Services to help you stay independent

Adult social care is changing to give people more choice, flexibility and control over their own lives to help them live independently. This change is called Personalisation. It’s about making sure you are involved in your care plan so that you get the support you need in a way that best meets your needs. Making an individual, personalised support plan may mean you get the support you need in your own home. Or, it could mean the chance to build a more personalised support package if you choose to live in a care home.
Assessment The starting point for creating a personalised package of support is asking for an assessment. To do this you can call Social Care Direct on 0845 8 503 503 or speak to your social worker. An assessment finds out whether you have support needs that are eligible for funding from Wakefield District Council, under the Fair Access to Care Services Criteria. If you are eligible you will then write a care plan with your social worker. An amount of money will be set aside to pay for the care needs outlined in your plan. This money is called your personal budget. You can then choose: • who will manage your budget • how you spend the budget in order to meet your care needs. In addition to the personal budget funding you are allocated from Wakefield Council, you can choose whether to add any money to the personal budget, perhaps from other allowances or grants you receive or from your personal money. These amounts combined are known as your individual budget.
5
Who will manage your budget? The thought of managing a care budget may seem daunting but there is help to do this. Alternatively, you may decide that you would prefer Wakefield Council to manage this for you. Either way you will have support to make sure you are getting what you need from your personal budget.
How to spend your budget? The key to a personal budget is that you have a choice of the service you ‘buy’ and a choice of where you buy it from. You can receive services directly from Wakefield Council, or buy them from Housing, local NHS services, or from an independent organisation or person. There are many different services available that can support you to remain as independent as possible. Some are well known, such as personal assistance services at home – that can help with the housework and personal care, equipment and adaptions, meals at home and emergency alarm systems.
New ways to find support - Connect to Support You can find out about these services and many more by visiting a new website called Connect to Support to help you choose and buy the support you need from a range of care providers. The website address is www.connecttosupport.org/wakefield. Using the Connect to Support website service users, carers, the general public and social work staff can: • view and buy goods, products and care or support

services from local and national providers. This includes accommodation, home support, leisure and activity opportunities, equipment, health and wellbeing services. As an example of what’s on offer, you could arrange for a carer to support you to social occasions you regularly attend, take part in new social opportunities, receive companionship at home or arrange a telephone medication prompt service • find information, guidance or advice around support • connect to the PA Framework to find a Personal Assistant (PA).
Finding a Personal Assistant You may decide that employing a PA best meets your needs. This might be because you feel the services on offer from other agencies don’t do what you need. Or it could be that choosing your own PA is important to you. A PA can help you carry out the tasks that have been identified in your support plan. This could be help with personal care, household tasks or getting out and about. The PA Framework is an online database from which people with a personal budget can find a PA to meet their social care needs. You can place an advert on the website or view the PAs currently available. All of the PAs on the database have a current DBS check (formerly known as a CRB check). There is also general information available to help you understand your role as an employer. If you would like to find out more about employing a PA visit www.wakefield.padatabase.org or call 0300 111 1977. Your social worker will also be able to help you find out more. The two websites are not provided or managed by Wakefield Council and therefore the Council cannot recommend specific providers.

Social Care Personalisation – a quick guide
8
Personalisation: this is the move across all public services to meet the individual needs of customers by helping people have a say in what they need and by helping them take control of their care and support. Personalisation includes health and social care services and includes the care people receive in residential and nursing care homes. It means you have a greater choice on what support you receive and how you get it.
individual budget is spent.
Personal budgets: following an assessment, this is the amount of money you may receive to pay for your care and support, as identified through your care plan. This can be used to buy services from Wakefield Council, Housing, local NHS services or by an independent organisation. A budget can be managed for you by the Council, or you can choose to manage it yourself.
Assessment: an assessment aims to build a full picture of you, your current situation and what things might be like in the future. It looks at the support you currently receive and what you may need help with in addition to this. The assessment is also gives time for you to say what you want to change or improve and what support you think would help.
Individual budget: this is the total amount of money you choose to spend on your care and support needs. It may include your personal budget amount, plus money from other funds or an amount from yourself.
Care Plan: this is the plan that you make with your social worker after your assessment. It describes the things you would like to be able to do, the support you need in order to do that and who will provide that support.
Self-directed support: this is one way to get personalised support. This is where you have financial control of your budget and choose and buy the support you need to meet your health and social care needs. Self-directed support gives you as much on-going control as you want on how your
Connect to Support: this is a new website where people can choose and buy services from their personal budget.

Direct payment: a direct payment can be used to employ staff- for example a PA. You can buy services from a local authority, an agency or a charity. Examples of services you can buy are meals delivered to your home or joining a weekly social group. Or you could buy equipment to help make it easier to get around your home or outside.

Shared Lives Shared Lives is another way that you can be supported to live more independently in the community. It is a flexible scheme that works by matching you with a Shared Lives carer recruited from the community who can give you the support you need. It can be used in a variety of ways, from short breaks to living with a carer permanently.
Shared Lives can also be used as a long term placement where a vulnerable person can live with a Shared Lives carer or family on a long-term basis.
The scheme can be used for a few hours each week to support you while your usual carer or family member has a short break.
If you would like more information about Shared Lives visit www.wakefield.gov.uk/SharedLives or call 01924 302 681.
Notes
10
If you are an older person, you can use Shared Lives for a short break, staying as a guest in the home of a Shared Lives family or single person. This gives you a change of scene and gives you a break perhaps from being on your own, while receiving some support.
