Northern Westchester Hospital is focused on providing birthing mothers with a vast array of resources, ranging from OB navigators and a prenatal care clinic, to cribs and car seat offering programs. The hospital’s goal is to reduce the percentage of 30-day readmissions in Black birthing people with hypertension from 5.7% to < 1.5% by January 2026.

As a committed community partner, SJRH leads in providing culturally-centered prenatal care. Their upcoming integration of TeamBirth, along with their PREM survey outcomes demonstrate that they are hearing the voices of patients and are continuing to shape responsive care practices for birth equity in the county.

BEST PRACTICE PRESENTATIONS

Westchester Medical Center is a pioneer in birth equity improvement. The hospital is improving and creating more health services, while involving patient education and advocacy to reduce C-section rates. Their OB/GYN team provides consistent updates to patients about birthing options, while actively tracking and improving C-section outcomes with a focus on reducing unnecessary interventions.

WCHC provides direct clinical services to underserved birthing people and connects families to nutrition, dental, housing, lactation and doula programs, low cost medicine, and behavioral health resources. Their care approach is tailored to your unique health concerns, stage of life, and daily challenges.

Healthy Families assigns a home visitor that a family can relate to who will make weekly in-person home visits after a birth. They are currently conducting several workshops and initiatives, while collaborating with community partners to improve birth equity.

service. Their mission is to empower and create strong families that know their bodies, know their choices and know their voice through education, comprehensive training, inclusion and holistic care.

LHVPN improves Maternal & Child Health

outcomes for Black & Hispanic women in Westchester County via doula, breastfeeding/lactation, and at home

visiting services. They collaborate with local hospitals and community partners, and host both in-person and virtual Breastfeeding

Baby Café meetings.

The Westchester County DOH presented on Maternal and Child Health, and launched a public dashboard to visualize local birth outcomes data. The dashboard allows users to

explore outcomes by mother's race, ethnicity, health insurance type, education level, and whether the mother is U.S. -born or foreign-born.
