‘Medicine Champions’ encourage children to move from liquid medication to tablets

Welcome to the Summer issue of Bulletin!

Young patients at Leeds Children’s Hospital are being encouraged to take tablets instead of liquid medicines, leading to a better and safer patient experience.

Through a ‘Medicines Optimisation’ project the Paediatric Pharmacy team are supporting parents and children who want to make the switch to tablets.

Taking tablets over liquid medicine is often preferred due to a number of reasons. The volume of liquid medicines can be large, and can be difficult to carry outside of the home, whereas tablets are much more portable. The texture and flavour of liquid medicines can often leave an unpleasant taste, and children may refuse to take them.

By contrast, tablets once swallowed rarely leave a bad after-taste and remove many of stresses associated with medicine taking.

The main challenge to taking tablets is learning to swallow them. To help their young patients overcome this, the team set out to create a culture whereby medicine taking is not just the responsibility of the Pharmacy team but also the nursing, medical and play teams. The play team have become ‘Medicine Champions’ in providing children with the tools, confidence and ability to swallow tablets and capsules.

Our play specialists provide parents with information and the tools to help young patients master swallowing techniques at home, often practising with sweets before progressing to the tablets themselves.

Children who have successfully mastered swallowing tablets are awarded a special ‘Star Award’ certificate to recognise their hard work and to give young patients something to aspire to.

Anna Kinsella, Paediatric Clinical Pharmacy Team Lead, said: “We have shown that by working together with families, and with a

little creativity, all of us have a role to play in creating an easier experience for not only the patient but also for the patient’s parent or carer who is responsible for managing their medicines safely at home.”

Andreanna Kosmirak, Specialist Clinical Pharmacist, said: “Liquid medicines often have a shorter expiry than tablets, so by switching to tablets, the Paediatric Pharmacy team have reduced the amount of wastage. Tablets often cost less than the liquid preparations available. By supporting and encouraging families in making the switch from liquids to tablets, the team have been able to reduce the overall spend on medicines.

Anna said: “We have achieved a muchimproved patient experience as a result, combined with a decreased spend on medicines. We now have more children than ever before taking tablets, even as young as three years old - this is a really positive example of ‘Medicines Optimisation’ in practice, putting patients at the heart of everything we do with medicines.”

Also in this issue of Bulletin we have something of a waste-reduction special that looks at how our teams across the Trust are developing a lean culture of continuous improvement that puts patients at the centre of finding better ways of working. There are some really great stories so I hope you find them a source of inspiration for starting your own journey of improvement. And if you do, I would love to hear about how you get on!

Reducing waste, improving efficiency Leeds Teaching Hospitals is one of five trusts in the UK to work with NHS Improvement and the Virginia Mason Institute in Seattle, USA, on an improvement programme using Lean methods to improve the efficiency and flow of our services, with patient and staff experience and respect for people embedded at the centre of this work. This work is known as the Leeds Improvement Method (LIM).

What is the Leeds Improvement Method?

Our waste reduction programme is clinically-led, delivered through the CSUs. Thanks to the hard work in the CSUs we are making really good progress. An example of this is the Medicines Management Pharmaceutical Services team who have been working to not only generate savings through smarter prescribing but also reducing wasted medicines.

This process is now fully embedded in Pharmacy and in doing this the Trust is expected to save around £200,000 on HIV drugs alone.

The Leeds Improvement Method (LIM) is a system based on Lean methods supporting Kaizen, which means continuous improvement. LIM requires us to understand what really adds value from the perspective of our patients, ensuring the patient voice is central in all of our improvement efforts.

Initially, four work areas across the Trust were chosen, known as Value Streams. This describes the entire set of activities that are involved in transforming a patient experience or product from beginning to end. Today there are six active Value Streams with three having made the ‘transition to operations’, meaning that their focussed improvement work has now become ‘business as usual’.

The three transitioned value streams are Elective Orthopaedics at Chapel Allerton Hospital, Time to first Consultant Review on the Acute Floor in Emergency & Specialty Medicine CSU and the TURP (Transurethral Resection on the Prostate) pathway in the Abdominal Medicine and Surgery CSU.

Leeds Improvement Method Success All of our Value Streams have seen some fantastic improvements whether they are at the beginning of their journey or are now ‘business as usual’. At Chapel Allerton the team has reduced the number of repeat preassessments from 66% to 13% as well as reducing the time taken to step up for a knee replacement procedure from 59 minutes to 9 minutes. 10 hours of nursing time has been released each week by removing unnecessary handovers on the post op unit and patient stay on the unit is now at 4 hours as opposed to 17 hours previously. All of these achievements have supported a better patient experience and saved time for staff too.

Similar success has been seen in the ESM CSU Value Stream when looking at time to first Consultant review on the Acute Floor. The team has reduced the number of patients not seen by a consultant within 14hours of admission from 83% to 7% as well as reducing time to complete the medical clerking of patients from 77 to 46 minutes which releases over 9 hours of clinical time each day.

A number of CSUs have already embraced LIM with the following value streams currently in operation: • Adult Critical Care Step Down – Neurosciences • Ophthalmology Outpatients • Adult Cardiac Surgery • Acute General Surgical Admissions • Emergency Department LGI • Cancer MDT

Linking the Leeds Improvement Method to our Waste Reduction Programme The values-based approach taken here in Leeds, underpinned by the

The WRP’s focus is on identifying, tracking and delivering schemes that release cash to re-invest in patient care. This is different to LIM but still links to the ‘identification of waste’ which often initiates WRP schemes.

Pharmacists Emma Wrench and Kelly Atack recently implemented a process for reducing waste linked to HIV medicines. They identified that in some cases patients were being given prescriptions for further supplies of medicines well in advance of them needing a repeat prescription. This was leading to patients having duplicate supplies at home.

The team set about implementing a new system whereby the prescriptions and supplies are tracked and patients only receive supplies within 28 days of them running out.

As part of the process, the pharmacy team now reconciles new prescriptions against the medicines tracker as well as discussing medicine quantities with the patient during their consultation.

Although the primary aim of LIM is in generating efficiencies, the success of linking this method to the WRP has meant that the financial value of the Value Streams proves its worth and offers incredible spread across the Trust.

