2017 CELEBRATION AWARDS DINNER

By Shannon Boyle, CMS Communications and Events Coordinator The sixteenth annual Celebration Awards Dinner honored Dr. Richard Zorn and Dr. Wayne Batchelor.

The event was held in honor of Doctors’ Day on March 30th at the FSU University Center Club. There was a room full of proud supporters of the award winners. Dr. Richard Zorn was awarded the I.B. Harrison, M.D. Humanitarian Award, and Dr. Wayne Batchelor was awarded the Outstanding Physician Award. Our CMS President, Dr. Tracey Hellgren did a great job emceeing the event.

Dr. Hellgren thanked the Celebration Awards Dinner Steering Committee and the Capital Medical Society staff for their hard work in organizing and coordinating this event.

Dr. Hellgren recognized six CMS members who retired this past year: Dr. Susan Cross, Dr. James Davenport, Dr. John Mahoney, Dr. Margaret Havens Neal, Dr. Steven Ostrov, and Dr. Jitendra Padalia. We wish them well in their retirement.

A moment of silence was held to honor several physicians who passed away this past year: Dr. Edwin D. Crane, Dr. Lee Elgin, Dr. T. Bert Fletcher, Dr. Nelson Kraeft, Dr. Stuart Smith, Dr. Robert P.C. Whittier, and Dr. Karen Wilkens.

2017 CMS CELEBRATION AWARDS DINNER

Dr. Hellgren thanked the referring and donating providers in the We Care Network. She noted that We Care Network donating providers donated more than $7 million in care last year and a cumulative total of more than $80 million in the last 25 years. We Care Network is celebrating their 25th year of service to the community through its provider partners this year.

After explaining that the Outstanding Physician Award honors a physician who has demonstrated outstanding leadership in the profession of medicine, Dr. Hellgren asked all past recipients of this award to stand and be recognized. Dr. Wayne Batchelor joined these esteemed physicians, as well as those who could not attend, as the 2017 recipient of this prestigious award.

Dr. Thomas Noel, friend and colleague of Dr. Wayne Batchelor presented him with the Outstanding Physician Award.

Dr. Hellgren then discussed a brief history of the I.B. Harrison, M.D. Humanitarian Award. The criteria for the award are predicated on the values that came to be associated with the late Dr. I.B. Harrison: compassion, service, respect, wisdom and integrity.

She asked all past recipients of this award to stand and be recognized. Dr. Richard Zorn joined these esteemed physicians, as well as those who could not attend, as the 2017 recipient of this prestigious award.

DR. DON ZORN AND DR. AUSTIN ZORN’S PRESENTATION TO DR. RICHARD ZORN, RECIPIENT OF THE 2017 I.B. HARRISON, M.D. HUMANITARIAN AWARD Dr. Don Zorn…

First, congratulations to you, Wayne. Your contributions have been truly awesome, and we are blessed, our patients are blessed, to have the benefits of your wisdom and care.

Beeper goes off. Sometime in the night. I call the ED. In my mental fog, I finally realize that this is a surgical case. “Excuse me,” I interrupt. “I’m sorry, but you need the real Dr. Zorn.” The real Dr. Zorn is who we’re talking about tonight.

Richard Zorn was born 1947 in Macon, GA, the middle of 3 boys. His dad was a Methodist preacher and his mother was a schoolteacher turned full time mother and preacher’s wife, who ruled the household strictly, lovingly.

He was the best looking of the 3, at least until middle school, when his vision required significant refraction.

He has always claimed the thick framed glasses of the day cramped his style with the ladies.

Richard was athletic, too, believe it or not, briefly putting his skinny frame on the line for high school football in Tifton, and playing JV basketball where he was the star of the team. However, after his family moved to Macon, he suddenly found himself at Lanier High, a public high school, 1600 boys and zero girls, and compulsory ROTC for all. He wore military uniforms 3 days per week, and quickly ruled out a military career.

