The following pages have a detailed explanation of each area and how this useful publication is distributed to people requiring help, advice and support on living independently or accessing care options.
• The content includes editorials on the assessment process, intermediate care, home enablement services, motability, health services, referral to the social work team, care homes, continuing NHS care, CQC, and help and advice from healthcare professionals.
• The A4 publications are distributed freely to elderly people on a bi-annual or annual basis.