Supporting CSUs to make a real difference As part of the waste reduction programme our Finance Business partners have been working with CSUs to support them in making changes that can make a real difference.

Last year the Trust achieved a surplus of £18.9m, the first achieved in 20 years at the Trust. This was followed by an even more impressive £53m in 2018/19. Marie Dearman, Senior Finance Business Partner, explains how Finance have been working with CSUs to support them in waste reduction work: “As well as the WRP, we introduced a new Financial Performance Framework that allows effective CSU-led forecasts that are monitored and refreshed monthly - teams feel a sense of ownership and know that they can make real changes in their own department.

“We’re also the first teaching hospitals to support Day One reporting which means our CSUs know their financial position on the first working day of the month and plays a big role in identifying issues and taking actions before they become a problem.

“A number of other initiatives including successful partnerships with commissioners, close partnership working with NHS Improvement and the development of a strategic partnership with Leeds Cares has led to the impressive surplus.”

Medical Records & Libraries Optimisation The Medical Records team have been working on a comprehensive optimisation project looking at the streamlining of library processes and the culling of redundant records. Previously, the culling team were having difficulty identifying and locating the correct casenotes to cull. This was wasting valuable time and a waste walk showed this was not a lean process.

The optimisation project centred on designing and implementing a new database suite which has been used to improve and streamline the processes involved in identifying redundant patient records for culling. The database produces daily reports, accessible through a portal that refreshes every night.

The results of using this new database have been a significant increase in accuracy and productivity. Previously, a culling report was produced monthly, which rapidly became out of date and only resulted in about 40% of casenotes on the report being found. Now, thanks to the new system, 100% of health

This has enabled the team to significantly reduce the overall footprint of the libraries in the Trust. As of June, 87% of the LGI library has been culled, with a total of 76,073 casenotes removed.

This is supporting the project to transfer all medical records to the library at St James’s. As of June, 54,262 notes have been transferred to St James’s, which is now 69% complete.

Lorraine Tennyson, Health Records Implementation Manager, said: “The difference this has made to the libraries’ operational work is huge. Staff can now file in the culled areas more efficiently which frees them up for more important tasks. Productivity has increased and morale in the library is at an all-time high. Previously it could take up to 12 months to complete a full cull. Now, the culling team are removing everything on the report and feel they are really making a difference.”

Lesley Barr, Trust Health Records Manager, said: “To embark upon such a large project staff engagement was crucial as they are the process experts and helped to identify and deliver changes and improvements as well as maintaining the day-to-day operational work required in a busy Health Records Service. We will endeavour to ensure we roll out our core beliefs of lean and increase value, decrease waste improving quality and flow.”

Reducing Theatre inefficiencies in the Neck of Femur pathway Hip fracture is considered to be the commonest reason for admission to an orthopaedic ward and occurs more in older patients. At LTHT, more than 700 patients a year undergo operations for a fractured neck of femur (a specific type of hip fracture). The operation should be performed within 36 hours of arriving at the Emergency Department to ensure the best chance of recovery for patients.

A project was undertaken by teams in Trauma Related Services and Theatres and Anaesthesia to address the challenge of late theatre starts and cancelled operations, which were impacting on the teams’ ability to deliver the best patient care.

The work stream focused on making improvements both on the ward and in theatres. To achieve this, a number of teams worked collaboratively using Lean methodology to develop a Standard Operating Process. This required a multidisciplinary team to ensure the correct patient was identified as first on the list (the surgeon), that they were ready and fit for surgery (nursing and ward staff), that they arrived in theatre at a standard time (the theatre team), and that everyone in theatre was ready for the procedure.

Working in this new, standardised way has significantly improved the patient experience and reduced cancellations by eight per month.

It has also reduced the average length of stay for patients with fracture neck of femur by 3.6 days, and led to over 2,000 bed days saved, in addition to saving around £400,000 during 2018/19.

Critical Care redesign improves patient flow Last year, a fantastic collaborative effort in critical care led to significant improvements in the care that we offer to our sickest patients.

The Critical Care CSU identified a number of challenges that were preventing them from being able to deliver outstanding care. This included cancellations, delayed stepdowns, staffing shortages and bed closures.

Using data from more than eight different sources including audits, Friends and Family Test, the team’s own monthly dashboard and the ICNARC national database, they were able to establish the need for change and test theories and improvement ideas.

In early 2019, the CSU was rated as ‘Outstanding’ by the CQC. The CSU set about redesigning their patient flow team approach which enabled them to address the biggest

challenges and implement a sustainable approach to patient flow. Their work has contributed to: •A 12.5% reduction in elective patient’s cancelation at St James in 2018 compared to 2017; and •A n 18.5% reduction in delayed discharges which has saved 539 bed days in the financial year 18/19 compared to 17/18.

The Critical Care CSU has since implemented a continuous improvement programme to maintain their excellent standards and continue to respond to emerging challenges in a sustainable way.

In early 2019, the CSU was rated as ‘Outstanding’ by the CQC making it the first core service in the Trust’s history to achieve this rating, and ranking it among the top 5% nationally.

Success in the HIV Antiretroviral Therapy Generic Switching Strategy

Meeting the changing demand for palliative care

In 2018 the HIV Outpatient team, HIV Specialist Pharmacist and Pharmacy Procurement team undertook a significant waste reduction project to ensure the safe implementation of generic switches for three products used in HIV care - Truvada, Atripla and Eviplera. A switch that could save the Trust up to £3m each year.

An increase in referrals and the complexity of patients’ needs were the driving force behind the Specialist Palliative Care Team applying the Leeds Improvement Method to review, refine and optimise the delivery of care to patients and their families.

One of the first steps in waste reduction is identifying a need and opportunity for change. In the 2017/18 financial year overall drug expenditure for HIV outpatient service was over £9m, which equated to a 5% increase on the previous year. The predicted HIV drug spend for 2018/19 was £10.8m.

The team recognised that this could be improved by procuring and implementing a generic supply of Truvada and Atripla and by switching patients on Eviplera to its component parts. Since October 2018 the team have managed to procure generic versions of Truvada and Atripla which are now purchased at around 10% of their original cost per patient per month.

