FLORIDA DEPARTMENT OF HEALTH UPDATES

MQA has launched a new Online Services Portal.

It incorporates updated technology and additional services that enable you to manage your Florida medical license. The ability to complete online license renewals is now available. In order to access the Online Services Portal, a one-time registration is required. Re-registration is necessary for licensees who have previously used Online Services.

The use of CEBroker is required to record CME course credits. CEBroker reports CME completion to the MQA portal for renewal compliancy.

License holders are encouraged to personally complete the one-time registration in the Online Services Portal. Verification of your Practitioner Profile and the completion of two surveys can only be completed by the license holder.

License holders should be familiar with accessing and using the Online Services Portal in order to manage their license.

It is up to the license holder’s discretion to share their log-in information after the initial registration process is complete.

Visit www.FLHealthSource.gov/mqa-services to begin the registration process. Follow the steps provided or reference the online tutorial and/or webinars for additional instructions.

WAKING TO A NIGHTMARE

“You’re going to need to come down here for this one,” said the resident, my first warning that this four a.m. interruption of my sleep would be the final one of a long night. Most calls had been to check out his impressions and treatment of an endless series of triage patients. Several that night had thought they were in labor, but weren’t – those I had to check with him to be sure they could safely be sent home. Others had been worried about some symptom, or about decreased baby movement, or spotting (after vigorous intercourse at 31 weeks), and a simple phone call to check out and guide the treatment was enough. This call is different, and the very tension in his tone of voice is enough to wake me up fully. Pain and bleeding, heavy, at 27 weeks, and the nurses unable to get heart tones. We hang up and I am on my way downstairs.

This patient had refused to see the physician who had done all of her prenatal care, and this is an early red flag that lodges in my thoughts with a “why?” attached.

A quick look at the very brief intake data reveals she is just past the middle of her tenth pregnancy, and has six living

children. The fetal monitoring display screen is ominously blank as I pass the nurse’s desk.

Other than that, we know little about her as yet. Her cubicle is obvious, the one with the three sets of feet scurrying about below the level of the curtain.

Inside, there is a youngish man slumped in the chair and a skinny woman with a very large belly slowly writhing in the bed. She is clearly in pain, but not severely so. She is quiet, shifting constantly to try to find some comfort, and she doesn’t look as worried as I would have expected. She looks more blank than anything, dull, hidden even. Lifting the sheet as I introduce myself, I confirm that the bleeding has slowed to a trickle. In medical school we are taught to introduce ourselves, explain our presence and what we are going to do, and ask permission. With the brand new resident just behind me it occurs to me that I might not be setting the greatest example, but I need to do an ultrasound of her belly which is already well coated with gel from the attempt of the nurse to find heart tones, I begin immediately. Part of my brain is thinking how crowded everything seems and how big she is for 27 weeks, another part is watching her reaction to the images (still blank), and my mouth is trying to narrate to the

resident, when the nurse asks the patient almost casually (and clearly for my benefit) “you did say you were carrying twins, right?”

Click. That is why the ultrasound shows so many arms and legs and so much cord. I seem to find the head wherever I look. That is why she is so big, and that placenta looks huge. On the downslope of that thought I am searching for the chest cavity, following the spine down from the first head I found to the ribs, holding my breath.

There is the heart. Not moving, and my face assumes the fixed feeling that it gets at the times I have bad news but am not ready to share it. Back up to the head, over to the other head, and down again. Another heart, also not moving. Glancing up at the nurse and the resident I see the understanding on their faces. Two thoughts come to mind, seemingly in conflict.

First is a misplaced relief that there is no need to rush now, as long as she remains stable, followed closely by the strange quiet sadness that comes with the death of the unborn child in such circumstances. I already know what has happened. She has abrupted, meaning the placenta has come off the wall of her uterus, depriving the CONTINUED ON NEXT PAGE

babies of their blood supply, and putting her in danger of bleeding to death as well.

