Working as a Team with Health Care Providers

As a family caregiver, it is inevitable that at some point you are going to need to work in partnership with members of the health care system to assist with decision-making and appropriate planning on behalf of the elder care recipient.

The person receiving care can become increasingly reliant on family members to provide more complex types of oversight, either in their home or seniors’ residence.

Ultimately, as additional care assistance is integrated, better service can be obtained all around if everyone is working collaboratively and in a mutually respectful manner.

To achieve this end, often an elder requires someone to act as their voice in order to advocate for appropriate levels of care and help interpret their individual needs to those providing the care. As a family caregiver, your awareness

and knowledge of the elder is invaluable to health care practitioners, many who will not know the person and his or her unique needs. Your insight into what their wishes are with respect to personal privacy, emotional well-being, diet, lifetime interests and how the care recipient reacts to medication and certain types of procedures or treatments will provide information that leads to the best decisions.

March, 2013 TEAM… from page 3 are willing or able to do. Trying to “do it all” and make considerable changes all at once threatens to result in unhappiness on both sides and caregiver burnout.

As in every relationship, good communication takes time, effort and a willingness by all parties to work through differences of opinion together. Working within the health care system can sometimes be frustrating; feeling a lack of control over the care being provided, scheduling and wait times for procedures, and working with a revolving door of health workers who are providing the care, are some of the issues that may be encountered.

Needless to say, using assertive, rather than nonassertive communication with health care providers is generally more productive.

Assertive techniques that are beneficial are: active listening, taking responsibility for your own statements, dealing with problems, not people and expressing your thoughts, feelings and beliefs in a direct, honest and appropriate way.

Always respect the rights of others while making your request clear and to the point. For example, ask: 1) How do you feel about that? 2) Are you able to do that? 3) When can you give me a reply?

Having an open discussion with health care providers about the kind of relationship that you would like to have, and the level of involvement you want, is a good first step. A “Circle of Care” or teamwork approach implies that both parties are contributing equally to the care of the person.

Teamwork often involves building

rapport, which means establishing a level of trust and open communication. This allows you as a family caregiver to feel comfortable discussing your concerns, questions and needs around the elder care receiver and to also feel supported in your role.

How do you function as part of a team on behalf of your loved one? The answer to this depends on how you feel about your caregiver role, the level of involvement you wish to maintain and also in the degree of comfort you have in communicating with health care providers. An indicator of a satisfactory relationship is a feeling that: – information is being shared – when appropriate, you are being included in decision-making – there is someone you can contact when the need arises – the services are responsive to your needs and to those of the elder care recipient Effective teamwork requires a mutual understanding of the roles and responsibilities of each member of the team. If you are not satisfied, look for ways to resolve the problem. Sometimes an intermediary who isn’t as close emotionally to the situation can be a helpful benefactor.

Your role as advocate, especially for elders who are vulnerable, is essential to ensure access to necessary services, but also to ensure that the individual’s voice and needs are respected and heard.

Senior moments explained

Well if you are someone who is into the second half of your life, chances are you have noticed the occasional memory lapse. Simple things like phone numbers or the location of your keys slip your mind a little more often than they once did.

These ‘senior moments’ as they are somewhat endearingly called, are typically just an annoyance.

They do not seriously affect a person’s ability to function on a daily basis but they can sometimes cause anxiety. Often, people have an unsettling feeling that minor memory slip-ups could be the beginnings of something more serious.

A study released just recently has shed some interesting light on the subject of age-related memory loss and may lead to the possibility

of reversing it.

Columbia University researchers examined the effects of a particular protein in the brains of humans and mice. They found that a deficiency in this protein in the hippocampus area was associated with age-related memory loss.

Their research showed that this forgetfulness is distinct from the more serious memory loss associated with dementias such as Alzheimer’s disease and that the one is not a precursor to the other.

MOMENTS… from page 5 We have known for some time that Alzheimer’s disease is characterized by plaques within the brain and these are not present in those with the more minor age-related memory loss. This study pinpointed what is happening when we experience those pesky senior moments.

It turns out that levels of a particular protein (RbAp48) decline steadily in people as they age. This was found across the board in human subjects. A similar decrease was noted in the brains of mice – the older the mice, the less of this protein they had in their brains.

Further, when scientists reduced the amount of the protein in the brains of young mice, the mice experienced the same memory loss that occurs naturally in aging mice. When the protein levels were returned to normal, the memory did as well.

Perhaps most interestingly, when this protein was increased in the brains of aging mice, their memory improved dramatically.

This finding inspires hope that we could eventually use this

information to come up with ways to reverse memory loss associated with aging. Whether the solution comes in the form of a pharmaceutical treatment or natural methods to increase production of this protein within the hippocampus, it will be profoundly helpful to understand the mechanism involved in this condition.

Of course, much more research will be necessary before any kind of treatment could be realized, but it certainly is promising to have something concrete to work with.