The HIV Outpatient team have worked extensively over the last year to maximise savings and in doing so the team have saved over £3m this year which can be reinvested into patient care and will be a recurrent annual saving. This impressive saving has, in addition, been achieved whilst the HIV service has grown by 25% from 1,500 to 2,000 patients over the last two years.

As well as saving money, the team were keen to ensure that the project was also patientcentred, an approach which was fundamental

to the success of this waste reduction scheme.

The team worked collaboratively with Third Sector organisations including Mesmac, Brunswick and Skyline, discussing the proposed implementation plan and the potential impact on services and service users.

To further ensure patient-centred implementation of and engagement with the changes, information regarding generic switches was discussed at the HIV service user group as well as being included in the six-monthly newsletter for the service. Patient information leaflets for each switch have been produced to ensure relevant written information is available to patients following their consultation.

Critical to the success of this project was substantial clinical engagement. Over 1,000 face-to-face clinical discussions took place to explain the changes to patients’ HIV medications, which has been critical in ensuring the generic switches were implemented safely and effectively.

To empower the clinical team during the switches, the North of England prescribing algorithm was adjusted specifically to facilitate the position the organisation took in switching to generic Truvada and Atripla and no longer stocking Eviplera. The Trust’s relationship with its NHS commissioners has been key in realising savings through the “Aligned Incentive Contract”.

Looking forward, the team intend to share this learning with regional and national services to ensure the savings are made within other HIV services across the UK.

A whole team approach was adopted with the team mapping out existing service provision, exploring ideas for change and testing out new ways of working.

Changes included: enhancing the clinical coordinator role, going digital where possible and implementing a daily ‘board round’ to focus on patient outcomes. The team concentrated on ensuring the skills of the whole multi-disciplinary team were applied as effectively as possible to make the greatest impact for patients.

By adopting a lean approach to MDT meetings, streamlining processes to reduce waste, the team were able to make the most effective use of professionals’ time and expertise whilst ensuring that patients who needed MDT input continued to benefit from discussion.

Thanks to this work the team is in a stronger position to work with colleagues throughout the Trust to meet the increasing demand for palliative care.

PLICS and Costing Team Supporting Colleagues

Standardising trolleys improves efficiency at Leeds Children’s Hospital The Junior Doctor Forum at Leeds Children’s Hospital has been working on a project to standardise cannulation/blood taking trolleys across the hospital to save doctors’ and nurses’ time.

Leeds Cares funded the trolleys which have been standardised and stocked in the same way across the hospital thanks to our fantastic junior doctors. Photos of all the equipment which needs stocking in the drawers are placed inside the trolley.

The aim of standardising cannulation trolleys is to reduce the time it takes to gather equipment for cannulation/blood taking.

On some wards it was taking 10 minutes to do this and three nursing staff to help get all the equipment. With the standardised cannulation trolley on the CAT unit/ward 9 it is now taking two minutes and no additional nursing staff to do the same.

Based in Trust HQ is a small team who, as part of the Finance Department, are supporting teams across the organisation to transform patient services through collaboration and pioneering development.

Hospital and Doctor Fosters, the tool helps to signpost where variations exist both internally and in comparison with external peers, and then helps with understanding the reasons behind this.

Earmarked as a key component in achieving financial sustainability, Patient Level Information Costing System (PLICS) allocates all of a trust’s costs and income to the individual patients that they provide care for. To do this, the system takes activity information from a wide variety of sources and uses a number of drivers to ensure costs and income are apportioned appropriately. The resulting data set can then be viewed and aggregated at various levels, from patient bills all the way up to individual medical specialities.

This provides an in-depth understanding of the trust’s financial position and helps to contextualise it at different levels.

Andy Hogarth, Lead Clinician for Cardiology, has worked closely with the PLICS and Costing team and has seen a number of benefits within his specialty. He said: “As a clinician working in a high cost speciality, I am acutely aware of the need to get the sums right. Working with the PLICs team meant we could address a problem with financial waste that we’d struggled with for a long time.

PLICS is as much a managerial tool as it is a financial one. It provides clinicians and managers with a comprehensive data set that should greater inform their decision-making processes. Therefore, the use of PLICS not only increases the viability and sustainability of our services but also contributes to improved and more efficient care for our patients.

As part of the waste -reduction initiatives clinicians have engaged with the Trust’s Costing Team to roll out our Patient Level Information and Costing System (PLICS) helping us identify and eliminate areas of waste. Their collaboration has also helped the team to develop its new ‘PLICS Opportunity Scoper’ tool that has made PLICS and the clinical information used in its creation far more compelling and accessible. The problem was how to translate that into pounds and pence and crucially to make sure we kept on doing it right.

This was too time consuming and we couldn’t do it in real time.

“This is where PLICs came in, helping us to really define a project area for real waste reduction - the team helped us use the system to articulate exactly what our programme needed to be.

“Working collaboratively we have been able to address the problem, reduce waste and have continued to monitor the performance of activity and expenditure. Along the way we’ve developed close working relationships with our partners in finance and PLICs which is beneficial for the future.”

Children’s outpatient clinics offering more appointments The children’s outpatient department at Martin Wing is one of the busiest services at Leeds Children’s Hospital. The department caters for young patients who are having their first appointment as well as those having follow-up care across a range of clinics including congenital heart services, endocrinology, medicine and gastroenterology.

In 2018, Leeds Children’s Hospital identified a number of challenges within the outpatients’ service. Although patients reported a positive experience of their care, there were unacceptably long waits for both new patients and follow-up appointments. Patients were waiting longer for appointments with some specialties than with others leading to inequitable care.

The children’s team reviewed a range of aspects of the service to identify where positive changes could be made to streamline processes and make best use of the time available in the clinic. They included: • Reviewing the clinic templates used by each different team; • Reviewing the numbers, types and lengths of appointment slots; • Clinic start and finish times; • Looking at capacity versus demand for clinics; and • Reviewing the consultant job plan to compare the outpatient

activity to see if the clinic hours matched what they were contracted to provide.

A number of different teams took part in the project with the goal to improve the experience for all children and young people using the outpatients’ department and to speed up their care.

Working with the consultants and the outpatients’ department team, service managers discussed the challenges of running multiple clinics and how different

departments could work together to make outpatients work better. Job plans and clinic templates were changed in agreement with teams so that clinic times could be allocated fairly and in the best way to maximise capacity.

