In Deep Shift:

The Courage of Facing Transition I was watching a show a few days ago on OWN called “In Deep Shift”. It is about how a challenging occurrence in your life can bring about a shift in consciousness to a higher level. It usually involves having a breakdown then a breakthrough, but it brings new strength to use in your life as a result of facing adversity. These difficult challenges are like obstacle courses that force us to twist and turn in a way that creates a new found sense of resilience and once embraced, can positively affect not only your life, but that of others.

I loved the title as it really does seem to express the feeling we all have when there is a desire within us to make a change, but the process of it initiates some deep inner muck we have to traverse through. Bringing consciousness to ourselves is focusing awareness on what is really going on inside of us, not outside. We stop external striving and make a choice to get off the train and rest at the station for awhile. We take time to linger in quietness and really connect with our own inner self, listening to what we truly need, want and desire. Alas, this is when resistance can often appear.

Resistance is anything that you revert to doing in order to avoid going deeper into yourself. It is natural because most of us are afraid of really being who we are. Resistance can take many forms and we can come up with many interesting reasons why we must get busy again or not take the time right now to just be still.

In actuality, they are all reasons we avoid listening to our intuitive and knowing self. Listening might just take us down a path that seems unfamiliar, out of control or it can even present us with a 360 degree turn from what “we think” is the right answer to ensure our happiness, success, well-being or recovery.

Resilience is an important quality to develop in life, but a revisiting of what it means is probably even more important as the transitions of “advancing maturity” become more apparent in your life. I consider it one of the important ingredients in coming to live life in grace and it is one of the qualities you need to manoeuvre all of the ups and downs of aging. Dr. Eva Selhub is a resilience expert and clinical instructor of medicine at the Harvard Medical School. She says, “Resilience is about the deep knowing that one has the resources to handle the uncertainties that come with life. She thinks that some people are naturally more resilient than others and better able to handle the pressures of change, but that resilience is also something that can be learned. By committing yourself to shoring up physical, mental and emotional resilience, you’ll be better able to face any challenge, big or small, that comes your way.” continued on page 3

You have to take care of yourself first to be able to do this. Many of us, especially when a caregiving role is taken on, tend to forget this principle very quickly, and succumb to the mounting needs of another to the detriment of our own well-being. Recognizing that a propensity for martyrdom is a pattern worth addressing can initiate a type of breakdown where you must lose your chosen identity. A resistive confusion of who you are without this seeming sense of external value can result. You must come to grips with the adage, “Change your outlook, change the outcome.” This takes some real inner connection to the truth within you and a focused commitment to making a shift. And yes, you encounter reluctance to face it. However, the simplicity of it is this: you just have to be willing to be willing and really desire a shift to happen. You begin to sense a physical sensation of something changing. When you make a choice to focus consciousness on yourself, you activate a healing process. If you keep allowing it, consciousness will move its healing presence to the place that really needs it.

It helps to have tools to do this. One tool is called the Insight Meditation. You use your breath to bring mindfulness to this desire to let go of figuring everything out

and practicing how to let go. Sit comfortably in a sitting position and focus on your inhale breath. Breathe in deeply and as you do, imagine the word Rise. Hold your breath, then slowly begin to blow your breath out through gently pursed lips and as you do, imagine the word Fall. This creates a type of breathing mantra that quiets the mind and also takes you into your inner presence, which is where you want to be to truly listen. If your mind starts thinking, as it loves to do, say the word thinking, thinking then return to the gentle inflow and outflow of your breath, imagining Rise, then Fall.

As external circumstances happen or are about to happen in our lives, we get a sense of it. We have a knowing that a transition is coming. We don’t always like it, because on a deep level we understand that we are being called to make a shift in our inner awareness, usually one that is very needed in our lives.

I was recently asked to give a talk at Cottonwoods gerontology unit on the benefits of exercise, programs I offer and the value of community programs.

I wanted to share this information as it relates not only to Cottonwoods but all seniors.

New volunteer organization helps seniors to live independently

Mike Straus, Contributor When Gwenda Tempel was searching for a way to help her elderly mother stay in her home, the right solution wasn’t immediately obvious. s “It’s hard to find the right provider,” she says. “It’s not easy to connect the dots as to what a person needs at different times.”

