2016 CELEBRATION AWARDS DINNER

By Shannon Boyle, CMS Communications and Events Coordinator The fifteenth annual Celebration Awards Dinner honored Dr. H. Avon Doll, Jr. and Dr. Michael W. Forsthoefel. The event was held in honor of Doctors’ Day on March 24th at the FSU University Center Club. There were many proud friends, colleagues, and family members in attendance supporting the award winners.

Dr. H. Avon Doll, Jr. was awarded the I.B. Harrison, M.D. Humanitarian Award, and Dr. Michael W. Forsthoefel was awarded Dr. Faisal Munasifi with the Outstanding Dr. Nancy VanVessem, Title Sponsor, Physician Award.

Dr. Forsthoefel was unexpectedly admitted into the hospital the day before the event and was unable to join us, but we honored him just as if he were there. Our CMS President, Dr. J. Daniell Rackley did a great job emceeing the event.

Dr. Rackley thanked the Celebration Awards Dinner Steering Committee and the Capital Medical Society staff for their hard work in organizing and coordinating this event.

2016 CMS CELEBRATION AWARDS DINNER

Dr. Rackley recognized three CMS members that retired this past year: Dr. Charles F. Manning, Jr., Dr. Nola Munasifi, and Dr. D.J. Underwood. We wish them well in their retirement.

A moment of silence was held to honor several physicians who passed away this past year: Dr. Donald Alford, Dr. Baxter Byerly, Dr. Jo Dalal, Dr. Evan “Dickie” Dussia, Dr. Wendell Heaton, Dr. Vildan Mullin, Dr. Eric Nicholson, and Dr. Thomas Wood.

Dr. Rackley thanked the referring and donating providers in the We Care Network. He noted that We Care donating providers donated more than $4.5 million in care last year and a cumulative total of more than $64 million in the last 22 years.

After explaining that the Outstanding Physician Award honors a physician who has demonstrated outstanding leadership in the profession of medicine, Dr. Rackley asked the past recipients of this award, in attendance, to stand and be recognized: Dr. Thomas Hicks, Dr. Larry Deeb, Dr. Charles Williams, Dr. Dean Watson, Dr. Alma Littles, and Dr. Armand Cognetta. Dr. Forsthoefel joined these esteemed physicians, as well as those who could not attend, as one of those honored with this prestigious award.

Dr. Joseph Camps and Dr. J. Orson Smith, friends and colleagues of Dr. Forsthoefel, presented the Outstanding Physician Award to Dr. Forsthoefel’s wife, Dr. Jana Bures-Forsthoefel. Dr. BuresForsthoefel accepted the award and made a few remarks on his behalf.

Dr. Rackley then discussed a brief history of the I.B. Harrison, M.D. Humanitarian Award. The criteria for the award are predicated on the values that came to be associated with the late Dr. I.B. Harrison: compassion, service, respect, wisdom and integrity.

He asked all past recipients of the award to stand and be recognized. Those in attendance were: Dr. J. Brian Sheedy, Dr. J. Orson Smith, Dr. David Craig, Dr. James Stockwell, Dr. Lisa Jernigan, Dr. Louis St. Petery, Dr. Whit Oliver, Dr. Don Zorn, Dr. James Geissinger, Dr. David Miles, and Dr. Al McCully. Dr. H. Avon Doll, Jr. joined these esteemed physicians, as well as those who could not attend, as the 2016 recipient of this prestigious award.

Dr. Lisa Jernigan, friend and colleague of Dr. Doll, presented Dr. Doll with the I.B. Harrison, M.D. Humanitarian Award. (You can read Dr. Jernigan’s remarks on page 6 and Dr. Doll’s remarks upon receiving the award on page 9.)

The music provided by FSU College of Music’s talented students, Jia-Rong Gan and Tim Stephenson made this an extra special evening.

DR. LISA JERNIGAN’S PRESENTATION TO DR. H. AVON DOLL, JR., RECIPIENT OF THE 2016 I.B. HARRISON, M.D. HUMANITARIAN AWARD

It gives me great pleasure to provide a brief and certainly inadequate introduction for Avon Doll, this year’s I.B. Harrison, M.D. Humanitarian Award winner. Having known Dr. Harrison and profited greatly from observing his approach to patient care, I can assure you that Dr. Doll and Dr. Harrison share many attributes in this area.

