The MedTech Radar informs our readers about different issues in the medical sector and markets of interest. Using examples and industry case studies, it shows the complex journey a medical product makes from initial research through to use by patients.

Germany is a well-established leading location for developments in medical technology. In addition to research carried by the industry itself and its product launches, university research also supplies innovative products that play an important role in medical care. The journey from invention to market is a long and a difficult one, however.

ei der Entwicklung neuer Dienstleistungen oder Produkte für das Gesundheitswesen gehen Firmen und Investoren oft hohe Risiken ein. Die kritische Phase beginnt, wenn sich das Produkt im Markt beweisen muss. Diejenigen, die darüber wesentlich mitentscheiden, sind Leistungserbringer und Krankenversicherungen. Bislang waren sie am Innovations- und Finanzierungsprozess kaum beteiligt – auch, weil das deutsche Sozialrecht enge Grenzen setzt, wie mit dem Geld der Versicherten umzugehen ist. Der Wagniskapitalgeber Earlybird hat jetzt mit der gesetzlichen Krankenkasse BARMER dennoch einen Weg der Kooperation gefunden und die Versicherung als strategischen Investor für seinen neuen Health-Tech-Fonds an Bord geholt. Insgesamt soll der Fonds ein Volumen von 100 Mio. Euro aufweisen, damit 12 bis 15 Firmen finanziert werden können. Über die Kooperation mit der BARMER sind die ersten 15 Mio. Euro gesichert.

Zu den weiteren Investoren gehören die staatliche NRW.Bank, der Medizingerätehersteller Miele und die Versicherungsgruppe Generali. Über die Hintergründe der neuen Zusammenarbeit sprach MEDTECH RADAR mit Dr. med. Christoph Straub, Vorstandsvorsitzender der BARMER, und Thom Rasche, Partner bei Earlybird.

Herr Rasche, Sie haben sich mit der BARMER erstmals eine gesetzliche Krankenkasse als Partner für Ihren Fonds geholt. Warum war dieser Schritt nötig?

Thom Rasche: | Dieser Schritt war längst überfällig. Bislang werden Krankenkassen hierzulande erst viel zu spät in den Innovationsprozess eingebunden – also erst nachdem die Entwicklung neuer Produkte stattfand und womöglich schon erste kleine Umsätze generiert wurden und sich die Firmen lediglich fragen, wie sie etwa eine Erstattung bekommen oder wie sie mit einer Krankenkasse ein Modellvorhaben aufsetzen können. Zudem gibt es zwischen Medizinern und Krankenkassen gewisse Differenzen, sodass oft nicht effizient zusammengearbeitet wird. Mit unserem Fonds wollen wir Brücken bauen und alle Beteiligten von Anfang an zusammenbringen. Dass dies klappt, zeigen im Übrigen auch Bespiele aus anderen Ländern.

Health-Tech-Fonds Bis zu 3.000 Ideen aus Diagnostik, Digital Health und Medizintechnik wollen die Partner des Health-Tech-Fonds bei Earlybird in den nächsten fünf Jahren bewerten. 12 bis 15 Firmen sollen finanziert werden. Insgesamt wird ein Volumen von 100 bis 120 Mio. Euro anvisiert. 15 Mio. Euro will die BARMER beisteuern. Weitere Investoren sind die NRWBank, die Versicherungsgruppe Generali, der Medizingerätehersteller Miele und das Bundeswirtschaftsministerium, das Anteile des European Recovery Program (ERP) einbringt.

MEDTECH RADAR | Herr Straub, welche Motivation gibt es bei der BARMER, als strategischer Investor in einem VC-Fonds aufzutreten?

Dr. Christoph Straub | Als Herr Rasche vor dreieinhalb Jahren mit der Idee auf uns zukam, war ich sofort begeistert. Gleichzeitig wusste ich: Das bedeutet das Bohren dicker Bretter. Klar ist: Unser erstes Interesse gilt dabei gar nicht so sehr dem Investment, sondern dem möglichst frühen Zugang zu einer Vielzahl neuer Entwicklungen in der Gesundheitsversorgung – vor allem mit Blick auf die Digitalisierung der Medizin. Über die Beteiligung am Fonds erhalten wir die Möglichkeit, eine große Zahl vielversprechender Geschäftsideen frühzeitig auf ihr Potential hin zu überprüfen, sodass unsere Versicherten davon auch möglichst früh profitieren können. Zugleich bringen wir unsere Erfahrung mit ein – über den Gesundheitsmarkt, seine Funktionsweise und Bedürfnisse.

