District 850 Family Event

A huge thank you to Diana O’Bryant and Hancock Whitney for sponsoring the Capital Medical Society’s first in-person family event in over a year. Over 80 happy guests attended the District 850 event on July 17, and played arcade games, bowled, entered the Escape Room, and took the Ropes Course Challenge! PROFESSIONAL NOTES Dr. Steve Currieo was presented the Teacher of the Year Award by the Tallahassee Memorial Family Medicine Residency Program on June 24, 2021.

This is Dr. Currieo’s second time receiving this award. This award is in recognition of Dr. Currieo’s dynamic and dedicated voluntary teaching.

Dr. Rohan Joseph received the 2020-2021 Excellence Award from the FSU College of Medicine General Surgery Residency Program at Tallahassee Memorial HealthCare.

In July 2021, Dr. Charles Williams was honored at the state meeting of the Florida Radiological Society at the Ritz Carlton-Orlando. The Florida Radiological Society established the Charles D. Williams, M.D. Legacy Lecture to attract national speakers to society meetings. Dr. Williams also spoke at the event.

ELECTRONIC PRESCRIBING REQUIREMENTS •

House Bill 831 (2019), Electronic Prescribing, was signed into law by Governor DeSantis. The effective date is January 1, 2020. The bill provides important new requirements for prescribers to generate and transmit all prescriptions electronically upon licensure renewal or by July 1, 2021, whichever is earlier.

The law requires prescribers to generate and transmit all prescription electronically, unless:

The practitioner and the dispenser are the same entity; The prescription cannot be transmitted electronically under the most recently implemented version of the National Council for Prescription Drug Programs SCRIPT Standard; The practitioner has been issued a waiver by the department, not to exceed 1 year, due to demonstrated economic hardship, technology limitations that are not reasonably within the control of the practitioner, or another exceptional circumstance demonstrated by the practitioners;

he practitioner reasonably determines that it would T be impractical for the patient in question to obtain a medicinal drug prescribed by electronic prescription in a timely manner and such delay would adversely impact the patient’s medical condition; The practitioner is prescribing a drug under a research protocol; The prescription is for a drug for which the federal Food and Drug Administration requires the prescription to contain elements that may not be included in electronic prescribing; The prescription is issued to an individual receiving hospice care or who is a resident of a nursing home facility; or The practitioner determines that it is in the best interest of the patient, or the patient determines that it is in his or her own best interest to compare prescription drug prices among area pharmacies. The practitioner must document such determination in the patient’s medical record.

REFLECTIONS FROM THE EDITOR

BERMUDA SHORTS AND SURGERY

By Frank Skilling, M.D. My father, who had to work to pay his way through college, was insistent that his own children didn’t work while they were attending college.

However, the three of us were expected to work every summer and Christmas vacation. During my first summer home from Spring Hill College in 1965, I worked at what was then the main Sears and Roebuck store on Biscayne Boulevard in Miami. I was hired as a salesclerk at $1.50 an hour (the minimum wage then was $1.25 per hour) and assigned to the boys clothing department. My first boss there was a Tennessean named Don Little who was about forty and had twenty years with Sears. He was a doggedly hard worker and explained to me the ins and outs of being a salesman. Bermuda shorts, which were extremely popular in South Florida, had to be stacked the same way every time: folded into halves with the left front pocket on the top. This was mandated so that the size and price label, which was attached to the belt line, could be seen immediately.

So, in my mind, for over 55 years, all shorts should be folded and stacked this way.

Now that I’m retired, I’m in charge of storing my own shorts once they have been washed. My wife tends to fold shorts differently, essentially whichever way they come out of the dryer. I always re-fold them myself with the left front pocket facing up. Now, you may ask, what does clothing have to do with the practice of medicine? It’s because learning surgery, especially, is like folding clothes: it’s instilling manual habits that must become second nature to the surgeon. (Some detractors call this “muscle memory,” but I don’t think that’s a fair assessment of memory or muscles.)

I was fortunate to attend the University of Alabama in Birmingham School of Medicine when John Kirklin, M.D. was chairman of the department of surgery. Kirklin was a world-famous heart surgeon who had essentially “written the book” on surgical correction of congenital heart disease. During my senior year at UAB, 50 years ago, I took an elective on Kirklin’s cardio-vascular surgery service. The duties of a fourth-year medical student on his service were few, mostly assisting in the transport of patients and preparations in the operating room. However, I was allowed to “scrub in” on any case he was doing.

was started, I had an up-close view of everything that was going on. Kirklin was a noted teacher, and he had a set of dictums that had to be adhered to by every surgeon who trained under him. Among them: never rush; never pick up an instrument unless you’re going to use it; never make a motion unless there’s a purpose; always be kind to surgical tissue; if you get to a point and forget what you’re doing or run into an unusual situation, stop, step back from the table and consider what your next step should be. This was drilled over and over into the residents who worked with him, and he demanded it even of surgeons who had many years’ experience. I obviously never became a heart surgeon, nor did I see him again after being on his service. But for the rest of my surgical career, I could hear his voice sometimes when I was operating, especially when I hit an unexpected complication during an eye operation.

