This guide is intended to help you, your carer and your relatives understand how your local health services can help you after discharge from hospital. Also information to help you to live independently or access support and care services in your area.

The right place

for your care

Your local hospital in partnership with other hospitals in the region, and your local authorities recognise and support each patient to receive the right treatment, at the right place and by the right professional.

An acute hospital provides consultant led health services within the National Health Service. Acute care is for patients who require emergency, medical or surgical services within a hospital environment. The Royal Liverpool and Broadgreen Hospitals provide care for patients who require this level of care.

Hospitals are the right place to be when you are in need of specific medical or surgical treatment.

However, when your treatment has been completed, it is important that your stay is not delayed for the following reasons: • Beds are needed for people who are very unwell, and who may be waiting in the Accident and Emergency department for a bed to become available.

• People awaiting surgery, both urgent and nonurgent, may have their operations cancelled if a bed is unavailable.

• There is a risk of acquiring infections in hospital, so leaving at the earliest opportunity means this is less likely.

• Some people find it harder to return home the longer they stay in hospital.

It is good practice to start planning for your discharge as soon as you come into hospital or even before you arrive. This means that we can begin to: • Assess what your needs are likely to be when you are ready to leave.

• Involve relevant staff who can help in meeting those needs (e.g. social worker, occupational therapist, physiotherapist, district nurse).

• Make arrangements for equipment or services which need to be in place when you are medically ready to leave.

• Make sure that your carer is given any

information, help or advice that is needed to aid your recovery.

All staff will work with you and your carer or relatives, as a team, in order to discuss your discharge with you.

We need to discuss any concerns you have at the earliest opportunity, e.g. your safety at home, managing your personal care or domestic arrangements, your ability to move around and any equipment you may need.

You will be given an expected date for discharge based on when it is expected that your treatment will be completed. It is important that everyone involved is aware of this date so that we can all work towards it, and so that the necessary arrangements can be made.

These may include: • Transport home. Patients are normally expected to arrange their own transport.

• Suitable clothing and footwear if you are not already using them in hospital.

• Access to a key to your property.

• Adequate basic food stocks.

• Adequate heating in your home.

• Adequate support for your carer or family members.

We aim for patients to be discharged before 10am but if this is not possible, it is likely that you will move to the Hospitality suite which enables us to prepare the bed for the next patient.

The Hospitality suite is a comfortable and safe place to wait for your transport.

Relatives or carers coming to collect you can meet you in the Lounge or a nurse can escort you to the main reception area.

Finally, we understand that any hospital stay can be stressful and we need to support you and your carer during your time here. You can also help us by moving from hospital when you no longer need our services. We will discuss with you the sort of help you and your carer may need when you return home, to make the transition as easy as possible.

Your Hospital Case Manager is the member of the team who will meet you on the ward and, along with the ward staff will begin planning your discharge from hospital.

For most patients, being discharged from hospital to their home is straightforward.

Some may need information and advice or may need the support of social services.

Some people have ongoing health and social care needs which require a more detailed assessment. Staff from health and social care will work together to plan and deliver services to support such people after discharge.

This guide explains some of these processes and explains the role of the hospital and Social Care team in the hospital discharge process.

Leaving hospital You will be transferred from hospital when your consultant led team decide that you are clinically ready to leave hospital and that you no longer require an acute hospital bed.

For the majority of patients, you will go home from hospital with no additional support required. In some cases you may require some extra support to regain your independence. Your hospital team will help you, your family and carers to access these services if you need them.

Your safe and timely transfer from hospital will also allow new patients who need acute hospital treatment to be admitted without delay.

We fully support your transfer out of hospital for the following reasons: 1. To support you to regain your independence 2. Some people find it harder to return home the longer they stay in hospital 3. Acute beds are needed for people who are very unwell, and delays in transfer may result in patients waiting in the Emergency department for a bed to become available.

4. Although we work hard to ensure the highest standards of care there is a risk of acquiring infections in hospital. Leaving hospital as soon as you are medically fit means this is less likely and reduces the risk of any complication.

Your Hospital

Team

A Multi – disciplinary team including your Hospital Case Manager, Consultant team, Matron and ward nurses, Occupational Therapists, Physiotherapists and Social Workers will begin to plan your transfer from hospital as soon as you are admitted.

