HIV is still here Diagnoses are increasing, while funding is decreasing We’re less than five years away from the 2030 target of eradicating HIV/AIDS as a public health threat – and at the rate we’re going, it’s likely Canada will miss this threshold; unless something bold is done, and fast

For nearly four decades we’ve invested in groundbreaking Canadian research that has changed HIV science and led to incredible discoveries that have changed the prognosis for people living with HIV.

We helped develop highly-active antiretroviral therapy in the mid-90s, funded studies that pushed PrEP through Health Canada approval in the 2010s, and even helped get HIV self-test kits approved for public use in 2020.

Over the past several year’s, we’ve sought to enhance our impact and to invest in underfunded areas that are critical to ending this epidemic, whose first case in Canada was discovered in March 1982

This year marked a significant shift for CANFAR with the introduction of our Strategic Implementation Fund, the outcome of our National Strategic Task Force that was convened throughout 2023 and 2024

Implementation research is an evolving field that explores how realworld evidence can inform and improve the adoption of evidence-based practices, interventions, and policies particularly within healthcare and public health settings.

For CANFAR, this means working with community partners across the country to get HIV testing to those who need it most through the adoption of GeneXpert testing instruments These rapid and mobile testing machines can be deployed nimbly and remove the burden of access for under-resourced populations

It also means installing more Our Healthboxes In 2025, CANFAR funded four machines Our Healthboxes are free, accessible 24 hours a day, 7 days a week, and distribute HIV self-test kits . and other sexual health supplies anonymously.

The benefits of these programs cannot be overstated: with GeneXpert testing, we can screen and diagnose new cases of HIV in 90 minutes, and subsequently connect people to antiretroviral medications within less than 24 hours. With HIV self-test kits, a screening diagnosis can be had in less than 2 minutes; and follow-up care is just a phone call away

We have the tools today to end the HIV epidemic in Canada. But the resources aren’t where we need them to be. CANFAR is following the lead of the World Health Organization and helping Canada decentralize HIV testing so we can bring these services directly into the communities that need them most

As we ramp up our investments in implementation science, we hope to have an even greater impact on our domestic epidemic by filling gaps in health inequities for priority populations Alongside continuing to fund Canada’s brilliant researchers as they search for a cure, and increasing national awareness for key demographics, CANFAR is acting swiftly and boldly in coordination with our community partners to provide innovative solutions that reach people in culturallysensitive safe spaces so they can access the healthcare services they need without stigma or discrimination.

WE’VE INVESTED OVER $26 MILLION AND AWARDED OVER 575 GRANTS TO HIV/AIDS RESEARCH PROJECTS THAT HAVE LED TO TREMENDOUS BREAKTHROUGHS IN HIV PREVENTION, TESTING, TREATMENT, STIGMA, AND THE SEARCH FOR A CURE. HIV & SEXUAL DYSFUNCTION AMONG GAY, BI & OTHER MEN WHO HAVE SEX WITH MEN

Sexual dysfunctions are more common among men living with HIV than men living without HIV. In current research on sexual dysfunction, only expert voices are often heard. Dr. Lebouché and team know we need to hear directly from patients.

Dr. Lebouché, PhD student Francesco Avallone, and the research team aim to conduct 30 semi-structured interviews with gbMSM living with HIV in Montreal who have experienced sexual dysfunction This project will recruit participants through HIV clinics, community groups, magazines, and online platforms

Dr Lebouché and Mr Avallone will gather recommendations from participants to create a patient-centered model of sexual dysfunction care

DISCOVERING WHY THE IMMUNE SYSTEM CAN’T GET RID OF HIV?

HIV stays in some of our immune system cells even with antiretroviral therapy, making it nearly impossible to eliminate completely from the body. Dr. Ostrowski and his team posit the body could get rid of HIV reservoirs with our immune ‘killer cells’. But there’s a catch: Killer cells in people living with HIV and on antiretroviral treatment are often not as effective as killer cells found in people not living with the virus This phenomena is called ‘T Cell exhaustion’

Dr Ostrowski and his team will analyze cells in a 3D culture system that mimics the lymph nodes in the body and track the performance of killer cells If we can get to the heart of why these cells are exhausted, we can try to improve killer cell functionality for better immune responses

HIV & MENOPAUSE: ADDRESSING THE GENDER GAP IN IMMUNE RESEARCH PRINCIPAL INVESTIGATOR DR. JULIE LAJOIE

The ‘gender data gap’ refers to the lack of adequate, accurate, and comprehensive data on women and gender non-conforming folks, leading to underrepresentation and skewed perspectives in data collection, analysis, and decision-making.