Thom Rasche | Wir rechnen damit, dass wir innerhalb der Investitionsperiode von fünf Jahren gemeinsam sicher rund 3.000 innovative Firmen anschauen werden. Und bei der Analyse dieser Ideen werden wir insbesondere aufgrund der Marktkenntnis der BARMER zu sehr vielversprechenden und spannenden Erkenntnissen kommen, davon bin ich überzeugt. Denn das Interessante an der Kooperation ist die Datenlage, die Versicherungen in der Ge-

in der Abrechnung? Welche Wahrscheinlichkeit der Marktakzeptanz gibt es? Da lernen alle Beteiligten sehr viel und das allein ist sehr gewinnbringend und genau das, was wir wollen.

MEDTECH RADAR | Wie haben Sie die Beteiligung rechtlich geregelt?

Dr. Christoph Straub | Das war ein langer Diskussionsprozess mit dem Bundeswirtschafts- und dem Bundesgesundheitsministerium sowie unserer Aufsicht. Denn normalerweise gibt das Sozialgesetzbuch 4 Versicherungen wie uns enge Grenzen vor: Man darf Versichertengelder nicht spekulativ einsetzen. Gleichwohl hat die Bundesregierung in ihrem Koalitionsvertrag klar gemacht, dass sie nicht nur Start-ups und Innovationen im Gesundheitswesen am Standort Deutschland, sondern auch Public-Private-Partnerships fördern will. Genau hier setzt unsere Initiative an. Nach ausführlichen Gesprächen haben wir ein zufriedenstellendes Modell gefunden. Dieses sieht vor, dass das von uns eingesetzte Kapital in Höhe von 15 Millionen Euro vollständig abgesichert ist. Es ist also im eigentlichen Wortsinn kein Wagniskapital mehr, sondern eine abgesicherte Einlage. Dafür haben wir vom Bundesversicherungsamt die Genehmigung erhalten.

Thom Rasche | Es gehören offene Ohren bei allen Beteiligten dazu und dann ist die Zeit irgendwann reif für eine solche Konstruktion. Das Modell der Absicherung sieht vor, dass wir zum einen das ERP-Sondervermögen, verwaltet durch das Bundeswirtschaftsministerium, als Bürgschaft nutzen. Zudem gehen wir als Partner von Earlybird selbst in die persönliche Haftung.

MEDTECH RADAR | Nach welchen Innovationen wollen Sie insbesondere Ausschau halten? Was wird im Fokus des Fonds liegen?

Thom Rasche | Inhaltlich ist der Fonds auf klassische Medizintechnik, Diagnostik, Health IT und Digital Health ausgerichtet. Spannend auch mit Blick auf die Kooperation mit der BARMER sind vor allem digitale Innovationen, da sie Versorgungsprozesse ändern können. Dies gilt vor allem für das Ende der Versorgungskette, den ambulanten Bereich und insbesondere der Schnittstelle zwischen erstem und zweitem Gesundheitsmarkt. Hier passiert besonders viel, auch wenn hier – angesichts der hiesigen komplexen Erstattungswege – der Marktzugang eine besonders schwere Aufgabe ist.

Dr. Christoph Straub | Unser Interesse ist primär: Wir wollen mit neuen, innovativen Gesundheitsleistungen den medizinischen Fortschritt aktiv gestalten. Unser Ziel ist, aussichtsreiche Technologien der gesetzlichen Krankenversicherung zugänglich zu machen. Hier lohnt auch der Blick darauf, wo es sinnvolle Produkte oder Dienstleistungen außerhalb der etablierten Erstattungswege gibt, wenn sie dem Patienten nachweislich helfen.

Thom Rasche | Unser gemeinsames Ziel ist die Stärkung der Patientenversorgung. Was zählt, sind Produkte, die dem Patienten tatsächlich helfen.