With so many boys at Lanier, Richard was no longer the star, but (L-r) Dr. Richard Zorn, Dr. Austin Zorn, and Dr. Don Zorn he did make the varsity basketball team, had a sweet J, and a presence in the paint. No excuses: the three of us were expected to do well in school. The star, of course, was Mom’s favorite, Richard, who never seemed to get into trouble. The typical story: Richard would race thru a year’s worth of assignments in a month, then for the rest of the year he would help the teacher to teach the class.

Richard followed his older brother George to Atlanta (George, I’m happy to say, is here tonight. He started all of this, and is to blame for all the Zorns in medicine). At Emory, Richard would spend an astonishing 13 years in college, medical school, and general surgery residency.

In college, like his brother, he pledged Sigma Chi, and played varsity tennis for the Emory Eagles.

In medical school, he was elected to AOA honorary society and graduated with honors. When asked what kind of doctor he was to be, he always said “a rich one.” He had already spent a month with a rural family doctor in SC. “No way in hell,” was his response to that. Among the giants at Emory with whom he worked was J. Willis Hurst, the world famous cardiologist, who pushed the new Weed System, the Problem Oriented Medical Record, SOAP notes and all. Richard saw general surgery as a refuge from all of that.

In 1978, Richard settled in Tallahassee, largely thru the influence of the vascular surgeon Tommy Lawhorn. He joined Hank Watt and Jim Cavanagh, and when Jim retired in 1998, Richard joined Dr. RD Snyder, a close and valued colleague, with whom he still practices.

His leadership posts include Chair of the TMH medical staff 1989-90, and President of the Tallahassee Physician Association, which was the parent organization for HealthPlan Southeast. “I was the one supervising the demise of TPA”, he notes.

Along the way he has served numerous committees, and continues to do so. As a reliable and effective teacher, he has helped many Family Practice residents and FSU COM students, and has taken on new roles with the FSU COM and the nascent General Surgery Residency Program.

But the true measure of the man lies in 2 things: his love of and faithfulness to his family, and the care and compassion he has shown his patients.

Richard’s family has had its share of challenges, including unspeakable tragedy, the loss of son Brian.

Thru it all, he has been there, standing tall, putting one foot in front of the other, refusing to give in, faithful to the end. In this, for the past 13 years, he’s had the loving help of his wife Kathy. His children and grandchildren stand witness, as do we, to the power of Richard’s love to positively influence the direction of his family.

Those who know him best know this. Much can be said. Being 6 feet 4 inches tall doesn’t begin to say it.

To his family, he is a giant of a man.

But it is the day to day, night to night, meeting

patients at their point of need which we celebrate most tonight, 39 years on the front lines of surgical disorders and illness, where he has made his mark.

He had not been in Tallahassee long before one of his ER call nights brought in a young inebriated motorcycle driver who did not see that Apalachee Parkway ended at the old Capitol. The collision with the tree at high speed left him with multiple injuries including a badly lacerated liver. After transfusion of 120 units of blood, multiple surgeries, and months of care, the young man died. It was tough all around, especially for a young surgeon new to town, but Richard took solace in the help and support of fine nurses in ICU and surgery, and the willing and able physicians of the TMH medical staff. He felt convinced that in Tallahassee he had found his medical home.

Dr. Austin Zorn…

Before I say my piece, I would like to say that my Uncle Richard operated on me at the age of 2…and I turned out fine. I’ll leave that up to you all to decide.

But I actually want to share with you all what it’s like to know Richard both personally and professionally.

We are both the middle child in our respective families, and we’ve always had a bond over that.

Outside the hospital, Richard is easy-going. He’s the supplier of junk food on our beach trips. He’s hosted numerous large family gatherings at his house. He’s supplied Kelly and me with innumerable football/ baseball/basketball tickets. He’s always making sure the rest of us are having a good time in our own way.

At the beach, he is an avid reader, perhaps even the fastest I’ve seen. He’s known for bringing a 3-400 page book for every day he’s on vacation, a fact he will shrug off as “Oh, these are just trash…” There are a great many more stories about Richard I could tell.

Most would make you laugh, and some would make you cry. But I’ll sum up by saying this. To know him personally is to know a truly kind and gentle spirit.

