This guide is intended to help you, your carer and your relatives understand how your local health services can help you after discharge from hospital. Also information to help you to live independently or access support and care services in your area.

Caring for you in hospital and in the community For people living in south Cumbria and north Lancashire, there are three general hospital sites; Furness General Hospital in Barrow in Furness, the Royal Lancaster Infirmary in Lancaster and Westmorland General Hospital in Kendal.

University Hospitals of Morecambe Bay NHS Foundation Trust (UHMBT) operates these three sites, providing planned care for patients with a wide range of medical and surgical needs. These include outpatient services, diagnostics, therapies, day case and inpatient surgery. Furness General Hospital and Royal Lancaster Infirmary also have Accident and Emergency departments, while Westmorland General Hospital has a Primary Care Assessment Service.

Hospitals are the right place for patients that require specialist or urgent treatment. However once this treatment is complete, the best place to continue your care is often at home or in a community setting.

The “Supporting Patients’ choices to avoid long hospital stays” policy, which was launched locally in August 2017, helps hospital staff to effectively manage and plan for any ongoing care patients might need once they are well enough to leave hospital. This process begins from the moment of admission and continues once you have left hospital.

It is a good idea to start planning for your discharge as

soon as you arrive in hospital, or even before you arrive if possible. This will help the hospital discharge teams to accurately assess what your needs are likely to be once you leave hospital, and allow enough time for any arrangements, such as equipment, services, a domiciliary care package or care home to be in place for when you are ready to leave hospital.

If you are admitted to hospital, you will be made aware of how long you are expected to be in hospital at the beginning of your stay so you will be able to plan better with your family and/or carer.

For most patients, they will return to their own home when they leave hospital but for those with additional health and care needs, they may need further support out of hospital.

For these patients, the hospital staff will work closely with them and their families and/or carers to help them understand their options.

Whilst the hospital Trust will do everything possible to ensure patients are discharged to their preferred setting, this may not always be immediately available. In these cases, an alternative option will be explored. Although this may not be ideal, our teams will continue to work towards the patient’s preferred choice of accommodation in order to transfer patients at the earliest opportunity.

This new way of working will help you and your family to make informed decisions about your next steps after leaving hospital.

What Is Discharge Planning?

When you are admitted to hospital, your treatment plan, including details for discharge or transfer, is developed and discussed with you. A discharge assessment will determine whether 4

you need more care after you leave the hospital.

• You have any medication or other supplies you’ll need

Discharge planning is the process used to decide what a patient needs for their transition from hospital to their home or another community care setting. This process is completed by the hospital discharge team, with support from social workers, nurses, case managers, and other relevant teams in the hospital and wider community.

• You’ve been trained how to use any equipment, aids or adaptations needed

Will I be discharged before my care has ended?

No, we will never discharge a patient that still requires acute hospital treatment. The Consultant in charge of your care will assess your health regularly during your time in hospital, and will only give approval for your discharge once you are well enough to leave. Once you no longer need acute hospital treatment, you will be discharged to the most suitable setting for your needs. This could mean that your treatment continues at home or in the community, for example if you need physiotherapy sessions or a course of antibiotics.

• You have money and any keys necessary for your destination A family member or care should also ensure that: • You have appropriate clothes to wear to your destination • Suitable transport is arranged to get you there If you are being discharged to a care home, the care home should also be told the date and time of your discharge, and have a copy of the care plan.

What happens once I have been discharged from hospital?

Your care should be monitored and reviewed as set out in your care plan. The care plan should also include details of who to contact if things don’t work as planned, this is usually a next of kin.

Once people no longer need hospital care, it is best to get home or to another community setting as quickly as possible because:

If your care plan includes community care services from a local authority, it should check that their care package is working well within two weeks of discharge. If you live alone, this should take place within the first few days of discharge. Following this, your care plan should be reviewed at least annually.

Why do I need to leave hospital?

•O nce people are aged 80 years and over, ten days spent in a hospital bed equates to ten years of muscle wasting

What will happen when I am ready to leave hospital?

Once you are well enough to leave hospital, the person co-ordinating your discharge should make sure that: •Y ou (and a carer if you have one) have a copy of your care plan. Your care plan should include details of any treatment or support you will be getting once you have left hospital, including who will provide it and how often • Any carers or family members will be available if needed • Your GP is notified in writing

You have several options to raise concerns, but first of all we would ask that you speak with the Doctor involved in your care, Ward Manager or Matron in the department. Many problems can be resolved quickly by talking things over.

If you do not want to discuss your concerns with the staff or their manager, or if you have tried this and are unhappy with the outcome, please ask to speak to the Patient Advice and Liaison Service (PALS).

Providing health and wellbeing services across Lancashire Lancashire Care provides health and wellbeing services for a population of around 1.4 million people, including community services such as health visiting, podiatry, sexual health and dentistry as well as inpatient and community mental health services. The Trust covers the whole of the county and employs around 6,500 members of staff across more than 400 sites.

Here are some of our community services for adults in Central Lancashire:

Intravenous Therapy Service – Home Administration This service is an extension of the current outpatient parenteral antimicrobial therapy (OPAT) service that allows us to treat a limited number of patients at home. It is particularly helpful for frail or immobile patients, and people who are unable to return regularly to hospital for outpatient antibiotics.