Tempel was aware of the existing agencies, but finding one organization that could meet all of her mother’s needs was a struggle.

That is, until she found Elder Helpers.

The Elder Helpers of the Central Okanagan is part of a North America-wide network of Elder Helpers organizations that provide the elderly with volunteer services. Elder Helpers connects the elderly to local volunteers through an easy-to-use online database.

Volunteers simply make a profile on the site and post icons of the ser-

vices they wish to provide. Seniors can access this database and search by region or by service. Within minutes, seniors can see a list of local volunteers who will help them get groceries, cook and clean, read to them, do handiwork and gardening, or simply socialize with them.

“The biggest thing I see this organization doing is providing seniors with companionship,” Tempel says.

“My job is to encourage people to live their best, happiest life. I see a lot of sadness in seniors because they’ve lost that. You don’t have to feel that way if you know you can reach out to someone.”

Tempel says there are lots of ways that seniors can benefit from the Elder Helpers program. Families can often get worn out as caregivers, but with volunteer support, a visit with elderly relatives can simply be a visit. In some cases, Tempel sees children living in other provinces using the service to check up on

For many seniors, Tempel says, the service is filling an urgent need and preventing seniors from being forced into assisted living facilities.

“There’s a long waiting period for assisted living facilities. There are some agencies that can provide care, but if there are little things that you need done, it’s not always affordable.”

“I used to tease my mom that when the time changes it would take two volunteers to change her 28 clocks. And sometimes it’s just the little things you need done to keep you in your home.”

Elder Helpers could also be a great tool for organizations, and cross-promotion efforts between agencies could help seniors to better access local resources.

“The Red Cross has lots of

devices they lend out to seniors free of charge, and a large number of volunteers, but people don’t often realize the program exists.” Tempel says. “With Elder Helpers, you could connect seniors to all sorts of different resources.”

Improving Access to Healthcare: Central Okanagan Division of Family Practice Mobile Assessment Unit Connects Seniors to Physicians Local program can connect senior citizens with family doctors in as little as 6 weeks.

Mike Straus, Contributor Senior citizens without regular family physicians can now find a local physician to monitor their ongoing health needs, thanks to a new local initiative that is changing the way people access healthcare.

The Mobile Assessment Unit was created by the Central Okanagan Division of Family Practice in October 2014 and is funded by the provincial A GP for Me initiative, a joint venture by the B.C. Government and local physicians.

Since November, the unit has helped nearly 160 people over the age of 65 to find family physicians.

Tristan Smith, Executive Director of the CODFP, says the program takes a three-pronged approach in addressing the local family physician shortage.

This includes attaching senior

When you have a family physician, your needs will be better managed, and you’ll cost the system less,” Smith says. “If you’re admitted to the emergency room, your hospital stay will likely be shorter if you have a family physician. It’s a worthwhile investment.

Recruiting new physicians to the

Okanagan is also a major priority for the CODFP’s A GP for Me program.

The organization estimates that 36,000 people in the Central Okanagan don’t have a family physician, and that 4000 to 6000 of these people are over the age of 65.

The Central Okanagan has 12 to 16 fewer physicians than a population centre of its size should. Nearly 40% of family physicians in the area plan to retire within the next 10 years.

In an effort to address this problem, the CODFP has instituted a campaign to attract new physicians to the area. The initiative identifies practice opportunities and shares them with physicians in advertisements while also talking with community members about designing family practices that will entice new physicians to settle

here.

This new campaign has been in place for only 6 months, and already it has attracted 3 new physicians to the Central Okanagan. Another 5 physicians have shown interest in the area.

The MAU program works by first taking a patient history and registering patients on a waitlist. As the organization finds physicians with capacity for new patients, they assign physicians to patients.

Most applicants can be assigned a family physician in as little as 4-6 weeks, although people with an urgent need or people who are deemed high risk patients – such as cancer patients - will be attached to a family physician as quickly as possible.

The Mobile Assessment Unit is planning to find family physicians for 1000 seniors by the end of 2015.