First, a commitment to excellence of care, from knowledge base to availability at all hours. Second, a compassionate approach to those needing care, understanding them as fellow humans in need.

More than perhaps any other physicians of my acquaintance, Drs. Harrison and Doll share the ability to find humor in the foibles of their patients as well as themselves, without ever violating the call to offer respect to those under their care. The patients at the Quincy dialysis unit are practically his family members after all of these years, and I have never met one that did not smile broadly when telling me that Dr. Doll was their kidney doctor.

Dr. Doll grew up in Pennsylvania, and went to Medical School at Oral Roberts University, where he met Louie. He also earned induction into Alpha Omega Alpha. He and Louie moved to Gainesville for residency in Internal Medicine, Chief Resident Year, and then Nephrology Fellowship at UF. Then they moved to Tallahassee where he joined the practice of Drs. Applegate and Hansen. Shortly after that is when we met, caring for the same HIV patient. God Bless him, he thought I was the Infectious Disease Specialist, not realizing that he had decided to practice in a town with no such specialty physician at that time! Louie continued to 6

work as a nurse, and was soon even busier with 3 growing boys.

He has served on multiple committees at both hospitals, and is or has been on the Medical Executive committee at both. He is the guy that asks the crucial question to help get to the crux of whatever the issue du jour is. He has served Capital Medical Society in many roles, currently chairing the Access to Care Committee.

Avon has been teaching residents and med students ever since he has been in town. He is clear, and humble, always concerned that he is being too basic, that they already know all of this stuff.

He has served the Family Medicine residency by teaching, and by offering his wisdom to the Graduate Medical Education Committee for years.

He has helped improve my teaching ability, a few words after witnessing a teaching moment on the wards, or simply observing how he gets to the bottom of what a resident does or does not know about a subject.

Dr. Doll has apparently developed the ability to live 30 hours in a 24 hour span, while still taking an occasional Sunday afternoon nap. There is no other way to explain the breadth of activity and accomplishment of this Renaissance man in our midst. With a gift for music, he learned to play the piano at the age of 7, and has been continuously using this skill ever since. As in the field of medicine, thousands of hours of hard work have added to the gift to produce excellence.

His academic life has always shared space with music, from Junior High through med school and beyond. He has never lived in a home without a piano, sometimes the piano being the only piece of living room furniture he and Louie owned. In turn, he has shared his talent for music generously, playing in every church he has ever attended, accompanying 8 years of Lincoln High students as they went to band contest, providing music for weddings and funerals. His interests in music are broad – one minute he will be playing Bach, the next Gershwin, then a hymn, next a show tune, then some very complex jazz number in a weird key with an unheard of time signature. Recently he has been heard practicing princess songs from Disney in anticipation of playing them for his grand-daughter Finley Rose. He enjoys all sorts of musical performance, and has actively encouraged music students through the University Musical Associates at FSU for almost 30 years, serving on the Board for 8. He and Louie were also very active in the Band Boosters at Lincoln during the years their kids were there.

As a father, Avon gets high marks from his 3 boys, Jarrod, Logan, and Colton, as well as his wife Louie. Colton wonders at how a guy with a busy medical practice managed to make it to every event of any importance for 3 sons who were themselves busy with academics, athletics, music, and church and community involvement. Supportive as a dad, able to communicate his high standards while providing a safe place for the boys to express their thoughts, he has earned the respect of all of his sons. When asked to provide any critique of their dad, the only subject that came up was his apparent difficulty in conquering casual fashion.

Avon is active in his church family, from music to home fellowship groups to youth events to mission trips. Most of the latter involve medical service in some distant and difficult to access location. Not limited in his usefulness to medicine, he enjoys doing construction or other types of service as well.

