This guide has been produced to help you in selecting a care home. It contains information and advice about what you need to consider when choosing a home and it provides a list of homes available within the Wakefield district.

We hope that you find this information useful and if you can think of ways we can improve this guide please let us know. We have tried to answer as many questions as possible and also tell you about what other services may be available to you that can help you stay independent.

The information in this guide was correct at the time of printing, July 2014. For amendments and updates to this information, please contact your social worker who will be able to help.

The care home list only includes care homes that have entered a contractual agreement with Wakefield District Council and the extended boundary area. For homes located outside of Wakefield district and the extended boundary area, or who do not have a contractual agreement with Wakefield District Council, please contact your social worker who will help you find an appropriate placement. Information on these can be obtained from the Care Quality Commission, or by contacting the relevant local authority.

Personalisation – your life, your choice A new way to plan your care and use the services you need If you think you, or the person you care for, needs some additional care and support, the starting point for creating a personalised package of support is asking for an assessment. To do this you can call Social Care Direct on 0345 8 503 503 or speak to your social worker.

An assessment finds out about your needs and whether you are eligible for funding from Wakefield District Council, under the Fair Access to Care Services Criteria. Once the assessment is complete you will then write a care plan with your social worker.

A personalised plan - Personalisation is about making sure you are involved in your care plan so that you get the support you need in a way that best meets your needs. Making your own individual, personalised support plan may mean you get the support you need in your own home.

Or, it could mean the chance to build a more personalised support package if you choose to live in a care home.

Personal budgets and personalisation After your assessment, you are allocated a personal budget, based on your needs. This budget can then be spent how you choose, in order to meet those needs.

Who will manage your budget?

The thought of managing a care budget may seem daunting but there is help to do this. Alternatively, you may decide that you would prefer Wakefield Council to manage this for you. Either way you will have support to make sure you are getting what you need from your personal budget.

How to spend your budget?

The key to a personal budget is that you have a choice of the service you ‘buy’ and a choice of where you buy it from. You can receive services directly from Wakefield Council, or buy them from Housing, local NHS services, or from an independent organisation or person.

There are many different services available that can support you to remain as independent as possible. Some are well known – such as help with the housework and personal care, equipment and adaptions, meals at home and emergency alarm systems.

Care homes Your assessment may show that a care home may be the best place for you to receive the care and support you need, in order to live a safe, comfortable and fulfilling life. If you agree with this, then you will be involved in choosing which care home you would like to live in.

What is a care home?

A care home can be a residential or nursing home or both. These homes can provide the personal help that people need 24 hours a day and seven days a week. Nursing homes also employ nurses to look after people’s medical needs. Some homes cater for people with different needs, for example people over 65, people with dementia, older and younger people with physical disabilities and people with learning disabilities.

How do I know which home to choose?

The best way to choose a home is to go and have a look round and talk to the people who live there. A list of homes that Wakefield Council contracts with and the different needs they cater for is included in this directory. You are welcome to arrange to visit them.

We have included a useful checklist that you can use when you visit the homes. It outlines the sort of things you should be looking for and the questions that you might ask.

All homes are inspected every year by the Care Quality Commission to make sure that they provide the care that people need. When you visit a home ask to see their annual inspection report. However, before you start to do this, you or the person you care for needs to have been assessed as needing to move into a care home.

Care at home Your assessment may show that some help and support may help you to stay independent in your own home. Wakefield Council and the NHS provide a range of services that may help. These include: • Home care services – personal assistance at home with washing, bathing, using the toilet, dressing and undressing, getting up and going to bed, laundry, shopping etc. You can expect that the person who comes to your home to help you with your personal care will be trained to do these tasks. You will be asked how you would prefer to be helped with very personal matters and your wishes will be noted and respected.

• Rapid response service provision in a crisis – This is a temporary service for around six weeks where a higher level of home care and support will be delivered to you to allow you to recover from a temporary illness.

• Day care and day time activities – Here you will receive a meal and be able to socialise or carry out activities away from your home.

• Short break provision and respite care – This service 8

allows the person who would normally take care of you a chance to have a break from their caring role.

• Provision of meals at home – the community meals service can provide you with fresh and frozen meals delivered to your door seven days a week.

• Rehabilitation services - includes services called ‘intermediate care services’ which helps to support people when they leave hospital and help to prevent inappropriate hospital or long stay admissions.

• Equipment or adaptations to your home - If you need your home altering to make it easier for you to manage daily tasks and personal care we may be able to help. Our Occupational Therapists can visit you to carry out an assessment of your needs and discuss with you ways to meet those needs by the use of specially adapted equipment.