Risks for dementia Those at risk of having a stroke may also want to pay attention to their risks for dementia. This is concerning news, but it may also lead to new approaches for preventing Alzheimer’s disease and other forms of dementia.

Dr. Sandra Black at the University of Toronto sees increasing evidence that strokes and dementia are connected. Both conditions, her research shows, are related to the health of the blood vessels – the arteries and veins – in your brain.

A growing number of researchers share Dr. Black’s view. This shift in opinion suggests an exciting possibility – namely, that controlling risk factors like hypertension and high cholesterol could help prevent or delay forms of dementia, such as Alzheimer’s disease.

The challenge will be to identify people in the early stages of the disease, so that they can take steps to reduce their risk.

Through the ongoing Sunnybrook Dementia Study, started in 1995, Dr. Black has collected brains scans

of more than 1,000 patients with dementia. In addition, she’s attempted to link the patients’ brain scans with changes in their memory and behaviour, and comparing them to scans of healthy aging brains.

“We’ve done scans in a way that’s enabled us to measure the brain changes in an objective way that can be linked to cognition and behaviour,” says Dr. Black.

“A quarter of Canadians 65 and older have these tiny areas of dead brain tissue,” says Dr. Black.

Unlike major strokes which result in dramatic, almost immediate paralysis or loss of speech, silent strokes can cause a gradual, cumulative decline in mental abilities. Another condition that affects the small blood vessels of the brain causes partial brain injury that appears as small white dots or patches, and is present to some degree in 95% of the elderly.

“I have a sense of urgency about the future,” says Dr. Black. “We need to find ways to set up care to prevent this disease.”

Chronic Pain Management Program Gives Seniors Better, Longer Lives Physiotherapist Helen Day says her unique 5 Point Protocol for Chronic Pain cured her own neuropathy and paraplegia after a debilitating surgical error left her in terrible pain.

Mike Straus, Contributor The daughter of an orthopedic surgeon, Helen Day graduated from the University of Alberta over two decades ago - and ever since she has been a practicing physiotherapist and avid athlete. So when she underwent a routine surgical procedure in 2002, she could tell almost immediately that something had gone wrong.

“For the kind of operation I was having, the anesthetist had to paralyze me from the waist down by injecting something into the muscles around my spine,” she says.

“But as soon as the needle went in, I know I was in trouble. I could see my nail beds turning blue.”

As Helen would later discover, the anesthetist had injected the anesthetic agent directly into her spinal column, where it traveled up to her brain and inflamed her spinal cord. She became an incomplete paraplegic and was in severe pain.

Helen knew that if she was going to have any chance of reversing her paralysis and treating her pain, she’d have to take her care into her own hands.

“I’d been doing acupuncture forever and ever. So I took one of my acupuncture needles and inserted it into my back. Then I took the tissue around the needle and formed it into a C shape with my hands. The relief I felt was amazing.”

Just prior to her surgery Helen had resumed mediation and diaphragmatic breathing, and after her surgery, she started using breathing and meditation techniques in conjunction with acupuncture to treat her pain.

After spending 8 years in pain and 6 years on disability benefits, Helen

was finally free of her pain thanks to her unique approach.

Today, Helen uses her 5-point protocol for pain to help chronic pain patients. Younger people stay for an average of 6 sessions, each lasting an hour, and walk out fully recovered from their chronic conditions, she says.

“The older patients tend to have hip pain or knee pain,” she says, “and they’re usually trying to prevent or at least delay a hip or knee replacement.”

“One of my patients is a 93-yearold man who comes in for his hip. He was going to quit golf, but ever since he came to see me he’s been back out on the golf course. He hasn’t had any problems for six months.”

“Divert some of your money to your own healthcare. All we can go on is research, and the research says that therapy for injuries will help you to recover. You don’t have to live the rest of your life with these injuries.”

Chronic Pain

By James Kaufman, R.Ac. Pain is considered chronic if it lasts 6 months or more and it can be debilitating to live with. The pain may be constant, or it can come and go, interfering with many aspects of daily life. The quality of the pain can be tingling, jolting, burning, dull, aching or sharp.