We have gone on a bunch of trips together, and that is how I know Avon is not merely a nephrologist, he is an everythingologist. From a septic HIV patient to an infant with scabies and everything in

between, he is working as hard as he can and as long as he needs to, and is able to communicate that he cares across language and culture barriers.

Long bus rides on mission trips is where I learned that there is no subject off limits for discussion with Avon. From eschatology to scatology, culture, science, and theology, everything is fair game.

Should you ever need someone to discuss issues of faith, including the thorny questions, you will find Avon to be discreet, gracious, accepting, able to say he does not know, challenging, well read, with no preachiness. Of course, in all of these activities, Avon is joined by Louie, perhaps his very best gift to medical missions in my humble opinion.

And then there’s the running. And running, and running. Running since college, he will complete his 17th and final marathon this April. While he may not be gunning for the long distances forever, he will always be running.

Avon and Louie are quite a pair – both interested in lots of things, and outwardly focused on the needs of others. He and Louie particularly enjoy travelling together, hiking. I have long legs but find keeping up with Avon to be a great challenge, even just walking, so my hat is off to Louie scurrying along at great speed with him and often the boys as well.

Avon is generous with everything he has - time, talent, wisdom, resources. His example in this would be a good one to follow for all of us. He is well deserving of the Harrison Humanitarian Award!

As I have shared with you, Dr. Doll has been supportive of music education at Lincoln High School and FSU College of Music. As a token of their appreciation to him, FSU College of Music is represented here tonight and have provided us beautiful music this evening. They have a special performance in honor of Dr. Doll. They heard that he enjoys the music of George Gershwin, and will perform his Three Preludes, composed in 1926.

DR. H. AVON DOLL, JR.’S ACCEPTANCE SPEECH UPON RECEIVING THE I.B. HARRISON, M.D. HUMANITARIAN AWARD

Thanks to God the Father, our creator and from whom comes all things good and true, To his son, Jesus Christ, our brother, savior, and king, whose teachings in the Beatitudes demonstrate how to be truly humanitarian, And to the Holy Spirit, our comforter and guide through all of life. I am utterly dependent on God’s love and grace.

To the Capital Medical Society, Capital Health Plan, the selection and steering committees and all of you, I am humbly appreciative of this recognition, although I remain unconvinced that I deserve to have my name added to the list of illustrative previous recipients. I may be the first recipient that did not know or work with Dr. Harrison, and therefore appreciate the reminder from Orson Smith and others about what he exemplified in our medical community. The cards and comments I’ve received from many of you, especially my friends’ thoughts posted on the slide show are all uplifting and meaningful, if not a bit exaggerated. And Shannon Boyle has gone out of her way to make all of this so special. Thank-you to all.

Thanks, Lisa, for the introduction. It was 10 years ago that you received this award, but you continue to practice, teach, and give of your many talents in a way that will put you in consideration to win it again. Lisa is my friend and colleague, and the one responsible for my participation in off shore

medical missions. As a role model, she continues to positively impact so many young doctors, and to teach them how to be humanitarian. I guess you may understand how I feel about her when I report that she is my family’s physician. And we appreciate Doug, who has loved her well, and polished some of the edges of this gem.

What an honor to share the evening with Michael Forsthoefel, a person I greatly respect for his dedication to excellent patient care, and one of the few primary care docs to still insist on rounding at the hospital. It is a pleasure to share patients with him, and he understands the value of good communication. Specialists commonly hear from patients about what is wrong with their PCPs, but I hear only praise and respect for Michael. I don’t mind at all winning the “Miss Congeniality” award tonight to his “Miss America!”

I’m thankful for Drs. Applegate, Hansen, my former professor Dr. Peterson, my girlfriend Dr. Gaboury, and Drs. Kolli and Patel for establishing and continuing a practice where lifestyle matters.

By lifestyle, I don’t mean how little work we have to do, but how much time is made available for extracurricular activities and trips. This in addition to being the sole providers of excellent nephrology care to our region. I understand that for many of you, time off for a medical mission trip would mean a large financial and workload sacrifice, but for us at NFNA, we come back to work knowing that all matters were adeptly addressed.

compassionate care. I’m proud that we can have a live human answering the phone, and that we have nurses rapidly responding to patients with in-depth advice that could inform even a nephrology fellow.