Putting that together with her unwillingness to see her own doctor (we later found he had dismissed her from his practice), her unusual thinness, and the haunted look of her, one word comes to mind. Cocaine. But I don’t say it, only murmur to her that I also have been unable to find any sign of a heartbeat in either baby. Trying to lessen or delay the blow I inform her that we will need to call in Dr. B. who can do a C-section if needed, and that his ultrasound will confirm our findings. The resident and I leave the room to discuss things. He hadn’t gotten much of a history yet, only of hypertension (again I think, cocaine). The nurse points out that she has already had two previous C-sections, one emergent for a miscarriage.

She’s done this before, I think, she must already know, she must. Returning to the room alone, I ask if she is taking any medicine, allergic to anything, smokes tobacco, drinks alcohol, or uses cocaine.

Somehow, running all those questions together feels better, like I am not accusing her, but that drug use is similar to an allergy, a condition you have.

Not looking at me, she says “alcohol, yes”. “And cocaine?”, I ask. “Yes…. cocaine”.

Her dead affect thickens to become a wall between us.

We are suddenly miles apart, and it almost feels like I need to shout to be sure she can 12

hear my next question about when she last drank or used cocaine. “Yesterday,” she says, “yesterday,” and turns away.

The man is still silent, slumped in his chair staring only at his kneecaps.

Then the resident and I get down to the business of writing orders, documenting our findings, and wait for Dr. B to do the C-section. I am very glad that she isn’t bleeding so fast, and he only has a fifteen-minute drive to the hospital. I ask the resident to see if she would like her tubes tied. She consents, and we add it to the operative permission. Dr. B. arrives, confirms the ultrasound, and again we look at the two babies.

They seem to be embracing one another on the screen, and I wonder if they somehow knew what was happening and reached out for comfort.

At surgery a large clot is found behind her uterus and two apparently perfect girls, not really embracing, in separate amniotic sacs. Once the fallopian tubes are removed and the abdomen closed, she is soon on the way to the recovery room. I have the dead feeling brought on by fatigue and events too large to be truly comprehended. A thought is running through my brain: should I now be more troubled, more something, anyway.

Angry? Should I be angry with this mother? IS she a mother?

The resident tells me that she told them at first that the pain

and bleeding started after her eight-year-old son put his knee into her stomach. As I am about to leave, I turn back suddenly.

Please, I ask the resident, again because I don’t want to do it myself, please be sure she knows this was not the fault of her eight-year-old, and that she knows it was the cocaine.

Yes, it was the cocaine. This woman has three dead babies from cocaine, and the killing of the spirit of an eight-year-old to protect her from knowing the truth, to make it his fault instead of hers, is a possibility that must be made impossible.

Would she sacrifice him for this purpose? Would she? Would she even know she was doing such a thing? Or is she so dead inside at this point that she is incapable of such knowledge? It is very early in the morning and neither my brain nor my heart can take any more, and I go home, and to bed. FLYING HIGH: MY EXPERIENCE AS A NAVY FLIGHT SURGEON

By George Slade, M.D. Ever since I was a kid, flying was the one thing I wanted to do for sure, even more than being a physician, a decision I came to after graduating from a Catholic boys’ high school run by the Marist priests. I grew up off Clifton Road in Atlanta within 2 miles of Emory University so it was natural that I’d go to undergraduate and medical school there. (Our family ate at the Emory cafeteria almost every Friday night, mainly because it was a source of baked, rather than fried, fish on those Catholic “no meat” days, even though Emory was a Methodist institution.) Pre-med studies seemed the thing to do since most other entering males were in it, including a skinny blonde kid from Valdosta, Georgia named Comer Cherry. I met him during freshman orientation; later he became one of the top two in our medical school class. Back then he spoke in mostly unintelligible South Georgia parables, an entirely new dialect for me, a transplant from New York’s Spanish Harlem, a “boricua”, from W. 136th Street and Broadway. So, after professing my love for math and chemistry (and of course 3 years of Spanish), I decided to take the MCAT and apply to med school with the rest of my fraternity brothers.

All things changed quickly in med school: by Christmas of the first year I was set to quit.

