YOUR PRESIDENT’S PERSPECTIVE ON THE 2017 FMA ANNUAL MEETING

By Tracey Hellgren, M.D., CMS President The recent 2017 FMA Annual Meeting was at Universal Studios’ newest resort, the Loews Sapphire Falls Resort in Orlando from August 4-6, 2017. As your CMS President, I was fortunate to be given the opportunity to attend the meeting as one of CMS’s 11 FMA Delegates.

Unfortunately for me, due to bad weather and cancelled flights in NYC (where I was vacationing pre-FMA Meeting), I missed the Friday evening (August 4th) Dessert Reception that CMS hosted for its own, Dr. John Katopodis, the incoming FMA President. The Dessert Reception was held at the resort, and multiple CMS members and guests came to welcome and honor Dr. Katopodis and his family (including his new 3-month-old baby), while enjoying desserts and drinks in a beautiful pavilion where the breeze and fan misters kept all who attended cool and comfortable while mingling. From what everyone has told me, it was a wonderful evening under the stars, enjoyed by all, especially Dr. Katopodis and his family.

Bright and early Saturday and Sunday mornings, the CMS FMA delegates met each day from 6-8am over breakfast and coffee with the other Northwest Florida Caucus members (Includes Escambia County Medical Society and Emerald Coast Medical Association) to discuss the pending resolutions that would be voted upon at the Sunday FMA House of Delegates (HOD) session. For those that do not know/understand, resolutions stem from Florida physicians and other

representatives in healthcare who try to identify the most important needs and challenges to the current practice of medicine. These resolutions give direction to the FMA for their Legislative agenda for the upcoming Legislative Session. During our Caucus meetings, we had several special guests, which included Dr. John Katopodis (incoming and now current FMA President), Mark O’Bryant (TMH CEO and first non-physician FMA Board Member), Dr. Andrea Friall (OB/GYN Specialty Society), and Dr. Christie Alexander (FMA Board Member/CMS Board Member). These pre-meetings with the Caucuses and Specialty societies help to prepare discussions for the next level, which is the Reference Committees, who then prepare a report for the HOD for Sunday’s HOD meeting, where the ultimate and final debate, amending, and rewording of resolutions occurs on the final day of the FMA Meeting. Initially this can be very confusing and daunting to the less-experienced delegates, but fortunately CMS has many seasoned and well-experienced delegates to help all through this prelegislative planning process.

The 2017 CMS FMA Delegates are Dr. John Bailey, Dr. David Dixon, Dr. Michael Forsthoefel, Dr. Rohan Joseph, Dr. Bill Kepper, Dr. Alma Littles, Dr. John Mahoney, Dr. Alfredo Paredes, Dr. Hugh VanLandingham, Dr. Gary Winchester, and myself.

Specialty Society Representatives (CMS members who attend to represent their Specialty Society) who were in attendance to help us through this process included Dr. Andrea Friall (OB/GYN), Dr. Brence Sell (Anesthesiology), Dr. Avon Doll (Nephrology), Dr. Andy Borom (Orthopedics), Dr. Christie Alexander (FMA Board of Governors Representative), and Mark O’Bryant (TMH). Two other invaluable members of our team were Pam Irwin (CMS Executive Director) and Shannon Boyle (Communications and Events Coordinator), who helped to keep us directed to the correct meeting places and times throughout the entire weekend to keep us on task. A special “Thank You” to all of these people who spent their weekend to help all of us in CMS and the Medical Community.

The FMA Annual Meeting is an amazing and very smooth process to watch in action, and to participate in. CMS is very fortunate to have such an incredible, knowledgeable, and well-read and well-informed group of delegates who provide tremendous insight and support for the FMA causes during the Pre-meeting, Caucus meetings, and HOD Meeting on the final day. I urge and challenge anyone interested in becoming part of this annual FMA Delegation (and ultimately the Legislative process, which is aimed at improving the field

of medicine), call CMS and find out how to begin the process of becoming involved.

This year CMS again had a strong presence on the Reference Committees and the Rules and Credentials Committee. Dr. Rohan Joseph was on Reference Committee I (Health Education and Public Policy), Dr. Michael Forsthoefel was on Reference Committee II (Finance and Administration), and Dr. Alfredo Paredes was on Reference Committee IV (Medical Economics).