Their names are Diane, Sharon, Laura, Pamela, Debbie, Lynette, and Chad.

We have some friends at our table tonight that mean the world to me, and I regard them as spiritual family. They are Dr. Michael and Christy Neal, and Dr. Paul and Vicki Harman. Paul is one of the founders and board members for “A Doctor’s Heart.” All of them have powerful spheres of influence and are humanitarian in their own right.

I never want to take for granted the importance of parents’ support. My blue-collar father found a way to financially help us out with medical school, and my nursing supervisor mother directed me towards medicine. She was old school- proud and firm- and I’ll always remember polishing white nursing shoes for her as she ironed and starched her nursing hat. She taught me that medicine is a life-long profession, as even into her 80s, she was the go to person for family and friends for medical advice and therapies. She lived her life as a “giver.”

Two of my three sons couldn’t make it tonight, for good reasons, including one awaiting the arrival of Zachary any day. My oldest son, Jarrod, and his beautiful wife, Rachel, slipped away from pressing work responsibilities to honor me by coming, and for this, I’m grateful. While none of my sons chose human medicine as a career, I am thankful that all three are wonderful persons. All three love and fear God and have a sensitive compassion for others, especially for those in need. What more could a parent desire?

Finally, I will always be amazed that Louie Leonard came into my life 40 years ago. We celebrate 35 years of marriage on Monday, but I’m on call, so, sweetheart, enjoy your night out.

She has influenced most every decision, including the one to pursue nephrology. As an ICU nurse, she would come home saying, “You, know, the nephrologists are the smartest doctors.” (Obviously, that was about to change.) She worked the 6PM to 6AM in CCU to put me through medical school.

up at UF. She taught me clinical medicine before I was supposed to know much, but mostly, taught me a humanitarian approach to patients and nurses, as only an ICU nurse can. After musing that my mission trips may just be extended vacations, she finally started doing them on her own. Now, I won’t leave her behind- unless it involves sleeping in a hammock. We are fortunate that so many of our interests coincide, and I’m thrilled that she is as comfortable in a 2 man primitive camp tent as she is at the Ritz Carlton. Whatever may be good in me is because of her loving input, and she has helped me many times when I have been less than humanitarian in my response, like after that 2 AM call for pulmonary edema.

Please remember that we have the best jobs in the world, with the most noble of callings; that is, the sacred trust of helping vulnerable people through difficult challenges. I truly believe that somewhat overworked sentiment. At the point when a person’s narrative is reduced to a click of a box, or her need for touch is replaced by a two dimensional screen, we are diminished. When the discussion about amputation concerns doesn’t take into account that he was a marine-turned-truck driver who coached city softball for 20 years, or when we don’t care that a stroke victim was a respected middle school teacher that loved the piano, our profession is diminished. The questions that clarify the narrative draw us closer to the person, allowing compassion and empathy to bathe any therapy we may have to offer. It’s why effective doctors are effective, and have been long before the scientific method was understood. Neither time restraints, nor financial pressures, nor computer keyboards can solely influence our connection with and compassion for the patient sitting in front of us. And I understand that these and other distractions are increasingly real for young physicians.

These relationships are what get me up in the morning, long after the stimulation of scientific innovations and successes have waned-after the frustrations of data entry and insurance forms have peaked. And I know that is true for many of you as well. By the grace of God, may we continue to focus on providing humanitarian health care.

Each year, the ACR Board of Chancellors awards the ACR’s highest honor, the ACR Gold Medal to an individual for distinguished and extraordinary service to the American College of Radiology or to the discipline of radiology.

THE NAVY AND ME, FORTY YEARS LATER

By Paul Sawyer, M.D. Interim Editor’s Note: For generations the military provided young men a way to mature while serving their country. Dr. Paul Sawyer recalls the forces that were influencing society while he was in medical training and his service as a ship’s medical officer in the U.S. Navy. The travails of a newly married young couple are remembered as well.