This team of clinical experts will explain their key roles to you if they are involved in your hospital care.

The assessment Your assessment begins as you are admitted to the ward area and is a way of working out what your needs are and which ones might be putting you at risk.

The assessment process will start with discussions with your Hospital Case Manager, with you and your family / carers. This process of information gathering and clarification of the situation will indicate which members of the multi disciplinary team need to be involved in your assessment.

Your Hospital Case Manager will work alongside you to: • Identify your needs • Establish which members of the Multi disciplinary team may need to be involved in your care and assessment • Provide you with information about services that could support you as you are discharged from hospital

Therapies You may be referred to a therapist to assist in planning for your discharge from hospital.

This could be someone from a number of different professions; a dietician, occupational therapist , physiotherapist or a speech and language therapist. They will discuss with you and assess what you are able to do; and what

you might need more help with. They will then discuss with you what treatment, help or support is available to improve your independence. The therapists will discuss with the other members of the hospital team their findings and together with you and your family make a plan for your discharge from hospital.

Dementia practitioner

During your hospital stay we have a dementia practitioner who is available to provide support for patient’s relatives and carers.

Intermediate Care Here in Liverpool, intermediate care can support people as they are discharged from the acute hospital who are not quite ready to return home.

• Over the age of 18

This period of intermediate care can provide rehabilitation from therapy staff or a period of reablement prior to returning home. Your hospital team will work with you to assess your level of need and which intermediate care service which would suit your needs should you require it.

Intermediate care is provided under Health and Social Care joint Commissioning arrangements and is free of charge whilst you are on the intermediate care pathway.

All patients who no longer need acute care but cannot return home and do not require specialist care are eligible for intermediate care if they meet the following criteria:

• Resident of Liverpool or • Registered with a Liverpool GP

People may be eligible for intermediate care following an episode of acute care for a further short period of recovery and or reablement.

This can benefit patients and enables a more accurate assessment of your ongoing care needs prior to returning home or before long term care is considered.

A wide activity programme is on offer both in the centre and off site, along with life skills and training opportunities such as employability skills and the opportunity for volunteer work placement schemes. A full pick up and drop off service is available in the Sefton area.

Intermediate Care

– Aiming to get you home

The aim is to help you to relearn daily living skills so you do not need long-term support or go back into hospital.

You need to know what you want to achieve and how this will be done in hospital to establish your reasons for going into Intermediate Care through your multi disciplinary team assessment. First route out of hospital should always be your own home. If this is not possible, then the Intermediate Care aims to get you home and to eliminate dependency on long term support.

Each facility supports intensive short-term community bed based services under the umbrella of re-ablement services.

• Provides a service to support hospital discharges and admissions for people who have had a stroke. Service delivery is supported by the acute trust through Physiotherapist, Occupational Therapists, Speech and Language specialists and a range of social and health support services for individuals and their carers.

An integral day facility has been commissioned and this will provide a unique opportunity to support people who have poor or limited mobility, are prone to falls.

Re-ablement services help to ensure people receive the right level of support at the right time.

Merseyparks Calderstones Unit (15 beds)

Liverpool City Council provides a model of health and social care support through community bed based centres. The model creates a circle of health and social care support for adults.

The Model of care:

We aim to support patients as they are discharged from hospital to reduce the length of time you stay in hospital and can offer a more realistic assessment of your future support needs.

We operate a Multi Disciplinary Team approach which comprises of Social Worker, Community Matron, Therapists, General Practitioner, Access coordinator (social care) and appropriate attendees related to your care programme.

day facility has been attached to the main building and is fully operational. The new build provides carer support, drop-in facilities, café, day support, community outreach work, advocacy, crisis intervention, health and well being advice and support.

99 Mill Street, Liverpool L8 5XW Tel: 0151 709 4791 Residential ICB unit • provides a generic re-ablement service for people, supporting hospital discharges and admissions for people recovering from a period of medical intervention. Granby works closely with the local community groups, schools and wider neighbourhood networks to ensure inclusive holistic approaches to providing and delivering localised services.

