What are adult care and support services?

Care and support services are the part of the Council that's responsible for providing care and support, protection and help for vulnerable adults. We aim to provide choice, control and independence for people in Sheffield and support and protection for those who need it the most. We share this responsibility with other services like local health organisations (the National Health Service), housing providers and the police, and we work in close partnership with them. People who might need help from social care and support services for adults include • Adults aged 18 and over: • who require short term support after an operation, illness or fall • with a physical, sensory or cognitive impairment • with a long term illness or health condition • with a learning disability • with a mental health problem • Carers for people in any of these groups.

The Council also has responsibilities for children and families.

Our services If you think you need our help please contact the Adult Access Team on (0114) 273 4567 and choose option 5, then option 2, or email adultaccess@sheffield.gcsx.gov.uk. Or you can speak to the staff in our First Point reception at Howden House in the city centre. First Point is open 8.30am-5.30pm Monday to Friday.

Staff will talk to you about the problems you are having and the issues you are facing and how you can get care and support.

Short term support and reablement The Access Team will ask you about your problems. They can give you information and advice about organisations that may be able to help you, and identify if you only need some short term support (up to a maximum of six weeks) to help you regain your independence. We may be able to provide some reablement to help you to find new ways to cope with the problems you are having. For example, we can help if you are struggling at home after a fall or an illness, either by helping you to learn new ways to do things or using equipment to support you. This can help you increase your confidence and make sure you can maintain your contact with family and friends and make the most of your community.

Assessing Your Needs If you need support longer than six weeks we may agree with you for a social care worker to assess your needs. The social care worker will visit you at home to discuss your needs and listen to your views, the views of your family and the views of other professionals involved in your care and support, like your doctor. This is called a care and support needs assessment.

Your social care worker will make sure you are able to be involved in your assessment and care and support planning as much as you are able. If you have substantial difficulty in being involved in your assessment and care and support planning (such as difficulty understanding or retaining information), and you have no family or friends who can help, we will arrange for an independent person (called an advocate) to help you so you can be fully involved.

Your social care worker will ask about the help you already have from family or friends and any more needs you have. They will also collect basic information about you, like your contact details and your date of birth, and some information we need to ask you to meet statutory equality duties, like your religion and ethnicity.

Your social care worker will record your care and support needs on a form called the Assessment Questionnaire. You can see a copy of this form on www.sheffield.gov.uk/asc. The form includes personal our website: information about your health and background, so we keep this information securely. If you have health, housing or social care and support needs we may need to share your information with other agencies. We will always ask you first, unless we need to act quickly to protect you from harm.

We will then decide if you are eligible to receive care and support from us. We use national rules (called the Eligibility Regulations) to decide if you have any eligible care and support needs. Care and support to meet those needs makes sure you are safe, and that you are able to maintain your independence. You www.sheffield.gov.uk/asc. can find more details on this from our website

After your assessment Whatever the level of care and support needs you have, we will work with you to identify how we can reduce your needs or prevent them getting worse.

We want to help you be as independent as possible, so we first look at how you can help yourself, and support you get from family, friends and your community.

If the assessment shows you still have eligible care and support needs, and there is no one available to help with those needs, we will provide support.

If you have no eligible care and support needs we will help you to remain safe, independent and well by giving you information and advice about community and voluntary services that can help you.

What if I don’t want an assessment?

If you don’t want to have an assessment you will not be eligible for support from the council. There are many other services in Sheffield that can offer you support directly without an assessment. However, you will have to pay any charges for these services. The Support Available section, on page 9, includes details for some of these.

Planning your care and support If you have eligible care and support needs we will help you to decide how your needs can be met and the outcomes you want to achieve. The activities and services you need to meet your needs are recorded on a form called a Care and Support Plan. You can see a copy of this form on our www.sheffield.gov.uk/asc. We will discuss with you whether website: we should arrange and pay for the services in your Care and Support Plan (this is called Council arranged support), or whether you have money paid to you or a family member or friend as a Direct Payment so that you can organise and pay for your care and support yourself. Your plan can include Council arranged support and a Direct Payment.