I loved my Dad. My Dad was a physician. My Dad was in the Navy in World War II. As you might surmise, I tried to emulate my father, and it made sense to me to join the U.S. Navy as a future Lieutenant in the medical corps in the summer of 1973. I wanted a way to pay for the last of my medical education. The Navy offered a two year Health Professions Scholarship which would help to pay my medical school expenses in return for a two year active duty commitment.

I was lucky. Active combat in Vietnam ceased in 1973, and I no longer faced going there when I graduated in 1975 from the University of North Carolina.

Active duty would start after a surgical internship at the University of Florida. This move was fortunate since it was there I met my life partner, Jonette, and received excellent surgical training.

Following my internship I was to see the real implications of joining the Navy to help keep my fellow countrymen “safe from the communists.” No longer would I be under the umbrella proffered by older, more experienced clinicians or residents. I would now be on the front lines assuming responsibilities for those shipmates whose care was entrusted to me. At this time I was also convincing Jonette to 16

marry me and then follow me to San Diego where my future ship was based. Happily, she agreed to marry me before I reported for duty, but as she would later point out, I had not thought out the consequences nor explained to her the implications of my decision vis a vis the Navy.

One of the great results of being in the military is the opportunity to learn how to make good decisions.

Having spent the previous eight years in the bosoms of Davidson College and UNC Chapel Hill, I was not in touch with the “real world.” Nevertheless, after our marriage, we took the first steps to active duty with a stop in Norfolk Naval Base for two weeks of intensive officer’s training school.

Jonette watched me as I lost my long hair in the officers’ barber shop getting a “regulation” haircut.

She later told me that was where she began to realize I was not as smart as she had thought. While in Norfolk I learned how to salute and, more importantly, who I had to salute first and who had to salute me. The Navy did not try to teach the medical officers how to march!

After training we set out for San Diego which was to be our new home as newlyweds and a newly minted naval medical officer.

It was, for two young people, a beautiful city with beautiful weather, beautiful people, beautiful vistas, and of course, beautiful military bases everywhere. Our first stop was at the Navy Detailer’s office. This is the officer in charge of personnel assignments, and he proved to be an older guy with Elvis Presley sideburns and a pearlhandled cigarette holder which he calmly puffed while telling me I would be leaving in six weeks for WESTPAC (Western Pacific).

I had previously thought that I would be away from my bride for perhaps two or three months, but suddenly the geography became apparent to us. The Pacific Ocean is a “big pond,” and it would take three weeks for our little task force to cross it each way. The detailer allowed that it would not be cost effective to come back after a cruise of only a month or so, and though Jonette allowed that she had always wanted to live in a large city, what she did not envision was being married and then left on her own for nine months…2700 miles from her old home and family.

My ship, the USS Dubuque (LPD 8), steamed off into the dense fog covering Southern California during the first days of September 1976. Over the next nine months I learned about the Navy, life, and medicine. I bonded with the men

[no women on board in those days!], learned how to function as a team, and recognized how each member of the crew was critical to the mission at hand. In Hawaii I paused to take in the sacrifices of those who had died at Pearl Harbor at the haunting memorial of the USS Arizona. Similarly, in the Philippines, I spent time on the Bataan Peninsula and Corregidor, again aware of the sacrifices of so many. In Japan I marveled at the close relationship between our two countries despite the Japanese defeat and the destruction brought about by the atomic bombs. In Korea I came ashore at Inchon, a place full of difficult memories surrounding the Korean War. I spent time at the Demilitarized Zone a scant few weeks after the North Koreans had cruelly beheaded one of our soldiers in the neutral zone between two nations who are still at war.

I spoke with Jonette only once during the nine months I was gone, and that call cost eighty dollars (approximately $333.00 in today’s money). We communicated with daily letters which chronicled our lives apart that year. The time after our Christmas reunion was incredibly difficult since we had not thought about how we would feel after being together in Yokohama. The deployment finally drew to a close in late May 1977. My ship was met in the harbor with a landing craft that carried our wives and dependents who came aboard to greet us. As I looked down from our deck high above the landing craft, I can still clearly see my wife in her bright blue trousers and white, blue, red, and yellow plaid shirt…she was at that moment a truly beautiful sight!