Granby (30 beds) 50 Selbourne Street, Liverpool L8 1YQ Tel: 0151 233 8631 • is a specialist intermediate care unit, designed specifically to help you regain mobility and confidence, in order to live independently again. Once admitted to Granby, you

will be assessed by a qualified team of a Physiotherapist, Occupational Therapist and Social Worker, who will develop an individual therapy programme specifically to meet your needs. This may involve staff working with you on a daily basis to complete exercises to improve your mobility and to ensure that all support you may need is in place.

Intermediate Care bed based services provide assessment, care and treatment for you and discuss with you and your family or carer what care and support you need. During your stay, you may move from one part of our service to another depending upon your assessed needs. This may include transfer to an alternative care setting.

During your admission you will be assessed by our therapy team who will discuss any treatment plans, goals and frequency of interventions with you. Therapy treatment may

be delivered on the ward or in the gym and will consist of chair based exercises, walking practice and functional activities, treatments will be carried out by a member of the multi disciplinary team where it is deemed appropriate to do so. It may be necessary to carry out a home assessment prior to your discharge and if so we will ask you to provide access to your home and ensure that you have appropriate outdoor clothing for the assessment.

If the Multi disciplinary team deems that you are medically fit for discharge but you are not ready for discharge home, you may move to another part of the service dependent upon your assessed needs. This may include transfer to an alternative care setting.

If required you will be assessed by a Social Worker or community care assessor to find out if you require ongoing support at home.

Intermediate

care at home:

Liverpool Out of Hospitals Service Liverpool Out of Hospital Service (LOOHS) is a multidisciplinary team of Health and Social Care professionals including Nurses, Occupational Therapists, Physiotherapists, Podiatrists, Social Workers and Support Workers.

LOOHS supports people and their carers when they are in health and / or social care crisis. The aim is to prevent people from being admitted to hospital if possible, support people to return home after a recent hospital admission, and enable people to live at home rather than in a care home, if they choose.

The rehab and reablement elements of the service aim to enable people to recover from recent loss of function, due to an illness,

or fall, for example. Therapists work in partnership with the local authority to develop a plan of care and rehab to enable people to regain their independence.

LOOHS tends to provide lots of support in the beginning and gradually reduces this support as people become more independent, but identifies that some people will need that care and support for the long term, and arranges that for them.

If you are assessed as requiring support from LOOHS you will be referred by a health or social care professional in the hospital.

What is Home First?

Home First is a partnership service provided

by Liverpool City Council, the Royal Liverpool and Broadgreen University Hospital, Aintree University Hospital, and Liverpool Community Health Trust.

Home First helps you to return home as soon as possible when you no longer need hospital treatment. The service will try get you back to doing all the things you were able to Every week, do before your the Home First hospital stay.

Research shows that people are less likely to need long-term care and will stay independent for longer if they are supported to continue their recovery at home instead of staying in hospital.

How does Home First work?

When you are almost ready to be discharged one of the hospital team, such as a physiotherapist or an occupational therapist, will check if Home First is right for you.

If it is, arrangements will be made for you to return home as soon as you are ready, including transport if you are otherwise unable to make the journey.

When you are at home, the Home First support team will help you to get back to doing dayto-day tasks. You will also get support from health staff if you need it.

As far as possible, we will do our best to provide support at a convenient time for you.

How long does Home First last?

Most people do not need Home First for longer than three weeks and it is not provided for more than six weeks.

Every week, the Home First support team will check how you’re progressing. When you are able to take care of yourself safely the service will end.

What if I still need care?

If it becomes clear whilst you are having Home First that you will need long-term support, your needs will be assessed by a social worker and a care plan will be put in place.

Liverpool City Council’s Help to Live at Home care providers will give this new support.

Will I be charged?

There is no charge for the Home First service.

You may be asked to contribute towards the cost of any Help to Live at Home support you need. To work out your contribution, you will be offered a financial assessment.

The assessment looks at your income and any savings you might have. You will only ever be asked to pay a fair amount that takes into account your income and any savings you might have.

If you refuse to have a financial assessment you will be asked to pay the maximum contribution.

We will only transfer you when the appropriate placement has been identified and confirmed.