As I’ve started my career here in town, I’ve also gotten to know him professionally. I remember one particular weekend, I was rounding on 3A, writing in a chart when I heard the unit secretary state “Dr. Zorn’s on the phone.” I looked up wondering who they were referring to, and quickly realized they meant Richard.

let’s just say that even though I wasn’t on the phone, I heard the entire conversation. Richard is a fierce advocate for his patients (ask anyone in the hospital).

As I have gotten into practice here in town, I have realized that referrals between PCP and surgeon are a two-way street. Occasionally in practice, I get patients that make me want to ask, “Where, again, did you hear about us?” After seeing one such patient, he beamed and said, “Your uncle gave me your name.” Well it just so happens that he has an equally interesting spouse, and you better believe that the first time she had a surgical issue, I sent her same-day over to Richard’s office.

On a serious note though, in our age of computers, templates, and electronic medical records, it is unbelievably refreshing to get a 9-line consult note. No fluff, nothing extraneous. A clear and concise plan for moving patients toward wellness. Many times, I have picked up the phone to discuss a case with Richard, and he has formulated a straightforward plan then and there. I have even sent patients to Richard, highly doubting that their problem was even surgical, simply for his diagnostic opinion. I’ve also found that many patients simply benefit from his calming demeanor, even when no surgery is done.

I’ll leave you with this. I remember vividly the first time I understood just how gifted Richard is. I was between my first and second years in college. My grades were flagging, my motivation was waning, and I was simply unsure where I was headed. I think my parents were concerned about me, and so they suggested I spend time with 2 people. One of those people

was Dean Watson; the other was the man we are honoring tonight.

I spent a great deal of time with my uncle that summer, but I still remember the first case I saw.

Bowel obstruction, 80+ year old patient in absolute agony, multiple prior abdominal surgeries. As he opened the peritoneal cavity, he let out a long slow breath (and maybe a 4-letter word or two… or three). Adhesions, scar tissue.

Everywhere. A two hour surgery had just become a five hour surgery. And then, slowly, (and to me at the time, it seemed VERY slow), he made his way through the tangled mess. Kathy (his first assist on this case) occasionally slapping his hands away from ‘danger spots’. It was miraculous for me to witness at the age of 19. The human body, in a most vulnerable position, in the skilled, trained, patient hands of a master. It was Mr. Fix-It, doing what he has done best for so long. That patient had an incredible outcome, lived long after that particular procedure with great quality of life. And Richard probably hasn’t thought much about that surgery, but it was one of the days where he stopped being only Uncle Richard to me. That summer I spent with you changed my life.

I have known him both personally and professionally, and in my totally biased opinion, there is nobody more deserving of this award than you.

the contributions of our local physicians, you have left own your mark on the fabric of both, with scars marking the healing you have promoted, and scars unmade in your better judgment testifying to the adage to first do no harm.

From that inebriated young motorcyclist your first year to last Thanksgiving afternoon when our shared 89-year-old patient Mr. M had spontaneously dehisced his cecum thru his abdominal wall at home. You, while driving north in GA to a family gathering that night, paused briefly, quietly, then turned around, and came back to spend several hours fixing the problem, along with Kathy, that night. Successfully I might add.

That was just so typical.

These cases serve as bookends near the boundaries of your career, with thousands of cases well-served in between. These are the patients, and many like them, who celebrate you and who grieve to think of your impending retirement.

Whatever the situation, you have brought your best: strong intellect, well-developed surgical skills, sound judgment, dedication, integrity, and compassion.

What you see with Richard Zorn is what you get: understated, yet excellence, consistently.

“Just taking care of folks,” as Dr. Harrison would say.

DR. RICHARD ZORN’S ACCEPTANCE SPEECH UPON RECEIVING THE I.B. HARRISON, M.D. HUMANITARIAN AWARD It is an incredible honor to accept the I.B. Harrison, M.D. Humanitarian Award for 2017. The list of previous award winners reads like a who’s who of Tallahassee area medicine. I have had the honor and privilege to work with many of these physicians over the years and it is a very humbling experience to have my name included with theirs. They were not only excellent physicians but they were really good people. It is worth noting that Tracey Hellgren was President of CMS in 2011 when my brother Don got the Harrison Award, she is now back for her second term when I got the award. If she comes back for a third term in say 10 to 15 years we may have a few more Zorn possibilities for her...I am just saying.

