NEW RESIDENT CMS BOARD MEMBERS

THE CAPITAL MEDICAL SOCIETY BOARD OF GOVERNORS WELCOMES THE NEW RESIDENT BOARD MEMBERS Todd Hewitt, M.D., Family Medicine Dr. Hewitt joins TMH after a successful career as a Chiropractor for 13 years in Pasadena, Calif. While in practice, he served on many professional, as well as volunteer health related organizations. Through his exposure and close work with other medical specialties (and with the support of an incredible family), he decided to fulfill a goal set as a child and left practice to enter medical school.

He considers being active one of the key tenants in achieving and maintaining a healthy lifestyle and has been extremely fortunate to be involved in and associated with a wide variety of athletic activities in his life. He was a college football player and attended East Texas State University for undergraduate education and he earned his Doctor of Chiropractic from Parker College of Chiropractic in Dallas, Texas. While there he was a member of the U.S. National Rugby League team and a student member of an organization that provided care to professional Rodeo cowboys. He received his Doctor of Medicine from the University of Medical Health Sciences in 2014. While a medical student, he was privileged to serve as one of the Team Physicians for the nation of Nevis’s Olympic Triathlon/Cycling team. He is extremely enthusiastic about returning to practice and thankful for the opportunity to continue his education in a dynamic community like Tallahassee. Through all of his experiences and training, Dr. Hewitt has maintained strong interest in Rural Medicine, Sports Medicine and Pain Management, but also appreciates the comprehensive role a Family Medicine physician is entrusted to play in the lives of their patients. Dr. Hewitt and his wife Michelle have been married for 18 years and welcomed a joyous and enthusiastic daughter, Campbell, to their family three years ago. As a family they celebrate any activity that allows them to be outside and on the water and are excited by the many unique opportunities in their new North Florida home. They share a passion for college football and thanks to Campbell, throw one heck of a “Fancy Tea Party.”

Meredith Maxwell, M.D., Family Medicine Originally from New Jersey, I moved to the southeast in 2003 to attend college at Emory University in Atlanta, Georgia. I received a bachelor’s degree summa cum laude with a dual degree in Psychology and Asian Studies. During my time at Emory I completed a neuroscience research project and publication on the effects of hormones on attention in songbirds. After graduation I worked for a year as a paralegal at a criminal defense law firm, followed by a post-baccalaureate pre-medical program at Agnes Scott College in Decatur, GA. I was subsequently part of the inaugural class of the Medical Partnership of Augusta University/ University of Georgia, in Athens. I am very happy to be starting my second year at the Family Medicine Residency at TMH. My plans post-residency are not yet set in stone, but I am currently considering a post-graduate fellowship in Palliative Care and Hospice. I spend my spare time with my husband and 2 year old daughter, Caroline. I am also a certified scuba diver who enjoys exploring wrecks (of the shallow variety) and marine wildlife in the Caribbean.

Vishal Dahya, M.D., Internal Medicine Dr. Dahya was born and raised in Okeechobee, Florida and graduated with honors from the University of South Florida with a bachelor’s degree in biomedical sciences and a minor in public health. As an undergraduate student he became involved with various volunteer organizations and premedical societies. He then moved on to medical school at the Florida State University and chose the school because of its commitment to aiding the medically underserved, its wonderful facilities and its focus on patient-centered health care. He completed his clinical rotations in South Florida and was also able to work on multiple clinical research projects before returning to Tallahassee to continue his medical training. In his free time, Vishal enjoys participating in all kinds of sports, outdoor activities, and spending time with family and friends.

Rachel Russell, M.D., Internal Medicine Dr. Russell was born and raised in the small town of Old Myakka near Sarasota, Florida. She graduated from New College of Florida with a bachelor’s degree in biology. Upon graduation from New College, Rachel then entered the University of South Florida’s medical master’s program and earned a Master’s of Medical Science with a concentration in women’s health. As an undergraduate she volunteered at Genesis Behavioral Health Clinic of Sarasota, as well as volunteered at Sarasota Children’s Clinic, and was an intern for the Sarasota County Health Department’s CHAT program. Rachel’s motivation to enter medicine is derived from her desire to serve the medically underserved, and thus she chose the Florida State University College of Medicine for its patient-centered mission. Finding a passion for serving the people of Tallahassee, Rachel returned to TMH for residency and looks forward to continuing to provide care to the Big Bend area as a hospitalist at Tallahassee Memorial Hospital.