Women experience hormonal changes for most of their lives – but especially in menopause. Menopause is a natural process that happens to women as they age This study aims to understand how aging as a woman living with HIV impacts the immune response Our immune system may be less responsive or more tired, which can increase inflammation This study will determine if menopause changes how T cells are activated and respond to viral infections Changes in their activation profile may increase susceptibility to HIV

A SILENT EPIDEMIC: LONELINESS, SEXUAL HEALTH & SUBSTANCE USE

Black sexual minority men are often sidelined in HIV prevention research.

Dr. Skakoon-Sparling and co-lead Garfield Durrant want to ensure their research takes a participatory action approach, which will allow them to work closely with community members to determine what resources, interventions, and supports are needed for providing culturally safe care. This research project will be conducted through an online survey with 200 Black immigrant gay, bisexual, and other men who have sex with men The results of this survey will help the research team to use community input in highlighting and exploring gaps in sexual health prevention for African, Caribbean and Black newcomers in Canada to inform systemic changes and improve sexual health outcomes

WHAT ARE HIV CLUSTERS & HOW CAN THEY INFORM PREVENTION STRATEGIES?

By examining patterns in HIV genetic data, we can better understand how the virus moves through populations– which can help inform better and more targeted prevention and testing strategies.

Dr. Brenner and her team believe that studying the genetic makeup of HIV can give us better insight into how HIV spreads, beyond traditional methods.

This approach combines genetic analysis with patient demographics and behaviours to identify factors driving the spread of HIV among MSM and racialized communities in Quebec

Phylogenetic network analyses will map the growth trajectories of HIV subtype epidemics that are occur across Quebec

DISCOVERING WHY THE IMMUNE SYSTEM CAN’T GET RID OF HIV?

DR. AMY GILLGRASS

Dr. Gillgrass and her research team are currently investigating improving the Shock and Kill approach for HIV cure using a technique that has been successfully used in cancer treatment. This approach combines our bodies’ special natural killer (NK) cells, with drugs that can reanimate dormant HIV. First, drugs called latencyreversing agents are used to reactivate latent HIV hiding in immune cells (the ‘shock’) Then, the reactivated cells can be targeted and killed by the body's immune system or anti-HIV drugs (‘the kill’) This study aims to gather evidence about the potential use of a novel kill agent in the HIV 'Shock and Kill' cure method, and based on its results, will hopefully allow Dr Gillgrass and other researchers to expand the scope of applying this method to finding a cure for HIV

UNLOCKING INNOVATIVE SOLUTIONS TO HIV’S IMMUNE EVASION

PRINCIPAL INVESTIGATOR

HIV-1's capsid is a protective shell that helps the virus avoid being detected by the immune system. Dr. Mouland and team will test if the HIV-1 capsid avoids detection by the immune system’s cGAS sensor – a protein that detects foreign DNA and triggers an immune response.

The team will examine whether HIV-1 infection suppresses cGAS activation using cell-based tests They will then study how HIV-1’s protective shell, the immune system, and viral genetic material (cDNA) interact after infection Using live-cell imaging techniques, the team can confirm these interactions This could help researchers understand how to enhance immune responses in people living with HIV and lead to making HIV more recognizable by the immune system

The CAHR-CANFAR Excellence in Research Awards are bestowed annually to mid-career researchers who, while at the prime of their careers, have achieved a track record of excellence and demonstrated a commitment to improving the lives of those living with HIV or at risk of acquiring the virus.

Awarded since 2017, the recipients are honoured each year at the opening ceremony of the CAHR Conference (this past year from May 1 to 4, 2025 in Halifax, Nova Scotia) to highlight the achievements of investigators in all pillars of HIV research

THE 2025 CTN+-CANFAR POSTDOCTORAL FELLOWSHIP AWARD WILL BE ANNOUNCED IN NOVEMBER 2025.