The new way of working developed as a result of the project resulted in some significant improvements in the way the outpatients department operates and therefore leading to better patient care.

The highlights of the project include: •T he waiting times for first appointments for new patients were reduced from 40 weeks to 32 weeks; •T he waiting time for follow-up appointments has reduced by over two thirds to 32 weeks; •M ore capacity has been created so that 1905 more appointments were offered in 2018/19 compared to the previous year; and •C linics are no longer over-booked and therefore much less likely to over-run.

By undertaking the outpatients’ department project, Leeds Children’s Hospital has been able to work towards its goal of providing outstanding patient care and helping children and young people to have the best clinical outcomes.

In addition, the project has reduced overtime payments and able to provide the activity without employing further clinical staff.

During August, Leeds Teaching Hospitals launched the NHS Rainbow Badge, which signalled the organisations’ commitment to promoting an environment that is open, tolerant and inclusive.

Wearing the Rainbow Badge is a responsibility. The badge itself is intended to be a simple visual symbol identifying its wearer as someone who an LGBT+ person can feel comfortable talking to about issues relating to sexuality or gender identity.

It shows that the wearer is there to listen without judgement and signpost to further support if needed.

In 2015, LGBT+ rights campaigners Stonewall released a report called Unhealthy Attitudes which clearly demonstrated that people identifying as LGBT+ still experience discrimination. It also showed that a ‘one-size-fits-all’ approach to healthcare provision creates gaps in the way LGBT+ staff are supported and patients are cared for. Only by acknowledging and tackling this behaviour will we create an environment where everyone feels free to be who they are, wherever they are.

Alison Conyers, Lead Professional for Learning Disabilities and Autism, was one of the first in the Trust to sign up to the pledge. She says, “I know how it feels to experience homophobia and I want staff and patients to know that I understand, and that they can talk to me.”

Extensive research has shown that people from LGBT+ communities are disproportionately affected by bullying, depression, self-harm and suicidal thoughts. The fear of being judged is often enough to delay individuals from seeking medical help and those that do

are often afraid to disclose their gender identity or sexuality. This culminates in a poor patient experience as well as poorer health outcomes for those people identifying as LGBT+.

The NHS Rainbow Badge initiative has been introduced to the Trust thanks to the LGBT+ Staff Network who recognised a need to promote inclusivity for the benefit of both patients and staff.

Network Chair James Greenwood is a physiotherapist based at Chapel Allerton

Hospital. He says, “As an openly gay man I feel extremely lucky to be in a supportive team where I can be open and honest about my life. However this isn’t always the case for all our colleagues or patients.

By wearing this badge it will show support to colleagues and patients who may be struggling, but it will also demonstrate pride in my role at the Trust and as a member of the LGBT+ community.”

LGBT+ Staff Network The LGBT+ Staff Network meets regularly at venues across the hospitals but also in Leeds city centre with all LGBT+ staff and allies welcome. The Network provides an opportunity for people to share their experiences, form friendships and offer support in a non-judgemental environment. The Network’s aim is to support Leeds Teaching Hospitals to be an inclusive organisation for all.

DFCAH ‘Oscars’ recognise fantastic work of Chapel Allerton Dermatology team In May, 47 staff members and patients came together to celebrate the Dermatology Friends of Chapel Allerton Hospital (DFCAH) Outstanding Skin Care Award Recipients (‘Oscars’) to recognise the great care delivered by the teams at Chapel Allerton.

There were ten nominees all of whom received excellent citations: Connie Bray, long time senior secretary, received the Lifetime Award Achievement, Senior Sister Bev Mousa, received the Gold award and Senior Sister Nicola Howcroft, received the Silver award. The Bronze award went to the Dermatology Outpatient Hub team (Gemma Thornton, Jade Lister, Lesley Tosh, Deborah Dearing, Erma Smith, Laura Stone and Gloria Worrell).

There was much anticipation within the department leading up to the event which was thoroughly enjoyed by staff and patients. Prof Bill Cunliffe - retired professor of dermatology and secretary for DFCAH - highlighted just how fortunate patients are in Leeds to have an excellent dermatology department at Chapel Allerton Hospital, which is entirely down to the great staff at all levels.

The Oscars were possible as a result of a generous donation from Victor Chairperson DFACH - who presented the awards, and Anne Boughton. DFCAH is a small registered charity who raise funds for medical equipment, support nurses to go to training courses, and purchase patient comforts.

Learning Disability Conference In June our Corporate Nursing team organised an event in support of Learning Disability Week which was attended by the new Lord Mayor, Councillor Eileen Taylor.

In place of the annual fashion show, the event was chance to showcase our Learning Disability and Autism Service. Attendees were able to meet with the team and other patients who also use the service.

Junior Doctor Awards 2019 On 3 July we held our second annual Junior Doctor Awards. Believed to be the only dedicated awards of their kind in the country, the Awards are organised by our Junior Doctor Body and Junior Doctor Forum (Children’s Hospital). They provide an opportunity to showcase some of the vital work undertaken by doctors in training to improve patient care.

There were hundreds of fantastic nominations submitted this year and our judges had a hard time selecting the winners! Well done to everyone who was nominated and a big thank you to the organisers of the event for making it such a brilliant occasion.

In June the Trust welcomed a cohort of fifteen nurses from the Caribbean Island of Jamaica as part of an international education partnership.

The Global Learning Practitioners have joined the Trust as part of a pilot scheme between Leeds Teaching Hospitals, Health Education England and the Jamaican Ministry of Health which aims to create a long-lasting reciprocal relationship between the UK and Jamaica, whilst globally showcasing the outstanding critical care that Leeds Teaching Hospitals provides.

The nurses will undertake a bespoke critical care education programme at the Trust over five months and will be evaluated against a framework of nationally recognised competencies.

This will enhance their critical care skills,

enable them to implement positive changes in practice back in Jamaica, and eventually allow the Jamaican Ministry of Health to open up more critical care beds.

Kimberly Reid-Ferro, 28, comes from St Thomas in Jamaica. She said: “I have been working on L06 so far and trust me, they are like family. On my first day I worked with Keren, she was such a sweetheart. She took really good care of me. Denise is my other mentor and she is like an angel. She looks after me very well. I am having a wonderful time with my new ICU family, just like I am used to back home.”