Integrated Neighbourhood Teams Integrated Neighbourhood Teams provide coordinated nursing and specialist care to patients with complex and long-term conditions in the community. The service comprises the following teams: Community Therapies, Community Matrons and District Nurses. District Nurses visit patients with various nursing needs that include general wound care, pressure area and catheter care, palliative and end of life care, and other medical and surgical interventions. Community Matrons use specialist clinical diagnostic and assessment skills for patients with long-term conditions, with the aim of them selfmanaging their conditions to reduce unnecessary hospital admissions. The teams work alongside GP practices, local hospices, nursing and residential care homes and treatment rooms.

24 hour, 7 day a week community nursing services Community nurses are available 24 hours a day, 7 days a week. They are able to conduct overnight visits to patients with a range of nursing needs that include symptom management, end of life care and wound care. The team meets any unpredicted nursing needs that might arise and works alongside GP practices, Community Matrons, local hospices, nursing and residential care homes and treatment rooms.

Rapid Response The Rapid Response team is therapy led, supporting patients with various support needs, such as patients who may need a frame to allow them to move around at home, or other clinical situations requiring home based support. The team is also able to access community beds where the person is unsafe to remain in or return to their own home.

Community Rehabilitation Services The Central Lancashire Rehabilitation service currently provides a range of therapy services across Integrated Neighbourhood team footprints. Therapy services provided by the team include: Occupational therapy and physiotherapy provision to residential rehabilitation beds, domiciliary rehabilitation; Rapid Response function for urgent and admission avoidance work; Falls Service/Steady On for patients at risk of falling or have previously fallen.

Cardiac & Respiratory The Cardiac and Respiratory Service was formed in 2013 and comprises the Respiratory Intensive Home Support (COPD IHS), Pulmonary Rehabilitation and Heart Failure services. COPD IHS is a community based nurse and physiotherapy service. The service is mainly for patients with COPD and is a sub-acute service designed to manage the patient for a period of stabilisation. Pulmonary Rehabilitation is a community based physiotherapist-led service providing Pulmonary Rehabilitation over six weeks.

Heart Failure is a community based nurse led service currently delivered by a combination of community clinics and home visits.

Diabetes Service The Diabetes Service covers the Preston and South Ribble area, and works with patients who live outside of this area in the acute setting at Royal Preston Hospital and also patients who are under the care of the local Diabetologist in the community. The service works closely with diabetes dietitians, consultants and podiatrists.

Rheumatology The Rheumatology service is a multi-disciplinary consultant led service caring for patients with a wide range of Rheumatological conditions. The service offers a wide range of treatment on site, including outpatient clinics, joint injection and intravenous infusion clinics, and a wide range of therapist interventions including physiotherapist and occupational therapist led groups and acupuncture.

The right place for your care Your local hospital in partnership with other hospitals in the region, and your local authorities, Blackpool Borough Council and Fylde and Wyre Councils, recognise and support each patient to receive the right treatment, at the right place and by the right professional.

An acute hospital Trust is a hospital that provides consultant led health services within the National Health Service.

Acute care is for patients who require emergency medical or surgical services within a hospital environment.

Blackpool Teaching Hospitals NHS Foundation Trust provides care for patients who require this level of care.

Hospitals are the right place to be when you are in need of specific medical or surgical treatment. However, when your treatment has been completed, it is important that your stay is not delayed for the following reasons: • Beds are needed for people who are very unwell, and who may be waiting in the Accident and Emergency department for a bed to become available • People awaiting surgery, both urgent and non-urgent, may have their operations cancelled if a bed is unavailable • There is less risk of acquiring an infection at home compared to busy public health buildings such as hospitals • Some people find it harder to return home the longer they stay in hospital.

It is good practice to start planning for your discharge as soon as you come into hospital or even before you arrive. This means that we can begin to: • Assess what your needs are likely to be when you are ready to leave • Involve relevant staff who can help in meeting those needs (e.g. social worker, occupational therapist, 8

physiotherapist, district nurse) • Make arrangements for equipment or services which need to be in place when it is deemed safe for you to be discharged • Make sure that your carer is given any information, help or advice that is needed to aid your recovery following your discharge.

speak to your pharmacist or a member of the nursing team. Relatives or carers coming to collect you can meet you or a member of the ward staff can escort you to the car park entrance.

All staff will work with you and your carer or relatives, as a team, in order to discuss your discharge with you.

You can also help us by moving from hospital when you no longer need our services. We will discuss with you the sort of help you and your carer may need when you return home, to make the transition as easy as possible.

We need to discuss any concerns you have at the earliest opportunity, e.g. your safety at home, managing your personal care or domestic arrangements, your ability to move around and any equipment you may need.

You will be given an expected date for discharge based on when it is expected that your treatment will be completed.

It is important that everyone involved is aware of this date so that we can all work towards it, and so that the necessary arrangements can be made.

Some of the arrangements may include: • T ransport home. Patients are normally expected to arrange their own transport • Suitable clothing and footwear if you are not already using them in hospital • Access to a key to your property • Adequate food stocks • Adequate heating in your home • Adequate support for your carer or family members.

A supply of your current medication will be ordered from the Hospital Pharmacy and delivered to you.

Finally, we understand that any hospital stay can be stressful and we need to support you and your carer during your time in our care.

Your Hospital Discharge Coordinator is the member of the team who is available to meet you on the ward and, along with the ward staff will begin planning your discharge from hospital.

For most patients, being discharged from hospital to their home is straightforward. Some may need information and advice or may need the support of social services.

Some people have ongoing health and social care needs which require a more detailed assessment. Staff from health and social care will work together to plan and deliver services to support such people after discharge.

This guide explains some of these processes and explains the role of the hospital and Social Care team in the hospital discharge process.

Leaving hospital You will be transferred from hospital when your consultant led team decide that you are clinically ready to leave hospital and that you no longer require an acute hospital bed.