The Motability Scheme The Motability scheme allows some disabled people getting DLA, PIP or War Pensioners’ Mobility Supplement to obtain a car, powered wheelchair or scooter.

The scheme uses some or all of your mobility benefit payments to cover the cost of “contract hire” or “hire purchase” of an appropriate vehicle. You can also pay an extra amount of money if you want a more expensive vehicle.

With hire purchase, the price of the car will be agreed directly with the Motability dealer, and you will own the vehicle outright at the end of the agreement.

With contract hire, you won’t own a vehicle, but you will get a new car every three years, full insurance for the driver and passengers, servicing, maintenance and repairs, vehicle excise duty, replacement tyres and breakdown cover.

Adaptations to the car can be made if needed, although there may be an extra charge for some adaptations. Under the Motability scheme, cars can also be adapted for people in wheelchairs.

If you don’t need or want a car, you can transfer your allowance to lease a scooter or powered wheelchair.

Sefton Council Adult Social Care Across the country Social Care has changed!

Within Sefton we have changed the way we commission and deliver services as well as how we work with partners, communities and providers.

This means that we deliver improved outcomes and reduce the longer term reliance on public sector services.

With the introduction of the new Care Act 2014, reduction in Government Funding and with our ageing population these changes were essential.

The Council has designed social care services and activities that are modern and flexible with self care and self management at the core. As a result social care staff will assess what people, families and communities can do for themselves with a focus on keeping active, healthy, well and living independently.

These changes have resulted in a social care service that is sustainable and meets the needs of our most vulnerable.

Improving your health and wellbeing Getting healthier is one of the easiest things you can do to make your life better: • You will be able to do more for yourself •Y ou can meet more people and build your social network by making new friends •Y ou can improve your wellbeing.

Wellbeing Sefton is a range of activities that support adults to manage their own wellbeing. The types of activity vary from information and advice to drop-in sessions or a regular weekly programme. The programmes encourage people to learn skills in coping and self-care.

Research shows that reading improves mental wellbeing. Mood-boosting books are aimed at adults, older people and teenagers, particularly those who might have experienced mild to moderate mental-health conditions linked to stress, anxiety and depression. If you are feeling stressed, depressed or just want a bit of a pick me-up, boost your mood with a good book.

The Active Lifestyles, Relax and Revive – mental wellbeing and Creative Alternative Programmes are well established in Sefton and demonstrate an increase in overall wellbeing. A positive sense of wellbeing has encouraged participants to change some of their other habits such as reducing smoking and taking up new interest such as volunteering. No smoking policy From 1st July 2007 it became illegal to smoke in virtually all public places. The Council needs to ensure steps are taken to minimise the risks associated with second hand smoke when staff visit private homes and residential accommodation.

Clinical Commissioning Groups There have been some important changes to the way your NHS plans and buys, or ‘commissions’ health services in Sefton.

NHS South Sefton Clinical Commissioning Group (SSCCG) and NHS Southport and Formby Clinical Commissioning Group (SFCCG) are the bodies responsible for ensuring that Sefton residents have the right health services for their needs, which are also effective, efficient and of the highest possible quality and safety.

These two organisations took over the majority of the local primary care trust’s work when it was abolished on 31 March 2013. They are made up of doctors, nurses, practice staff and members of the public from Sefton, who are best placed to know the health needs and views of people living in the area.

Their membership brings together all doctors’ surgeries in Sefton, stretching from Churchtown in the north, Bootle in the south and Melling and Lydiate to the east, and which serve around 280,000 people.

SSCCG and SFCCG are working closely with a wide range of partners - including local people, Sefton Council, hospitals and community and voluntary organisations to make sure that health and health services across the borough continue to improve in the future.

NHS continuing healthcare What is NHS continuing healthcare?

NHS continuing healthcare is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital and have been assessed as having a "primary health need".

Where is NHS continuing healthcare provided?

If you are eligible, you can receive NHS continuing healthcare in a variety of settings, for example: • in your own home – the NHS will pay for healthcare, such as services from a community nurse or specialist therapist, and personal care, such as help with bathing, dressing and laundry • in a care home – as well as healthcare and personal care, the NHS will pay for your care home fees, including board and accommodation NHS continuing healthcare is free, unlike social and community care services provided by local authorities. You may be charged for these depending on your income and savings.