OMAHA BEACH: A D-DAY REMEMBRANCE

My father in law, John A. Watson, was one of the most honest, direct and least pretentious people I’ve ever met.

He loved the woods around his Pennsylvania home, hunting, his family and his dogs. He never drank alcohol or used tobacco. He liked to kid with his old friends, eat breakfast at the Family Tradition Restaurant in Harrisville, and visit with the “shut ins” on his mail route. He was a rural mail carrier in his hometown after The War. He rarely spoke about his Army service in World War II, but when pressed he would let you know that he’d been in Europe.

He also survived landing with the US First Army on Omaha Beach on D-Day. After the film Saving Private Ryan was released I took my father in law and my two sons to see it. The depiction of the savagery and slaughter of young Americans on a French beach was searing, and when my younger son John asked his grandfather and namesake if that was what it had really been like, he softly responded, “It was much worse.” Unfortunately, by this time John Watson was

in the early stages of dementia and had developed expressive aphasia so he could not lucidly recount his experiences of that day or the subsequent incursion across France and Germany.

He was unable to tell my sons what it was like to huddle down in a Higgins boat under intense, unexpected machine gun fire while riding into the beach at 7:30 AM. Once ashore, he was told that the initial beachhead was going to collapse and he had to return to his transport ship. Three hours later his landing party was told that his unit was going back in since the infantry had gained a toehold and was hanging on. He’s the only person I knew personally who landed on Omaha Beach on D-Day, and he had to survive two landings.

John Watson had graduated from Slippery Rock State Teachers College (now Slippery Rock University) in Pennsylvania in 1940. He was immediately drafted into the US Army and was selected to go to Cartography School to become a mapmaker, draftsman and photographer. He was assigned to G-2, the intelligence section, and became a member of the group headed by Colonel Benjamin Dickson. By 1943 his unit had been transferred to England to prepare for the invasion of France. He

experienced the blitz in London and was involved with the planning of the invasion.

Since he was part of Dickson’s unit, he accompanied the colonel ashore that day. No one expected that the German forces would be still so lethal after the naval bombardment that took place beforehand. The beach master in charge of their landing zone perceived that the colonel was in danger of being killed or captured, and it was the duty of the men in his unit to get him alive back to the ship, hence the need for a second trip later in the morning.

On June 6, 2016 my wife and I stood on the seawall overlooking Omaha Beach.

A few of the hardened gun batteries still remain, but the barbed wire, beach obstacles, machine guns and mortar pits are all long gone. The sea wall where Allied troops hunched up on the shingle has been removed. There is a road that winds its way along the shore, and there is a modern stainless steel sculpture embedded in the sand near the exit where the US Second Division disembarked.

There are numerous memorials to individual units, but the ruined assault boats, vehicles, and barges have been swept away so that the only memories of the blood spilled and lives lost remain in photos. A few

of the original combatants visit even today. They are in neat blazers with their medals fastened to their chests, and they come in wheelchairs.

The enormity of a 62-mile front encompassing five individual beachheads is not easy to comprehend, even in person. The thought of a six thousand-ship armada, which materialized out of the fog over the English Channel that morning, beggars the imagination.

Just conceptualizing how to mesh individual transports, landing craft, airplanes and combat units kept thousands of men occupied for over a year beforehand. John Watson was one of them. But despite the impeccable planning, the success of the invasion still hinged on the resourcefulness of individual small groups of men who persisted in going forward in the face of certain slaughter to carry out their missions.

My father in law never spoke directly about the rest of his day on Omaha Beach, but for years after he returned home his wife Irene would find him waking up at night in cold shaking sweats with recurrent dreams of that day.

The cost to the Allies is not seen on the landing sites now.