For the majority of patients, you will go home from hospital with no additional support required. In some cases you may require some extra support to regain your independence. Your hospital team will help you, your family and carers to access these services if you need them.

Your safe and timely transfer from hospital will also allow new patients who need acute hospital treatment to be admitted without delay.

We fully support your transfer out of hospital for the following reasons: 1. To support you to regain your independence

2. Some people find it harder to return home the longer they stay in hospital 3. Acute beds are needed for people who are very unwell, and delays in transfer may result in patients waiting in the Emergency department for a bed to become available.

4. Although we work hard to ensure the highest standards of care there is a risk of acquiring infections in hospital. Leaving hospital as soon as you are fit and safe to do so means this is less likely and reduces the risk of any complication.

Your Hospital Team A Multi – disciplinary team which may include Hospital Discharge Coordinator, Consultant team, Matron and ward nurses, Therapists, and Social Workers will begin to plan your transfer from hospital as soon as you are admitted.

This team of clinical experts will explain their key roles to you if they are involved in your hospital care.

The Assessment Process The assessment process will start with discussions with you and your family/carers. This process of information gathering and clarification of the situation will indicate which members of the multi disciplinary team need to be involved in your assessment.

The team will work with you and your relatives/carers to:

• Provide you with information about services that could support you as you are discharged from hospital.

Therapies You may be referred to a therapist to assist in planning for your discharge from hospital.

This could be any of the following; a dietician, occupational therapist, physiotherapist or a speech and language therapist. They will discuss with you and assess what you are able to do; and what you might need more help with. They will then discuss with you what treatment, help or support is available to improve your independence.

The therapists will discuss with you and the other members of the hospital team their findings and together with you and your family make a plan for your discharge from hospital.

Out-patient appointments If your Doctor needs to see you in an outpatient clinic to review your progress following discharge, you will either be given a note of your appointment before leaving the ward or be notified by post. If you find that you are unable to make that appointment after discharge please let the appointments office know so that we can reschedule the appointment and offer your time to someone else.

Intermediate care Across Blackpool, Fylde and Wyre, we have access to services called Intermediate Care. These services can support people who no longer require acute care, but still require some further support to ensure they are able to function with their daily activities.

This period of intermediate care can provide rehabilitation from Therapy staff or a period of reablement prior to returning home. Your hospital Team will work with you to assess your level of need and which intermediate care service would suit your needs should you require it.

All patients who no longer need acute care but cannot return home and do not require specialist care are eligible to enter the intermediate care pathway if they meet the following criteria: ver the age of 18 •O •R esident of Blackpool, Fylde or Wyre or •R egistered with a Blackpool, Fylde or Wyre GP. Intermediate care is provided under Health and Social Care joint Commissioning arrangements and is free of charge whilst you are on the intermediate care pathway.

People may enter onto the intermediate care pathway following an episode

of acute care for a further short period of recovery and reablement. This can benefit patients and enables a more accurate assessment of your ongoing care needs prior to returning home or before long term care is considered.

Aiming to get you home The aim is to help you to relearn daily living skills so you do not need long-term support or go back into hospital.

You need to know what you want to achieve and how this will be done to establish your reasons for going into Intermediate Care. The multidisciplinary team assessment will help you make this decision. First route out of hospital should always be your own home. If this is not possible, then Intermediate Care aims to get you home and to eliminate dependency on long term support.

The Assessment & Rehabilitation Centre - ARC The Assessment & Rehabilitation Centre (ARC) is based in a quiet residential area of Blackpool and offers social care rehabilitation for people who need a short period of intensive support in a 24-hour setting.

Type of Support Both social care and health professionals work to provide individually tailored support programmes for anything from a few days to six weeks to help people maximise their independence.

The service is regulated by the Care Quality Commission.

Thornton House: Thornton House is owned

by Lancashire County 10

Council and managed by Care Services.

It is registered to provide care for 44 older people with varying needs of care and support, including specialist dementia care.

All residential and domiciliary services provided by Care Services are registered with the Care Quality Commission (CQC) and comply with Lancashire County Council’s preferred provider requirements.

Residential Rehab: This service provides residential care with specialist support to help individuals regain capacity and independence as far as possible in their own home with or without direct therapy involvement.

The service can be up to 6 weeks and is available for people who are resident in Fylde & Wyre. Access is via assessment from the Hospital Discharge Team or your Social Worker.

Rapid Response Team: The Rapid Response Service is a nurseled service for adults who meet the eligibility criteria in Blackpool, Fylde and Wyre. The service provides an alternative care option interfacing between primary and secondary care, aiming to prevent hospital admission or facilitate early discharge.

Rapid Response is an integrated team of health and social care professionals that prevent hospital admissions and facilitate early hospital discharge of patients who meet specific criteria with a clinical need.

Patients are required to have been seen by the referring personnel in the previous 24 hours. The service aims to provide an alternative care option to hospital.

Early Supported Discharge The Early Supported Discharge Team is a rehabilitation service for individuals within the Blackpool, Fylde and Wyre

areas. Patients are referred into the ESD service by therapists within both Blackpool Victoria and Clifton Hospitals.

The ESDT consists of occupational therapists, technical instructors, staff nurse, physiotherapists, administrator and a team leader. The service is for adults who are medically stable but who would require a longer stay in hospital in order for them to return to their previous level of independence / function. By facilitating an earlier discharge home the ESDT increase the throughput of patients within both hospitals, reduce in patient length of stay, reduce the risk of patients developing hospital acquired infections and provide intensive rehabilitation for up to six weeks within patients own homes.