In the meantime, it is also helpful just to know that normal age-related memory loss is physiologically different from dementias such as Alzheimer’s disease. At the very least, it can be a comfort to many older people who experience occasional ‘senior moments’.

Although it is irritating to misplace your wallet or forget a postal code you’ve been using for years, it is not necessarily a harbinger of more upsetting symptoms coming your way.

Society of B.C. needs Kelowna

volunteers to handle growing demand

Fred and Amelia look forward to Tuesday afternoons at the community centre. It’s where they come for Minds in Motion®, a fitness and social program for people experiencing early stage memory loss and their care partner.

The local Resource Centre of the non-profit Alzheimer Society of B.C. needs volunteers who can help with these weekly Minds in Motion programs.

“Our volunteers assist and engage with program participants, so a friendly personality, helpful demeanour and a sense of fun would be great assets,” says Lynnea Westlind, the Society’s Minds in Motion Coordinator for Kelowna and the North & Central Okanagan. “Volunteers are an important part of creating an atmosphere that is welcoming and inclusive to people with dementia

and their care partners.”

Minds in Motion offers a 45-minute gentle fitness program followed by an hour of socializing and activities such as board games, ping pong or puzzles designed to stimulate thinking, laughter and conversation.

In addition to helping others, volunteers have the opportunity to spend rewarding one-to-one time with participants and see first-hand how people’s lives are enriched by this program. Some experience working with older adults and basic knowledge of or

experience with dementia would also be helpful.

Volunteers must be physically able to participate in the fitness part of the program and have the ability to lift supplies, assist with the set up and clean up of refreshments and activities and move chairs.

The Society also needs administrative volunteers to help with a variety of duties, including mailouts, packaging education materials, assisting walk-in public, answering phones, sending out email notices and data entry.

Very few strata title owners realize that, in the event of an earthquake the results in even minimal damage, they will have no coverage for the deductible portion of the insurance policies carried by their strata corporations.

Such deductibles are sizeable and will mean significant special levies upon condo owners before their buildings can be restored and reoccupied. Most condo owners assume that there are sufficient

Virtually all strata corporations include earthquake as a “peril” in their standard insurance programs, however, all polices have sizeable deductibles for earthquake damages. These deductibles range from 10 to 20 per cent of the insured value of he strata corporation, technically referred to as the Cost of Replacement New. For example, if a strata corporation is appraised at $80,000,000 and the earthquake deductible is 15 per cent, the strata corporation is essentially uninsured for the $12,000,000, and this very lot of money will have to be raised by the owners to fund any repair program.

Levies and normal monthly strata fees are actually determined on the basis of “unit entitlement”- a measure based on size, therefore some strata lots pay more than the average and some pay less.

If in this example, the strata corporation consists of 200 units, the average levy would be $60,000—still a lot of money.

Many condo owners believe that their strata corporations have adequate contingency reserve funds which will absorb the heavy hit of the earthquake deductible.

In fact, it is rare to find a strata corporation with adequate reserves of this magnitude. The reality is that only special levies will be the source of funding and most condo owners are unaware of this significant potential financial burden.

Condo owners also need to realize that they will have no vote in respect to raising special levies to fund the earthquake deductible. Normally, strata lot owners

have a statutory right to vote for or against special levies, which require a 75 per cent approval vote.

In this case of an earthquake deductible, such a vote is not required; the strata council alone has the statutory authority to simply declare a special levy and it is certain that strata councils will do this very quickly in order to secure contractors for the reconstruction task as soon as possible following an earthquake that leaves even minimal damage.

Concerns about earthquakes in British Columbia have increased significantly in recent years for insurance underwriters. Advanced technology has permitted that industry develop probably models of what damages might be expected.

These models are sophisticated to the point where they can pinpoint specific geographic areas that will be heavily impacted.

Consequently, some strata corporations have insurance policies with 10 per cent deductibles, while others have 15 or 20 per cent.

CAPITAL NEWS It is not unreasonable to predict or assume that in the coming years, these deductibles will increase.

Apart from the deductibles issue, condo owners are also generally unaware that standard strata corporation insurance policies do not provide for hotel expenses, meaning that if an earthquake severely damages a strata building and it can’t be occupied until it is repaired, which could take months, there is no hotel benefits.

The owners are not beneficiaries for out-of-pocket living expenses such as hotel accommodations. They would have to absorb that expenses themselves.

Strata lot owners should always obtain and maintain personal policies for such benefits. Few actually do.

In 2006, an airplane departing from Vancouver International Airport crashed into a strata complex in Richmond, leaving dozens of occupants displaced for months while the damages were repaired. The strata corporation’s

insurance paid for the full restoration (several million dollars) but did not provide for hotel expenses.

This scenario will play out in similar fashion in the event of an earthquake.

It is important to remember that all of this doom and gloom applies regardless of the size of the earthquake or damage that occurs. The deductible applies to every event—large, small or medium.