There are a few small French museums scattered about that recall individual aspects of that day: a C-47 transport plane that carried US paratroopers; a Horsa glider that served the British airborne; a Sherman

tank; a German personnel carrier; rusted machine guns and mortars. The real price paid for the conflict is found in the cemeteries. British and American troops lie separately in memorial fields dedicated by their countries. The British cemetery is smaller, more individualized, with grave markers recalling the names, and their branches of service, but also preserving a small space at the base where the families can memorialize their fallen service member with a Biblical phrase, a tribute, or a verse. Flowers are planted in front of the gravestones. There are no sculptures. In contrast the American cemetery is large, spacious, and uniformly covered with crosses and stars of David that commemorate only the name, service and date of death in acres of closely mown grassy lawns. The memorial statue of the spirit of a rising young man at the center of the cemetery is bold, solitary, and tragically triumphant.

Karen and I wandered through the rows of crosses where we placed individual roses on random headstones. The remains of many teenagers lie there. I chose the grave of a young Floridian named Marvin Booth. He survived D-Day, but his future was gone the morning he crashed ashore, and he didn’t live through the summer.

Karen marked a Pennsylvania boy’s with her rose. A light mist covered the field even at mid-day. It veiled the sun and

blocked our view of the fateful beach that ranged several hundred meters beyond the perimeter of the cemetery. The calm belied the struggles and deaths that saturated a similar June day 72 years ago.

Before we left Omaha Beach Karen scooped up enough sand to give to each of our children and grandchildren.

Her father would be 100 years old in August. He’s buried beside his wife, to whom he was married nearly fifty years and who outlived him by seven years, in the family plot in Slippery Rock. Their son Bob has planted pansies, Irene’s favorite flower, at the base of the headstone. After we returned from Normandy, Bob sprinkled the sand Karen brought from Omaha Beach on their graves.

D-Day was the most significant day of her father’s long life.

None of us in his family would be here if he hadn’t made it off that beach on that day. His life was permitted to run its long course and to create the family and memories that we have been left with. There are no memories left for the boys who lie alongside each other in those cemeteries. It remains for us to remember them, what they did on that day, and who they might have become had they not landed on Omaha Beach. CMS

WINNING THE LOTTERY CHANGES YOUR LIFE

Much to the frequent consternation of my colleagues and friends, and even occasionally to the love of my life, Carol, I was born with a triple dose of conservatism.

My father was a career military man (surviving both WWII and Korea); my parents were from the South; and they were Presbyterian / Southern Baptist.

As they say in Texas, that’s a five-alarm chili!

Growing up on military bases at the height of the cold war, we used to practice crawling under our school desks in the event of a nuclear attack. I felt safe and confident there.

My father gave me one dollar a month allowance. I always spent it the same way – a model airplane to build and paint, plus ten cents for church. I would suspend the models from the ceiling of my room, and lie in bed at night dreaming of being up there.

After school it was a short bike ride to the runway, where I could watch the pilots come and go in their sports cars and fly their jets into the wild blue 12

yonder. We watched every Mercury and Gemini space launch on TV in school. My dreams were but a few years from fulfillment: until I was fitted with eyeglasses, that is.

As the saying goes, we make plans, and God laughs. The Air Force and Naval Academies required uncorrected vision of 20/20. The only person more disappointed than I was my father. Since I had poor vision, I decided to become a dentist.

But before that could happen, I had to win the lottery.

It was 1973, and my winning lottery number was 275 (out of 366). This number meant that I would not be drafted into the Army. How ironic – I wanted to be in the military. The previous year two of my dorm mates who were lottery losers and headed off for the military. That did not seem fair, and I did not like it.

After I finished dental school and residency, I was broke and newly engaged. I asked Carol if she would like me to join the Air Force for three years and pay off the debts. It would be a temporary diversion, and I would finally be able to do my part. Being the incredibly good egg that she is, Carol said yes.

My first hospital and dental commanders were just like my father – career Air Force and estimable rock solid conservatives. They were also consummate and inspirational healthcare providers. I was home again. Carol amazingly liked the military more than I did. Soon I asked the Air Force dental command to send me for Oral Pathology residency training – I had admired those mentors in dental school. The answer was an emphatic “No! We already have enough of those.”