‘Marble-ous’ idea helps young patients at Leeds Children’s Hospital celebrate their organ transplant The mum of a young liver transplant patient has teamed up with staff at Leeds Children’s Hospital to create an ‘end of treatment’ marble run to celebrate this important milestone for young patients.

Children like five year-old Imogen, who received the gift of a new liver in December last year. Mum Alex wanted to say thank you to the ward in a meaningful way, and after hearing about Megan’s idea, decided to help.

The marble run has been set up by the team on Ward L50, who care for children and young people requiring liver or kidney transplants. Once a child has finished their treatment and is ready to go home, they get to place a coloured marble - yellow for liver, red for renal (kidney) - in the run.

She said: “The marble run is something that symbolises what children like Imogen have been through and is something for them to look forward to.

Imogen really loved doing it and it really meant a lot to us - it’s my hope that other children and their families will get just as much out of it.”

Only a patient who is ready for discharge may place a marble in the run, which symbolises their successful transplant and will collect at the bottom of the track as a lasting message to future young patients that they are not alone in their transplant journey.

The idea came from Staff Nurse Megan Rees, who wanted something special for young patients to look forward to and that celebrates the fact they have had their transplant and are

ready to get on in life.

Megan said: “A lot of people suggested we shout about the number of transplants we do and how amazing the children are who go through this.

I came up with the idea for children to pick a marble that they place into the run on their discharge day as a celebration of how brave they have been and how far they’ve come. We have had many children who have come through this treatment and these marbles tell their stories of success.”

Keep on runnin’ You won’t get far in Leeds these days without seeing someone jogging along the pavement or groups of runners out for an evening session. It’s a free activity that’s taken the city by storm.

The emergence of the game-changing parkrun initiative in 2004 has really propelled running’s popularity among the masses and now nine of the 5km, timed runs operate across the city every Saturday. Following hot on its heels was junior parkrun for children aged 4-14 years, which are equally popular as the adult sessions.

Parkrun devotee, Dr Ai-Lyn Tan is an honorary consultant rheumatologist based at Chapel Allerton Hospital.

Having completed well over 300 runs herself, Ai-Lyn has taken to ‘prescribing’ the exercise to her patients in a bid to support them with their long-term health and wellbeing.

There is increasing evidence of the benefit of physical activity in helping people with arthritis and musculoskeletal conditions. In the rheumatology department, the team actively signposts patients to a wide range of free and low-cost exercise opportunities in the city through poster displays in the waiting areas. GP letters include a ‘referral to parkrun’ to indicate that the team has recommended this type of activity to the patient, which has helped to facilitate many discharges from hospital care.

“The most positive impact of parkrun for me is the social camaraderie and the opportunity to challenge myself athletically”, says Ai-Lyn, “My preparkrun self was like a stranger to me. I know it is a cliché, but I’m proud to admit that parkrun has given me a new life.”

Back in June, Ai-Lyn teamed up with another parkrun fan - diabetes consultant Dr Mike Mansfield - to organise a parkrun ‘takeover’ at Woodhouse Moor and Potternewton Park. Mike also ‘prescribes’ parkrun to many of his diabetes patients to increase their physical activity, reduce social isolation and improve health outcomes.

Ai-Lyn’s own experience has meant that she’s been able to promote parkrun to most of her patients, safe in the knowledge that it offers them the welcoming, non-judgemental and

Adrian Crellin awarded CBE Congratulations to Dr Adrian Crellin who has been awarded a CBE as part of this year’s birthday honours.

Adrian has worked at Leeds Teaching Hospitals for nearly 30 years as a Consultant Clinical Oncologist and he also provided clinical leadership for the development of the Leeds Cancer Centre.

Adrian also holds a number of highly regarded national roles, including as the NHS England National Clinical Lead for Proton Beam Therapy.

This is very well deserved recognition for the outstanding contribution Adrian has made to cancer care and the improvement of cancer treatment at LTHT and in the NHS as a whole.

Leeds Health and Care Academy Launched in Washington In May, Jenny Lewis, Director of HR and Organisational Development, headed to Washington DC to launch the pioneering new Leeds Health and Care Academy – a new and shared way of thinking, learning and working together across the sector that will include around 57, 000 people.

It is bringing together and transforming the learning and development for all who work in health and care in Leeds and in doing so, it underpins the city’s aim to benefit citizens by create ‘one workforce’ across health and care.

The city’s universities will help position the Academy at the forefront of learning, with a portfolio based on world-leading evidence, research and education.

The Academy is a groundbreaking project of Leeds Academic Health Partnership (LAHP), one of the biggest of its kind in the UK. The LAHP founding partners include three Leeds universities, the city’s three NHS trusts, NHS Leeds Clinical Commissioning Group and the City Council.

The Leeds Health and Care Academy is not about creating one big organisation, but about working better and smarter together beyond organisational boundaries to give Leeds people a more seamless experience of health and care services.

Jenny Lewis commented: “This is a very exciting step for Leeds and for Leeds

people. We don’t think any other city of our size, involving so many partners, has attempted such a comprehensive approach to transforming the culture, capacity and capability of its health and care workforce.

“As with any large scale ambition, we are starting by laying good foundations, such as improving support for people already in health and care roles and helping everyone understand the ‘one workforce’ approach.

“Technological developments are dramatically changing how we all work and it’s vital that our staff are equipped for this, not just to embrace the change, but to lead it with confidence, knowing they’re providing the very latest and best care possible.

Andy wins national award for improving care for tuberculosis Dr Andy Lowey was recently awarded the prestigious Excellence in Hospital Pharmacy Practice Award at the national Clinical Pharmacy Congress 2019.

Andy is our Lead Clinician for Pharmacy and Preparative Services and was recognised for his ‘significant national contribution’ after leading a piece of work to improve the safety and efficacy of liquid medicines for the treatment of TB. Working with colleagues at LTHT as well as trusts across England, Andy and the team identified disparities in the use of liquid medicines in UK hospitals. Liquid medicines are most commonly used in the treatment of children with TB but in some cases may be used for adults, where tablets can’t be swallowed.