Why I continued is beyond me, but my fate was sealed when I missed all of embryology while

bleeding from every orifice with Stevens-Johnson syndrome and erythema multiforme in the new DeKalb General Hospital for five weeks. At that point I discovered how important physicians were, somewhat less than the orderly who cleaned me up each day, changed my bedclothes and gave me baths. My first lesson in communicating with patients stuck with me…don’t suppose you are as important as you think because “you are” the physician…the sick tend to depend on the one most often caring for the more mundane needs of the day, not the pontificating physician.

After that first summer break between the freshman and sophomore years, I realized that running a service station at night wasn’t going to sustain me later in the clinical years, so I got an OB/GYN externship at Crawford Long Hospital in downtown Atlanta, not too far from Grady Memorial. By the end of my senior year I had delivered quite a few babies, used forceps for difficult deliveries and assisted in surgeries, an experience far beyond some OB/ GYN residencies at the time; but I spent my last four weeks in a neurology elective solidifying my eventual choice of specialty.

I spent my last year at Grady when I didn’t match with a Navy internship and “got” my second choice, a mixed internal medicine/OB-GYN internship at Grady.

I had joined the Navy Reserve the year before because I saw that as my best chance for aviation medicine, the right choice since flight surgeons learned to fly and actually soloed. The first plane I ever was in was the first plane I “flew” … in the front seat of a tandem seat T-34 with a maniacal Marine Corps Captain in the rear intent on killing me! Until 2000 the Naval Flight Surgeon was expected to learn to fly on the theory that a pilot’s family physician could better serve his squadron’s needs by understanding the stresses experienced by the pilot, flying crew members and the ground crew dedicated to keeping everyone safe. Like so many things in caring for others as a physician, a military pilot’s individual personal stresses affect his skills, whether in peacetime or in combat. In fact, military pilots are much like physicians: dedicated to their profession with a compulsive attention to details and willingness to make decisions affecting others.

I met Ray Bellamy in July, 1966 at the Naval Aero-Medical Institute (NAMI) in Pensacola during our flight surgeon’s course with its intense concentration in ENT, optics, ophthalmology, high altitude physiology and psychiatry. The flight school portion was a hoot with meteorology, aerodynamics, flight systems and emergencies, much like any other aviation ground school. In the Navy’s version CONTINUED ON NEXT PAGE

we crashed into the water, turning upside down and sinking while trying to remember the sequence of releasing restraints to get the hell out of a cockpit before descending beyond 10 ft! Another cute trick was being towed behind a landing craft in Pensacola Bay while wearing a parachute harness to simulate landing in a windy sea and escaping restraints by not being flipped so that you’re dragged face down by your legs and drowned.

Finally, a lower force, rocket ejection seat tower “exercise” supposedly acclimated the future flight surgeon to that event… just in case. We received ten to twelve hours of flight instruction including solo flight. The first two hours were purposely stressful to immediately wash out any “softies” who had no business flying. Very few did wash out or were not allowed to solo, and this was probably the first time any doctor failed in a school.

We then moved up to the back seat of a T-28 trainer without an ejection seat. We learned to open the canopy latch and stand on the seat before jumping toward the trailing edge of the wing in case we had to abandon the plane. This was finally followed by experiencing jet flying and aircraft carrier landings. Our final common experience was a 3-day survival trip equipped with a knife, canteen, no food and a parachute to help with various chores (fishing, tent construction, etc.). I can attest to the fine rattlesnake roast prepared by our distinguished future orthopedic tennis champion (Bellamy). But we hadn’t trapped it: some lost farmer in a truck ran over the viper on a dirt road and took pity on us.

I also acted as the flight surgeon OOD (officer of the day) for air/ sea rescue helicopter missions, crashes, crash investigation team duties and miscellaneous other responsibilities. The variety of experiences stimulated me (and made me fly with the squadron every chance available).

Once in the fleet as a qualified Navy flight surgeon with newlyminted wings proudly worn, the maneuvers I practiced became reality. More than three quarters of our class of December 1966 were assigned USMC billets.

