CONTINUING HEALTH Vancouver Coastal Health 604-278-3361 Richmond - Continuing Health The Richmond Hospital - 7000 Westminster Highway, Richmond, BC V6X 1A2 www.vch.ca For clients who have chronic health care problems that impede their ability to be independent. A home visit can be arranged to discuss health care needs. Call for more information or to book an appointment.

Adult Day Care 604-272-3237 100 - 6100 Bowling Green Road, Richmond, BC V6Y 4G2 Offers a Therapeutic Activity program every weekday for adults with health problems. Adults can enjoy healthy activities and contact with other people under the supervision of health care professionals.

Runs from 9:00am to 2:30pm and includes lunch and snacks. Call Richmond Continuing Health for a referral or call the Centre directly.

Case Management Arranges home support for those unable to care for themselves due to health problems. Also arranges adult day care and respite services.

Assists in finding alternative living arrangements for those who can no longer live independently at home. There is a cost for some services.

Home Care Nursing and Rehabilitation Provides services to those whose needs can safely be met while they remain at home. Arranges home support for those who need help with daily tasks. Offers nursing, and rehabilitation consultation and support to care providers and adult clients who have multiple physical and cognitive disabilities.

Home Support Provides help with daily tasks for people who have ongoing health problems, disabilities, or temporary medical or physical problems.

Case managers and professional care staff arrange home support.

Community health workers follow the plan developed by the case mangers. Service is delivered by neighbourhood teams within a time frame that is focused on completing the required tasks. The cost of service varies.

Long Term Care Facilities Residential care homes offer 24-hour care and supervision for adults who can no longer live independently. To be admitted, individuals must meet the criteria for complex care. Waiting period is approximately six months. There are five complex care homes in Richmond, including Fraserview Immediate Care, Pinegrove Place, Richmond Lions Manor, Rosewood Manor, and Minoru Residence.

Subsidized Assisted Living Includes rental accommodations, hospitality services, and personal care for seniors with low to moderate incomes that need help with personal care. Offers a living arrangement with communal meals and social activities. Clients must be assessed and are eligible if they receive home support services, are able to make decisions about daily activities and needs, and are able to communicate and be understood by others, but are not managing well in their home with available services. Waiting period is twelve months. The two locations are the Courtyard Gardens and S.U.C.C.E.S.S. Austin Harris Residence.

MENTAL HEALTH AND ADDICTIONS Vancouver Coastal Health - Richmond - Mental Health and Addictions www.vch.ca Mental health and addiction services are provided by the Richmond Mental Health and Addiction Teams and community partners such as CHIMO Crisis Services, Canadian Mental Health Association, and the Richmond Addiction Services Society.

Mental Health Anne Vogel Methadone Clinic 604-233-5699 at City Centre Health 100 - 8160 Cook Road, Richmond, BC An Addictions and Primary Care outpatient clinic that treats clients with complex addictions. Consists of a multi-disciplinary team including a psychiatrist, two general practitioners, and an internist who also specializes in addictions, a counsellor, and a social worker. Many of the clients receiving services also receive primary medical services at the clinic. Refers to other services such as residential care, detox, other outpatient addiction services, and hospital and outpatient mental health services.

Community Mental Health 604-675-3975 Programs for Older Adults 200 - 6061 No. 3 Road, Richmond, BC V6Y 2B2 Community based assessment and treatment for older adults with dementia-related behaviour problems or complex psychiatric and medical needs requiring multidisciplinary care including case management and rehabilitation services. Family support and education is also provided. Care is coordinated with primary care physicians, elder care facilities, and other community programs.

Services are provided on an outreach basis.

Provides support and understanding to help callers identify problem areas and work towards resolving them.

Crisis and Suicide Intervention 604-279-7077 120 - 7000 Minoru Boulevard, Richmond, BC V6Y 3Z5 Email: chimo@chimocrisis.com CHIMO Crisis Services offers a counselling service to people who are experiencing a crisis, who are at risk of suicide, or who have made a suicide attempt. Counselling service is available to those ages 19 and older. Limited counselling for those under 19 is also available. Offers a support group for those who have lost a friend or family member to suicide.