Und ich bin sicher: Am Ende des Tages folgt das Geld der Versorgung. ¤

ew companies and investors often have to share huge risks when developing new services or products for the healthcare system. The critical phase commences once the product has to prove itself on the market. Care providers and health insurers are amongst those responsible for decision-making. They have hardly participated in the innovation and financing process, to date, however. One reason for this is that the German Social Security Code (SGB) offers little in the way of freedom to make decisions on the use of insurance members’ money. Venture capitalist firm Earlybird managed to team up with BARMER and acquired the insurance provider as a strategic inves-

MEDTECH RADAR | Mr. Rasche, you brought in BARMER as a partner for your new fund. Why? Thom Rasche | This step has long been overdue. To date, health insurers in Germany have first gotten involved in the innovation process when it was too late, i.e. after the development of new products and the generation of initial turnovers, and at a stage when companies only began to ask if they could profit from a reimbursement or how to go about starting a pilot program with the health insurance provider.

In addition, there are certain differences between doctors and insurers that prevent them from working together efficiently. With our fund we would like to build bridges and bring together all stakeholders right from the beginning. Examples from other countries show that this model works.

Mr. Straub, what has motivated BARMER to act as a strategic investor?

Dr. Christian Straub | I was immediately enthusiastic when Mr Rasche came to me with the idea three years ago. I also knew that this would be a painstakingly difficult task. To make it clear: Our main

tion in the health care system in Germany, but also public-private partnerships. Our initiative falls precisely under this category. After longstanding talks we finally settled on a satisfactory result for all sides.

This meant that the capital of EUR 15m invested by us is secured in its entirety by someone else. It is not venture capital in the strictest sense of the word, but rather a secured investment. We were granted permission from the German Federal Social Insurance Office for this.

priority is not the investment, but enabling the earliest access possible to a broad number of new developments in healthcare, in particular in regard to the digitalisation of medicine. By participating in the fund we get the opportunity to evaluate a large number of promising business ideas early on, so that our insured members can profit from these as quickly as possible. In addition, we offer our experience – in the health market, its mode of operation and needs.

Thom Rasche | We assume that we will probably examine about 3,000 innovative companies. I am convinced that we will gain promising and exciting insights during the analysis, particularly as a result of BARMER’s market experience. The most interesting aspect of the partnership is the data set that insurances have regarding diseases. How often does a disease appear and how is that interpreted? How is the medical incident seen on the bill? How probable is market acceptance? All of the participants will learn and benefit from this.

MEDTECH RADAR | How did you regulate the partnership by law?

Dr. Christoph Straub | This involved a lengthy discussion process with the German Federal Ministry of Economics and the German Federal Ministry of Health as well as with our board. Typically, book no.

Which innovation will the

fund focus on?

Thom Rasche | Content wise the fund addresses classical medtech, diagnostics, health IT and digital health. Digital innovation is a particularly exciting aspect of the collaboration with BARMER as it could change healthcare processes. This applies particularly for the end of the healthcare chain, the ambulant sector, and here above all the cutting point between the first and second health market. Here currently happens a lot, although market access in this area is particularly challenging because of the complex reimbursement system in Germany.

Dr. Christoph Straub | Our main priority is to actively work towards progression of the medial sector using new innovative health services. We aim to make promising technologies accessible for statutory health insurance providers. In this case, it is worth examining where meaningful products or services exist outside of the established reimbursement pathways if they are to verifiably help patients.

Thom Rasche | Strengthening patient care is our common goal. Products that truly help patients are what counts. And I am certain that investing in the healthcare system will bring it long-term financial awards. ¤

Ob für den Patienten oder den Arzt, ob ambulante Versorgung oder im Krankenhaus – viele Start-ups bieten innovative Produkte und Dienstleistungen an, die die Gesundheitsversorgung entscheidend verbessern können. Doch der Weg in den Markt hat es in sich. | There are a range of start-ups offering innovative products and services that can significantly improve healthcare – whether they‘re for the patient or the doctor, in the ambulant or clinical sector. Market entry is a challenge, however.

ie Zukunft der Medizin hat begonnen – nicht zuletzt, weil zahlreiche Start-ups mit ihren Produkten die Gesundheitsversorgung verbessern wollen. „In der Gesundheitsindustrie sehen wir gute Ansätze im Zusammenspiel Old und New Economy“, sagt Michael Brandkamp, Geschäftsführer des High-Tech Gründerfonds (HTGF), und verweist insbesondere auf Bereiche wie Immuntherapie und Diagnostik, die auch von Firmen im Portfolio des HTGF abgedeckt werden.