Dr. John Mahoney was on the Credentials and Rules Committee.

Highlights of the FMA Meeting included remarks on the current state of healthcare challenges and successes by Dr. David Barbe, current AMA President.

The Maintenance of Certification (MOC) was a hot topic of discussion at the Caucus meetings, Reference Committee Meetings, and of course, at the HOD Meeting. Also, the Vaccination process and ability to attend school was a much discussed topic at the HOD meeting.

Other highlights included our own CMS Board of Governors’ member and first-time FMA Delegate, Dr. Rohan Joseph, being recognized as a graduate of the Karl Altenburger, M.D. Physician Leadership Academy, and Dr. Christie Alexander received the Gerold L. Schiebler, M.D. Advocate for Medical Students Award (nominated by a current FSU-COM medical student).

Finally, Mrs. Mollie Hill (former CMS Executive Director) received the Distinguished Laymen’s Award.

Go CMS! The biggest highlight of the FMA Meeting was the induction and election of our own CMS member, Dr. John Katopodis, as the 141st FMA President for the 2017-18 year. As many of you know, Dr. Katopodis is a long time FMA and FMA-PAC member who has been and remains very active in gathering support and lobbying for legislative issues important to Florida physicians. As part of his Presidency, he is determined to bring and maintain diversity in the FMA Delegation and Leadership so that all areas/specialties of medicine will be addressed and recognized in equal proportions.

Please remember to congratulate Dr. Katopodis when you see him on this very distinguished honor as our current FMA President. His speech at his Installation telling the story of his father’s journey and ultimately his own, was incredibly moving and speaks of his passion for medicine, the FMA, his family, and life.

I feel very fortunate to know Dr. Katopodis and see his commitment to medicine and his family—and I am grateful to him for taking the time to speak at my CMS Presidential Installation this past January. Dr. Katopodis’s Presidency will help to continue CMS’s unique involvement with the Florida Legislature and the FMA (which has its home base here in Tallahassee), especially in the upcoming year. We all enjoyed his Installation Dinner this past weekend which was wellattended by CMS members and spouses (30 in all) and the activities/festivities included a delicious buffet of Greek Food, Greek Music, and Greek Line Dancing (and yes, I tried it and it was very fun even though I don’t think I did the footwork correctly).

In conclusion, I started the FMA Annual Meeting a bit dazed and confused by the multiple resolutions (with their legislative wording), and the uncertainty of how in one weekend a group of physicians could come together to put together resolutions to go forward to the 2018 Legislative Session. As the weekend progressed, I now understand how this process works to help medicine thanks to seasoned delegates and physicians, outstanding FMA leadership, and the obvious commitment and dedication of these people to the current state and the future of medicine. I drove back home to Tallahassee no longer dazed and more optimistic and hopeful for a brighter future for medicine. Congratulations again to Dr. Katopodis (and thank you to his family for sharing him with the FMA and the medical community) on your FMA Presidency!

Schiebler, M.D. Advocate for Medical Students Award honoring outstanding service and advocacy of a FMA physician on behalf of the FMA Medical Student Section. It was established in 2003 and is awarded each year at the FMA House of Delegates Opening Session during the FMA Annual Meeting. The award recognizes a physician that is a member in good standing of the Florida Medical Association who has demonstrated a commitment to enriching the experience for medical students within organized medicine and who has a passion for building bridges between levels of training and experience. It distinguishes those with a selfless vision of the future of this noble calling and a commitment to the Hippocratic maxim of instructing pupils in its precepts. She was nominated by Stefano Leitner, third year medical student at the Florida State University College of Medicine (FSU COM). 6 – Dr. Rohan Joseph was recognized for successful completion of the 2016-2017 Karl M. Altenburger, M.D. FMA Physician Leadership Academy. The Leadership Academy is an intensive, 10-month program aimed primarily at FMA physician members ages 45 and under, who are selected annually through an application process. Participants come from a variety of specialties and practice environments but have the common objective of becoming effective advocates for the medical profession and patients.