Mine was with the 2nd Marine Air Wing in Cherry Point, NC and Marine Attack Squadron 332 (VMA 332). My first orders from the Marine Air Group commander was to join my squadron in Key West and insist on flying at night with my future squadron commander, since the older Irish Major had an aversion to night flying, a wellknown cause of “fear-of-flying” in mid-career pilots. Less known to me at that time was that avoidance of nocturnal “sorties” also indicated a proclivity for the bottle in the evening hours. This was my initiation into attack jet night flying in close air support, formation flying and nocturnal “dog-fighting”, all at 400 to 500 knots with an alcoholic skipper!

After leaving the Navy to begin my neurology fellowship, I continued to fly and obtained a civilian pilot’s license, commercial ticket and instrument rating. I lived that boyhood dream of seeing a different painting every day or night while flying my plane for the next 3000+ hours over the next 30 years for business and pleasure…

WOW! Why is medical service in the PHS, USN, USAF, Army or Coast Guard discouraged and frequently by those who never served and know the least about uniformed service? Many of us are risk-averse to a fault, but the potential benefits of service medicine are many. Foremost is serving with people who have chosen a way of life that few of us even ponder. Yes, I know all the arguments, most of which are false, misdirected or politically biased. Our uniformed services attract the brightest and most patriotic amongst us. Why is that anathema? Young physicians should seriously consider “service”, particularly if going to med school poses a parental/ spousal financial burden or debt.

Consider the financial help and “vacation-time” guaranteed, wellpaid job offered by the various services. Who knows? You might find a niche unmatched by any other to practice medicine. CMS

OPTIMIZE THE EHR: BETTER WORKFLOW MEANS BETTER WORK-LIFE BALANCE

By Chad Anguilm, Director of Professional Services, Medical Advantage Group Today, physicians have access to more data than ever before, and complying with new quality programs and pay-for-performance initiatives requires doctors to aggregate that data in various ways. Physicians and their office staff now spend a significant amount of time sorting through data in order to satisfy all the checkboxes—leaving them overwhelmed and dissatisfied.

But there are tools that can streamline patient care and payment workflows in the electronic health record (EHR). By dedicating time and effort up front to optimize the EHR, physicians can get relief from administrative tasks, reduce duplicate entry of data, and save time by not having to search for records in multiple places. Effectively maintaining an EHR is an ongoing process— the industry offers near-constant improvements—but the reward is a better work-life balance.

Enhancing the EHR means focusing on these four top areas: 1. Building a direct data interface. When all patient data, from both within the practice and from outside sources, is routed through the EHR in a structured format, a physician has all the information required at the point-of-care when treating a patient. The interface can include data from lab tests, radiology, and health information exchanges (HIEs); devices like blood pressure cuffs and health trackers; and other outside vendors.

Organizing patient data to provide a central point of access in the EHR. Immediate and organized access to complete patient information can help reduce unnecessary office visits, imaging, and labs tests.

Establishing predefined care alerts to guide physicians through the treatment portion of the visit. The parameters of each alert should be set by the physician, based on the Healthcare Effectiveness Data and Information Set (HEDIS) or other quality metrics or quality-based programs the doctor participates in. By setting up alerts that are triggered by the receipt of structured data instead of being manually triggered, a practice can see more patients and provide customized treatment plans without running recalls or prepping charts prior to the patient’s arrival. Setting up alerts is a one-time effort—then, armed with the right tools, the physician can rely on educated decision-making at the point-ofcare, when time is of the essence.

At a minimum, the EHR should identify gaps in care and alert the physician in real-time as to what the patient needs.

Integrating population health management reporting tools that allow the physician to see gaps in care or future care needs based on gender, age, and medical history, such as foot exams, immunizations, and colonoscopies. These tools—offered in many EHRs through the EHR vendor or a clinically integrated network (CIN)— analyze data from the EHR, HIEs, payer claims systems, and lab and radiology facilities, so that a physician can make decisions based on the patient’s longitudinal medical record. This offers the ability to bill for services during the office visit, send campaign messages (such as for a flu shot or annual physical) to patients, and report data in a useful format to referring physicians. In addition, a clinical rules engine takes action based on what is documented in the chart. For example, when a patient is diagnosed with asthma for the CONTINUED ON NEXT PAGE

first time, it automatically recommends a medication and a follow-up appointment.