Innovationen in der Diagnostik zielen insbesondere auf Schnelligkeit und Genauigkeit. So vermarktet die 2002 aus der Universität Regensburg ausgegründete Lophius Biosciences GmbH T-Zell-basierte Diagnostik-Tests im Bereich der Infektions- und Autoimmunerkrankungen – für künftiges Wachstum wurden im

Sommer weitere 4,25 Millionen Euro eingesammelt.

Mit dem frischen Kapital soll in einen In-Vitro-Diagnostik-Test zum besseren Nachweis einer aktiven Tuberkulose investiert werden. „Eine frühe Diagnose ist hier für Betroffene besonders relevant für eine nachhaltige Therapie, bisherige Methoden können das nicht liefern“, sagt Bernd Merkl, CEO von Lophius. Gleichwohl weiß er um die Hürden beim Markteintritt. „Die globale Vermarktung und die Zusammenarbeit mit den richtigen Partnern ist eine große Herausforderung für junge Firmen wie uns. Auch die rechtlichen Rahmenbedingungen für Erstattungen sind nicht optimal und sehr langwierig“, so Merkl. Ähnliche Erfahrungen hat auch die seit 2012 aktive oncgnostics GmbH gemacht. Das Start-up aus Jena setzt auf neue Diagnostiktests zum Nachweis von

Gebärmutterhalskrebs, die schneller und genauer als bisherige Methoden arbeiten. „Mit unserem Produkt GynTect® ermöglichen wir es Frauen, frühzeitig und nicht-invasiv Klarheit darüber zu bekommen, ob die Gefahr besteht, an Gebärmutterhalskrebs zu erkranken oder nicht“, sagt CSO Martina Schmitz. Denn der Krebs ist gut behandelbar, sofern er rechtzeitig erkannt wird.

Mit Crowdfinanzierung weiter wachsen Im Oktober 2015 wurde die Marktzulassung erreicht – noch bevor es Anfang 2017 eine neue, noch strengere Verordnung für die IVD-Zulassung gibt.

Schmitz: „Diese wird viele junge Unternehmen vor große Herausforderungen stellen.“ Bei ihrer Firma geht es nun um weiteres Wachstum. Dank einer Crowdfinanzierungsrunde, die bei Seedmatch 500.000 Euro eingebracht hat und von der bm-t Beteiligungsmanagement Thüringen GmbH um den gleichen Betrag aufgestockt wurde, ist die finanzielle Basis dafür gelegt.

Digital-Health-Firmen wie Sonormed müssen sich zunächst im Erstattungssystem zurechtfinden, weil ihre Lösungen nicht in das bisherige Raster der Regelversorgung passen. Das bedeutet: Der Dialog mit den Krankenkassen braucht viel Zeit. „Gerade kleine Start-ups haben aber oft gar nicht die notwendigen Ressourcen, die es für die

Healthcare Innovation: Sonormed GmbH develops digital medical products with a focus on audiology. The

company‘s first product “Tinnitracks” is a therapeutical app for tinnitus patients.

Systemintegration braucht“, berichtet SonormedChef Jörg Land. Er rät daher, sich nicht darauf zu verlassen, und das Geschäftsmodell entsprechend zu planen. Die Hamburger sind mit ihrer TinnitusApp inzwischen erfolgreich: Gestartet als Produkt für Selbstzahler, gibt es sie seit 2015 auf Rezept bei der Techniker Krankenkasse (TK) und mittlerweile auch weiteren Versicherungen. Unterstützung gibt es auch von den HNO-Ärzten. „Man muss die relevanten Mediziner mehrheitlich auf seiner Seite haben, sonst wird es schwer“, sagt Land. Geholfen hat dem Start-up auch, dass seine Software als Medizinprodukt zertifiziert ist und dass eine Wirksamkeit der Therapie durch wissenschaftliche Studien belegt ist. Schulwitz ist daher optimistisch: „Immer mehr Ärzte erkennen die Vorteile der Telemedizin.“

Mehrwert für Patienten und Ärzte schaffen David Schärf, Gründer und Geschäftsführer der OneLife, ist sich sicher, dass nur wenige Ärzte „digital komplett verloren“ sind. Problematisch sei die Vielzahl an verfügbaren Apps. Hier gilt es, herauszustechen und Mehrwert zu schaffen. Mit ihrer Femisphere-Lösung begleitet OneLife werdende Mütter durch die Schwangerschaft und die ersten zwölf Monate nach der Geburt. Die App merkt sich relevante Gesundheitsparameter, die hinterlegt wurden. Auf der Basis medizinisch-wissenschaftlicher Algorithmen werden die Eingaben in Echtzeit analysiert und Handlungsempfehlungen angeboten, auch mit dem Arzt kann sich vernetzt werden. „Gerade für Hochrisikoschwangerschaften ist ein regelmäßiger Check wichtig“, betont Schärf. OneLife bietet aber auch B2BLösungen. Gerade ist eine Kooperation mit einer arabischen Krankenkasse gestartet, die die Betreuung von Schwangeren verbessern will.