When I came to Tallahassee in 1978, Dr. Harrison was the Chief Medical Officer for Tallahassee Memorial Hospital. He always had an orientation session for the new members of the Medical staff. Drs. Bob Snyder, Gary Hansen, Frank Skilling and myself were among those at the meeting. There was not much give and take. He laid out what was expected of us regarding patient care and our role as physicians in the community. He presented the high expectations that he had for us; we listened and accepted what he said because we knew that these were the same standards that he lived by.

The medical community in Tallahassee in 1978 was incredibly different from the one that exists in 2017.

There was only one hospital. My medical record is 358. The last one given out at TMH was 2640. Dr. A.D. Brickler’s number is 109. Dr. Orson Smith’s is 175.

I started my office with one employee, Ms. Shirley Whittington. She told me what to do and I did it.

Capital Health Plan and Capital Regional Medical Center had not come into existence. Capital Medical Society under Ms. Diane Pickett ran out of two small rooms near the auditorium at TMH. Dr. Nelson Kraeft ran a tumor clinic for indigent patients with cancer out of those same rooms once a month. TMH had just ordered one of the first EMI scanners in the State of Florida.

There were very few hospital employed physicians. A very high percentage of local physicians saw their patients in the hospital and thus interacted with each other on a more regular basis.

Smoking was still allowed in the hospital. There was a small room in TMH near where Mr. Guidice’s office stands, where a group of doctors would meet after making morning rounds. Not a lot of problems were solved but a lot of smoke was inhaled, even by the non- smokers. That professional physician to physician interaction is one of the potential casualties of the modern medical scene. We do not make it easy for other physicians to reach us in our offices.

Electronic phone systems may be more efficient but not necessarily easier to navigate. I had one ask me for my birth date before it would put me through to the physician. This decrease in physician to physician interaction is given as one reason for the increase level of physician burnout. The January 2017 edition of The ACS Surgery News states that “peer support, a long-standing source of strength among surgeons and other physicians has been unintentionally eroded in many modern medical institutions. increasing productivity expectations, documentation requirements, and clerical burden.”

As a medical community we have had many labels over the years. Vendor and provider being two of the more recent ones. The term I prefer is colleague. I went to the dictionary expecting some very impressive terms that would inspire you but all it said was, “An associate in a profession.” I want to tell you that you have been much more to me and my family than that.

You have had our back and my patients back. When I needed you professionally, you were there. When we suffered personal tragedy, you were there. Dr. Snyder and I are in the process of turning things over to our new associate later this year. Part of that is planning follow up for a lot of our long-term breast cancer survivors. It has been hard and rewarding at the same time. I will miss these relationships greatly, but what I will miss even more is the daily interaction with you, my medical colleagues. Try to use every opportunity to interact with each other. The simplest conversation or gesture gives more strength than you will ever know.

There is a real art to receiving a gift to show you appreciate the thought, time, and effort that went in to it. Have you ever seen a child open a present? Now that is true appreciation. No thought to right color, cost, or how long it is going to last. Just unbridled joy and enthusiasm. My mother’s (God rest her soul) favorite line was “You shouldn’t have.” “But mom, do you like it?” “Yes, but you shouldn’t have.”

“Mom!” My wife says that I am not very good at receiving gifts, not the right something or other. I want you to know that I really appreciate this award and understand the history and the significance of it.