The cause of chronic pain often isn’t well understood. In some cases, there may be no evidence of disease or damage that points directly to pain. In other cases, pain may remain after the original injury shows every indication of being healed. Chronic pain can be due to a chronic condition, such as arthritis or fibromyalgia, but often there is still no clear understanding of the physical cause of the pain. Damage to a peripheral or spinal nerve may lead to chronic pain, where the damaged nerve, not the original injury, is causing the pain. Nerve damage can result from accidents, infections or surgery.

Researchers think chronic pain may be partly caused by sensitization, a process where the nervous system amplifies and distorts pain, resulting in severe pain that is out of proportion to the disease or original injury. Sensitization can affect all the pain-processing regions of your nervous system, including the sensing, feeling and thinking centers of your brain. When this occurs, chronic pain may be associated with emotional and psychological suffering.

Treatment for chronic pain often means managing the pain through

over-the-counter pain relievers, prescription opioids, cortisone, or NSAIDs (non-steroidal anti-inflammatories). Unfortunately for many, medications often have undesirable side-effects and may offer only limited effectiveness for dealing with the pain. Other treatments may include physiotherapy, massage, chiropractic, counseling, and acupuncture, therapies which aim more at resolving the pain. Often, a combination of approaches is needed to effectively treat this problem.

Acupuncture is an excellent option for chronic pain. Although Western medicine has not yet uncovered the mechanisms of acupuncture, it is clear that acupuncture is very effective for treating all kinds of pain, both acute and chronic. This goes beyond the “placebo effect”, as acupuncture works just as well on animals (who bring no expectation to the treatment table). One of the reasons that acupuncture is particularly good for treating pain is the way in which it stimulates the production of endorphins, the body’s natural pain relievers.

In addition, acupuncture focuses on correcting imbalances in the body and encouraging normal, healthy functioning of the body’s various systems, which may help the body to correct the sensitization process as described above, or changes or disruptions to the body as a result of injury or illness. In this way, it can help both to manage and relieve the pain in the short term, and help to address the underlying causes of the pain to help to resolve chronic pain

over time.

In my practice, I have seen patients with chronic pain respond very positively to acupuncture, with a reduction in pain symptoms and the overall level of pain. Questions for Nurse Kris Dear Nurse Kris; My parents live in Newfoundland, and I live in Summerland, BC. They are strongly considering moving to Kelowna? I’ve always thought of Kelowna as a pretty spendy place to live. What should I be telling my parents to look into? I don’t have a lot of time to help them, as I’m busy with my own kids and their problems, and I work and look after my own house. Can you give me any ideas to advise them about?

Sarah in Summerland, BC Dear Sarah; Kelowna consistently ranks near the top in UN-affordability of housing, in all of Canada. While the winters out East are brutish, the cost of living in Kelowna is similarly brutish.

I have come across a great study recently done by a Master of Arts in Interdisciplinary Studies student at UBC-O, by the name of Heather Catherine Brown (Brown, 2013). In her thesis called “Aging Gracefully: A case Study of Seniors in Kelowna’s Housing Market,” she finds that seniors already living here do say that Kelowna’s housing market is expensive and that these seniors recommend that the location of the new home, be close to amenities. They also recommend that a senior/senior couple, rent first for one year before deciding to buy a home. These study participants all recommend that the City of Kelowna add more affordable housing options. Recommendations were also made by these seniors that the three levels of government do more to make housing more affordable for seniors.

The main keys to success for your parents, according to this thesis (Brown, 2013), is maintaining social connections, having a good sense of personal well-being, and having available home support services.

Many respondents to this study said that they incorporated the help of realtors, family members and friends as well as doing on line research and enquiries in making decisions about what kind of housing

would best suit their needs. Many seniors feel that it is their ‘right’ to stay in their own homes for as long as possible (and in most cases, they are right about that).

Sarah, I think your parents should investigate the housing market with the assistance of someone they trust, to make non-conflict recommendations to them. There is a lot to keep in mind to be sure. Your parents should think of the things that are important to them (access to medical services, golf courses nearby, values around privacy and space, proximity to family etc), determine what their funds are and plan what the best living and housing situation is, based on the answers to those questions.

