This guide is to help you with selecting a care home. It contains important information and advice about what you need to think about when choosing a care home. It also has a list of the care homes that the Council contracts with including some that are located outside the Wakefield district.

We hope that you find this information useful and if you can think of ways we can improve this guide please let us know. We have tried to answer as many questions as possible.

Not all care homes in the Wakefield district have a contract with the Council.

Care Homes What is a care home?

A care home can be a residential or nursing home or both. These homes can provide the personal help that people need 24 hours a day and seven days a week. Nursing homes also employ nurses to look after people’s medical needs.

Some homes cater for people with different needs, for example people over 65, people with dementia, older and younger people with physical disabilities and people with learning disabilities and/or autism.

However, before making the decision to move to a care home it is really important that you have considered all the options available to you. Going into a care home is a major commitment for your future – it involves changing where you live and potentially committing to paying a considerable amount of money for your ongoing accommodation and care needs.

There are lots of services and support available locally to help you to remain living at home or to stay independent. Help could include things like assistive technology, adaptations to your home, provision of meals at home, help with personal care at home, help for when you leave hospital or support provided by a carer or from within your community.

Alternatives to care homes You may want to consider whether a move to an alternative home such as "extra care" housing schemes, independent living schemes or supported living accommodation may suit your needs better.

These options offer greater independence and choice with an increased level of care and support often on site. You will have your own secure apartment and front door with the piece of mind that there is care and support onsite often 24 hours a day.

Assessment of Needs This is how we work out if a care home is the right option for you and what type of care home you need. Where ever possible the Council will always try to support you so that you can remain living in your own home or stay independent.

If you think you need social care services, you have the right to ask us to assess your needs. Carers also have the right to an assessment.

The assessment is a discussion between you and a trained person from the Council such as a social worker or a care coordinator to get a better understanding of your care and support needs and the goals you want to achieve to maintain your wellbeing.

The assessment will find out: • what help you need

• if you’re eligible for help from us • if a care home is the right option for you • what type of care home would be best for you The Council will comply with the Care Act 2014 and other relevant legislation when assessing your needs and eligibility for support or services.

If following an assessment you are not eligible for any care or support from the Council, we will give you information and advice about what help is available to prevent your needs from deteriorating.

If you need someone to support you with the assessment you can have a friend or family member there. For some people they will be entitled to have someone independent to the Council to help them with their assessment – this will be through an independent advocate.

You will always be given a copy of your assessment. If, your assessment shows that a care home is the best and safest place for you to receive the care and support you need then you and your family will be asked to consider a appropriate care home or another type of residential setting such as

an extra care scheme. This guide and directory may help you.

Even if you are funding or arranging your own care you can still ask the Council for an assessment. The Council may be able to help you with identifying any unmet needs and give you advice on how best to meet them or prevent your health needs from deteriorating in future.

• You’re already getting help at home If you’re already getting help at home with your care and your situation has got worse or you can’t manage at home any more, you should ask for a review or re-assessment of your needs. Contact your social worker or contact Social Care Direct to ask for a review or re-assessment.

How much will I have to pay for my care and support?

Fees charged by care homes vary considerably. It is sensible to compare the fees of different homes and check what items and services are included within the fees and what they charge extra for.

It is important that you understand all the options for paying for long-term care and we strongly recommend that you get specialist advice about your situation e.g. from Age UK, Citizen Advice Bureau or from a qualified independent financial adviser. These can help you find out if you are entitled to any benefits and payments,

and help you to plan and make well-informed decisions about how best to pay for your care and support over the long term.

The amount you have to pay (or contribute) for your care and support will depend on the care you receive, your income, savings and other personal circumstances. If you move into a care home you most likely will have to contribute towards the cost of your care. The cost of a care is one of the deciding factors when choosing a care home. It is really important to

know your financial situation before choosing a home.

The Council will complete a financial assessment with you to work out how much you may need to pay towards the cost of your care and support.

If you have health or nursing care needs you may be eligible for help towards the cost of your fees through the NHS.

If you are eligible for financial help from the Council your Social Worker or Care Co-ordinator will tell you the amount the Council would expect to pay for the level of care, which you have been assessed as needing. This is called the Council’s allowable rate. If you choose a care home that costs more than the Council’s Allowable Rate, you will need to find someone, ‘a Third Party’ to pay the difference or ‘top up’. A relative or friend may agree to pay this for you. It is important that you make sure you or they are able to afford the fees for the foreseeable future.

If you are assessed as not being eligible for any financial help, then you will be responsible for paying the full cost of your care home fees. This is known as a being a self –funder.

When choosing a home you need to think about what will happen if your or your third parties financial situation changes in the future and you or they are no longer able to pay or contribute towards the cost of the home. The local authority may have no obligation to continue to fund the more expensive care home place and you may have to move.

It is worth thinking about this potentially difficult situation when deciding on care home options.

How do I know which home to choose?

The best way to choose a home is to go and have a look round and talk to the people who live there.

A list of homes (some that Wakefield Council contracts with) and the different needs they cater for is included in this directory. You are welcome to arrange to visit them.

Dementia An increasing number of people, particularly over 65, are being diagnosed with dementia. There are about 4,000 people in the Wakefield district known to have dementia – but there may be many more that have yet to be diagnosed. Although there is no cure for dementia, with treatment and support many people are able to lead active and fulfilling lives.

What is dementia?

Can dementia be prevented?

Dementia is not a single illness but a term used to describe a group of related symptoms caused by damage to the brain, the most common is Alzheimers. Different types of dementia affect the brain at different rates and in different ways.

Dementia is not an inevitable part of ageing.

Signs of dementia The condition develops over time and so it is not always easy to spot in the early stages. Other factors such as someone’s personal circumstances and the environment in which they live will affect their experience of dementia.

Over time the symptoms of dementia become more obvious. These can include situations where the person: • has difficulty in remembering things, particularly things that happened recently • repeats things or loses the thread of what's being said • has increased difficulty in understanding choices and making decisions • becomes confused about what is going on around them, even in a familiar place

• finds it more difficult to reason and communicate for instance difficulty in following a conversation or TV programme • feels anxious, depressed or angry about memory loss and confusion.

High blood pressure, heart problems, high cholesterol and diabetes can increase the chances of developing some types of dementia, so it’s important to maintain an active and healthy lifestyle.

The sooner symptoms are discussed with a doctor, other treatable conditions can be ruled out. If it is dementia, diagnosing early means getting the right support to help people live well for longer.

Recognising it, getting help and support to deal with it A diagnosis of dementia can be a shock. However, there is much that you can do in the early stages that can help to make life easier and more enjoyable, both now and in the future.

If you have concerns that you or someone close to you is experiencing symptoms see your doctor.

Your GP practice or medical centre may also have services that can help you.