Our commitment to you • P atients should receive the right treatment, at the right place and by the right professional •W e place a high priority on keeping your stay in an acute hospital bed to a minimum •O nce your consultant led team assesses that you are medically fit for transfer, we will aim to transfer you from hospital on the same day •Y ou will be transferred from the Blackpool Teaching Hospitals Trust when you are ready to leave hospital as there are more appropriate services and facilities available to you •W e will aim to transfer you from hospital before 10am on your day of transfer and if you require transport, we will arrange this for you.

When you leave hospital It is important to know that the majority of patients will be transferred home directly from hospital.

If you no longer require consultant led care and there is a delay in the start of your care package or preferred community based placement, you

cannot choose to remain in an acute hospital bed.

You will be transferred to a bed within a non hospital setting (within 48 hours) whilst this is arranged.

We will only transfer you when the appropriate placement has been identified and confirmed.

Giving the hospital feedback Our Trust is committed to listening and responding to the views of our local population. Whether you are a patient who has received treatment with us, a carer, or an interested member of the public, it’s important you tell us what you think about our services - good and bad. So we know what we’re doing right and what we could do to improve.

There are several ways in which you can give us your feedback including: The Friends and Family Test - Before you leave one of our sites you may be asked to fill in a very short survey called the ‘Friends and Family Test’ which asks if you would recommend our services to a friend or relative. You can also fill in this survey on our website or via telephone as long as it’s within 48 hours of your discharge.

Spot surveys - Patient experience volunteers may approach you on the wards or in day clinics to find out if you are satisfied with your care so we can address any concerns you may have whilst you’re still receiving treatment.

Postal surveys - A paper questionnaire may be sent to you at home shortly after an inpatient stay, an outpatient appointment, an A&E attendance or a home visit. It will be sent with a free

post envelope to return it to us.

Via staff: if you have a concern request to speak to the service or ward manager or the matron for the area if you are in hospital. For out of hours concerns please call the Trust switchboard on 01253 300000 and ask for the out of hours matron.

Via telephone: Contact the Patient Relations Team on 01253 65 5588 / 5589. The team offer impartial advice or support relating to any aspect of your care and deal with any concerns or complaints the Trusts receives.

NHS continuing healthcare Some people with long-term complex health needs qualify for free social care arranged and funded solely by the NHS. This is known as NHS continuing healthcare.

Where can NHS continuing healthcare be provided?

NHS continuing healthcare can be provided in a variety of settings outside hospital, such as in your own home or in a care home.

Am I eligible for NHS continuing healthcare?

NHS continuing healthcare is for adults. Children and young people may receive a “continuing care package” if they have needs arising from disability, accident or illness that can’t be met by existing universal or specialist services alone.

To be eligible for NHS continuing healthcare, you must be assessed by a team of healthcare professionals (a multidisciplinary team). The team will look at all your care needs and relate them to: • what help you need • how complex your needs are • how intense your needs can be • how unpredictable they are, including any risks to your health if the right care isn’t provided at the right time Your eligibility for NHS continuing healthcare depends on your assessed needs, and not on any particular diagnosis or condition. If your needs change then your eligibility for NHS continuing healthcare may change.

You should be fully involved in the assessment process and kept informed, and have your views about your needs and support taken into account. Carers and family members should also be consulted where appropriate.

A decision about eligibility for a full assessment for NHS continuing healthcare should usually be made within 28 days of an initial assessment or request for a full assessment.

If you aren’t eligible for NHS continuing healthcare, you can be referred to your local council who can discuss with you whether you may be eligible for support from them.

If you still have some health needs then the NHS may pay for part of the package of support. This is sometimes known as a “joint package” of care.

Information and advice The process involved in NHS continuing healthcare assessments can be complex. An organisation called Beacon gives free independent advice on NHS continuing healthcare.

NHS continuing healthcare assessments Clinical commissioning groups, known as CCGs (the NHS organisations that commission local health services), must assess you for NHS continuing healthcare if it seems that you may need it.

For most people, there’s an initial checklist assessment, which is used to decide if you need a full assessment. However, if you need care urgently – for example, if you’re terminally ill – your assessment may be fast-tracked.

Initial assessment for NHS continuing healthcare The initial checklist assessment can be completed by a nurse, doctor, other healthcare professional or social worker. You should be told that you’re being assessed, and be asked for your consent.

Depending on the outcome of the checklist, you’ll either be told that you don’t meet the criteria for a full assessment of NHS continuing healthcare and are therefore not eligible, or you’ll be referred for a full assessment of eligibility.

Being referred for a full assessment doesn’t necessarily mean

Full assessment for NHS continuing healthcare Full assessments for NHS continuing healthcare are undertaken by a multidisciplinary team (MDT) made up of a minimum of 2 professionals from different healthcare professions. The MDT should usually include both health and social care professionals who are already involved in your care.

You should be informed who is co-ordinating the NHS continuing healthcare assessment.

The team’s assessment will consider your needs under the following headings: • breathing • nutrition (food and drink) • continence • skin (including wounds and ulcers) • mobility • communication • psychological and emotional needs • cognition (understanding) • behaviour • drug therapies and medication • altered states of consciousness • other significant care needs These needs are given a weighting marked “priority”, “severe”, “high”, “moderate”, “low” or “no needs”.

If you have at least one priority need, or severe needs in at least 2 areas, you can usually expect to be eligible for NHS continuing healthcare.

You may also be eligible if you have a severe need in one area plus a number of other needs, or a number of high or moderate needs, depending on their nature, intensity, complexity or unpredictability.