(She remains so to this day!)

We spent long, boring hours steaming the South China Sea, loading and off-loading Marines who are part of our country’s rapid deployment forces. As the ship’s medical officer, I treated venereal disease and was responsible for insect control on board as well as the usual mundane aches and pains. I was a counsellor for the homesick and the depressed. We spent time in military exercises with the South Koreans planning for responses to provocations from the North Koreans. In Okinawa I listened to the U.S. election returns the night Jimmy Carter was elected President in 1976.

The officers shared Christmas in Yokohama with our wives who had arranged a group trip to see us. It was moving to share our first married Christmas with the other officers who had become close friends as well as professional

There I took care of active service personnel and their dependents.

Jonette and I had time to reacquaint and to prepare for a return to civilian life, residencies, children, and careers.

So looking back what did I garner from my time in the Navy? I helped my family by taking care of my own financial needs for school and living expenses. I realized that my wife was asked to make many more sacrifices than either of us anticipated when we got married and moved across the country. Our experience in the Navy reflected the loneliness and sacrifices that current members of our armed forces make for each of

us daily. I realized that the world, although vast in size, is quite small and that our country’s reach and power to influence events all over the globe is far beyond our daily perceptions. I learned the benefits of working together as a team, and the necessity to put personal aspirations aside, and to enjoy one’s fellow men.

In short, the Navy provided an opportunity to mature and to place in perspective what is really important to our personal growth.

When I returned to residency, I tried to incorporate what I had learned and experienced during my time of service. Change is never easy, but I found myself grateful for the exposure, responsibilities, and opportunities to work with lots of different people. I feel that my journey left me a better person and a more caring, less selfish, and more responsive physician.

My time in the Navy left me wondering if our country would be better off if we all had to share in some form of governmental service, military or otherwise. I am not at all sure that an all volunteer military force facilitates thoughtful dialogue about our national and international commitments. I am certain that some of our policies might be altered if all of our sons and daughters had the possibility of being placed in harm’s way.

At present, this burden is gladly handed over to the one percent who fight for us all over the world.

Despite those concerns, I am glad that I had the chance to serve, not as a patriotic zealot, but rather because my experiences helped me to appreciate all that we have to be grateful for in this country. My military service helped to prepare me to be a better husband, father, and physician, and hopefully a better member of society. CMS

A MAMA’S LOVE

By Charles D. “Pedro” Williams, M.D. Mother’s Day is the day we set aside to honor those who brought us into this world and cared for us.

“Pedro, you’ve outgrown my lap but you’ll never outgrow my love.”

If your Mama is still alive, go see her. Tell her she is appreciated. I wish I could.

Some of the best memories I have of Mama were at the supper table. The supper table was the center for teaching values, expressing feelings, sharing, and hearing Mama’s sayings. I think we’d all be better off spending more time with our own children at the supper table, even if you want to call it dinner.

So often we don’t appreciate the best moments of our lives and the most important people until it’s too late. Life is about change, and today’s joys may be tomorrow’s memories, but on Mother’s Day I’m at least thankful for the memories. We need to pay more attention to the treasures in our lives while we still have them. Mama once said to Pedro, “Enjoy the little things in your life, for one day you may look back and realize they were the big things.”

Mama had about as little as anybody I’ve ever known but I never heard her complain about anything she didn’t have. My Mama’s dresses were handmade and she was glad to have them. She frequently reminded Pedro not to make fun of other people’s clothes. She said that whatever a person wore was probably the best they had.

My Mama lived through a depression and a world war. She ate buttermilk and cornbread cooked at home in the same old black frying pan. She placed a value on truth and honor. She wanted a brighter future for her son and she delivered.

My Mama was a praying woman and I knew this ‘cause she walked through the storms of life with her head held up to the heavens. One time she was looking up and I heard her saying, “O Lord, someway, somehow help my son git the schooling I never got, which he needs to help him on in life.”

She listened to my prayers at night and she woke me in the mornings. She listened to my concerns and gave me encouragement. Pedro did not feel very smart until Mama kept telling him he was and then he was.