7 – Mollie Hill, former CMS Executive Director and Director of Community Clinical Relations at FSU College of Medicine, was the recipient of the Distinguished Laymen’s Award. In 1972, the Florida Medical Association House of Delegates authorized the creation of a special annual award to recognize distinguished service by a layperson. This award could be made to any individual not possessing an M.D. degree, who is a legal resident of Florida, and who has made significant and lasting contributions to the medical profession. MIRACLES HAPPEN

By Lisa Jernigan, M.D. County prenatal clinics are the highlight of my week: a chance to get out of Tallahassee and into our lovely surrounding rural areas to work with mostly pleasedto- be- there pregnant women.

Having a few doctors going out makes much more sense than expecting pregnant women, with no reliable transportation, to somehow make it to Tallahassee to obtain prenatal care. This effort is part of the struggle to reduce the unduly high perinatal mortality statistics in these counties. Our physicians visit five counties each week, and each has their own “personality”.

My counties are largely agricultural, and the patients are mostly polite and seem grateful for the ability to obtain convenient local care.

Although the setting seems bucolic, these women face all of the problems of their urban sisters, but they have considerably fewer local resources to call upon. Healthy Start and social work are vital services provided through these clinics.

Amy (not her real name), was a lot happier than I expected, seeing as she had been brought in to clinic today for follow up on her elevated Edinburgh score (a questionnaire having to do with depression in pregnancy). In fact, she was almost giddy. After the usual 23-weeksof-pregnancy questions, I gingerly brought this up. “Oh yeah, I’ve been depressed most of my life, at least since I was eight,” she said with a smile. This encouraged me to further pursue the history of this young woman who had a reputation in our clinic for angry dissatisfaction.

Out it came: her dad died when she

was eight; mom got a new fella very quickly. He rapidly began to sexually and physically abuse my patient.

Mom was no defense. At fourteen, Amy began seeing Joe, who was twenty- two at the time. This relationship was also abusive, and Joe was “cracked out” often. She had sexually transmitted diseases, but no injury serious enough to bring this minor girl to the attention of the authorities. All through middle and high school, she had a friend Mike, who was supportive and loyal, and unhappy about her treatment at the hands of others. Finally, nearing 20 years of age, she became pregnant (I was amazed that it didn’t happen sooner!), and this gave her a new perspective. Joe hit her again, and again, and she snapped – she called the police, got him charged, and instituted a restraining order. She had been coming to clinic, mad, downright mean, and not once divulging this information in the face of repeated questioning about her sense of well-being.

But today, she is radiant. Mike has proposed. They are engaged, Joe is abandoning his rights as a parent, and Mike wants to adopt the child as his. He doesn’t drink or use drugs, and he has a job. She is wearing his ring. They have not had sex, and are planning to wait. She is joyful at having survived all of the hell and come out to this place. Saying silent hallelujahs, I congratulate her, and I move on to the exam.

Her tummy is about the right size for gestational age, if you adjust a smidge for her moderate fluffiness.

Baby heart tones sound great, and the child is clearly active under my hands. Our midwife has joined me for this phase of the visit, due to the previous reputation of this woman.

In order to measure her uterus, I had to scoot her pants down a bit in

front, and noticed that her pubic area was entirely shaven, no mean feat given her pre-existing pannus and her advancing pregnancy. As usual, I advised that she stop doing this, to reduce the risk of wound infection should she require a C-section. Her immediate response was horror: “I can’t do that! You guys have male doctors that are going to see me down there, and they will be disgusted with me! They won’t want to have anything to do with me!”

Click. Suddenly her history jumped out at me. All the men who have had sex with this woman have preferred girls. She verified that Joe had wanted her shaved, and it is doubtful that her first abuser ever had to command such, as young as she was. We talked briefly about this, and in a moment of inspiration, I asked her what Mike would think of her au naturel, with hair. “Mike will love me however I am,” she triumphantly responded. She had never before considered that her assumptions about what men think had been entirely built upon the two men she most hated. Then our midwife jumped in: “You know, women used to have to fight for the right to have their babies without being shaved, because all the doctors wrongly thought this would prevent infection.” Amy laughed then, and seemed to see herself almost as part of a movement from long ago.

Still happy, and a bit more free, she sashayed out of the clinic.

DEAN’S CORNER FSU COLLEGE OF MEDICINE STUDENTS GIVE HIGH MARKS TO THEIR TEACHERS

By John Fogarty, M.D., Dean, FSU College of Medicine Here at the College of Medicine’s central campus, we’re back in full swing. First- and second-year students are entrenched in the fall semester, while our first class of 40 PA students has begun studies. We have a full house.