Evidence showed that three of the main liquid medicines - ethambutol, isoniazid and pyrazinamide - were being used at different concentrations. A survey showed ethambutol was being used in at least eight different concentrations across UK hospitals, increasing the risk of harm to patients and leading to inequitable outcomes.

Working alongside hospitals, organisations and patient groups nationally, Dr Lowey and his colleagues devised and agreed a common approach to the administration of liquid medicines - meaning that all hospitals now follow the same clinical guidelines and use medicines in the same concentration.

Dr Lowey also worked directly with both NHS and commercial pharmaceutical manufacturers to ensure that all TB liquid medicines are now manufactured in batches and to standardised concentrations.

The new approach is much safer for patients because it reduces the risk of errors in prescribing, dispensing and administering medicines, particularly if a patient was to move from one hospital to another or from a hospital back to a community setting.

Biomedical Science Day celebrations are at the heart of healthcare Leeds Teaching Hospitals was privileged to be chosen by the Institute of Biomedical Science (IBMS) as the lead hospital in England for Biomedical Science Day in June. St James’s showcased the world’s first fully digital pathology laboratory to visitors from IBMS, as well as Daragh Corcoran from BBC Radio Leeds who interviewed Biomedical Scientist Chloe Knowles on a tour of the lab.

Patients, visitors and the public were invited to tour the blood sciences labs at the LGI and find out what happens to each sample. There were also interactive displays from a number of areas, showcasing the broad range of disciplines that make up biomedical science in hospitals.

There was also a special Harvey’s Gang visit, where Libbie, an oncology patient at Leeds Children’s Hospital was able to

learn more about the laboratory aspects of her transfusion treatment. Harvey’s Gang started in Leeds in 2017, and since then many young patients have visited the Blood Sciences labs to become trainee biomedical scientists for the day and tour the labs with their families.

The tours enable young patients to be more involved in their treatment, lessen their fears and answer any questions they may have.

Catherine Penrose recognised for organ donation campaigning Congratulations to Dr Catherine Penrose, consultant in PICU, for being recognised in the Inaugural UK Awards for Excellence in Organ Donation and Transplantation.

Leeds Celebrates International Homebirth Day The Trust’s Homebirth team got together to celebrate as part of International Homebirth Day on 6 June. The special afternoon tea was a chance for mums, families and midwives to get together and share their experiences.

Following the globally celebrated International Day of the Midwife in May, International Homebirth Day aimed to educate people about homebirth as a birthing option for women and to celebrate its many benefits.

LTHT has one of the UK’s busiest maternity units with around 10,000 babies born each year. In 2018, 205 babies were born at home and the city’s homebirth team has ambitions to increase that to over 300 this year, with 57 babies born at home so far.

Naomi Robinson, Homebirth Team Leader at LTHT said “It’s really important that we take every opportunity to celebrate homebirths and the benefits that they can bring.

Dr Penrose was Highly Commended in the category Exceptional Clinical Lead for Organ Donation 2019, which is a huge achievement.

Catherine has been an outspoken advocate of organ donation over the years, including championing the Trust’s Be a Hero campaign in Leeds, as well as appearing on BBC Look North to talk about the importance of signing up to the donor register.

There are also no restrictions on family and friends being present in your home to support you which means a lot to mums-to-be who often want their family and friends to share in the experience.

“It is important that women have a choice regarding their place of birth.

In Leeds we have a dedicated team of experienced homebirth midwives who are able to provide care during labour and birth at home - something mums-tobe may not have considered.”

In Leeds, the majority of mums-to-be choose to have their babies in hospital with just a small number opting for a homebirth. However, research suggests that having your baby at home can increase the chances of having a normal birth and reduce the risks of intervention in birth, such as caesarean section, assisted delivery, episiotomy and the need for medicinal pain relief.

What’s the best thing about your job?

Working as part of a finance team that is recognised as possessing strong financial skills and understanding and having a culture that promotes and demonstrates the right values, attributes and beliefs necessary to deliver great patient care and value. This is most evident with national recognition such with the HFMA award as ‘finance team of the year’ and recently Leeds Teaching Hospitals finance team attaining level 2 of the Future-Focused Finance Accreditation.

Richard Norman, Head of Programme Management Office, Finance, Trust HQ has worked at the Trust for just 3 months! (but loving it) Summarise the main aspects of your role?

My job is to support the finance Programme Management Office in its objective to help CSUs meet their financial targets through programme management of the Waste Reduction Programme.

I support PMO programme managers who work closely with CSU triumvirate teams, Finance staff, CSU Service and Business Managers to help manage WRP schemes. We help CSUs maintain the quality impact assessment process that ensures that schemes identified have their impact on quality assessed and tracked therefore maintaining the best possible care and outcomes for patients whilst still delivering waste reduction programme targets.

The PMO deliver an ‘advice and assurance’ model – balancing support with challenge and we work closely with finance colleagues to provide additional areas of support to the waste reduction programme.

What else have you done in your career so far?

I’ve worked in the programme management office environment for over 20 years and across

What is the one thing you would change at LTHT to help you do your job better?

I feel that the Trust is leading the way in sharing with other Trusts our experiences and learning.

As a finance team we have hosted a number of Trusts here at Leeds to share the successes of our financial approach. This has been a great opportunity for shared learning with other NHS Trusts. We very recently welcomed a group from University Hospitals Birmingham who were keen amongst other things, to understand our successful approach to management of the waste reduction programme and this gave us the opportunity to gain a valuable understanding of what schemes other Trust are delivering in their CIP and waste reduction programmes which helps inform some of our potential schemes here at Leeds - more of this can only help me in my job.

What’s your best advice to a new starter at the Trust?

I joined Leeds from a much smaller Trust and my advice is that the scale and size here offers invaluable opportunities to meet new people and talk to teams who are really living The Leeds Way through all they do.

Over 300 hardworking staff enjoyed thoughtprovoking and inspirational talks from Andy Cope, Doctor of Happiness, and Christie Watson, author of The Language of Kindness.

Under the theme of Caring for the Future, teams presented their work to their colleagues and many created posters which everyone could enjoy reading through the breaks. The conference was also an opportunity to recognise the fantastic work of nursing staff, with awards being presented

£2.4 million target reached for MR Simulator Appeal A celebration was held to mark the closure of the MR (Magnetic Resonance) Simulator Appeal, after the £2.4 million target was achieved following a milestone donation of £250,000 to Leeds Cares

by Sovereign Health Care.