Following every minor earthquake in B.C. the rattles people’s nerves, there is considerable advice forthcoming to the public about being prepared for such an event ( i.e. bottled water, first aid kits, etc.).

While this is all true, it should also be noted that condo owners are also ill-prepared for the significant financial impact they will face in respect to the insurance policy deductibles.

For others, that loss of independence weighs heavy on them, as does the constant pressure from family and friends that it’s time to re-adjust your life, time to abandon what has been routine for so many years, and in many cases are the very things that give you the desire to wake up every morning and carry on.

I have seen this transition up close through my parents and see how difficult it can be. My mom is 86 and my dad is 90, and they live in the same home they built brand new in 1970 in the Lower Mainland.

They still live in that same house because the activities that surround keeping their home functioning—i.e. the work in the garden, the upkeep inside, etc.— has kept them healthy and active during the past 25 years of their retirement.

But in holding on to that independent way of life, those things they valued 10 or 15 years ago are now beginning to be an anchor rather than a buoy to maintaining an active life.

My dad has begun to deal with stability issues, and was recently told by his doctor that he is obligated to start the process to revoke his driver’s license. For someone wanting to maintain their independence, losing a driver’s license is a tough pill to swallow.

My mom has had a hip replacement a year ago and has her own mobility issues.

What has kept them in their house in recent years is the goodwill and support of their neighbours, who keep an eye out of them, help shovel the sidewalk in the winter and are quick to respond if my dad has any ideas about climbing a ladder to deal

before it becomes your Reality with any outside house maintenance issues.

So now my parents face the pressures of selling a home and relocating to some form of retirement resort or independent care setting that may not be entirely of their own choice or desire. It is not a choice they will make willingly so much as be forced into. It has gone from being inevitable to now having to do something.

The option of downsizing to a nice condo has passed them by, not something that is particularly tragic for them because they enjoyed their independence at home, but still an opportunity of admittedly both pluses and minuses they won’t get to experience.

You read about these family scenarios all the time, where the family steps in and arbitrarily does what’s in the best interest

of the parent(s), a decision often complicated by a wealthy estate anticipated to be handed down to the surviving children.

Or they make a decision to downsize late in life of their own freewill and often get swindled out of their money by opportunistic real estate agents or financial advisors not always acting solely in the best interests of their clients.

I see how my parents have faced some hard choices in making this transition in their lives, but it is one that comes with having been able, through relatively good health and stable financial income, to enjoy their early retirement years in their own home, bringing a lot of joy and satisfaction to their daily lives.

As a family, we will be called upon to support them in the transition that I think both my parents see now as inevitable, even if both are at different levels of realization

and stubbornness in accepting it.

That’s all we can do.

While we are living longer now and more active at our later stages in life, we can’t change the ultimate reality. We do age in our mind and body, our ability to lead the lives we did in our younger years begins to escape us, and we should embrace that inevitability with wide open eyes well before it confronts our lifestyle choices.

That’s not a bad thing. Just different, not about opportunities in life that are lost, but welcoming new things to keep our minds active and engaged. Instead of focusing so much on retaining our youth, maybe we should spend a little more energy learning how to cope with the loss of our physical and mental state freedoms we all take for granted until we lose them, and how we can still lead full and interesting lives despite age catching up with us.

Stories I have written about health retirement resorts in the past always raise a similar comment from residents of a given facility: “I wish I had done this years sooner” in embracing the lifestyle change from a house to condo living. I wonder if in time my parents will feel the same way.

In the end, I think it comes down to making that transition of lifestyle to our senior years as one of choice and not a forced decision, one we should think about when that prospect looks far off still on the horizon rather than right before our eyes.

We need to collectively realize we are not throwing the book away on our lives as we embrace our retirement years, just writing a new chapter.

Travel Insurance considerations

There are five misnomers or mistakes that travellers often make when making a decision about travel insurance before heading out on holiday outside Canada, says a Kelowna insurance broker.

Greg McGill, with the Western

Financial Group in Kelowna, says people too often take unnecessary risks with travel insurance decisions that can turn out to be costly mistakes.

McGill said the five travel insurance missteps are:

1) Don't set aside enough time to research what travel insurance policy will best suit the needs of you and other family members. "I often get phone calls from people on the way to the airport asking me to get an insurance policy in place," McGill said. "That is not the way to ensure you have adequate coverage."

2) Vacationers make assumptions that aren't always true. While there are insurance options from travel groups, banks, credit cards and retirement plans, they don't always meet the potential needs based on planned holiday activities.

3) Pre-existing health conditions can't be overlooked. McGill says people who have pacemakers or a stent in an artery to to have coverage in case problems arise while away from home.

4) Not all travel insurance policies cover all holiday activities. "It depends on your particular appetite for adventure, but if you go scuba diving, for instance, and you get injured or hurt, you want to ensure your policy will cover that," McGill said.

5) Don't select a travel insurance policy on price alone. McGill advises to be careful you don't get what a low cost policy does or doesn't pay for.