POSTDOC AWARD

The CTN+ Postdoctoral Fellowship Awards Program was established to empower earlycareer researchers, providing them with invaluable opportunities to expand their knowledge, and craft their own research programs and trials. The fellowships provide career support and are renewable

for up to two years. The CANFAR/CTN Postdoctoral Fellowship Award offers $70,000 per fellow, with an additional travel allowance of up to $5,000 to support research-related expenses. Calls for applications were open through February 2025.

STRATEGICIMPLEMENTATIONFUND

CANFAR’s strategic implementation program was developed from our Bold Actions national strategic plan, which aims to increase access to HIV testing and care, and fund implementation research to end the epidemic This program aims to support community-based models and programs that improve healthcare accessibility for priority populations.

Implementation research is an evolving field that explores how real-world evidence can inform and improve the adoption of evidence-based practices, interventions, and policies particularly within healthcare and public health settings. Its primary goal is to enhance the uptake of research findings by identifying and addressing the barriers and facilitators that affect implementation.

By developing effective strategies to support the integration of proven approaches into routine practice, implementation research helps bridge the

gap between research and real-world applications Ultimately, this leads to improved health outcomes and greater well-being at both individual and population levels

To date, CANFAR has funded the Express Testing Laboratory at HQ Toronto to facilitate same-day STBBI results, and funded four Our Healthboxes, a national initiative in collaboration with REACH NEXUS.

We have also supported Indigenous-centred initiatives at the Dr. Peter Centre and CBRC in Vancouver. The newest implementation program will be the GeneXpert rapid and mobile-testing instruments and cartridges to eligible community-based organizations. The GeneXpert system shifts access to HIV testing and care from the traditional laboratory setting and directs it to where people are; facilitating same-day diagnosis and/or monitoring of HIV

GENEXPERTRAPID &MOBILETESTING

PROJECT PARTNER

The GeneXpert system is a rapid and mobile testing instrument that uses individual cartridges to test for a variety of infectious diseases, including HIV It is a compact, on-demand diagnostic platform that operates as a closed system polymerase chain reaction (PCR) machine – widely regarded as the gold standard in molecular diagnostics. The GeneXpert machine can provide accurate and efficient testing in just 90 minutes or less. This means that community-based organizations can use this system to test, diagnose, and link patients to HIV treatment and care ondemand and on-site.

The GeneXpert system is compact and easily transportable, making it possible to bring testing closer to communities. It shifts access to HIV testing and care from the traditional laboratory setting to meeting people where they are; facilitating sameday diagnosis and/or monitoring of HIV

Benefits of the GeneXpert system include:

Reliable, decentralized testing bringing diagnostics closer to patients and communities that need it most Rapid turnaround times delivering results in 90 min or less (same-day diagnosis), closing the gap between diagnosis and treatment, and reducing the risk of further transmission

Flexible deployment does not require specialized lab staff and can be operated by trained community personnel

Multi-disease testing capability can provide a range of tests on a single platform, which supports broader access to care

Community-driven care allow testing to be delivered by trusted local providers, increasing comfort, trust and participation

Promoting HIV prevention supporting education, harm reduction, and engagement through community-based approaches

CANFAR began the process of working with the Public Health Agency of Canada to obtain 28 GeneXpert machines with the goal of mobilizing these units among key community partner organizations across Canada over the next 18 to 24 months

CANFAR’S FY25 INVESTMENT: $125,000

THE GENEXPERT MACHINE IS ABOUT THE SIZE OF A DESKTOP PRINTER.

OUR FIRST UNITS WERE DEPLOYED TO WOMEN’S HEALTH IN WOMEN’S HANDS IN DOWNTOWN TORONTO, AND WILL BE IMPLEMENTED FALL 2025.

OURHEALTHBOX

OUR HEALTHBOX IS A SMART VENDING MACHINE THAT PROVIDES FREE AND ANONYMOUS ACCESS TO HIV SELF-TEST KITS, SEXUAL HEALTH SUPPLIES, AND SAFE INJECTION EQUIPMENT TO REDUCE HIV TRANSMISSIONS. IT’S A PROJECT LED BY REACH NEXUS AND THE MAP CENTRE FOR URBAN HEALTH SOLUTIONS AT ST. MICHAEL’S HOSPITAL, WITH SUPPORT FROM VARIOUS FUNDERS INCLUDING CANFAR.