What was I to do now?

On a whim, I called the physician office at command headquarters, and asked if I would be eligible for a military scholarship to medical school and pathology residency. A few days later, with a voice that implied “you’re nuts,” the answer was “yes.”

Durham, North Carolina was Carol’s hometown, and for some reason the Duke University School of Medicine really liked my Air Force scholarship money. Medical school there became a “short” four year break from active duty.

The Air Force sent Carol and me far and wide. I was general dental officer and Chief of Hospital Dentistry (Malmstrom AFB, Montana); Chief of Base Dental Services (RAF Fairford, England); resident and chief resident in pathology (Lackland AFB and Ft. Sam Houston Texas); Chief of Pathology and Chief Medical Officer (Offutt AFB Nebraska); Chief of Pathology (RAF Lakenheath England); fellow in Dermatopathology (Armed Forces Institute of Pathology Washington DC); and Chief of Dermatopathology (Lackland AFB and Ft. Sam Houston Texas). As consulting pathologist and medical director of the laboratory, I semiannually visited the hospitals at Whiteman AFB Missouri, Cannon AFB New Mexico, Aviano AFB Italy, RAF Croughton England, Incirlik AFB Turkay, and Ismir AFB Turkey. I was assigned temporary duty stints at Sheppard AFB Texas, Brooks AFB Texas, Travis AFB California, and Elmendorf AFB Alaska. I conducted College of American Pathologists laboratory inspections for the Air Force in Charlottesville Virginia, Lincoln Nebraska, Edinburgh Scotland, Brussels Belgium, Lajes Azores, and Keflavik Iceland.

Combine all of this with living as a child in Bermuda, Georgia, Colorado, Hawaii, the Pentagon, and North Carolina, and you can see I’ve literally been around the world.

second reason was the people: there were sterling characters everywhere, many of them young kids from less privileged backgrounds who were trying to better their lives. I was able to mentor them, and many went on to become nurses, doctors, pilots, and dentists themselves. I also served the spouses and children of those deployed around the world in our protection. How cool is that?

Call me crazy, but I love being a part of the rich heritage and professional family of Washington, The Minutemen and those at Valley Forge, Lee and Grant, Rickenbacker, Doolittle, the Red Tail squadron, and Eisenhower, to name a few. I pray that one day God will give me the privilege of saluting Pat Tillman and Chris Kyle. These guys and gals are heroes. How privileged Carol and I were to serve among them and in their footsteps.

We all have regrets in life – mine are monumental. But my military career is not one of them. I wish everyone could be so blessed. CMS

After twenty five years, our military career ended, and I was unbelievably fortunate to find a position with KWB Pathology Associates here in Tallahassee. What a wild ride and amazing string of good fortune it all was!

Still the question remains – why voluntarily make the Air Force a healthcare career? There are many answers to that question, but I’ll mention the two most important ones. First, as our clergy and police often say, I revered being part of something larger and far more important than myself. Like my Christian walk, I was attempting to serve a greater good and felt blessed by God to be doing so. As far back as I can remember I have always had a problem with my weight. I was raised in a family with a weight problem. My mother and father both were very large; obese I guess it would be considered. I am sure their weight had plenty to do with their health issues later in life; my father having been diagnosed with diabetes and high blood pressure and my mother having diabetes for most of her life. Now as I am getting older I find myself with some of the same problems. Growing up south of Tallahassee my family ate all the usual southern style food, loads of fried fish and chicken with sweet tea along with other family favorites. My sister and I did things outside; it was not like we just sat indoors watching television all day. But maybe we just did not do the type of exercise that is required to lose weight or to keep from gaining weight.

I would have to say that my weight definitely affected my life during grade school. I did not like school as a child. I’m sure my weight was a major part of my problems in school. I can easily recall some of the other kids, on the bus mainly, saying things about my weight. I think this became a vicious cycle because I would come home and just snack the rest of the day and not do much else. I figured I was not athletic enough to participate in any school sports so I did not ever try out for anything. During physical education class I would walk some but never really played any group sports. As I got older though, it got better later in high school. I think this was probably due to adapting a sense of humor and not dwelling on my weight. I became the class 14 CAP SCAN - A CAPITAL MEDICAL SOCIETY PUBLICATION

clown so to speak. I have continued to carry a sense of humor with me throughout my life.

