Submit a letter of credit from an eligible bank or other performance bond in accordance with NTIA guidance contained in the BEAD Letter of Credit Waiver. Submit business plans and related analyses, such as pro forma statements, that substantiate the sustainability of the proposed project.

Submit a project plan that includes a network design, diagram, project costs, build-out timeline and milestones for project implementation, capital investment schedule, and attestation that the proposed network can deliver broadband service to all locations the project area serves.

Compliance with Laws Demonstrate capability of complying with all applicable federal, state, and local laws. Demonstrate compliance to the following legal topics, among others: employment, safety and health, underground plant protection, consumer privacy, cybersecurity, public utilities, and nondiscrimination. Verify that the organization permits worker-led health and safety committees.

Project Funding Areas Select each project funding area you wish to apply for funding to serve. Specify the technology to be used. Submit supporting documentation to demonstrate the validity of the proposed project, including budgets and letters of support.

Proposed projects on Tribal lands must include resolutions of consent from the relevant Tribe.

BIG BEAD Registration and Pre-Application Information 1

Financial Capability Submit a wet-signed letter certifying that the applicant organization is financially qualified, has resources to cover the financial difference between award amount and project costs, and that it will comply with program requirements. Submit audited financial statements from the previous fiscal year and a balance sheet through Sept. 30, 2024. Disclose any public funding received or applied for to deploy broadband.

Technical Capability Submit current licensing documentation, workforce credentials, and attestation of Utah licensing of engineers. Disclose any contractors or subcontractors you plan to engage or a description of your procurement process to identify such support.

Managerial Capability Submit resumes and employment information for key personnel, an organizational chart, a narrative regarding your organization’s readiness to manage a broadband network, and report whether or not your organization has existing risk management policies.

Ownership Informtion Disclose parties with a controlling interest in the applicant organization. Disclose any party holding 10% or more interest in the applicant organization. Disclose the organization’s corporate structure. Submit supporting documentation to verify this information.

Operational Capability Provide information relating to your organization’s services, pricing, and experience. Disclose any penalties incurred relative to broadband deployment projects. Disclose criminal proceedings or civil litigation the applicant has been involved with in Utah.
