When you’re ready to go home, if you are over 50 and a Liverpool or Sefton resident then we are here to help and support you.

Information & Advice

If you have any queries or concerns we provide a free information and advice service and if we don't know the answer we will signpost you to the perfect local agency who will be able to help. We can even recommend traders from handymen to decorators from our trusted local trades register to deal with those jobs around the house which need to be done. If you need any advice or information, please give us a call.

Sefton Reablement Service

Our Sefton Reablement service provides excellent one to one support at home to tackle your issues and concerns, regain skills and confidence and get you back involved in your community. If you are a Sefton resident our talented Befriending and Reablement Officers will work with you to identify the challenges you face and then work with you over the coming weeks to address them.

Befriending and Companion Groups

Linked closely to our Sefton Reablement service is our Befriending Service and Companions Group. Loneliness and isolation can be an issue when you leave hospital and are recuperating or struggling with long term health issues. We operate a befriending service in Sefton and Liverpool so if you would benefit from a regular chat on the phone or a visitor to share a cup of tea and a chat please get in touch. We also hope to re-establish our regular Companion Group meetings and fun activities across Liverpool and Sefton this year once Covid restrictions are eased, so if you would like to get involved please call with your details and we will let you know when they are up and running.

Adjusting to New Circumstances

Leaving hospital can sometimes be a time of reflection for some older people when they find that their home no longer meets their needs or they are unable to cope or feel comfortable at home. You may feel that parts of your home have become inaccessible and a stair lift may help. You might want to downsize or be more supported in your home. Age Concern Liverpool and Sefton provides, through its trusted partners, advice and support on a variety of services, ranging from stair lifts or home care aids to legal services, equity release and our trusted traders register.