Humor has been something I rely on quite often.

After high school I tried junior college for a couple semesters but decided I would rather work instead. So I started working at a retail store in Tallahassee. It was around that time when I was diagnosed with my first life-changing health problem, diabetes. I had started noticing at work I was always hungry and had to go to the bathroom far more often than usual.

After a couple of weeks with these symptoms I visited the doctor. I remember talking with him about how my weight was an issue and that it most certainly was part of the problem.

This was upsetting for me because I remember how my mother struggled with her blood sugar when I was growing up. I did not what to go through the same problems. Over the next few years, even though I knew better, I did not try to do much about my weight. I was living in a small apartment and working quite a bit. I often did not take the time to cook and usually would pick up fast food for lunch and dinner.

My weight continued to go up. Eventually my blood pressure started to rise and I was having a lot of trouble breathing and coughing. My blood pressure was climbing higher and higher each time I went to the doctor for my diabetes visits. Finally my doctor diagnosed me with hypertension. He had me start taking a new medication for my blood pressure and he wanted me to see a lung doctor as well for my breathing.

that due to my obesity it had become hard for my lungs to get the amount of air that my body needed. I eventually had to start wearing an oxygen cannula and a CPAP at night. It did not take long for me to learn just how different my life would be. Now I always have to make sure I have oxygen in my tank before I leave the house and I cannot stay out very long.

Now at that point I was in my late thirties and was taking a handful of medications throughout the day. I began to feel down about my health.

I was having trouble getting out of bed in the mornings. All I wanted to do was sit in my house and snack. I was diagnosed with depression and started taking an antidepressant in addition to my other medications. Shortly thereafter, my insurance provider contacted me about switching my care over to a specialty care center. I was told that my health might benefit if I switched providers. When it came time for me to see my new doctor we sat down and discussed a lot of my health issues. We discussed how important it was for me to exercise and watch what I eat.

We also discussed bariatric surgery for me to lose weight, something I had been reading a lot about.

But my doctor said that I would have to try several different options before I could potentially get the procedure, a few things being exercise and diet.

to get the operation. Everything went well for the first visit, I was overjoyed. But at the follow up visit before my operation I was told I was no longer eligible for the procedure due to my insurance. How could this be? Needless to say I was devastated. I stopped exercising and started eating fast food again. I was feeling down about everything. I discussed this with my doctor who believes there must have been a mistake with my insurance. Now we are working together to try and become eligible for the procedure again.

So here I am back on my exercise routine, my weight has dropped even further than where it was at the time of my first pre-operative visit.

I think it’s probably the least I have weighed in many years. I am spending a great deal of time with my niece and nephew, who have been instrumental in helping me stick with my exercise and diet. They have been an inspiration throughout this whole ordeal. I am going to continue to try for the operation and I will remain hopeful for the future.

Inspired by the hope that I could eventually qualify for bariatric surgery I began to walk and tried the gym in my neighborhood. It was so difficult at the beginning, mostly because of my breathing. Over the next few months, with encouragement from my niece and nephew, I added more time to my daily walks and was exercising most days of the week. I felt as if I was making quite a bit of progress. I even began going to the Diabetes Center since my blood sugar would need better control for the procedure.

During the next visit to my doctor I discovered I had lost over ten pounds. That was great news!

OBESITY: A NATIONAL HEALTH CRISIS AND A THREAT TO SAFE PATIENT CARE

Troxel, M.D., Medical Director, The Doctors Company Obesity continues to be a national crisis: Current research has found that 35 percent of men and 40.4 percent of women in the United States are obese.1 The obesity crisis not only contributes to growing health costs but also raises serious patient safety risks.