Each Our Healthbox machine collects data in real time to ensure we are reaching the people who are the most underserved – and providing them with access to what they need without facing judgement, racism, or stigma. As part of CANFAR’s Bold Actions strategy, Our Healthbox is an implementation research program that addresses the health gaps faced by Indigenous peoples, African, Caribbean and Black people, racialized women, and people who use substances and inject drugs

With this initiative, we are helping to make vital resources available to everyone, including HIV self-test kits, no matter someone's circumstances. Our Healthbox will help normalize conversations around sexual health and reduce stigma in our communities

Our Healthbox falls under implementation science, which is the scientific study of how to best promote the adoption and integration of evidence-based practices, interventions, and policies into routine healthcare and public health settings to improve population health outcomes

COMMUNITY PARTNERS FOR CANFAR’S OUR HEALTHBOXES:

URBAN INDIGENOUS PEOPLE FACE NUMEROUS CHALLENGES IN ACCESSING CULTURALLY RESPONSIVE SERVICES, PARTICULARLY IF THEY ARE NON-STATUS OR DO NOT HAVE BAND MEMBERSHIP, WHICH OFTEN NECESSITATES THEM ACCESSING SERVICES AT NONINDIGENOUS ORGANIZATIONS SUCH AS THE DR. PETER CENTRE. MANY NONINDIGENOUS URBAN FRONTLINE HEALTH PROVIDERS ARE ASKING FOR SUPPORT IN ADAPTING THEIR SERVICES FOR INDIGENOUS PEOPLE SEEKING CARE.

This project builds on the Dr Peter Centre's Indigenous Wellness Program, which currently supports over 150 Indigenous participants-approximately onethird of Day Health clients While not an Indigenous organization, the Centre is one of Vancouver's largest Indigenous-serving agencies and plays a unique role in modelling culturally safe HIV care.

Through this grant, CANFAR has made it possible to explore why the Centre’s culturally safe model works, how it fosters trust in healthcare settings, and how its Balanced Wellness Approach can be documented, evaluated, and adapted by non-Indigenous organizations across Canada contributing to a more integrated and culturally grounded model of care.

The Culture as Medicine initiative grew out of CANFAR’s national task force, where the Dr Peter Centre joined leaders from across the country to co-design a new generation of implementation projects

building national capacity The current project explores why the Centre's culturally safe model works, how it fosters trust in healthcare systems, and how its approachnow formally named the Balanced Wellness Approach-can be documented, evaluated, and adapted by non-Indigenous organizations The grant from CANFAR is supporting:

MEDICINEBUNDLE PROGRAM

The Medicine Bundle Program is an Indigenous-led HIV and STBBI prevention and health promotion project within the Two-Spirit Program at the CommunityBased Research Centre (CBRC) The project was developed by and for Two-Spirit and LGBTQQIA+ Indigenous Peoples, integrating Indigenous knowledge, medicines, and culture in combination with Western biomedical sexual health knowledge and tools, including HIV selftesting kits.

As a community-led, evidence-based, and culturally informed intervention working to address HIV knowledge, access, and support needs among Indigenous and TwoSpirit Peoples and communities across Canada, the Medicine Bundle is closely aligned with CANFAR’s strategic funding priorities

To support this program, CANFAR funded a national series of in-person sharing circles, organized in collaboration with the CBRC’s

five local Indigenous community partner organizations, in Spring & Summer 2025

The overarching goal of this project is to partner with these five local community organizations who will be supported in designing, delivering, and evaluating the impact of the Medicine Bundle project within their local community and/or region. What is learned from these sharing circles, as well as the local adaptation process more generally, will be used to create a guide for communities and organizations in Canada to develop their own communitybased programs and interventions.

IN JULY 2022, CANFAR FUNDED THE EXPRESS TESTING LABORATORY AT HQ TORONTO FOR THREE YEARS – AN INVESTMENT OF $500,000.

