Benefit Coverage for all lines of coverage will begin on the 1st day of month following date of hire.
Maximum Allowable HSA contributions are federally defined each year; for 2025 the maximum contributions are $4,300 for individ ual and $8,550 for family. Individuals over 55+ may make a $1,000 c atch-up contribution. Biomerics will contribute $800 (employee only) or $1,600 (employee + dependents) per year towards your HSA. Em ployer contributions count towards the Maximum yearly allowed amounts.

The maximum contribution amount is $5,000 if you are married and filing a j oint return OR if you are a single parent. If you are married b ut filing separately, the annual maximum contribution is $2,500. Individu als over 55+ may make a $1,000 catch-up contribution.
All full-time eligible employees are provided with 1 X Annual E arnings up to $250,000 Basic Life and AD&D.
All full-time eligible employees are provided with $10,000 incr ements to a maximum benefit of Lesser of 5 X Annual Earnings or $500,000 with a Guarantee Issue Amount of $200,000.
Spouses are provided with $5,000 increments to a maximum benefi t of $250,000 with a Guarantee Issue Amount of $50,000. Children are provided with $10,000 maximum. Rates calculations are age based.
All full-time eligible employees are provided with 60% of weekl y salary up to a maximum of $1,500 per week for 11 weeks. The elimination period for Accident and Illness is 14 days. Claims may be subject to pre-existing conditions guidelines.
All full-time eligible employees are provided with 60% of month ly salary up to a maximum of $6,000 per month for 2 years with 3/12 pre-existing conditions. The elimination period for Accident an d Illness is 90 days.
