30 June 2020 Dear Staff member/ parent/guardian, Re:

COVID-19 exposure associated with Camberwell Grammar.

The Department of Health and Human Services (the department) is asking all staff and students who attended Camberwell Grammar since June 1 to undergo testing for coronavirus (COVID-19). Please attend a testing site that is convenient for you as soon as possible. The following link gives information on where to get tested. https://www.dhhs.vic.gov.au/getting-tested-coronavirus-covid-19#where-to-get-tested Please present this letter when you attend for testing and ensure that the hospital reads the information on page 2. You will need to provide your name, date of birth and a mobile phone number so that you can be contacted with the results. The results will be provided to you by the testing provider. If the test comes back positive, you will need to self-isolate. The department will contact you to advise on next steps. Even if you receive a negative result, if you have been identified by the department as a close contact and have not yet completed your 14-day quarantine (isolation) period you must stay at home until 14 days after you were potentially exposed to COVID-19. You will be told by the department when you can leave quarantine. If you have any questions or wish to discuss this information in more detail, please call 1300 651 160.

Yours sincerely

Dr Simon Crouch Deputy Public Health Commander COVID-19 (Case, Contact and Outbreak Management) Department of Health and Human Services