he future of medicine has begun and this is not only because a great deal of startups aim at improving healthcare with their new products. “We envisage positive approaches through the combination of old and new economy in the health care industry,” says Michael Brandkamp, CEO of High-Tech Gründerfonds (HTGF), who is in particular pointing at areas such as immunotherapy or diagnostics in which several of the HTGF portfolio companies are active. Innovation in diagnostics often aims at speed and precision. Lophius Biociences GmbH, which was founded as a spin-off

of the University of Regensburg in 2002, markets Tcell-based diagnostic tests in the area of infectious and autoimmune diseases. Over the summer, the company raised EUR 4.25m towards further growth.

That fresh capital will be invested in a new in vitro diagnostic test for the improved detection of active tuberculosis. “Early detection is crucial for a sustainable therapy for those involved. Existing methods cannot provide that,” says Bernd Merkl, CEO of Lophius. He knows the challenges of market access, however. “Global distribution with the right partners is a big challenge for a young company like ours,” continues Merkl. “In addition, the legal environment for reimbursement is not optimal and very lengthy.”

Growth throught crowd financing Oncgnostic GmbH, a company operating since 2012, has had similar experiences. The Jena-based startup focuses on new diagnostic tests to detect cervical cancer faster and more accurately than existing methods. “With our product GynText we enable women to determine early on and using a non-invasive procedure whether or not there is a risk of cervical cancer,” says CSO Martina Schmitz. This type of cancer responds well to treatment if detected on time. Market approval was received in October 2015 – in time before a new, stricter regulation for IVD approval will come into place. Schmitz: “This will be very challenging for a lot of start-ups.” Her company

is now on its way to further growth. Thanks to a crowd financing round at Seedmatch.de that brought in EUR 500,000, plus the same amount which was matched by investor Beteiligungsgesellschaft Thüringen GmbH bm-t, the financial foundation has been laid.

Compatibility with the reimbursement system .Digital health companies such as Sonormed have to be compatible with the German reimbursement system. This can be tricky if their services do not fit into the existing system of standard care. This means communication with health insurance providers is very time consuming. “Small start-ups in particular do not have the necessary resources for

The Hamburg-based firm has already enjoyed success with its tinnitus app: it began as a product for private customers, but is now available under prescription from the health insurance group Techniker Krankenkasse (TK) and further insurance providers.

The start-up has also been supported by ENT doctors. “You should have the relevant medical doctors on your side, otherwise it will be hard,” says Land.

The fact that the company had certified its software as a medical product and the effectiveness of the therapy was verified by scientific studies offered another additional advantage. Sonormed will focus more on digital process innovation in the long-term.

tor’s surgery. “Doctors still have trouble integrating the video consultation hour in their sequence of work,” states Patientus-CEO Nicolas Schulwitz.

However, it is progressing slowly. The service will resceive an own reimbursement code in summer 2017. The Berlin-based company works with health insurance providers such as the TK, AXA and AOK Nordost, as well as with the professional organisation of German dermatologists, a general practitioners union and many clinics. Schulwitz is optimistic: “More and more doctors are seeing the advantages of telemedicine.” David Schärf, founder and CEO of OneLife GmbH is certain that only a few doctors are “completely lost digitally”.

Treating chronic pain with electro-therapy The Bomedus GmbH, in operation since 2012, focuses on collaborating with external partners such as doctors and clinics. The spin-off from the University Clinic Bonn works in partnership with the life science incubator developing medical products for the treatment of chronic pain based on the electro-therapeutical procedure Small Fiber Matrix Stimulation (SFMS).

“Our products provide drug-free, non-invasive therapy for pain treatment,” says Bomedus-CEO Tobias Weigl.

“Thanks to clinically tested and certified algorithms we are able to significantly reduce different kinds of pains, ranging from acute knee pain to chronic back pain and so-called phantom pain.