In all cases, the overall need, and interactions between needs, will be taken into account, together with evidence from risk assessments, in deciding whether NHS continuing healthcare should be provided.

The assessment should take into account your views and the views of any carers you have. You should be given a copy of the decision documents, along with clear reasons for the decision.

Fast-track assessment for NHS continuing healthcare If your health is deteriorating quickly and you’re nearing the end of your life, you should be considered for the NHS continuing healthcare fast-track pathway, so that an appropriate care and support package can be put in place as soon as possible – usually within 48 hours.

Care and support planning If you’re eligible for NHS continuing healthcare, the next stage is to arrange a care and support package that meets your assessed needs.

Depending on your situation, different options could be suitable, including support in your own home and the option of a personal health budget.

If it’s agreed that a care home is the best option for you, there could be more than one local care home that’s suitable.

Your CCG should work collaboratively with you and consider your views when agreeing your care and support package and the setting where it will be provided. However, they can also take other factors into account, such as the cost and value for money of different options.

NHS continuing healthcare reviews If you’re eligible for NHS continuing healthcare, your needs and support package will normally be reviewed within 3 months and thereafter at least annually. This review will consider whether your existing care and support package meets your assessed needs. If your needs have changed, the review will also consider whether you’re still eligible for NHS continuing healthcare.

Q.

I have a local authority support package that works well. I’m now eligible for NHS continuing healthcare – will my support package change?

If you’re concerned about changes to your care package because of a move to NHS continuing healthcare, your CCG should talk to you about ways that it can give you as much choice and control as possible. This could include the use of a personal health budget, with one option being a “direct payment for healthcare”.

Q.

Can I refuse an assessment for NHS continuing healthcare? If I refuse, will I be able to get services from my local authority?

An assessment for NHS continuing healthcare can’t be carried out without your consent, so it’s possible to refuse.

However, if you refuse, although you’ll still be entitled to an assessment by the local authority there’s no guarantee that you’ll be provided with services. There’s a legal limit on the types of services that a local authority can provide.

If you refuse to be assessed for NHS continuing healthcare, the CCG should explore your reasons for refusing, and try to address your concerns. If someone lacks the mental capacity to consent to or refuse an assessment, the principles of the Mental Capacity Act will apply and in most circumstances an assessment will be provided in the person’s best interest.

Q.

Refunds for delays in NHS continuing healthcare funding

My relative is in a care home and has become eligible for NHS continuing healthcare. The CCG says the fees charged by this care home are more than they would usually pay, and has proposed a move to a different care home. I think a move will have a negative effect on my relative. What can we do?

CCGs will normally make a decision about eligibility for NHS continuing healthcare within 28 days of getting a completed checklist or request for a full assessment, unless there are circumstances beyond its control.

If there’s evidence that a move is likely to have a detrimental effect on your relative’s health or wellbeing, discuss this with the CCG. It will take your concerns into account when considering the most appropriate arrangements.

If the CCG decides you’re eligible, but takes longer than 28 days to decide this and the delay is unjustifiable, they should refund any care costs from the 29th day until the date of their decision.

If you’re not eligible for NHS continuing healthcare If you’re not eligible for NHS continuing healthcare, but you’re assessed as requiring nursing care in a care home (in other words, a care home that’s registered to provide nursing care) you’ll be eligible for NHS-funded nursing care.

This means that the NHS will pay a contribution towards the cost of your registered nursing care. NHS-funded nursing care is available irrespective of who is funding the rest of the care home fees.

If the CCG decides to arrange an alternative placement, they should provide a reasonable choice of homes.

Q.

Is it possible to pay top-up fees for NHS continuing healthcare?

No, it isn’t possible to top up NHS continuing healthcare packages, like you can with local authority care packages.

The only way that NHS continuing healthcare packages can be topped up privately is if you pay for additional private services on top of the services you’re assessed as needing from the NHS. These private services should be provided by different staff and preferably in a different setting.

Personal budgets If your local council agrees to pay for some or all of your home help, it must offer you choice and control over how your needs are met.

You’ll be given a personal budget, and can choose to receive it as a direct payment.

Personal budgets

Alternatively, you may prefer your personal budget to be managed by the council or by someone else, such as:

A personal budget is the amount of money the local council allocates for your care, based on its assessment of your needs.

• a friend or family member (the local council must agree to this)

You can be put in charge of this budget either by telling the council how you would like it spent, or by the council giving you the money so you can directly pay for your own care (a direct payment).

• a broker, independent social worker or an advocate

It can also be given to a separate organisation (such as a usercontrolled trust) that will spend the money on your care as you see fit, if you prefer.

You can choose a combination of the above (for example, a direct payment with some council-arranged care and support), often called a mixed package.

Direct payments Direct payments give you the most control over your care.

If you’re unhappy with the care you’re getting, you can decide to change it without having to go through the local council.

But you have to be able to account for how you spend the budget. And you have responsibilities as an employer if you hire a personal assistant with your direct payment.

The pros and cons of taking charge of your personal budget Being in control of your own personal budget means you have to spend some time and effort thinking about your care and support needs and the outcomes you want.

There may be extra responsibilities. For example, if you decide to request a direct payment to cover the cost of homecare, you could use the payment to hire an individual, giving you the responsibilities of an employer.

Alternatively, you could hire care workers through an agency. This removes the legal obligations of being an employer, but may cost more and may remove some of the benefits of having the same person provide your care.

How to receive your personal budget Clarify how you want to receive your personal budget.

If you choose to receive it as a direct payment, the local council may pay the money straight into a bank account that you control (you must set up a new bank account to do this) or they may give you a pre-paid card.