We just received the results of the AAMC Annual Graduation Questionnaire completed by the Class of 2017 in May. Our graduating students, once again, are pleased with their educational experience at Florida State. One question asked graduates to agree or disagree with this statement: “Overall, I am satisfied with the quality of my medical education.”

More than 97 percent of our graduates responded with “agree” or “strongly agree,” compared with an average of 89 percent for graduates of the 140-plus other AAMC-member medical schools. That puts us at the 90th percentile for all medical schools.

The College of Medicine will be undergoing a reaccreditation site visit in April 2019, so we’ll need to start our self-study this fall. As a member of the Liaison Committee on Medical Education (LCME) for the past six years, I know that student impressions and evaluations are one of the areas that reviewers take very seriously in

assessing the quality of the education and the teaching environment.

The GQ is a large document with lots of information to help us assess and improve our program. Students rate their clerkship experiences above the national average and give high marks for their perceived preparation for residency programs in the areas of clinical skills, communication skills, clinical decision-making, understanding social issues in patient care, and understanding ethics and professional values.

Why do I share this? It’s always nice to share good news about our program. But, as a community-based medical school, we depend on our more than 2,500 community faculty to deliver our patient-focused curriculum in their offices in a one-on-one professional educational experience during the third and fourth years. That’s in addition to community faculty who provide preceptor experiences for our students during the first and second years.

You, as our faculty, do that very well. And our students benefit, matching at great programs and returning to Florida to practice in large numbers. Thank you for all you do to contribute to the College of Medicine’s success. We couldn’t do it without you.

NORTH FLORIDA MEDICAL RESERVE CORPS When disaster strikes, volunteers play a key role in response and recovery. From assisting communities after severe weather disasters, providing medical care during a mass casualty event or operating points of dispensing of emergency medications, the North Florida Medical Reserve Corps (NFMRC) supports the medical community in many different capacities. NFMRC volunteers are vetted and trained to assist with public health crises.

The NFMRC also supports non-emergency public health initiatives, including administering influenza vaccinations, assisting with school health screenings and staffing first aid stations at community events.

done by paper form and transmitted within the hospital by fax and/or conveyed by phone with paper forms to follow. But that would only be a workaround. Patient care overall would be affected, with registration slowed, he noted.

Many clinicians and staff would be challenged to adapt to non-digital processes, as happened in the March 2016 cyberattack on the MedStar Health system, which has 10 hospitals and more than 250 outpatient clinics. When hackers seized control of their computer data, senior staff had to assist their younger counterparts with learning how to use paper messages and recordkeeping.

The ER could be hit hard by a cyberattack, but the physicians and staff there might be best prepared to respond, says Roneet Lev, MD, FACEP, chief of emergency medicine at Scripps Mercy Hospital in San Diego, California, and president of the Independent Emergency Physicians Consortium.

“Emergency departments have all experienced downtime with computer systems,” Dr. Lev said.

“At our facility, we call this ‘Code White.’ When we hear ‘Code White’ on the speaker system, we know to get out the white board and the markers, and that things will be slower. It’s annoying and no one likes it, but we’d manage by keeping track of patients the old-fashioned way.”

Even so, a “Code White” still leaves clinicians without a way to refer to any medical records that are stored electronically. Not knowing a patient’s allergies or medical conditions is not optimal, she said, suggesting that all patients should always carry a list of their medications, allergies, and pertinent medical history on paper or on their smartphone.

Workarounds can only accomplish so much, Dr. Lev noted. A cyberattack could affect all computer-related hospital activities such as labs, x-rays, patient tracking, operating room scheduling, access to previous medical records, and treatment recommendations.

“While the emergency department would function using ‘Code White’ procedures, this is not sustainable for long-term operation of a hospital,” she said.

What these experts all seem to agree on is that in the face of an attack, the best way to protect patients is to return to practices that worked before computers.

As Ralph Gambardella, MD, orthopedic surgeon and president of the Kerlan-Jobe Orthopaedic Clinic (affiliated with Cedars-Sinai) in Los Angeles, so aptly stated: “Rather than relying on computers, I still believe that talking to—and communicating directly with—my patients is the best way to impact patient safety.”