HQ Toronto (790 Bay St ) offers tailored sexual health, mental health, substance use, and social support services for gay and bisexual men, trans and AMAB non-binary people.

The lab provides confirmatory tests for Sexually Transmitted and Blood-Borne Infections (STBBIs), including HIV, chlamydia, gonorrhea, and syphillis; with guaranteed same-day (24-hour) results. This innovative technology, which provides accurate, rapid, laboratory-grade test results, means that people who are travelling from outside the city can receive care and be linked to treatment or prevention in the same day

The CANFAR Express Testing Laboratory represents a total cumulative healthcare system savings to-date of $1,099,875 It also fills critical gaps in our healthcare system Since the lab opened, 2,784 individuals who were uninsured or precariously insured (no OHIP or IFH, 16 85% of visitors) accessed care at HQ FROM JULY 2022 UNTIL APRIL 2025,

SEXFLUENT (YOUTHAWARENESS)

Sexfluent is CANFAR’s national youth digital resource dedicated to ending Canada’s HIV epidemic by equipping youth ages 15-25 with the knowledge and tools necessary to prevent HIV Sexfluent is transforming the conversation around sexual health to be free of judgment and shame, so that youth can be empowered to embark on their own journeys of selfacceptance and exploring pleasure.

Over the last year, 6 Sexfluent Squad members, representing diverse youth from the 2SLGBTQIA+, Indigenous, and racialized/ newcomer communities, as well as youth who have experiences with substance use, served as peer educators for Sexfluent’s digital platform, amplifying our health education messages online. Together, they have a combined audience of 2.5 million followers Through these ongoing partnerships, Sexfluent continues to deliver a youth-led and community-driven content strategy that creates strong digital engagement to connect youth to local health resources

IN THE LAST YEAR ALONE, SEXFLUENT HAS:

Reached more than 4,785 at-risk youth through in-person educational engagement across 25 events in 11 cities, 5 provinces and 1 territory. Distributed over 7,090 safer sex supplies (i e condoms, dentals dams, and lubes) and 141 HIV self-test kits, thereby expanding access to HIV prevention and testing

Grown our online audience by 63% and increased our digital engagement by 33%, thereby enhancing access to HIV and sexual health information online.

As of June 2025, Sexfluent reached a combined online audience of 2,526,853 across our primary educational platform, Sexfluent.ca, and our social media channels. Between July 2024 and June 2025, 41,000 new users accessed our website a 9.1% increase from the previous year demonstrating our growing impact in expanding access to HIV knowledge.

SEXFLUENT (YOUTHAWARENESS)

Since launching in 2022, an average of 95.8% of users have rated our Sexual Health pages as helpful, with key pages like Prevention and Harm Reduction holding a 100% positive user rating, demonstrating the ongoing relevance and educational impact of our health resource In FY25, new tools such as a search bar and resource referral links were added to the home page and each subject area landing page, respectively, to enhance user engagement, learning and connections to care.

Between July 2024 and June 2025, Sexfluent received 56,208 organic engagements (i.e. likes, comments, shares, saves) on our educational posts, a 5.1% increase from the year prior.

This demonstrates that our content continues to resonate with youth today Users are taking the time to read through our posts and share our content, highlighting the relevance of our messages

Sexfluent now holds the highest engagement rate among our peer organizations. This establishes Sexfluent as a leader in online youth-driven sexual health education and prevention, further reinforcing the quality and impact of our work.

Demand for Sexfluent’s educational materials grew this year, with Sexfluent developing 16 new educational print resources for educators, community health workers and youth workers across Canada

Between July 2024 and June 2025, CANFAR’s resource centre distributed 7,528 educational print materials across 42 cities in 11 provinces and 4 territories a 3% increase from the previous year

Additionally, in December of 2024, Sexfluent launched 12 new educational postcards in partnership with CATIE

Over the next two years, Sexfluent will focus on investing more in analytics, collecting testimonials, and connecting youth to care.