However, you are the reason that I am standing up here this evening. The people in this room and the ones that could not be here tonight, although I do think going to Norway to see the Northern Lights or going to Cabo San Lucas just to keep from coming tonight is a little excessive. To my wife, Kathy who squeezed my arm or made some other gesture when my voice got too tight on the 2 am phone calls, and to my brothers, George and Don, especially Don since people have trouble telling us apart and I probably got credit for some of the things he has done, and to my kids and their wives and husbands and to my grandkids who make life more exciting, THANK YOU. 14 CAP SCAN - A CAPITAL MEDICAL SOCIETY PUBLICATION

To partners and former partners I still think we checked the right box. To office staff, past and present, who still tell me what to do and keep me out of trouble. KAREN WILKENS, M.D.: A REMINISCENCE

By Frank Skilling, M.D. Karen Wilkens didn’t look like a trailblazer. She didn’t have the dashing, devil-may-care demeanor of Amelia Earhart. She didn’t roll up her sleeve a la Rosie the Riveter to show her muscles.

She didn’t march in the streets like the suffragettes in the early 20th century. She was just Karen Wilkens, M.D. And she got things done.

I first encountered Karen soon after I opened my practice in Tallahassee. She was the first female family medicine resident to rotate on my ophthalmology service, and she presented herself as polite, quiet and obviously interested in medical care. She didn’t say much initially, but she had a way of gently asking questions about patients that showed her concern of how their particular problems affected them.

Perhaps because she had trained as a speech-language pathologist and audiologist, she seemed to perceive eye problems from a patient’s point of view better than some of the other residents I encountered over the years.

After she opened her private practice, she would call me to ask my opinion about a particular patient who had an eye complaint.

Her concern was usually if the patient needed to be seen immediately by me or if she could refer for a later appointment.

I don’t recall exactly how she came to be CMS president, but it was at a time of change when employed

physicians were being brought to town and the old guard of solo practitioners was being phased out. The CMS meetings then were more open and raucous affairs that were held at the old Silver Slipper Restaurant in the ground floor of what was then Northwood Mall.

Some of the older physicians would use the open session to make pronouncements from the floor endorsing or rejecting items of general interest to physicians.

I don’t recall that any of these disturbed her equilibrium, even though they stirred up reactions among the membership.

After being president in 1993-94, she continued her service as a delegate to the FMA meetings, and she was helpful in moving the FMA headquarters from Jacksonville to Tallahassee.

as a physician depends on one’s communication skills. Most of us have to develop and polish them in a lifetime of medical practice.

Many times we fail and don’t know it. Karen seemed to have the ability to connect with her patients who rewarded her with the loyalty and devotion that few of us ever achieve.

It was especially saddening for me to follow her decline as Parkinson’s Disease ravaged her brain. In addition to the motor dysfunction of her limbs, Parkinson’s dementia interfered with her ability to communicate with those who loved her. I could sense her frustration at losing control of her mind and body even as her friends and family tried to rally around her. For me her death came with a sense of relief as well as sadness.

Many of the younger physicians will not recognize the unusual position for women in medicine a generation ago. Karen’s persistence yet peaceful intensity helped make the transition easier for those who followed her, even though she didn’t seem like a trailblazer at the time.

God bless you, Karen. You lived the type of life and achieved the professional acclaim that few of us will ever attain.

UP FOR REVIEW: FIVE KEYS TO MANAGING ONLINE CRITICISM

By Kevin Pho, MD, Founder and Editor, KevinMD.com Patients have more avenues than ever to express themselves online, whether on social media or through physician rating sites like RateMDs, Vitals, and Healthgrades. No matter how professional and caring a doctor you may be, eventually you will face criticism on the web. No doctor will receive universally positive reviews. So when a patient posts critical comments about you, it’s important to know how to respond. Here are five keys to managing that criticism.

1. Listen to the criticism. Patients may leave online reviews because they feel this is the only way they can have a voice. After patients leave your exam room, often you don’t know what they thought about you or your practice. The criticism might not even be about you. You don’t know what patients thought about the nurses or medical assistants, or if they were concerned about the parking or whether the waiting room magazines were up to date. These are issues you may not be aware of—but they matter to patients. By listening to online criticism, you can identify and fix easily correctable situations and improve patients’ satisfaction scores.