The state-of-the-art machine will transform the way care is delivered for radiotherapy patients undergoing treatment at the Leeds Cancer Centre. The MR Simulator can produce high quality scans of the affected area; providing images with better soft tissue definition which allows the areas for treatment to be identified with greater clarity – minimising the risk of damage to healthy cells.

Over the last two years, money for the appeal had been raised through individual donations, support from corporate partners and fundraising by staff and patients through Leeds Cares.

Everyone who has been part of this fantastic achievement is thanked for their contribution, which will make a real difference to the patients and teams working in radiotherapy and across the Cancer Centre.

Alison has been with the Trust since 1980. Starting as a student nurse she completed her training in 1984, where she started as a student midwife after a spell as a staff nurse in coronary care.

Qualifying in 1985 she has since worked for the Trust and its predecessor for nearly 39 years. Alison has worked in all areas of maternity and has worked as a community midwife for many years, becoming their team leader around 18 years ago. She worked as a Community Team Leader for 8 years, and just over 10 years ago became Community and Outpatients Matron.

Anne Sanz, Clinical Support Worker, has retired after 28 years of service which started in the eye clinic at Leeds General Infirmary.

With the amalgamation of the hospital groups into LTHT, Anne continued to work within the eye clinic during the centralisation of ophthalmology department to St James`s. Throughout her time, Anne leant her support to the growing services, working at St George’s Eye Centre, Rutland Lodge Eye Clinic and assisting with the further expansion of the service to accommodate the Macular Treatment Centre at Seacroft Hospital. Her colleagues wish Anne a fond farewell and a long and happy retirement.

Professor Peter Selby has retired after working at LTHT for over 30 years. During this time he has made an outstanding contribution to the understanding of cancer and the improvement of cancer care at a local, national and international level. He has provided exceptional care to his patients as a Consultant Physician specialising in Medical Oncology, and has helped the Trust to dramatically improve the cancer services provided in Leeds including setting up the Teenage and Young Adult Oncology Service. He has also provided valuable clinical leadership for the Trust, as Director and Lead Clinician for the Leeds Cancer Centre and as a Non-Executive Director on the Trust Board. Peter is a well-respected expert in his field, and his broad research portfolio has always focussed on work that provides very clear benefits for patients in Leeds but also worldwide. His colleagues and all who know him wish Peter a very happy retirement.

Since then the job has grown, and not only did she take on the specialist teams, which she has been instrumental in developing, Alison also took over the fertility team. Whilst juggling all of her areas, she has also managed to raise a family - Lucy with her husband Tony - and is even doing a Master’s Degree! Her colleagues all agree it will be difficult to replace such a wonderful talented woman.

Heather Lewis Heather commenced her nurse training at the LGI in 1972 and qualified in Spring 1974. Heather has worked in the Outpatients clinic since 1979. Previous to this she worked in Coronary care and also for the ambulance service. Her colleagues wish Heather a healthy and happy retirement, which will mean she can spend quality time with her son, daughter and granddaughter.

Sharon Crawford, Senior Sister on J47 at St James’s, has retired after 36 years of service. Sharon started her nurse training in Harrogate in 1983 and qualified in 1986. The following year she moved to Seacroft Hospital and from there worked in clinical roles at various hospitals in Leeds including St James’s, Wharfedale Hospital, and LGI. In 1995 Sharon became Junior Sister in Vascular Surgery, where she has remained, moving to St James’s during the vascular service reconfiguration, and becoming Senior Sister. She has been on J47 since 2014. Sharon was known for being a very organised, kind and calm person, diplomatic in nature, as well as an expert clinician and strong leader. She will be missed by all her colleagues who wish her a happy and healthy retirement.

Fiona has worked as part of the Trust for 25 years. She started her working life in the Trust as an outpatients clerk at St James’s where she worked for three years. She then moved on to work for Professor David Jayne as his secretary in what was then the Professorial Surgical Unit. Fiona has continued to work for Professor Jayne until the date of her retirement and has seen many changes along the way. She has always been a valued member of the colorectal department and will be missed by her friends and colleagues who wish her a long and happy retirement.

Pat Mawson

Patricia Sneddon Patricia Sneddon retired in June after working for the NHS for 47 years. She worked as a ‘Tea Lady;’ TSW; Enrolled Nurse and later trained as an Operating Department Practitioner where she remained until retirement. A colleague said: “Patricia has experienced many changes in the NHS during her time with us and deserves to have a shining star award for her commitment to the LGI. You would have thought that this would have been enough but she will be back with us after a few weeks break. Patricia is a rarity and theatres won’t be the same without her and her many stories.

She has been an asset to us and will be missed from full time duty.”

John came to Leeds in 1990 from Edinburgh where he had been involved in developing some of the first cardiac MRI images in the world.

In Leeds he started work at St James’s providing research support and clinical development on the first NHS-funded MRI scanner in the country. John’s research work mainly focused on liver MRI and cardiovascular imaging and he was a co-author on several papers in those areas. In 1995 he moved across Leeds to work at Leeds General Infirmary providing scientific support to two new scanners. Cardiac MRI and cardiovascular work also transferred across to the LGI and John kept supporting these.

During this time he collaborated in developing a technique for imaging the peripheral arteries using MRI. In 1999 he was one of three principal investigators who were awarded a multi-million pound British Heart Foundation grant to facilitate further research into the use of MRI in the heart. This grant provided a 1.5T cardiac research system also located at the LGI. Following the creation of LTHT in 1998 John became the head of MR physics, a team which eventually expanded to include 6 people. He oversaw MR physics’ advice to numerous new MR systems and centres including Bexley Wing at St James’s, Leeds Biomedical Research Centre (LBRC) at Chapel Allerton and the Advanced Imaging Centre at the LGI. Throughout his time in Leeds John has been instrumental in setting up the safety frameworks to ensure all MR imaging is provided safely.

Pat Mawson retired from her role in admin and clerical in Children’s Therapy Services in May. She has had numerous admin roles within the trust during her 30 year career all within Children’s services. Her love of children was evident and she was fantastic at putting children and their parents at ease when they arrived for their outpatient appointments. Pat was committed to her work, great at supporting staff and exceptionally kind. How staff viewed her was evident in the many messages wishing her all the best for her retirement.