KNOWLEDG MOBILIZAT

STARTING HOME FIRES

This national Indigenous-focused HIV campaign centres Indigenous ways of knowing and storytelling to promote HIVrelated wellness and self-determination Visioned by an Indigenous leadership committee, which includes Indigenous People Living with HIV/AIDS (IPHA), and supported by CATIE, CANFAR and Gilead Sciences Canada, this national campaign will include a digital social media and radio strategy and feature a curated online collection of relevant community-based HIV resources. These components will ensure Indigenous peoples across Canada have access to reliable and culturally appropriate information to make informed decisions about their HIV-related health, to support their autonomy and selfdetermination in HIV health-related decision-making, and to support equitable access to HIV knowledge and messaging that is meaningful, appropriate and resonates with Indigenous peoples

PROJECT PARTNER

In November of 2024, CATIE & CANFAR held a two-day national stakeholder meeting convening 16 Black HIV leaders from across Canada to discuss how to reenvision Black-led HIV knowledge mobilization to create more meaningful impact in Black communities affected by HIV The session aimed to generate a collective vision for what “Black-led knowledge mobilization” (BLKM) means, and what is required to initiate, sustain, and resource it effectively.

The group agreed that Canada will not reach its 2030 UNAIDS goals if Black communities remain sidelined. Nine principles of BLKM were identified through this process, and seven critical shifts (representing areas of meaningful change) were generated as requirements to achieve a just future

THE RETURN OF THE RED RIBBON CAMPAIGN: ACTING UP BY GOING GRASSROOTS. ONCE AGAIN WITH THE SUPPORT OF GILEAD SCIENCES CANADA, AND IN PARTNERSHIP WITH OVER 76 COMMUNITY PARTNERS, WE WENT BACK TO THE ROOTS OF HIV/AIDS ADVOCACY AND REANIMATED THE RED RIBBON CAMPAIGN. WE BROUGHT BACK THE CANS, THE RIBBONMAKING, AND THE ACTIVISM POSTERS THAT ARE SYNONYMOUS WITH THE RED RIBBON CAMPAIGN OF THE LATE 80S AND EARLY 90S.

The campaign was shared across Canada by 69 community partners We provided a comprehensive toolkit that included physical and digital collateral to make participation seamless Our campaign and its companion activations generated over 15 million media and social media impressions, and included coverage from outlets like CTV, CityNews, CBC News, Healthing, Toronto Star, 680 News, and Newstalk 1010.

The campaign spotlighted the iconic AIDS awareness ribbon and helped raise national awareness that HIV is still here

We broke the first record on Tuesday, November 12 at The 519 Community Centre, with Billy Newton-Davis acting as official judge.

Then on Wednesday, November 13, we unveiled the World’s Largest Awareness Ribbon Mosaic made entirely of awareness ribbons – over 20,000 hand-stapled to a wooden platform in downtown Ottawa.

This is what community action looks like, and beyond the milestone, it's a call for more collective awareness about HIV and AIDS, and its impact on marginalized and vulnerable communities, including 2SLGBTQ+ people

With almost 450 people in the Aria ballroom at Four Seasons Hotel Toronto, the hush of the crowd was palpable as Valerie interviewed Billy Newton-Davis about his four decades living with HIV after his diagnosis in 1986.

Everyone listened intently as Randy Davis (no relation to Billy) recalled his own diagnosis story, and how he found strength, empowerment and community by advocating for other people living with HIV in the ten years since Dr Alice Zhabokritsky highlighted how conditions that often come with aging – hypertension, bone disorders, diabetes, immune system decline, kidney disease – arise more rapidly and more frequently among people aging with HIV. Mike Chalut and the always fabulous Jeanne Beker helped us raise over $30,000 with the live auction.

LEADERSHIP TRANSITION

ON BEHALF OF THE BOARD OF DIRECTORS, WE ARE WRITING TO SHARE IMPORTANT NEWS REGARDING A LEADERSHIP TRANSITION AT CANFAR. AFTER YEARS OF DEDICATED SERVICE, ALEX FILIATRAULT HAS FORMALLY STEPPED DOWN FROM HIS ROLE AS CHIEF EXECUTIVE OFFICER AS OF MAY 9, 2025. WE EXTEND OUR DEEPEST GRATITUDE TO ALEX FOR HIS EXCEPTIONAL LEADERSHIP AND TRANSFORMATIONAL WORK OVER HIS TENURE.