Paying towards your care and support We will also carry out a financial assessment to work out how much you can afford to pay towards the cost of your care and support.

The general rule is that you either pay the total cost of your care and support, or you contribute what your financial assessment shows you can afford and the Council pays the rest. Organising your support

When your plan is complete we will send you a copy, together with a copy of your Assessment Questionnaire. We will help you to arrange your care and support, and let you know if you need to pay towards the cost of your care and support. We will then check with you after a few weeks to make sure that your care and support is working and meeting your needs. This will provide an opportunity for you to let us know how your arrangements and services are working.

Reviewing your support Your Care and Support Plan is then reviewed regularly to make sure you still need care and support, and that your support is meeting your needs. You can also ask for a review if you find you no longer need care and support or your situation changes. There's more information about reviews on our www.sheffield.gov.uk/asc. website.

Support available The next sections in this guide give information about the different types of support available in Sheffield to help people live independently. Some contact details have been included however this is not a complete list.

Home support There are many different home support services available in Sheffield that may help you meet your needs. These may include personal care (including dressing, washing, bathing and toileting), mobility and moving around your home, meals, medication, laundry, some cleaning tasks, managing your money (such as collecting your pension), care at night time or a shopping service.

Meals delivery service To make sure you get regular meals and nutrition, you can have your meals delivered to your door.

Frozen meals You can have frozen meals delivered to your home once a fortnight to store in your freezer. Equipment and technology There is a wide range of equipment and technology available to support you to do the things you want to do more easily and with less risk. You can buy equipment from shops or catalogues or online. You can also request some equipment from your doctor. There is also technology available to help you to feel safer and more secure in your own home, such as a care alarm service.

Buying equipment If you’re struggling to do everyday things around the home there are lots of pieces of equipment that might help. Many stores now sell useful equipment to make life easier at home. At the following local specialist shops you can see and try out equipment for making daily life easier.

Equipment and Adaptations Service Our Equipment and Adaptations service can assess people who are experiencing difficulties. There are policies in place to make sure we meet the needs of those who need it most. We may be able to help you if: •Y ou can’t manage to do essential things and there’s no other way of getting them done.

•Y ou’re struggling to manage but you, or a carer, are at serious risk of injury, and there’s no other way of getting things done.

If it seems you would be better off getting help from someone else we’ll tell you who to get in touch with.

You can ask for help from the Equipment and Adaptations service by ringing our Adult Access Team on (0114) 273 4908. Depending on the risks you face, it can take up to three months to assess you at home. You may need a financial assessment and pay towards the cost of your care (see page 7).

You will need to be assessed as needing some equipment on loan, and qualify for this free service. The British Red Cross can buy, deliver, install, demonstrate and maintain loan equipment.

Help from your doctor Making daily life easier can be as simple as a visit to your doctor or community nurse. They can offer lots of help and advice to staying independent, including •M obility aids to help you get around, such as walking frames, crutches or a referral for a wheelchair assessment.

• I ncontinence equipment to help with problems getting to the toilet on time, such as incontinence pads or a commode, either through the district nurse or continence service.

•S pecialist equipment for someone who needs nursing care at home, such as a specialist bed or hoist.

Loan or hire of equipment Sometimes you only need equipment for a short time. You can borrow equipment for up to 8 weeks from the British Red Cross, Unit 24, Jessops Riverside, 800 Brightside Lane, Sheffield S9 2RX. Equipment is mainly wheelchairs but includes other items. There is no charge for the service; however you will be invited to make a donation.

Wheelchairs can also be loaned for free from Shopmobility, for a day or for a longer time. They also have a wide range of three and four wheeled electronic mobility scooters available for loan for a small fee, starting at ÂŁ3.00.