In Kelowna, there are a wide range of options; from living in an owned detached home, to apartments that are subsidized, to ‘campuses of care’ which have both Assisted Living condos and residential care beds, in the same large building. Or, supportive housing, where most meals are taken in a common dining room, and some who also have a variety of specialized purpose rooms such as a wood working shop, a movie theatre, a hair salon, a general store, a gift shop etc.

Housing affordability will determine where your mom and dad can live.

The cost of housing, should be no more than 30% of their available income (Brown, 2013).

Kelowna also has one or two professional geriatric care managers who know the seniors housing industry well. They could be contacted and asked for assistance as they know the seniors housing market well and have no conflict of interest in deciding which housing option is best.

sweeping his porch, a neighbour found him there a few hours later and called the ambulance. He went to the emergency room and was told he had a broken hip. If that isn’t bad enough, they didn’t put him in a real bed, they put him in a stretcher in the hallway. I was really shocked when I arrived there later that day.

He said he couldn’t get any rest because the lights from the nursing station, kept him from sleeping and the staff were noisy. He said he had wet the bed once because no one came to help with a bedpan. This really upsets me! I talked to the nurses and they said they were at “overcapacity” and unfortunately dad would remain on the stretcher til a bed came open, or surgery… whichever came first.

What has our health care system come to??

Dear Dealing: Your Dad is considered a “Frail Senior”. His experience is not unusual of patients in hospitals in BC, and in many places across Canada.

Being a “hallway” patient is not uncommon to hear about. Hospitals in BC are usually at overcapacity, which correlates to sick patients like your dad, sleeping on stretchers in the hallways; often without privacy, or the capability to readily summon help for pain control, or the wants of nature.

Canadians have claimed for five decades that we have the best health care system in the world. It may have been true at one point, it is true no longer. Canada spends 12% of its GDP on health care; it ties for first place with Iceland, yet has only mediocre health care outcomes in the 32 country OECD {Organization for Economic Development}. Canada is the only country in the world to outlaw the private purchase of health care services that are publicly administered, as per the tenents of the Canada Health Act. No other country in the OECD has replicated our single payer system. Our health care is not ‘free’, it is expensive to procure and manage. Its cost is hidden in our high taxes; free it is not.

When Medicare was developed in

the 1950’s it was designed to meet the acute care needs of a relatively young population, not keeping the elderly safe at home or housed in facilities. As such, the health care system can be compared to the Medieval Groaning Board; the health care system is buckling under the weight of burden of caring for its aging citizenry.

As Dr. Simpson, new leader of the Canadian Medical Association

said, “Canada’s health system ranks next to last (after the US) on virtually every measure of quality and access to care”. He continues on to say that no one would ever create a health care system like this today. (Andre Picard, 2014). Dr Simpson maintains that caring for people in their own homes to the extent safely possible would save the health care system in Canada, $2.3B per year.

Eat Well 9-5 for Nutrition Month March is Nutrition Month here in Canada, and this year’s theme of Eating Well 9-5 is designed to inspire Canadians to make positive changes for a healthier workplace environment. Research shows that eating well at work can boost concentration and productivity, and reduce sick days by helping to improve overall health. If you are like the 45 per cent of Canadians who say that eating healthily at work is challenging – according to the Dietitians of Canada – then check out these tips to improve your nutrition during the workday: Rushed Mornings – Almost 40 percent of Canadians skip breakfast, even though that can lead to weight gain and increased appetite later in the day. Missing breakfast also leaves you less alert and unable to concentrate at work. Instead, prep breakfast the night before: put frozen fruit, Greek yogurt and almond milk in the blender and store in the fridge overnight. As a convenient protein option, hard-boiled eggs can be refrigerated for up to one week.

Snacking at Work – One in four Canadian workers are highly stressed on most days. Studies show that stress can increase the desire to eat high-fat, sugary snacks. While you might be trying to save time, eating in front of your computer can lead to overeating because you’re not paying attention to your food. But did you know that a large (563 mL) coffee shop “double-double” has the same calories and more sugar than a chocolate glazed donut? Prevent the dreaded mid-day slump by staying hydrated and keeping healthy snacks in your desk drawer, such as pre-portioned packs of nuts, crackers or granola bars. Sleep patterns change as we age If you don’t sleep like a baby any more, you’re not alone. As we age, our brains change, which affects how we sleep. While many changes are normal, scientists think that helping older people sleep better may one day help rejuvenate brain functions like concentration and memory.