• Care Link emergency alarm system - Care Link is an emergency alarm system that provides peace of mind and promotes independence for anyone living in Wakefield. Care Link has a range of community alarms and equipment that can help you keep your independence and stay in your own home.

09 • Community health services, support from District Nurses, Community Psychiatric Nurses, your GP, etc.

• Sheltered Housing and Extra Care Sheltered Housing services – These consist of a group of homes that have been specially designed with the needs of older people in mind. A sheltered scheme may be a group of flats or bungalows, or sometimes a mixture of both.

Every flat or bungalow has modern facilities and is smaller and easier to maintain than normal housing.

In sheltered housing you will also have the reassurance of knowing that the Scheme Manager is on hand in an emergency.

• All sheltered schemes have a communal lounge and social activities organised by the Scheme Manager. You can join in the activities or enjoy the company of friends in the communal lounge - and of course you can retreat to the privacy of your own flat or bungalow whenever you wish.

• Community transport/bus passes/ car badges – This allows you the opportunity to get out of your home and to go shopping or visit a friend. (Please note that this is not a complete list of services available).

Services may be provided directly

by Wakefield Council’s Adults, Health and Communities Services, Housing, local NHS services or by an independent organisation under contractual agreement. There may also be a charge for some of these services but you will always be informed how much you will have to pay on your assessment. New ways to find support – Connect to Support Wakefield now has a website where you can find out about and buy services and support offered from a range of organisations. The address for the Connect to Support website is www.connecttosupport.org/wakefield.

Using the Connect to Support website service users, carers, the general public and social work staff can: • view and buy goods, products and care or support services from local and national providers. This includes accommodation, home support, leisure

and activity opportunities, equipment, health and wellbeing services.

As an example of what’s on offer, you could arrange for a carer to support you to social occasions you regularly attend, take part in new social opportunities, receive companionship at home or arrange a telephone medication prompt service.

Finding a Personal Assistant You may decide that employing a PA best meets your needs. This might be because you feel the services on offer from other agencies don’t do what you need. Or it could be that choosing your own PA is important to you.

A PA can help you carry out the tasks that have been identified in your support plan. This could be help with personal care, household tasks or getting out and about.

Shared Lives Shared Lives is another way that you can be supported to live more independently in the community.

It is a flexible scheme that works by matching you with a Shared Lives carer recruited from the community who can give you the support you need.

It can be used in a variety of ways, from short breaks to living with a carer permanently.

If you are an older person, you can use Shared Lives for a short break, staying as a guest in the home of a Shared Lives family or single person. This gives you a change of scene and gives you a break perhaps from being on your own, while receiving some support.

Shared Lives can also be used as a long term placement where a vulnerable person can live with a Shared Lives carer or family on a long-term basis.

Dementia An increasing number of people, particularly over 65, are being diagnosed with dementia. There are about 4,000 people in the Wakefield district known to have dementia – but there may be many more that have yet to be diagnosed. Although there is no cure for dementia, with treatment and support many people are able to lead active and fulfilling lives.

What is dementia?

Dementia is not a single illness but a term used to describe a group of related symptoms caused by damage to the brain, the most common is Alzheimers. Different types of dementia affect the brain at different rates and in different ways.

Dementia is not an inevitable part of ageing.

Signs of dementia The condition develops over time and so it is not always easy to spot in the early stages. Other factors such as someone’s personal circumstances and the environment in which they live will affect their experience of dementia.

Over time the symptoms of dementia become more obvious. These can include situations where the person: • has difficulty in remembering things, particularly things that happened recently • repeats things or loses the thread of what's being said • has increased difficulty in understanding choices and making decisions • becomes confused about what is going on around them, even in a familiar place • forgets the names of everyday objects and places • finds it more difficult to reason and communicate for instance difficulty in following a conversation or TV programme • feels anxious, depressed or angry about memory loss and confusion.

Can dementia be prevented?

High blood pressure, heart problems, high cholesterol and diabetes can increase the chances of developing some types of dementia, so it’s important to maintain an active and healthy lifestyle.

The sooner symptoms are discussed with a doctor, other treatable conditions can be ruled out. If it is dementia, diagnosing early means getting the right support to help people live well for longer.

Recognising it, getting help and support to deal with it A diagnosis of dementia can be a shock. However, there is much that you can do in the early stages that can help to make life easier and more enjoyable, both now and in the future.

If you have concerns that you or someone close to you is experiencing symptoms see your doctor.

Your GP practice or medical centre may also have services that can help you.