• What inbound interfaces can be set up to accept ADT data, structured data from other physicians, or vaccine registries?

Getting Help in Optimizing the EHR When optimizing their EHR, physicians would benefit from working with an experienced consulting team that understands the tools available as well as the flow of the practice. An outside firm with wide knowledge and perspective on the industry and various EHR systems will be most aware of the optimization opportunities available. The firm might also be able to use economies of scale to integrate the EHR systems of multiple practices within a physician organization, physician-hospital organization, CIN, or accountable care organization through a HUB or HIE—increasing the flow of data or decreasing cost. The EHR vendor will often be involved in the optimization process, but an outside consulting firm should lead the project.

Many EHR vendors offer a complete EHR solution but are ineffective at optimizing workflow during implementation. And once the EHR is implemented, an inexperienced office manager may be charged with managing the vendor relationship remotely to ensure the full functionality of the EHR. At this point, the office manager or physician may not be aware of the full capabilities of the EHR and may not be equipped with the questions to ask a vendor to determine if the vendor can help the practice get the most out of the EHR—or if it’s time to hire a consulting firm. Here are the questions a practice should ask the vendor: • What local laboratory/radiology/report interfaces are available in the area that may help streamline the flow of data in and out of my practice?

• What data can be submitted electronically through a commonly accepted file for incentive programs like PQRS, Meaningful Use, and other payer-directed programs in order to decrease duplicate entry?

• What devices such as trackers, kiosks, or EKGs connect directly to the EHR to decrease manual entry?

• What type of remote access capabilities— such as smartphone or tablet apps or web-based accessibility—are available in order to complete tasks without being tied to the office?

Maximizing Workflow to Minimize Work Time Ultimately, when an EHR system is optimized, physicians can focus their time on caring for patients and maintaining a good work-life balance.

Take the example of Michael Little, MD, a pediatrician in Clinton Township, Michigan. Dr. Little was staying at his practice until late at night to enter data due to an inefficient EHR system that caused him to take notes by hand and then enter them manually into the EHR. By working with an outside consultant to redesign workflows, optimize templates, and complete a HIPAA analysis, Dr. Little reduced the amount of time he spent entering data from 30 minutes per patient to 2 to 3 minutes. He now has more direct face-to-face interactions with his patients and no longer stays late to update his charts, providing him more time at home with his wife and small children.

A STUDENT’S PERSPECTIVE: COMMUNITY MEDICINE AND SOCIAL JUSTICE

By Acton Pifer, Florida State University College of Medicine, Class of 2019 In 2015, the Capital Medical Society (CMS) Foundation graciously donated $5,000 to help fund The FSUCares Chapman Community Health Program (CCHP) at the Florida State University College of Medicine (FSU COM). Founded in 2014 by then-first year FSU medical student, Susanna Zorn, CCHP is a student run health service at the FSU COM that introduces medical students to the various aspects of patient-centered, community medicine. The program seeks to work with and empower underserved communities in the form of medical and non-medical programs by emphasizing a humanistic approach to medicine that strives to implement sustainable solutions to numerous social determinants of health faced by such communities1. We at CCHP would like to extend our sincerest gratitude to the CMS Foundation for its generosity as we continue to fulfill our mission.