Who is eligible for NHS continuing healthcare?

To be eligible for NHS continuing healthcare you must be over 18 and have substantial and ongoing care needs. You must have been assessed as having a "primary health need", which means that your main or primary need for care must relate to your health.

Eligibility for NHS continuing healthcare does not depend on: • a specific health condition, illness or diagnosis • who provides the care, or • where the care is provided If you have a disability or if you've been diagnosed with a long-term illness or condition, this doesn't necessarily mean that you'll be eligible for NHS continuing healthcare.

How will my eligibility for NHS continuing healthcare be assessed?

For most people, the first step is to have an assessment with a health or social care professional using a screening tool called the Checklist Tool. If this screening suggests that you may be eligible for NHS continuing healthcare, a full up-to-date assessment of your needs will be arranged, using a tool called the Decision Support Tool.

You should be fully involved in the assessment and decision-making process. Your views about your needs for care and support should be taken into account. If you feel that you need help to explain your views, you may want to ask a friend or relative for support.

Initial screening with the Checklist Tool The Checklist Tool is used to decide whether you should be referred for a full assessment. It helps health and social care professionals to work out whether your care needs may be of a level or type that indicate you may be eligible for NHS continuing healthcare.

The Checklist Tool is usually completed when a nurse, doctor, other qualified healthcare professional or social worker is assessing or reviewing your health or social care needs.

For example: • a nurse or doctor could carry out the initial screening before you leave hospital • a social worker could do the screening when carrying out a community care assessment After the initial screening, your local clinical commissioning group (CCG) will write to you to confirm whether or not you will be referred for a full assessment for NHS continuing healthcare.

Full assessment with the Decision Support Tool If the Checklist Tool shows that you may be eligible for NHS continuing healthcare, the person who completed it will contact your CCG to arrange a full up-to-date assessment of all your care needs.

The full assessment will be carried out by a multidisciplinary team made up of a minimum of two different health or care professionals who are already involved in your care, to build an overall picture of your needs. In some cases, more detailed specialist assessments may be required from these professionals.

The information from your assessment will be used to complete the Decision Support Tool. This tool is used to assess whether your main or primary care needs relate to your health, by looking at the following types of care need: • behaviour • cognition (understanding) • communication • psychological / emotional needs • mobility (ability to move around) • nutrition (food and drink) • continence

your own home or in a care home • which organisation will be responsible for meeting your needs As part of the discussion about how your needs will be met, your wishes should be written down and taken into account.

What happens if I’m not eligible?

If you’re not entitled to NHS continuing healthcare, your local authority will discuss with you whether you may be eligible for support from them.

If you’re not eligible for NHS continuing healthcare but you still have healthcare needs, the NHS may still pay for part of the support that you need. This is sometimes called a joint package of care.

Care provided by local authorities Local authorities, such as town, city, borough, metropolitan and county councils, provide social and community care services, including:

• skin (including wounds and ulcers)

• personal care at home, such as help with bathing, dressing and getting up and going to bed

• breathing

• delivery of hot or frozen meals

• symptom control through drug therapies and medication

• day centre facilities

• altered states of consciousness

What if I still have health needs?

• other significant needs

If you’re not eligible for NHS continuing healthcare, but you have health needs, the NHS may still pay for part of your care. This is sometimes known as a joint package of care.

For individuals who need an urgent package of care because their condition is deteriorating rapidly, the Fast Track Tool may be used instead. This enables the CCG to arrange for care to be provided as quickly as possible.

What happens after my assessment for NHS continuing healthcare?

After your eligibility for NHS continuing healthcare has been assessed, the multidisciplinary team of health and social care professionals who carried out the assessment will make a recommendation to your clinical commissioning group (CCG).

The CCG will then write to you advising whether you’re eligible, giving reasons for their decision.

What happens if I’m eligible?

If you’re eligible for NHS continuing healthcare, the next stage is to arrange a care and support package that meets your assessed needs. The CCG will discuss with you: • how your needs and care will be managed • where your care will be given, for example, in

• care provided in a care home

One way this may happen is through NHS-funded nursing care.