Getting things fixed Minor Works Grants for repairs up to ÂŁ2000 are available to people aged 75 and over and claiming qualifying benefits. Contact the Adaptations, Housing (0114) 273 4646 for further information. and Health Team on If there are little jobs that need doing to make your home safer, you could get in touch with the Handyperson Service. They work with older and disabled people and can do small jobs, like fit hand rails on stairs or grab rails, for about half the usual cost.

City Wide Care Alarms Our City Wide Care Alarms service can help people to remain safe, secure and independent in their own home, 24 hours a day, 365 days a year. We provide a portable alarm button and a base unit, which you can hear and speak through, thanks to its powerful microphone and loudspeaker.

If you need help, all you have to do is press the alarm button and it automatically connects you to our call-monitoring centre. Our trained staff have instant access to your information and will quickly provide the most appropriate help, whether that is your friend, relative, doctor, our specially trained mobile support workers or the emergency services.

The full service, including installation, maintenance, repairs and response from our mobile support workers, if needed, is just £4.93 (ex VAT) per week, or £5.50 (ex VAT) per week to provide a service via mobile phone technology.

Additional automated alarms We can also provide a range of automated alarms, which work with the main unit and can help manage specific risks in the home, such as the risks associated with falling or memory loss. These alarms could provide the extra help needed to make sure people can stay in their own home for longer.

Respite Respite care is provided for people to give them a break, or to give their carer a break. This can be provided: • • • •

in your own home in a residential or nursing home by our Sharing Lives service (see next page) at a lunch club or community support service (for day time and evening activities).

LEARNING DISABILITY: Short Break service The Learning Disabilities Service provides a Short Break Service to adults with a learning disability. Short breaks are a way for you or your carer to have a break, and can include: • Support in your own home • Overnight stays • Supported holidays or weekends away • Days out • Evening or weekend activities in the community A short break can be provided in one of our short break care units, where our staff are available 24 hours a day. We can provide regular breaks throughout the year, such as one weekend every month.

We have specialist facilities to support people with profound and multiple learning disabilities.

Sharing Lives This service offers opportunities for adults, who need additional support, to live with, or spend time with, approved and trained carers and their families. This can help you to live as independently as possible with help from your approved carer.

Criteria - You need to have been assessed by a social care worker and have needs that are eligible for Sharing Lives support.

What you can expect - You will receive a service that is tailored to your identified care requirements. You will be carefully matched with an approved carer, based on your needs, mutual interests and the carer’s skills and abilities.

You will be able to give feedback and make comments to help improve and develop our service.

All potential carers are assessed by qualified staff. All approved carers are Disclosure and Barring Service (DBS) checked and trained to national standards. Our approved carers are reviewed and supported on a regular basis. The service offers: • Long term accommodation and support in the carer’s home.

• Short break respite care in the carer’s home.

• Day time support in the carer’s home.

• Sitting service in your own home to give your usual carer a break • Befriending, where a carer helps you do the things you like to do If you are interested in accessing any of our services please contact your social care worker who will arrange for an assessment of your needs.

Supported Accommodation Sheltered Housing Sheltered housing is specially designed for older people who wish to live independently. Sheltered schemes vary but can provide • s elf-contained flats, studio apartments or bungalows • communal facilities such as a lounge for social activities, gardens, kitchen, laundry and security and safety features • warden or scheme manager support service • caretaking service for the communal areas • 24 hour emergency response through an alarm service.

Care Homes (Residential and Nursing) Residential care Residential care is for people who can’t continue to live in their own home, even with support. You can stay in a home for a short-time (known as respite care) or live there permanently. You would get your own room, and have staff available to help you 24 hours a day. Homes normally provide accommodation, meals and personal care such as help to wash and dress, and with going to the toilet. Staff at the home can also help you gain skills to help you be independent. There are specialist homes for people with a learning disability, a physical disability or sensory impairment, for older people and for people with a mental health problem.

Nursing Care Nursing homes provide care like residential homes, but they also provide 24 hour nursing care by trained nursing staff. The person in charge must be a qualified doctor or nurse.
