Compassionate Care Benefit 1-800-206-7218 www.servicecanada.gc.ca This is an Employment Insurance (EI) special benefit that will pay up to six weeks to claimants who provide care or support to a gravely ill family member who has a significant risk of death. The claimant must have sufficient hours of insurable employment to qualify. Up to six weeks of leave may be taken, with job protection, to provide compassionate care to a family member. Disability Tax Credit 1-800-959-8281 www.cra.gc.ca This tax credit is available to people with a severe and prolonged mental or physical impairment. All or part of this credit can be transferred to a spouse or supporting individual. Fill out form T2201-E and submit it with the income tax of the person. The doctor must sign the form under “Particulars of Disability”. People with a permanent disability (e.g. Alzheimer Disease) only need to submit this form once to get the tax credit in future. Veterans Affairs Canada 1-866-522-2122 www.veterans.gc.ca Veterans Affairs Canada offers a range of services and benefits to qualified, eligible veterans (and their dependents), members and ex-members of the Armed Forces and of the RCMP. Benefits may include: • war veterans allowance - an income tested and non taxable benefit • disability pension • health care e.g. dental, glasses, hearing aids, medication, equipment, contributions toward home care • funeral and burial expenses If veteran was receiving housekeeping, home maintenance or personal care help at time of death, widow may continue receiving benefit. Patrons on the Homebound Services Program may place reserves on materials of their choice or allow the Community Services Librarian to select materials for them. A profile of reading interests and a list of materials read or desired is kept for each Homebound patron. This information is confidential and available only to Homebound Services staff.