Local authorities are allowed to charge for the services they provide, and most of them do so. If your local authority is part-funding your care package, you may have to pay towards the cost of their part of the care, depending on your income and savings. Some authorities have set maximum amounts that you can be charged, which varies from area to area.

When will my needs for NHS continuing healthcare be reviewed?

If you receive NHS continuing healthcare or any other long-term care funded by the NHS, your needs will be reviewed no later than three months after the initial decision was made about your care. After that, your care needs should be reviewed at least once a year. Some people will need reviews more often than this.

The review will consider whether your existing care and support package meets your assessed needs. If your needs have changed, the review will also consider whether you remain eligible for NHS continuing healthcare.

What is NHS-funded n NHS-funded nursing care is care provided by a registered nurse for people who live in a care home. The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

Who is eligible for NHS-funded nursing care?

You may be eligible for NHS-funded nursing care if: • y ou are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse • you live in a care home registered to provide nursing care How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Most people don't need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you.

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home. Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If your care needs have changed, your package of care may need to be altered. This may mean that responsibility for the funding of your care could also change.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, you can ask your CCG to review the decision.

Neither the NHS nor your local authority should withdraw from an existing care or funding arrangement without a joint review and reassessment of your needs.

You should be informed about any proposed changes, and the NHS and local authority should ensure that alternative funding or services are in place.

What is the rate of payment for NHS-funded nursing care?

NHS-funded nursing care is paid at the same rate across England. For 2016/17, the rate is £156.25 a

nursing care? week (standard rate).

The increase will be backdated to April 1, 2016 for people who are getting NHS-funded nursing care from that time. This new rate is under review and may change on the January 1, 2017.

Before October 1, 2007, there were three different levels or bands of payment for NHSfunded nursing care – low, medium and high.

If you moved into a care home before October 1, 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1, 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. For 2016/17, the higher rate is £215.04 a week. You’re entitled to continue on this rate unless: • you no longer have nursing needs • y ou no longer live in a care home that provides nursing • y our nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £156.25 a week, or • y ou become entitled to NHS continuing healthcare instead.

Paying for your own

care and support

Many people who use care and support services will pay for all of the costs. This is known as being a "self-funder".

The cost of your care will vary depending on its type, intensity, specialisation, location and duration. For example, a place in a residential care home will cost hundreds of pounds a week.

To make decisions that have such major financial implications, you may want to seek independent financial advice and it’s always worth researching the costs of alternatives first. For example, if you are considering a care home place, the cost should be weighed against the cost of care and support that may help you remain in your current home, such as homecare.

How much will care cost?

If you are thinking about your future care needs or are facing immediate decisions about care options, it can be helpful to get an idea how much care can cost.

Inevitably, the price you would pay will depend on your particular circumstances and needs. The costs also vary depending on where you live. Unfortunately, care homes and homecare agencies tend not to provide this information publicly but you may find it helpful to search for and contact care services in your area to get some idea of likely costs.

The ‘cap on care costs’ Currently, it is not easy to plan for your future care needs, as it’s hard to estimate how long you will need care for and how your circumstances may change. From April 2020, a "cap" on the costs of meeting your eligible care needs (but not accommodation associated with care or nursing costs) is being introduced. The cap means that, once reached, the local authority will take over paying the cost of their eligible care needs.

The cap will not cover your daily living costs. These costs include expenses such as rent, food and utilities and the costs you would face even if you did not have care needs. It is important to note that daily living costs will be a nationally set figure. This figure will be a "notional" amount rather than the actual costs. This is designed to support consistency

To benefit from the cap once it comes in, you will need to contact your local authority to see if your needs are eligible. If you are seen to have eligible care needs, the local authority will open a "care account" for you. This account records your progress (the amount of costs you have incurred) towards the cap.

The cap does not begin until April 2020 and any costs incurred before this will not count towards the cap.

The BBC’s care calculator can estimate how much you may have to pay for care services depending on where you live in England, once the new rules are in place. This will give you an idea of how the cap will work for you, but actual costs will vary to reflect your individual circumstances and needs and how they change over time.

Ask for help from your local authority It's worth checking whether you're eligible for means-tested support from your local authority or other financial support – for example, through a care needs assessment and a financial assessment.