Patients who have experienced an adverse medical event leading to a medical malpractice claim are frequently noted to be obese (based on documented height and weight). A review of 7,065 claims from 2011 to 2013 by The Doctors Company, the nation’s largest physician-owned medical malpractice insurer, revealed that 28 percent were identified as having one or more comorbidities, and obesity was the most common (8.3 percent of total claims and 19.2 percent of total claims with a comorbidity). When closed claims were analyzed, 26 percent of claims that resulted in indemnity payments listed obesity as a comorbidity.

available at night or on weekends should have transfer agreements with open facilities in place so there are no delays in urgent MRIs or CT scans. The failure to transfer an obese patient to a facility with an open MRI machine or CT scanner in a timely fashion may result in a delay of diagnosis and/or surgical treatment—and, ultimately, in a malpractice claim.

Practices should have appropriately sized furniture in the waiting areas and exam rooms to meet the needs of obese patients. They should also have equipment—such as blood pressure cuffs, needles, and wheelchairs—designed for obese patients.

It’s key to understand the importance of talking about weight with patients—the conversation should take place early for better prevention and treatment. Many factors can arise that inhibit a practitioner from speaking frankly about weight with a patient. As obesity rates continue to increase, it is worthwhile for doctors and other healthcare professionals to recognize that they might have their own barriers to such communications.

Increased Risks: Complications and Access Issues Obese patients commonly have a variety of comorbidities. Many are associated with a metabolic syndrome, such as hypertension, dyslipidemia, and hyperglycemia, which increases the risk of stroke, ischemic heart disease, and diabetes mellitus. These patients also have increased risk of obstructive sleep apnea (which often contributes to opioid-induced respiratory depression), susceptibility to nosocomial and postoperative infections, and weight-associated wear and tear on joints that can lead to osteoarthritis. Additionally, bariatric surgery can be associated with both surgical and metabolic complications.

In addition to the risks of comorbidities, healthcare facilities also face risks if they are unprepared to accommodate obese patients. An inability to fit a morbidly obese patient into a conventional MRI machine or CT scanner is a unique problem necessitating use of an open MRI or CT. Healthcare facilities that are unable to accommodate morbidly obese patients in their MRI machine or CT scanner or if their MRI or CT isn’t

Sensitive treatment of obese patients involves attending to their needs for comfort, safety, and respect. Obesity can be viewed as one of the many chronic health conditions afflicting patients. The person, not the obesity, should be the focus of treatment. As with any patient with a chronic health condition, a relationship with respectful caring forms the bedrock of medical care.

CAPITAL MEDICAL SOCIETY FOUNDATION PROVIDES GRANTS TO IMPROVE ACCESS TO HEALTH CARE Pam Wilson, Executive Director, Capital Medical Society Foundation presented grant checks to Mark Baldino, Executive Director of Elder Care Services and to Kimberly Galban-Countryman, Executive Director of Lighthouse of the Big Bend and her Development Director Yolanda Robles on June 16, 2016 on behalf of the CMS Foundation’s Board of Directors. The two $5,000 grants were given in fulfillment of its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.”

And quietly with little fanfare, that is exactly what it has been doing since 1975, when it was first incorporated as The Foundation for Medical Care of the Big Bend Area of Florida. Elder Care Services will use its grant funding to provide basic in-home services to low income seniors. Lighthouse of the Big Bend will use its funding to provide aids needed by the visually-impaired to manage their health and medical care independently.

CMS Foundation was recently recognized as a Finalist for the Nonprofit Category of the Tallahassee Chamber Award and is a past recipient of the Blue Foundation’s Sapphire Award.

In addition to this coordinated care, Capital Medical Society Foundation, Inc. raises funding and distributes up to $50,000 annually in scholarships to FSU College of Medicine students. Priority is given to those students who come from a rural background, have financial need, and have expressed an interest in returning to the Big Bend to practice medicine upon completion of their residencies.

The Impact of Private Support: Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.”

• Access to healthcare is increased through gifts to We Care Network and the We Care Network Endowment Fund. Over $77 million in donated specialty medical and dental care has been provided to low income, uninsured patients since the program was created in 1992. In fiscal year 2015-16, which ended June 30, 2016, over $5 million in health care was donated. Gifts support the salaries and benefits of the case management team who coordinate donated care.

Our intention is to build a world-class cancer center.