• your care manager or social worker Discuss these options with your social worker or carer to work out which option is best for you.

If someone else will be looking after your money, you may need to create a decision-making agreement. This should state how they’ll look after your money and what decisions they can and can’t make on your behalf.

Keep receipts The council may want to check what you do with your money to make sure you’re spending your budget appropriately, and your care and support needs are being met.

You may need to keep receipts (especially for large purchases) so you can show them you have spent your money responsibly.

If you choose a direct payment, the council will give you a direct payment agreement that sets out the terms and conditions.

If you’re struggling to manage your money, the council can advise you on how to make your money work best for you.

Disagreements about care plans and personal budgets If you have been told that you’re not eligible for services, or you don’t agree with the amount allocated to you in your personal budget, you have the right to complain.

Speak to your social worker about being re-assessed, or phone your local council social services department and request a complaints form.

Your council should have a formal complaints procedure on its website.

If you’re not happy with the way the council handles your complaint, you can take it to the local government and social care ombudsman. An ombudsman is an independent person who’s been appointed to look into complaints about organisations.

NHS-funded nursing care NHS-funded nursing care is when the NHS pays for the nursing care component of nursing home fees.

The NHS pays a flat rate directly to the care home towards the cost of this nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, ask your CCG to review the decision.

Who is eligible for NHS-funded nursing care?

NHS-funded nursing care is paid at the same rate across England. In April 2018, the rate was set at £158.16 a week (standard rate).

You may be eligible for NHS-funded nursing care if: • you’re not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse • you live in a nursing home

How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

The cost of NHS-funded nursing care

Before October 1 2007, there were 3 different levels or bands of payment for NHS-funded nursing care – low, medium and high.

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

Most people don’t need a separate assessment for NHSfunded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you.

If you moved into a care home before October 1 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. In April 2018, the higher rate was set at £217.59 a week. You’re entitled to continue on this rate unless:

Outcome of the assessment

• you no longer have nursing needs

If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home. Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

• you no longer live in a care home that provides nursing • your nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £158.16 a week, or • you become entitled to NHS continuing healthcare instead.

Paying for your own care (self-funding) You will not be entitled to help with the cost of care from your local council if: • you have savings worth more than £23,250

The needs assessment is free and anyone can ask for one.

Benefits can help with care costs

• you own your own property (this only applies if you’re moving into a care home)

You may be eligible for benefits, like Attendance Allowance and Personal Independence Payment (PIP), which aren’t means-tested.

You can ask your council for a financial assessment (means test) to check if you qualify for any help with costs.

You can use them to pay towards the cost of your care.

You can choose to pay for care yourself if you don’t want a financial assessment.

How to arrange your care as a self-funder You can: • arrange and pay for care yourself without involving the council • ask the council to arrange and pay for your care (the council will then bill you, but not all councils offer this service and they may charge a fee)

Find out what care you need Even if you choose to pay for your care, your council can do an assessment to check what care you might need. This is called a needs assessment.

For example, it’ll tell you whether you need home help from a paid carer for 2 hours a day or 2 hours a week and precisely what they should help you with.

Can I avoid selling my home?

You won’t have to sell your home to pay for help in your own home.

But you may have to sell your home to pay for a care home, unless your partner carries on living in it.

Sometimes selling your home to pay care home fees is the best option.

But there may be other ways to pay care home fees if you don’t want to sell your home straight away.

Releasing money from your home (equity release) Equity release lets you take money that’s tied up in your home without selling it. It’s available if you’re over 55.

But you have to pay interest on the money you take out, which can be expensive.

A typical hourly rate for a carer to come to your home is around £20, but this will vary depending on where you live.

Having a carer who lives with you costs from around £650 a week. But it can cost as much as £1,600 a week if you need a lot of care.

Paying for a care home There are 2 types of care home: • residential homes have staff that help with everyday tasks such as getting dressed and supply all your meals • nursing homes also offer 24-hour nursing care A room in a care home costs: • £600 a week in a residential home • £840 a week in a nursing home The price will vary according to where you live and the type of care you need.

Help around the home comes in many forms and there are many names used to describe it, including home help, homecare, care attendants, and “carers” (not to be confused with unpaid family or friends who care for you).

Home help can suit you if you need: • personal care, such as help with getting washed and dressed • housekeeping or domestic work, such as vacuuming, cleaning and doing laundry • help with cooking or preparing meals or “meals on wheels” • nursing and healthcare • companionship Home help can be very flexible. The same person or agency may be able to provide some or all of these options for the duration of your care: • long-term 24-hour care • short breaks for an unpaid family carer

Funding homecare If you believe you might benefit from some help at home, contact your local council’s social services department to ask for a needs assessment.

If you’re eligible for help at home funded by your local council, your local council may provide or arrange the help themselves.

Alternatively, you can arrange your own care, funded by the council, through direct payments or a personal budget.

If you choose direct payments or a personal budget, or you aren’t eligible for council funding and want to get care privately, you can arrange it in several different ways: • using a homecare agency • hiring a personal assistant • getting homecare from a charity, such as Age UK

• a list of national homecare organisations • s upported living services – these are services that can help you stay safe and well in your home on a long-term basis, including financial help, help with medicines, advocacy, social support and practical support • a place to live with a family who will care for you – this is known as shared lives services or adult placement services

Home help agencies If you use a homecare agency, you or the person looking after you has to find the care agency and pay them.

The agency will provide a service through a trained team of care workers, which means you may not always have the same person visiting your home, although the agency will do its best to take your choices into account.