“Sleep patterns change as we get older,” explains Dr. Julie Carrier, a scientist with the Centre for Advanced Research in Sleep Medicine in Montreal. Many older adults go to bed and wake-up earlier, take more naps and sleep less at night. They also tend to wake up more often and sleep more lightly.

“We’ve all seen how a baby can sleep right through a Christmas party,” she says. “By the time we reach the age of 50, it’s just not possible.”

As we sleep, our brains get a chance to recharge. But gradually, our brains lose that capacity, which can affect our ability to learn and remember. Dr. Carrier and her colleagues are looking at ways to stimulate the brain to give older adults back the qualities of sleep they’ve lost.

“Some people think of it as

searching for the Fountain of Youth,” she says, laughing. Her research is still in its early stages. In the meantime, if you’re tired of counting sheep, try these six tips for a better sleep: 1. Cut back on smoking, as well as coffee and alcohol.

2. Stay active and eat well, but don’t eat too much or exercise near bedtime.

3. Make sure your bedroom is quiet and dark with a comfortable temperature.

4. Go to bed at the same time every night.

5. Try to avoid stress at bedtime.

6. Don’t panic. Many changing sleep patterns are normal. But if you’re not sleeping well, talk to your doctor. Your insomnia might be caused by a medical condition.

The Centre for Advanced Research in Sleep Medicine has several labs where researchers use advanced technology to explore different aspects of sleep. These include how chronic pain affects our sleep, why we sleepwalk, how sleep patterns change after a brain injury and the sensitivity of our brains to light.

Hands-on learning opportunities A local tradesperson’s generous donation of thousands of dollars of highly sought-after precision measurement tools will give Okanagan College students some heavy-duty hands-on learning opportunities.

Bill Schoof, a retired journeyman, has donated his collection of machinist tools to Okanagan College after learning about the renovation and expansion of the Trades Training Complex at the Kelowna campus.

“If you’re learning a trade you need more than books, you need the right tools in your hand as well,” said Schoof. “The college already has great equipment, of course, but I know that good tools are always in demand, always appreciated. I was glad to hear that they would benefit students and I’m happy to see them put to good use.”

“It’s been wonderful to see the support from the community as our new Trades Training Complex comes together,” said Steve Moores, dean of OC Trades and Apprenticeship. “We greatly appreciate donations of equipment like this. They enrich our programs. They help us ensure that students have opportunities to work on and learn on a wide variety of equipment that they’ll encounter out in their industries.”

“I want to extend a sincere thank-you on behalf of Okanagan College to Mr. Schoof for this generous gift. As a journeyperson machinist and tool and die maker myself, I feel a special connection to these tools and the trade, and I appreciate that Mr. Schoof has chosen to share an important part of his life’s work to inspire our students.”

Schoof’s donation is one of several major donations of equipment to the Bright Horizons Building for Skills fundraising campaign, which was launched last fall by the Okanagan College Foundation, with the goal of raising $5 million for capital construction costs and $2 million for program and student support. In addition to over $3.5 million in funds raised, several local individuals and organizations have contributed equipment, including Art Salt, who donated a Caterpillar front-end loader to the college last year.

According to Schoof, the tools took half a life-time to acquire, spanning the course of his 40-year career as a tool and die-maker. He reflected on the difficulty of acquiring them, piece-by-piece, while also working full-time during his training.

“I took a little out of my paycheque each month to buy them,” explained Schoof. “It meant scrimping and saving, but it was a labour of love. So I know how hard it is for students starting out, and I believe we need to support them.”

Born in Windsor Ontario, Schoof moved to the Okanagan in 2002 with his wife, Mary, after retiring from the tool and die industry.

“It’s been wonderful to see how Okanagan College has grown over the past 15 years,” added Schoof. “These students are going to work hard and put so much back into the area.”