These examples demonstrate that innovation is already beginning to change modern healthcare.

Joachim Schmitt from BVMed sees potential for improvements, however. “For the innovative capacity of the sector to continue, we need to readjust our reimbursement and evaluation systems according to the dynamic of technologies, so that we can immediately ensure that future patients will begin to profit from these. ¤

Creating additional benefits for doctors and patients One issue is the high number of apps available on the market. The company hopes to tackle this by rising above the masses and creating additional benefits. Its Femisphere solution OneLife accompanies expectant mothers throughout their pregnancy and in the first twelve months after birth. The app remembers any relevant health parameters that have been stored on it. The user receives an individual advice on recommended actions based on real-time analysis that is driven by medical algorithms. Im September 2016 hat der BVMed bei seinen Mitgliedsunternehmen eine Online-Befragung mit insgesamt 26 Fragen durchgeführt. Demnach sind die Medtech-Unternehmen sehr erfolgreich im Export.

Das Wachstum liegt hier bei rund 6%. Die Entwicklung im Inland ist dagegen mit einem Umsatzwachstum von 4% leicht rückläufig. (2015: 4,3%). Das Umsatzwachstum bleibt damit in Deutschland auf einem guten, wenn auch leicht sinkenden Niveau.

Erfolge im Export Die weltweite Umsatzentwicklung ist für die Unternehmen nach wie vor besser als die Entwicklung im Inlandsmarkt. 87% der befragten Medtech-Unternehmen rechnen weltweit mit einem besseren Umsatzergebnis als im Vorjahr. Aus den Umsatzangaben ergibt sich ein weltweites Wachstum der BVMedUnternehmen um 5,9%. Im Vorjahr waren es 6,8%. Die international tätigen Medtech-Firmen wachsen da-

mit im Ausland deutlich stärker als in Deutschland.

Die unterschiedliche Entwicklung spiegelt sich auch beim Ausblick auf das kommende Jahr wider. Für den deutschen Markt erwarten nur 26% eine bessere Geschäftslage. 20% erwarten schlechtere Geschäfte.

Der Ausblick fällt damit deutlich schlechter als im Vorjahr aus. Etwas besser sieht es beim Blick auf die weltweite Geschäftslage aus: 51% der Unternehmen erwarten hier eine bessere Entwicklung.

Standort Deutschland Als größte Stärke des Standorts Deutschland sehen die befragten Medtech-Unternehmen die im Land vorhandene Infrastruktur, beispielsweise die Verkehrswege, 84% bezeichnen diesen Aspekt als „sehr gut“ bzw. „gut“. Auch das hohe Versorgungsniveau der Patienten (72%) und gut ausgebildete Ärzte (65%) werden geschätzt (siehe Grafik unten). Schwächen des Standorts Deutschland sehen die Unternehmen vor allem bei der Erstattung. 60% bezeichnen das Erstattungsniveau in Deutschland als „eher schlecht“ bzw. „sehr schlecht“. Das spiegelt die Tatsache wider, dass Deutschland beispielsweise durch die Mechanismen des G-DRG-Systems weltweit die niedrigsten Preise für Implantate hat. Nur 16% schätzen das Niveau der Erstattungspreise in Deutschland als „gut“ ein. Als große Schwäche werden auch die Rahmenbedingungen für die Erstattung beurteilt, beispielsweise die sehr langwierigen Entscheidungsprozesse. Als wichtigste Hemmnisse der aktuellen Rahmenbedingungen in Deutschland werden der Preisdruck durch Einkaufsgemeinschaften und innovationsfeindliche Einstellungen von Krankenkassen bezeichnet. Als Belastung werden von der mittelständisch geprägten Branche die Anforderungen der neuen europäischen Medizinprodukte-Verordnung empfunden. Der seit

ie Medizintechnik-Branche ist sehr forschungsintensiv. Im Durchschnitt investieren die Medtech-Unternehmen rund 9% ihres Umsatzes in Forschung und Entwicklung.

Rund ein Drittel ihres Umsatzes erzielen die deutschen Medizinprodukte-Hersteller mit Produkten, die nicht älter als drei Jahre sind. Die MedtechBranche ist also sehr innovativ und hat sehr kurze Produktzyklen. Die Branche ist zudem sehr mittelständisch geprägt. 95% der Medtech-Unternehmen beschäftigen weniger als 250 Mitarbeiter. Insgesamt bieten die Unternehmen der Medizintechnologie in Deutschland fast 200.000 Menschen eine Beschäftigung. Jeder Arbeitsplatz sichert dabei zusätzlich 0,75 Arbeitsplätze in anderen Bereichen. Die Exportquote liegt derzeit bei 68% – Tendenz steigend.