Homecare agencies can also: • take over the burden of being an employer – for example, payroll, training, disciplinary issues and insurance • train their homecare workers through national qualifications and service-specific training • replace workers when they’re ill, on holiday or resign • put things right when they go wrong An agency will want to see you and the person looking after you so they can assess your needs.

Questions to ask when using a homecare agency

This also means you can make a joint decision about the right type of care and support.

Before deciding to go ahead with an agency, you should ask questions about the fee and what it covers.

How much does a homecare agency cost?

These include:

Using a homecare agency can be expensive. The agency will charge a fee on top of the payment made to the care worker to cover their running costs and profits.

You normally have to make a regular payment to the agency, which includes both the worker’s earnings and the agency’s fee.

• what training and supervision do they provide?

• what’s their complaints policy?

• who’s responsible for insurance?

• i s there any out-of-hours or emergency contact if needed?

•w ill they be able to provide staff if your own care worker is ill or away? If an agency is contracted to provide care every day, it must do that

Hiring a personal assistant Instead of using an agency, you can hire a personal assistant to act as a homecare worker for you.

Personal assistants can offer you all that you’ll get from an agency worker, but you’ll also get continuity, familiarity and an ongoing relationship with an assistant.

But if you employ a personal assistant, you have the legal responsibility of an employer. This includes arranging cover for their illness and holidays.

Home help from charities Charities such as Age UK and Carers Trust can provide home help.

Carers Trust also supports carers by giving them a break from their caring responsibilities through homecare services.

Marie Curie Nurses can provide practical and emotional support for people near the end of their lives in their own homes.

Complaining about homecare You have the right to make a complaint about any aspect of homecare.

It’s a good idea to ask a homecare agency or charity about their complaints policy before you start to use their carers.

If you are finding it difficult to make a complaint, find out how an advocate can help you.

Driving and using public transport Although mobility problems make it harder to get around, transport has become more accessible for disabled and elderly people.

You can get help: • using public transport • getting discounts on public transport • from the NHS with travel costs • finding community transport schemes If you have mobility problems and you need a car to get around, you may be able to get help with costs through: • discounted or free road tax • a Blue Badge parking permit • help with the cost of leasing a vehicle through the Motability scheme

Using public transport with a disability All public transport vehicles have to be “accessible” so disabled passengers can use them. Public transport also has to accept guide dogs or assistance dogs.

But if you’re using public transport, it’s worth contacting the transport operator before you travel to make sure they’re able to offer the assistance you require.

Buses and trains usually have priority seating for older people and people with disabilities. They also usually have space and wide doors for wheelchairs. Some buses, trains and trams are fitted with automatic ramps.

London Underground is being upgraded to improve step-free access.

Staff at Underground stations are also trained to help people move around the underground system – for example, by helping you avoid escalators and calling ahead to arrange for assistance at your destination.

Discounts on public transport for older people and disabled people Older people and people with disabilities can travel free on local buses anywhere in England between 9.30am and 11pm Monday to Friday, and at any time during the weekend and on bank holidays.

Some authorities offer free travel for longer, and some allow a companion to travel with the pass holder for free.

You may have to apply through your local authority, but in most areas you can apply online for an older person’s bus pass or for a disabled person’s bus pass.

If you often travel by train, it’s probably worth getting a Disabled Railcard Card. This gives you a third off the price of rail tickets.

Check the criteria to see if you’re eligible.

Children aged 5 to 16 with disabilities are eligible for a Disabled Person’s Railcard. This lets an adult to travel with them for a third of the cost of an adult fare, while the child pays the normal child fare.

Taxi and private hire companies can provide wheelchairaccessible vehicles if you ask for one when you book a vehicle.

Some councils also give free taxi vouchers to people who find it difficult to use public transport because they’re frail or disabled.

Cars and parking Road tax reductions If you’re disabled or have a serious long-term condition, you might be eligible for a reduction in your road tax, or even be exempt from it altogether.

Blue Badge disabled parking scheme If you have severe mobility problems that make using public transport difficult, you may be able to get a Blue Badge parking permit for your car.

This lets you park closer to places you wish to visit, such as in marked disabled parking bays.

You may also be able to: • park for free within certain time limits in some places • park on single and double yellow lines • stay longer in on-street time-limited parking bays Blue Badge schemes are run by councils. Most councils will let you apply for a Blue Badge online.

Central London is exempt from the national Blue Badge regulations and the central London boroughs of Kensington and Chelsea, Camden, Westminster and City of London don’t fully operate the Blue Badge scheme.

But if you’re exempt from road tax or have a Blue Badge permit, you may be able to get an exemption from paying the central London congestion charge.

NHS help with travel and transport costs If you pay to travel to a hospital or other NHS premises for NHS-funded treatment or diagnostic tests, you may be able to claim a refund of reasonable travel costs.

Community transport schemes These schemes provide transport to and from hospitals, doctors’ and dentists’ surgeries, and opticians and chiropodists.

Many areas offer a free “Dial-a-Ride” service, offering door-to-door transport for people unable to use normal buses.

Motability scheme The Motability scheme allows disabled people to use their mobility benefits to lease a car, powered wheelchair or scooter.

You can also pay an extra amount of money if you want a more expensive vehicle.

The Motability Scheme is open to anyone who gets: • the higher rate mobility component of Disability Living Allowance (DLA) or Personal Independence Payment (PIP)

Some schemes require you to register and may charge a fee. Carers may be able to travel with you for an additional charge.

• the War Pensioners’ Mobility Supplement (WPMS)

Check with your local council whether your area has a community transport scheme.