Few of us will have the income or ready access to the cash to pay for our ongoing care needs, and you may need to look at selling or remortgaging any property you may own. The new Care Act means more people may be able to benefit from "deferred payments".

Deferred payments can help people avoid being forced to sell their home in a crisis in order to pay for their care by having the council temporarily cover the cost – usually until you sell your property.

Following the Care Act, every local authority in England has to make deferred payment agreements available.

As an alternative, you may be able to enter an "equity release scheme" with a financial organisation. Equity release can pay for the fees from the value of property you own. However,

you should consider which of these options best meets your needs, and what the overall costs to you will be.

Before taking such significant financial steps as equity release, you might want to get independent financial advice. You can find information on equity release for care at home from Which?

If you’re planning ahead, you may consider arranging an investment or insurance plan to fund your care. Again, it may be worth taking independent advice on financial arrangements before making major changes. Because of the new rules, there are likely to be more financial products emerging that are designed to help people pay for care.

You may also want to explore whether the NHS would meet some or all of your care and support costs, or you may have entitlement to benefits that may help you meet costs.

Advice on paying for care Even if your local authority is not able to help fund your care, it will be able to make an assessment of your care and support needs. From this, the local authority can provide you with access to a range of information and advice available locally.

• Age UK: has great advice for older people and those planning for their later years.

• Carers UK: an excellent resource of advice for carers who need to help someone else.

Deferred payments if you are unable to pay for care services Care home fees are a big financial commitment, and the decision to go into a care home is often made at a moment of crisis or urgency, such as when being discharged from hospital. This can make finding the money to pay for fees (usually several hundred pounds a week) challenging for people funding their own care.

In particular, if you own property outright but have little in the way of savings, you may be expected to fund your own care but have little immediately available money to pay for it. Some people going to stay in a care home for a long time find that they have no option but to sell their property to pay the care home fees.

Your council may be able to help you if you are at risk of having to sell your home. Where there is a delay in selling the property, or you don’t want to sell the property immediately, you may be eligible to have your care fee payment "deferred".

This is where the local authority helps you to pay your care fees temporarily, and you repay the costs to the local authority at a later date.

If the local authority agrees to the deferred payment and pays the care home fees, it will take payment from the money raised once the property is sold.

This can be: • during your lifetime if you choose to sell your home

• once you die, the local authority can be repaid from your estate All councils must offer people the chance to defer payment if they meet a set of eligibility criteria.

A deferred payment scheme is only available if you don't have enough income to cover your care home fees, and you have less in savings than £23,250. In these circumstances, the savings don't include the value of your property, but does include money in bank accounts.

The local authority will put a "legal charge" (similar to a mortgage) on the property and will then pay the remaining care fees in full. You will then be assessed to see whether they are able to pay a weekly charge to the authority. Your ability to pay is based on your income less a set "disposable income allowance" (currently £144 per week).

Before considering deferred payments, you should look into whether the property will or won't be counted in how your capital is calculated. For instance, it may be disregarded because your partner still lives there, and if it is a deferred payment agreement wouldn't be necessary.

It's wise to get independent financial advice before agreeing to a deferred payment, and it’s worth bearing in mind that choosing deferred payments can impact on some welfare benefits.

Protection for self-funders While you may have the savings in place to afford care services, if you lack the capacity to make the arrangements, the local authority can step in to help.

The local authority can also help people who lack capacity by negotiating fees with a care provider and paying them directly. The local authority will need to be reimbursed.

Anyone entering into a contract for care services should be given adequate information about the fees. Care providers should supply: • information about the fees charged for various services provided • arrangements for paying the fees • the fees charged for any additional services If your capital falls below the set levels for local authority funding (currently £23,250), you will be eligible for reassessment for help with funding your care.

Care services in your home If you need help around the home, a good option is to have a care worker come in to your home to help you.

themselves. Alternatively, you can arrange your own care, funded by the local authority, through direct payments or a personal budget.

Types of care and support

If you have chosen direct payments or a personal budget, or you aren’t eligible for local authority help and want to get care privately, you can arrange it in several different ways.

Care and support comes in many forms and has many names used to describe it, including home help, care attendants and “carers” (not to be confused with unpaid family or friends who care for you).

