a fellow physician, you hold in high esteem or has taken care of you or your family. The CMS Foundation will send an acknowledgement letter to those you memorialize and honor.