• the enhanced rate mobility component of Armed Forces Independence Payment

The British Red Cross runs a similar transport service in some areas.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

CQC’s Chief Inspector for Adult Social Care, Andrea Sutcliffe always uses ‘The Mum Test’: is a care home safe, caring, effective, responsive to people’s needs and well-led? In other words, is it good enough for my Mum (or anyone else I love and care for)?

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive. They should also be capable and confident in dealing with your particular needs. You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

Welcome to Age UK Lancashire We are a local independent charity dedicated to providing support and information to all older people living in Lancashire.

We believe that it is important to enable older people and their carers to have support to remain independent and to live interesting and fulfilled lives. We offer a range of services to help older people across Lancashire make more of their lives. Our care services are available to help frailer older people or those who feel lonely or isolated. These include access to social groups, exercise classes, activity groups, Home Help services and much more assist older people in remaining active, independent and linked in to their local community.

Our mission is to make Lancashire a great place to grow old and aim to support as many older people across the county as we can. We are able to provide services by generating income from; shops, trading, fundraising, donations and grants. Without the support of the general public we would not be able to continue the vital work we do.

Information and Advice Age UK Lancashire provides an information and advice service for older people aged 65+ and their carers. The service is free of charge, confidential, independent and impartial. We are able to help you with a range of issues.

This may be about claiming benefits, housing or health and care issues or local services. You may be facing a crisis, or

just considering your options. We value diversity, promote equality and challenge discrimination in all that we do. We respond to an individual’s needs without regard to their gender, sexual orientation, disability, race or religion.

Hospital Aftercare Service This service provides short term practical and emotional support to adults, 18 years and over. Our well trained staff and volunteers provide help and support to individuals on discharge from hospital and rehabilitation units. We provide support such as crisis shopping, assistance with light household duties, emotional support, providing information and advice etc. We also offer supported discharge from hospital to your own home. This service can be accessed by self-referral or through hospital and NHS staff.

Daytime support We provide support in a comforting and welcoming environment to encourage social interaction, inclusion and engagement in a variety of meaningful activities. This is a full day with lunch and refreshments provided. Enhanced and dementia support is also available from some of our centres, which can provide carers with much needed respite during the day. This service is provided by qualified support workers. There is a cost to this service.

Lancashire Older Veterans The Service aims to improve the quality of life and wellbeing of older veterans aged 65+. Our team of Navigators will help older veterans to identify their needs and overcome a range of barriers to accessing support.

We can provide access to social activities and groups, practical support at home, information and advice.

Dementia Support This service enables individuals and their carers with memory problems to maintain or re-develop daily living skills, have opportunities to be involved in educational, leisure or creative activities and be supported in maintaining or developing contacts in their own community.

Home Help The service includes household chores, cleaning, shopping, assisted shopping and help with general tasks.

We accompany to appointments, social events, activities; we support with interests and hobbies; Just tell us what you would like us to do, as we can be flexible in meeting your needs. There is a cost to this service.

Charity Shops Our shops are crucial to the work of our organisation.

They help us raise much-needed cash which we can use to provide services which are not funded by other agencies. They are also a great place to find good quality clothes, books and other items and very reasonable prices and also provide information on a range of issues relevant to older people. Anyone interested in donating items to be sold can drop things in to their local shop.

Donations & Fundraising To continue our work for older people across the county, Age UK Lancashire is dependent on the generous support of the people of the local area and beyond.

Although we receive grants for some of our activities, we have no regular guaranteed income.

Ways to support us are: making a one off or regular donation, donation in memory, participating in our fundraising events, taking on your own challenge or organising your own fundraiser, leaving a gift in your Will or a legacy. 100% of the money donated and raised stays in Lancashire.

Case Study

One way in which a person can feel lonely We recently met a gentleman & his wife at the Hub in Lancaster.

Mr K explained that his wife had recently been diagnosed with Vascular Dementia. (They are both in their early 70s) It was in September this year that Mr K started to notice that Mrs K was becoming forgetful in particular with times and places. It was this that led to Mr & Mrs K‘s GP making a referral to the memory clinic. Having now had the appropriate assessments, Mrs K is being supported in her own home and Age UK Lancashire’s Dementia Support Team are providing access to appropriate events in their own local community, which are all working very well.

The sad part to this synopsis is what Mr K stated to me that he has now “lost the women he married to vascular dementia & that there are many occasions that he feels very lonely.

Advice Line

Mrs P was living on her own since her husband of 47 years passed away. With no family close by Mrs P felt very isolated and low on confidence. This led to Mrs P not wanting to go out and socialise or even visit the local shops. Having previously been quite active and involved with local community groups, the affect to Mrs P’s mental wellbeing became progressively worse until her daughter, who lives abroad, decided to contact Age UK Lancashire to look for support.

The Advice line team arranged a telephone meeting with Mrs P and were able to provide her with information about Age UK Lancashire and other services that would help her to get out of the house and gain confidence.

Mrs P decided to try out the Home Help service to help with jobs around the house and as a consequence the regular visits helped her regain confidence to speak to people and want to start going out. Age UK Lancashire was able to put her in contact with a local library group who meet weekly and encouraged her to go.

Mrs P now enjoys the weekly group and has met some new friends who act as a support network for each other.

What is a Dementia Friend?

A Dementia Friend learns a little bit more about what it’s like to live with dementia and then turns that understanding into action – anyone of any age can be a Dementia Friend.

What is a Dementia Champion?

A Dementia Friends Champion is a volunteer who encourages others to make a positive difference to people living with dementia in their community.

They do this by giving them information about the personal impact of dementia, and what they can do to help.

What is a Dementia Friendly Community (DFC)?