2. Take critical conversations offline. Whenever you see criticism on the web, there’s a strong temptation to respond to it immediately. You want to set the record straight and clear the air. Instead, take the conversation offline. An online argument is unlikely to result in anything productive. Post a standard reply thanking the patient for the comment and asking him or her to call the clinic. Be careful not to reveal any private patient information. If you can resolve the dispute over the phone or in person, the patient may take down the comment or even add an addendum stating, “You know what? This office is actually listening to what I have to say.” That can turn a negative situation into a more constructive one. Take the same approach whether the patient’s comment is on a ratings site or on social media. If you’re employed by a hospital or healthcare system, coordinate your efforts with your marketing or public relations team, who are likely to see an offline conversation as the most beneficial solution for both you and the organization.

3. Read the fine print. If you believe any online comments are suspicious, contact the rating site to see if the comments violate the terms of service agreement. For example, a patient left my practice a little disgruntled. Shortly after that encounter, dozens of negative ratings appeared on a rating site that could have conceivably come from this one patient. I reported the comments because the rating site has a terms of service agreement that prohibits anyone from posting multiple ratings on a single doctor. The company investigated and found that all of the ratings came from a single computer. The site then removed the comments. Always read the terms of service agreement and report any possible violations.

4. Ask more patients to rate you online. Most patients generally like their doctors, and dozens of studies show that a majority of online ratings are positive. By asking more patients to rate you online, you can make negative ratings look more like outliers. In the surgical world, there’s a saying about 22 CAP SCAN - A CAPITAL MEDICAL SOCIETY PUBLICATION

irrigating an abscess: “The solution to pollution is dilution.” The same principle applies to physician rating sites. If you ask more patients to rate you online, the positive comments can dilute the negative ratings by placing them lower in search results and making them less visible. Your patients just need to be encouraged to write reviews. Ask your patients to post a review if there’s something they like about you or what your practice is doing, or if they have any suggestions for your practice. Don’t cherry-pick patients or pressure them to say something positive about your practice, but ask for a rating from every single patient in a low-key and low-pressure way. Many practices even hand out cards with specific instructions on how to rate their doctors online. On the whole, the reviews will be positive.

5. Resist the urge to sue. Only rarely have doctors successfully sued rating sites, which may argue that removing negative ratings is an infringement of a patient’s right to free speech. Also, suing patients for bad reviews may backfire.

A doctor once sued a patient for a negative review and made front-page headlines in a newspaper. Now whenever you search online for that doctor’s name, the newspaper story comes up as the first result. By suing patients over criticism, you will only bring more attention to it and highlight the negative reviews.

Final Thoughts Doctors by nature take all patient interactions very seriously—and often take criticism personally. We are trained to take a one-on-one approach to patient care and to make sacrifices for our patients. That makes negativity especially hard to hear. It may be difficult to regard online criticism as an inevitable part of the job, but that’s what it is. Patients don’t expect us to be 100 percent perfect, and patients are more likely to see an 89 percent positive rating on a website as more authentic than a 100 percent rating.

Try to manage your patients’ expectations—and also try to manage your expectations for yourself. Recognize that you may not be the right fit for every patient, and that sometimes a patient simply has different expectations than you do.

We now live in a world where doctors are rated like professionals in many other industries, a trend that will continue to grow. Many doctors dislike being rated at all, but to succeed in the online world you shouldn’t ignore reviews. Instead, approach online ratings proactively. You’ll find yourself better able to influence the online conversation about you, fix any shortcomings in your practice, and engage critical patients in a positive, constructive way.

The Impact of Private Support: Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.”

• Access to healthcare is increased through gifts to We Care Network and the We Care Network Endowment Fund. More than $79 million in donated specialty medical and dental care has been provided to low income, uninsured patients since the program was created in 1992. In fiscal year 2015-16, which ended June 30, 2016, more than $7 million in health care was donated. Gifts support the salaries and benefits of the case management team who coordinate donated care.

• To provide incentives to keep medical students trained at FSU College of Medicine in the Big Bend after completion of their residencies and help meet the growing demand for physicians in Florida, gifts to the Scholarship Fund provide scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice after completion of their residencies. $47,000 was distributed to eight medical students in 2016.