Einheitliche europäische Nutzenbewertungsverfahren wünschen sich 67%. Außerdem sollten Versorgungsforschungsdaten transparenter gemacht und stärker genutzt werden (63%).

Hintergrund Der BVMed hat bei seinen Mitgliedsunternehmen im September 2016 eine umfassende Online-Befragung mit insgesamt 26 Fragen durchgeführt. Von den angefragten 230 BVMed-Mitgliedsunternehmen haben sich 81 Unternehmen beteiligt, darunter v. a. größere Hersteller von Medizinprodukten aus Deutschland und den USA. Die Unternehmen, die sich an der Umfrage beteiligten, haben ihren Hauptsitz zu 64% in Deutschland und zu knapp 20% in den USA. Die restlichen Unternehmen kommen aus dem europäischen Ausland (14%) und aus Japan (2%).

Weit oben

Ausblick Medizintechnologien werden stetig weiterentwickelt – unter aktiver Mitwirkung der Anwender, der Ärzte und Pflegekräfte. Die Innovationszyklen sind sehr kurz – im Gegensatz zum Arzneimittelbereich.

he medical technology sector is very research intensive. On average, medtech companies invest 9% of their turnover in research and development. German firms gain about one third of their turnover from products that have been on the market for less than three years, demonstrating that the Medtech sector is very innovative with short product cycles. In addition, the sector is characterised by small and medium sized companies. 95% of companies have less than 250 employees.

In partnership with its member companies, the BVMed carried out a comprehensive online survey with a total of 26 questions in September 2016. According to the survey, companies are very successful at exporting. Growth is at approximately 6%. Domestic growth is decreasing slightly, however, with a turnover of 4% (2015: 4.3%). As a result, growth in turnover on the German market is at a positive level,

but is declining somewhat. Growth in global revenue is better than that of its domestic counterpart for the companies surveyed. 87% of these companies believe there will be better global turnover, compared to the previous year. global growth at a rate of 5.9%. Growth was at 6.8% in 2015. Those companies that operate internationally are thus experiencing significantly more growth on a global scale than at home in Germany. These different growth rates are also reflected with regard to the forecast for 2017. While only 26% of companies expect an improved business situation, 20% think it will worsen. The outlook is therefore significantly bleaker than in the previous year. The global situation seems to be slightly better: 51% of companies expect improvements.

Germany as a location The greatest strengths specified by the companies in relation to the German healthcare system were good infrastructure (84%), high level of patient care (72%), and well-trained doctors (65%). Above all, firms see reimbursement issues as the greatest obstacle facing them. 60% complain the level is “rather bad” or “very bad”. This is also reflected by the fact that Germany has the lowest prices for implants globally because of the DRG-System. Only 16% consider the level of reimbursement as “good”. Above all, companies see reimbursement conditions as presenting the greatest challenge. They criticise increasing pricing pressure coming from purchasing groups and anti-innovative policies of health insurance providers. Most SME companies complain about the requirements of the new European Medical Product Regulations. BVMed

has been surveying the innovation climate index since 2011 and found it is declining this year. The areas considered to be most innovative are cardiology (48%), oncology (41%) and neurology (29%). In regard to health policy demands, the firms want better recognition of studies in the framework of benefit analysis for medical technologies. 81% consider this “very important” and “rather important”. 67% are calling for consistent procedures at a European level. Further priorities include introducing innovation at a quicker pace (77%) and increased industry involvement in the Federal Joint Committee G-BA (70%). In addition, data produced in health care research should be more transparent (63%). In spite of the complex domestic situation, the medtech sector continues to provide additional jobs. 66% of companies have created more jobs compared to the previous year and only 7% have made job cuts. 92% of the firms rate the job prospects for professionals in the medtech industry as “very good” or “good”. This is particularly true for economists, engineers and medical technicians. 85% of firms say they have open positions.

Medical technologies are continuously developed in combination with users and doctors. To preserve the innovative capacity of the sector, there is a need to readjust reimbursement and evaluation systems according to the dynamic of technologies, so that it is ensured that patients will benefit in future. ¤