CCHP is now entering its third year working with Maryland Oaks Crossing (MOC). MOC is a subsidized housing community in Tallahassee, Florida, that consists of low income residents who are predominantly single mothers, disabled citizens, refugees, and those dealing with chronic homelessness. In its beginnings, CCHP sought to address barriers to care facing MOC through needs assessments, social gatherings, and health screenings; however, it was soon realized that there were fundamental needs that had to be met before the goals of narrowing the gap to healthcare access could be realized. To best address these diverse needs, a Resident Advisory Committee (RAC) was established. The RAC consists of residents from MOC who help provide recommendations for CCHP’s services and interactions with the community. This has allowed active participation within MOC and has begun to facilitate sustainable, community ownership.1 Through feedback and partnership with MOC, CCHP has expanded beyond the traditional medical model of health screenings to help address the social determinants FSU COM students from a Fall 2015 screening at MOC. of health of the Included are (front row) 2016 president, Stefano Leitner; community. FSU undergraduate organization Nole-Med, a local basketball tournament with participation and coaching by the FSU Women’s Basketball team, and the distribution of cookbooks made by medical students and faculty consisting of affordable and healthy meals. In striving to model a team-based approach to healthcare, interdisciplinary care has also been introduced to the community through work focusing on children’s education by the FSU College of Nursing, additional needs assessments and social services from the FSU College of Social Work, medical health screenings by the Tallahassee Memorial Family Medicine and Internal Medicine Resident Physicians, mental health screenings provided by FSU COM psychologists, as well as smoking cessation and education courses provided by the FSU COM Area Health Education Center (AHEC). Our future endeavors will also include expanding our services through a partnership with Grace Mission Episcopal Church to better serve the homeless and impoverished populations of Tallahassee.

As a student, the real-world experiences I have had through CCHP cannot be replicated in a classroom. Having been able to directly learn from and grow to develop relationships with so many Maryland Oaks Crossing community members and other healthcare professionals has shown me that leadership in medicine is more than what occurs in the clinical setting on a day to day basis. Investing in a community takes time, effort, frustration, learning from failures, and innovative thinking; but, ultimately leads to greater satisfaction and better outcomes for those in our society who need it most. Whether they be unfairly beneficent to some or unfairly maleficent to others, the structural policies intact within our governing and social systems influence our patients in numerous ways. Seeking to promote social equality through innovative healthcare on multiple levels is vital to our identities as both future and current clinicians. Taking time to invest in those communities which face the greatest social determinants to not only health, but also in areas which we may not associate with traditional healthcare, is vital. Social justice is health justice.

The Capital Medical Society Alliance presented a $500 check to Pam Wilson for the Capital Medical Society Foundation Holiday Auction at their October 26, 2016 meeting.

Your gifts to Capital Medical Society Foundation transform lives in the Big Bend.

• Access to healthcare is increased through gifts to We Care Network and the We Care Network Endowment Fund. Over $77 million in donated specialty medical and dental care has been provided to low income, uninsured patients since the program was created in 1992. In fiscal year 2015-16, which ended June 30, 2016, over $5 million in health care was donated. Gifts support the salaries and benefits of the case management team who coordinate donated care.

• To provide incentives to keep medical students trained at FSU College of Medicine in the Big Bend after completion of their residencies and help meet the growing demand for physicians in Florida, gifts to the Scholarship Fund provide scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice after completion of their residencies. $50,000 was distributed to seven medical students in 2015.

• Gifts to Capital Medical Society Foundation help provide operation resources for Capital Medical Society Foundation. In 2015-16, funds were used for a roof repair, purchase of a computer firewall upgrade, rental unit maintenance, exterior painting, and office renovations to accommodate additional staff. In addition, modest grants to area non-profits whose programs improve access to health care for the underserved.

In 2016, CMSF awarded $10,000 in grants to Elder Care for their In-Home Services Program and to Lighthouse of the Big Bend to provide tools to the visually impaired to improve their access to health care.

Gifts to Honor and In Memoriam: You can make a donation to the CMS Foundation in memory of someone who has passed away or in honor of someone, such as a fellow physician, you hold in high esteem or has taken care of you or your family. CMS Foundation will send an acknowledgement letter to those you memorialize and honor.

How to Give: Through the generosity of donors, funds have been created in order to establish financial stability to meet our mission. You can donate at any time to one of CMS Foundation’s current funds: 1) Capital Medical Society Foundation-area of greatest need 2) The Capital Medical Society Foundation Endowment 3) We Care Network 4) We Care Network Endowment 5) Scholarship Fund Checks should be made payable to the CMS Foundation and indicate on the memo line of your check to which designation you would like to donate.