Julian Hartley attended her retirement do and was able to present her with her 30 years Long Service certificate. She was given a good send-off attended by many who have worked with her over the years; her colleagues wish her an enjoyable retirement in Scarborough with her family.

Donations Dickie Bird donates to CHSF Legendary cricket umpire and Yorkshire icon, Dickie Bird OBE, recently visited the Congenital Heart Unit at Leeds Children’s Hospital to make a fantastic donation to charity partner’s Children’s Heart Surgery Fund (CHSF).

The generous £20,000 donation will be invested in the specialist care, treatment and support provided by the Congenital Heart Unit for young patients with congenital heart disease and their families.

Intensive Care Unit relatives room Following patient feedback, the team on Ward L2/3 have transformed the relatives room, with the addition of new furnishings and wall art. The funds for a new sofa were kindly donated by the Crabtree family, who wanted the room to be a more comforting space for loved ones who are going through such an emotional time.

Well done to everyone involved in this transformation, which will make a real difference to patients and their families for many years to come.

J93 day room fundraising

L21 Stroke Rehab

J93 have refurbished the large day room for patients and the Butterfly room for relatives and carers, which provide tea and coffee and a place to contemplate in comfortable surroundings. The day room has been used to encourage patients to mobilise and use the TV, reading and games facilities. This has been due to the fundraising from Christmas and Easter raffles which was led by Nurse Debra Wilson on J93. The prizes were kindly donated by the staff on the ward and tickets were bought by patients, visitors and staff passing through.

Moira Wallace and Rita Branford White have donated £1,000 to Ward L12, the Stroke Rehabilitation unit, at Leeds General Infirmary. The donation was made in recognition of the excellent care the staff at the unit had given to Dr Eric William Wallace - who died in June, 2018 - after he suffered a stroke.

Most of the money has been raised by runner Danny Cooney through a series of marathons and half marathons that he has completed in Spain and Portugal over the past nine months.

With additional charitable funds from Leeds Cares, the team were able to purchase some furniture and a television, as well as pictures for the wall, a fully stocked bookcase and other forms of entertainment for both adults and children. Two leather sofas bought from a selling site were also kindly given away for free when the seller discovered they were for use on an Oncology ward. The rooms have provided a home-from-home experience for patients and relatives on the ward.

Leeds Cares ambassador Lizzie Deignan visits patients ahead of UCI Road World Championship In June, World champion cyclist Lizzie Deignan visited patients at Leeds General Infirmary and Leeds Children’s Hospital in her new role as ambassador for Leeds Cares, the charity dedicated to supporting exceptional healthcare for the people of Leeds, Yorkshire and beyond.

The Yorkshire pro-cyclist visited the neonatal wards of Leeds Children’s Hospital then moved on to the trauma centre at Leeds General Infirmary. She also met a team of anaesthetists who have all committed to ride the official sportive of the 2019 UCI Road World Championships to raise money for Leeds Cares.

Lizzie said: “It was quite an emotional return to the place where I was born as I got to see first-hand the contributions made by Leeds Cares to the wards.

“It’s everything from major pieces of equipment to building refurbishments.

It was lovely meeting the new parents and babies on the neonatal ward, but also learning about the challenges that premature babies can bring.

“The projects Leeds Cares supports are life changing. Supporting families,

improving access to counselling services, making the wards better – it all helps in a very real way.

As the fundraising partner of the 2019 UCI Road World Championships, the charity is hoping to raise hundreds of thousands of pounds from the event towards new projects across the hospitals to further help people from Yorkshire and beyond.

The eight-day 2019 UCI Road World Championships from September 22 to 29 will see some of the world’s best cyclists travel to Yorkshire for men’s and women’s races, across age groups from Junior to Elite. Every day will see a start in different towns and cities throughout Yorkshire, with each finishing in the main competition town of Harrogate.

60 seconds Kathryn Essex What do you do to wind down and relax after a hard week at work?

I always enjoy a nice glass of wine (or three) and chilling in front of the telly with my fiancé. We’ll usually be watching a boxset of some sort on Netflix, although recently I have to say he’s been indoctrinated into Love Island…

What is your ideal holiday destination and why?

Kathryn Essex, Senior Programme Manager has worked at the Trust for 14 Months

We went to Bali last year and that was incredible to see a completely different culture. As much as I love chilling on a sunbed, it’s always good to see somewhere a bit different. I’ve always wanted to go to Cuba and see the old cars in Havana, so I think that would be my ideal holiday destination.

What is your favourite food?

I love anything spicy so I’d have to

say a curry – the hotter the better!

What did you want to be when you were growing up?

I always wanted to be an author; I love reading and have an English degree. Who knows, it may be still on the cards at some point.

What is your favourite film?

I love the adaptation of Nick Hornby’s High Fidelity. I’m a bit of a music nerd and I like that combined with the rom com elements. Jack Black is also at his most hilariously Jack Black.

Tell us something we don’t know about you …

I can read Braille. I used to work as a teaching assistant supporting blind children so learned so I could support making resources for their lessons. Can’t say I use it much in my current role!

Celebrating International Clinical Trials Day Research teams from every hospital in the Trust did a fantastic job of sharing the wonderful work they do for International Clinical Trials Day in May. This demonstrates the real commitment to research in Leeds and its role in improving care for patients.

The number of people benefiting from clinical research in England has reached record highs and this is echoed in outstanding results for Leeds Teaching Hospitals - in 2018/19 more than 20,000 people were recruited into high quality research studies.

The event was led by the Leeds Clinical Research Facility (LCRF) team who coordinated the day’s celebrations.

Displays featuring demonstrations, information, and much more were held in reception areas, inviting patients, staff and visitors to gain a deeper understanding into research and how to be part of it. Sheila Walker, a patient at Leeds Cancer Centre also shared her experience of how clinical trials have improved her quality of life.

She said: “Taking part in research has been amazing and I feel honoured to be giving something back. My care in Leeds has been tremendous and the love and personal care I have received from the team has made it a more pleasant journey. I feel like I am part of a family and I appreciate the little things the team do and ask about when I visit for treatment.”