The novelist and short story writer Flannery O’Connor’s letters were published in a posthumous collection entitled, The Habit of Being. In them, she discusses the motivations and habits that she developed to become a creative writer. Her works are still read and taught today, many years after her death at age 39 in 1964. As with many great authors, it’s the habits of writing every day that are so important to producing the works that they desire. Learning surgery, and surgeons should always be learning, is similar.

All surgeons are egotists; they must be. They must be convinced that what they’re doing is so important that the human they’re working on will be better for having had a surgical encounter with them than if they didn’t have it. Yet, I have watched surgeons who I know could never satisfy the Kirklin Dictums, especially the one about always making purposeful movements.

Some surgeons handle the tissue too much, or they’re too cavalier with their motions, or they use the wrong instrument or use it at the wrong time. Surgery is a demanding and unyielding art that most of us can’t do, and few can become experts at.

So how does this apply to shorts in my drawer? It’s because for me there is only one way to fold Bermuda shorts: creased, in half, and with the left pocket facing up. Like surgery, folding clothes is a habit that has become so ingrained that I reproduce the movements time after time.

SIMPLER TIMES

GRANDMA, QUILTS, AND MEMORIES

By Charles D. Williams, M.D., FACR, FAAP (“Pedro”) Pedro had been up at Moultrie visiting Millard when he took it on himself to stop by and seta-spell with Willard’s daughter, Judy. He hadn’t seen her in a coon’s age. Upon arrival Judy said she had picked up a quilt from over at Uncle Dillard’s and Aunt Annie Mae’s that Grandma had made for Pedro before she died.

When Pedro saw that quilt that Grandma had made specially for him, it brought a lump in his throat and it put him to thinking. He remembered as a child that Mama would put a quilt on the floor for us kids for afternoon naps.

Mama would also put a quilt down for a pallet in the front room when kin folks and friends came to spend the night. Nobody was turned away as long as Mama had a quilt and a floor. That’s what quilts and floors were for.

When Pedro saw that quilt and all those tiny pieces of cloth that had been sewed into a pattern, he knew how hard and how long and how much love she had put into it. She had spent hundreds of hours of work putting together a thang of beauty. Grandma had saved all scraps from makin’ dresses and shirts and she had saved up all flour sacks and Guano sacks. Each piece could even have its own special memory. One piece of scrap could have been from a special Sundy-go-to-meetin’ dress or another patch could have been left over from a family reunion outfit.

Pedro knew that this quilt would be passed along to each generation and he suspected Grandma knew that too and that each generation would have remembrance.

When Pedro thought of Grandma, he thought of the love she showed and gave to her family, to her friends and to Pedro. He thought of her wit and her humor. It was these qualities that brought her family closer together and helped them to overcome tragedy and hard times. He

remembered one time on a windy day in March in Colquitt County, someone had asked Grandma if the wind blowed this way all the time. Grandma studied on it and replied, “No. Sometimes it blows the other way.”

Pedro continued thankin’ and remembered Grandma’s wit and wisdom and she said that we should not cut down the tree that gives us shade and that we should not be all vine and no taters.

Most likely what she meant was don’t be all talk and no action.

Through Grandma Pedro learned not to go back on his raisin’. He learned of love and loss and how fleeting were the moments of our lives. He learned that the most precious thangs were the most simplest thangs and it was the simplest thangs that formed permanent pictures in our memories. Robert Browning once said that, “If you get simple beauty and naught else, you get about the best thing God invents.” That’s what Grandma gave and that was what the quilt reminded me of and it was mine to keep.

We cannot turn back the pages of time and time won’t stand still, but Pedro has his quilt, his memories of Simpler Times, and his memories of Grandma and “memories are our treasures here on earth.” Through telling these stories, it is hoped that Pedro leaves a glimpse of Simpler Times for others to remember and to enjoy and maybe trigger a special feeling, a special moment, and a special time in one’s own Simpler Times.

The Impact of Private Support: The Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.” ➢ Access to healthcare is increased through gifts to the We Care Network and the We Care Network Endowment Fund. More than $120.9 million in donated specialty medical and dental care has been provided to low-income, uninsured patients since the program was created in 1992. In fiscal year 2020-21, which ended June 30, 2021, more than $8.6 million in health care was donated. Gifts support the operational expenses of the program including salaries and benefits of the case management team who coordinate the donated care. ➢ To provide incentives to keep medical students trained at the FSU College of Medicine in the Big Bend after completion of their residencies and help meet the growing demand for physicians in Florida, gifts to the Scholarship Fund provide scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice after completion of their residencies. $31,000 was awarded in 2020. ➢ Gifts to the Capital Medical Society Foundation help provide operational resources for the Capital Medical Society Foundation.