My Mama loved me enough to teach me right from wrong. She loved me enough not to give me ever’thang I wanted. She gave me more hugs than switches but enough of the latter to keep me in line.

She made sure I knew the importance of education and that gift has taken me places I never dreamed of.

Most of all she gave me love. One time, Mama said, 18

Pedro learned a lot from Mama at the supper table.

First, I learned I had a place in the family and that I belonged. I also learned to share and had to pay attention to the number of folks at the table so I could take the proper portions. I was grown before I realized the chicken neck wasn’t really Mama’s favorite piece of chicken.

Mama watched our manners and I cudn’t fill my plate until ever’body was seated and the blessing had been said. When you wanted something passed you said “please” and then “thank you.”

One time, Mama asked Pedro if he wanted some beans. Pedro replied, “No.” Mama said, “No what?”

Pedro replied, “No beans.” Mama then said that she wanted us to say “no ma’m,” “yes ma’m,” “no, thank you,” and “please.” She didn’t want us young’uns to grow up with people thanking we were Yankees.

Mama’s sayings at the supper table still ring in my ears even on this Mother’s Day. “Being nice don’t cost a thang and is worth a fortune.” “‘Please’ and ‘thank you’ will take you further than a Cadillac, for a lot less money.” One time she said to Pedro, “I know you licked the plate clean but I still have to wash it.”

Of all of Mama’s sayings, the one I remember the most and that meant the most was “I love you, son.”

This probably will always be the favorite saying of all Mamas.

On this Mother’s Day, I wish with all my heart I could once again go back to the old homeplace, see Mama and say, “I love you too, Mom.” Happy Mother’s Day. Reprinted with author permission from More Simpler Times, Tales of a Southern Boy.

By John Fogarty, M.D., Dean, FSU College of Medicine The Florida State University College of Medicine is in the process of establishing a new physician assistant program. Our founding director, James Zedaker, is currently working with the College of Medicine leadership team to prepare the program for a site visit this December by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).

Developing a program is a challenging process that includes, but is not limited to, curriculum design and approval, course development, identifying clinical faculty who will precept the students during the clinical phase of their training, and identifying teaching faculty and staff to prepare for the arrival of students in the fall of 2017. Current faculty include Ph.D. science professors from the Department of Biomedical Sciences, as well as physicians and physician assistants from other departments within the College of Medicine.

The PA program is an intensive, 27-month, seven-semester program consisting of 111 credit hours of academic content. Students will spend the first 15 months on the College of Medicine’s central campus in Tallahassee.

During an additional training phase, students will spend 12 months throughout Florida working with physicians and PAs in various clinical settings to include emergency medicine, family medicine, general surgery, geriatric medicine, internal medicine, pediatrics, psychiatry, women's health and one elective clerkship.

The PA program will begin accepting applications for its first class July 1 (with a deadline of Oct. 1).

THE THREE OPTIONS IN A RANSOMWARE ATTACK: RESTORE IF POSSIBLE, PAY, OR LOSE PATIENT INFORMATION

By Craig Musgrave, Senior Vice President, Information Technology, The Doctors Company The news made national headlines: Hollywood Presbyterian Medical Center’s computer systems were down for more than a week1 as the Southern California hospital became yet another victim of ransomware—an attack where a business or individual’s computer system is held hostage by cybercriminals until a ransom is paid. Hollywood Presbyterian Medical Center ended up paying $17,000 to restore its systems and administrative functions.

“The quickest and most efficient way to restore our systems and administrative functions was to pay the ransom and obtain the decryption key,” said Allen Stefanek, president and CEO of the medical center. “In the best interest of restoring normal operations, we did this.”

No healthcare provider wants to be in Mr. Stefanek’s position. Once ransomware is in your medical practice or hospital system, there are only three basic options: 1. If you have performed frequent backups, restore your system.