Hospital Inpatient Program 604-244-5504 7000 Westminster Highway, Richmond, BC V6X 1A2 An 18 bed inpatient psychiatry unit providing assessment and treatment to adults with psychiatric diagnoses who require 24 hour management in hospital. Staffed by an interdisciplinary team who work with the patient, their family, and community supports to provide individualized patient-centred care. Discharge planning may include a referral to community resources for follow up.

Mental Health Emergency Services 604-244-5562 (MHES) A mobile service providing community assessment and intervention to individuals experiencing a mental health crisis. Services include assessments, and referrals to follow-up mental health services, health care professionals, or other agencies. Evening and weekend intervention and support are provided to individuals during non-office hours through the crisis line. Hours of operation are 11:00am to 1:00am, seven days a week.

Needle Exchange Locations For disposal of used needles in a safe and legal way to reduce the spread of HIV and Hepatitis C and to prevent other health complications. Reduces the number of discarded needles in public places and the incidence of needle-stick injuries. Encourages safer injecting practices by providing sterile water, alcohol wipes, and related health information. Services are provided in a safe and nonjudgmental manner. Clean needles are available. Locations are at City Centre Health (8160 Cook Road, Richmond, 604-233-5699) and Richmond Public Health (8100 Granville Avenue, Richmond, 604-233-3150).

Peer Support, Mental Health 604-675-3977 200 - 6061 No. 3 Road, Richmond, BC V6Y 2B2 In partnership with the Richmond Mental Health Consumer and Friends Society. Provides a peer support program for mental health consumers. Provides funding which aims to increase the effectiveness of the mental health system while helping mental health consumers develop skills and gain confidence. Also offers a peer support program for Chinese-speaking clients.

Mental Health & Richmond Hospital Psychiatry Emergency Unit (PEU) 604-244-5475 A four bed brief stay (72 hours or less) unit providing assessment, stabilization, and treatment to individuals experiencing a psychiatric/ mental health crisis. Provides specialized mental health and addictions services in an environment that supports a patient and family centered approach. Provides discharge planning, case consultation, and linkage with appropriate community resources for follow up.

Richmond Addiction Services 604-270-9220 Society (RASS) 200 - 7900 Alderbridge Way, Richmond • www.richmondaddictions.ca Provides a variety of services for young people and adults who have alcohol, drug, and gambling problems. Assists family members and others who are affected by substance misuse. Services include individual counselling, an education program, therapy groups, relapse prevention, public education presentations, seminars and workshops, prevention and community outreach programs and services, and older adult services.

Richmond Bridge House 604-244-7840 A ten bed residential short stay crisis intervention facility for clients who are experiencing a mental health crisis but do not require an acute psychiatric hospitalization. Clients are cared for by an interdisciplinary team, 24 hours a day, 7 days a week and can be admitted for a maximum of 14 days. Provides a safe environment where clients can learn new skills to better manage their illness.

Referrals require a physician assessment prior to acceptance and are screened prior to admission.

Richmond Mental Health and 604-675-3975 Addictions Housing Provides housing services to individuals with severe and persistent mental health conditions. Includes Licensed Specialized Residential Care Facilities, Enhanced Supported Housing, and Supported Independent Living placements.

Group therapy programs and psychiatric consultation and treatment services are offered. Psychiatry Clinics include: reproductive mental health, older adult mental health, and cross-cultural services.

Ambulatory Care Services 604-278-9711 Services provided at the hospital for people who are well enough to be treated as outpatients include blood transfusion, intravenous antibiotics, rehabilitation services, cast clinic, hand clinic, dietetic counselling, endoscopy, and oncology clinic.

Cardiopulmonary Neurodiagnostic 604-244-5124 Services A variety of tests are offered to help diagnose and treat conditions related to the heart, lungs, muscle, and brain. Services can be ordered by family doctors.

Emergency Department Open 24 hours a day. Does not answer health questions. If you have questions about health issues contact your Family Physician or BC Nurse Line at 811. If you are experiencing a health emergency, go to the Emergency Department or call 911. Does not give out information about patients in the department. If you need information, call a family member, friend or support person who has been with the patient in emergency.

Intensive Care For patients recovering from life-threatening illness or injury who require intensive, around-the-clock monitoring and care.