2. If you have not performed frequent backups, pay the ransom.

3. Put your system back to the default setting and lose everything.

If before the attack you’ve performed incremental backups, you can restore the areas affected, with minimal data loss (for example, an hour). If you have point-in-time backups, you can restore with increased data loss (for example, a week). If you have no reliable backups, you can reset the technology back to its “out-of-box,” or default,

state and lose all the data, if no paper records exist. The only other option would be to pay the ransom.

The key to handling any type of attack is to stop the spread once it’s identified. For example, Ottawa Hospital in Canada took the right steps when four of its 9,800 computers were hit by ransomware.2 The hospital was able to find the virus, isolate it before it spread, and wipe the drives clean on the infected computers. The hospital was able to prevent loss of any patient information and avoid paying any ransom because it had saved critical data on servers instead of desktop computers.

Besides loss of business, inconvenience to patients, and damage to reputation, a ransomware attack also poses liability risks. The possibility of adverse events and subsequent claims for professional negligence increases when computerized systems necessary for various functions such as CT scans, documentation, lab work, and pharmacy needs are offline. If hospital systems are down for any significant period of time, certain patients should be transported to other hospitals.

Adverse events can occur when healthcare workers do not have access to EHR systems.

However, if this type of case was litigated, the patient would have to prove that something in the records may have had a bearing on the treatment being provided. In the case of emergency care, the claimant would have to successfully argue that the staff should not have undertaken the care until the medical records could be accessed.

Another risk involves theft of patient records during the attack. If patients’ personal information such as social security numbers and addresses are stolen, the physician practice or healthcare facility may be subject to claims for damages due to identity theft. If a HIPAA violation occurs because patients’ healthcare information is compromised, the practice or healthcare facility would face an investigation by the federal government and could face fines.

Much of the decision to pay or not to pay the ransom is based on the circumstances surrounding the attack, the extent to which all or part of the systems have been compromised, and the degree to which recovery or restoration of the system can be achieved. Any decision must be viewed in light of all of the information and made on a case-by-case basis.

Hospitals, medical practices, and businesses should take full precautions to prevent a hack that results in ransomware being installed. Prevention strategies include:

• Provide security awareness for all employees.

Over 80 percent of attacks are made possible by human error or human involvement. Train staff members to avoid downloading, clicking on links, or running unknown USB on computer systems.

• Block the malware at the firewall, by using intelligent firewalls to stop the malware from downloading.

• Install intrusion detection software to monitor illegal activities on computer networks.

• Stop the malware from executing on desktop computers by installing application whitelisting software, anti-virus, or anti malware.

• Perform regular system backups.

- Ensure that critical systems and business data are backed up—even backed up hourly for critical systems.

- Test that the backup restore process works.

• Avoid relying solely on encryption.

Encryption does not protect a business from a ransomware attack. If a cybercriminal has your login, encryption doesn’t do anything to stop the hacker.

• Perform penetration testing on a regular basis to determine any existing vulnerabilities that should be patched.

It feels like a homecoming, and I’m excited to work with our amazing team.”

Most recently, Rosemary served as the Financial Compliance Manager for the Leon County Office of Human Services and Community Partnerships, where she provided oversight of grants and contracts for community programs, such as the We Care Network.

Rosemary was born and raised in Tallahassee. She graduated from Florida State University with a Bachelor’s Degree in Social Work. She is currently pursuing her Master’s Degree in Social Work with a concentration in Social Leadership at Florida State University. She is married to her husband of 12 years, Tim. They have an 11-year-old son, JP and a 9-year-old daughter, Bella.

Rosemary enjoys spending time with her family, reading, cooking, and being in the mountains.

The Impact of Private Support: Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.”

• Access to healthcare is increased through gifts to We Care Network and the We Care Network Endowment Fund. $72 million in donated specialty medical and dental care has been provided to low income, uninsured patients since the program was created in 1992. Gifts support the salaries and benefits of the case management team who coordinate donated care.

In 2014, CMSF was able to repave its parking lot after 17 years, install a front door security and intercom system to improve the safety of staff, and purchase a new computer server. $11,000 in grants were awarded for Big Bend Hospice’s Bereavement Services Program, Elder Care’s In-Home Services Program, and FSU College of Medicine’s Chapman Community Health Program were awarded in 2015.