Care and support can suit you if you need: • personal care, such as washing or dressing • housekeeping or domestic work, such as vacuuming • cooking or preparing meals • nursing and health care • companionship Care and support can be very flexible, in order to meet your needs, and the same person or agency may be able to provide some or all of these options for the duration of your care: • long-term 24-hour care • short breaks for an unpaid family carer • emergency care • day care • sessions ranging from 15-minute visits to 24-hour assistance and everything in between If you already know what you want, you can search NHS Choices directories for: • local care and support services and agencies • a list of national care and support organisations • services that can help you stay safe and well in your home on a long-term basis; these services, often known as “supported living services”, can include financial, help with medication, advocacy, social and practical support • a place to live in a family who will care for you, known as “shared lives services” or adult placement services

Independent care and support agencies If you use an independent care and support agency, you or the person you’re looking after has to find the care agency and pay them.

The agency will provide a service through a trained team of care workers, which means you may not always have the same person visiting your home, although the agency will do its best to take your choices into account. Independent care and support providers are regulated by the Care Quality Commission (CQC). Care and support agencies must meet CQC’s national minimum standards and regulations in areas such as training and record-keeping. The CQC has the power to inspect agencies and enforce standards.

Care and support agencies must vet care and support workers before engaging them by taking up references and carrying out Disclosure and Barring Service (DBS) checks on potential employees. Care and support agencies can also: • take over the burden of being an employer – for example, payroll, training, disciplinary issues and insurance • train their care and support workers through national qualifications and servicespecific training • replace workers when they are ill, on holiday or resign • put things right when they go wrong

If you believe that you might benefit from some help at home, the first thing to do is to contact your social services department to ask for an assessment of your care and support needs. To contact social services, go to GOV.UK: find your local authority.

An agency will want to see you and the person you’re looking after so that they can assess your needs. This also means that a joint decision can be made about the most appropriate type of care and support.

What are the disadvantages of using a care and support agency?

The main disadvantage is the cost of using an agency. The agency will charge a fee on top of the payment made to the care worker to cover their running costs and profit.

You normally have to make a regular payment to the agency, which includes both the worker’s earnings and the agency’s fee.

Questions to ask when using a care and support agency The fees some agencies charge can be quite high.

Before deciding to go ahead with an agency, you should ask questions about the fee and what it covers, including: • Does the agency check references?

• What training and supervision do they provide?

• What is their complaints policy?

• Who will be responsible for insurance?

• Is there any out-of-hours or emergency contact if needed?

• Will they be able to provide staff if your own care worker is ill or away? (If an agency contracts to provide care every day, it must ensure that it does.)

Safeguarding vulnerable groups The DBS makes decisions about who is unsuitable to work or volunteer with vulnerable adults or children. It makes this decision based on information held by various agencies and government departments.

The service decides who is unsuitable to work or volunteer with vulnerable adults or children.

If someone who is barred from working with children or vulnerable adults is working, volunteering or trying to work or volunteer with these groups, they are breaking the law. They could face a fine and up to five years in prison.

Employers must apply for an enhanced DBS check (formerly known as a CRB check) when taking on new employees or volunteers to work with vulnerable adults or children. This includes a check of the barred lists. If an organisation fails to make the relevant checks, they can be penalised.

Hiring a personal assistant

If an organisation dismisses an employee or volunteer for harming a child or vulnerable adult, they must tell the DBS. The DBS must also be notified if any employee or volunteer harms a child or vulnerable adult, but isn’t dismissed because they leave voluntarily. If their organisation does not tell DBS, they will be acting illegally.

You can hire a “personal assistant” to act as a care and support worker for you.

Personal assistants can offer you all that you’ll get from an agency worker, but you’ll also get the continuity, familiarity and ongoing relationship with your assistant.

Employing a care worker on a private basis

However, if you employ a personal assistant, you will then have the legal responsibility of an employer. This will include arranging cover for their illness and holidays.

Care and support from charities Charities such as Age UK and Carers Trust can provide home help and domestic assistance services. The Carers Trust supports carers by giving them a break from their caring responsibilities through care and support services.

Marie Curie Nurses can provide practical and emotional support for people near the end of their lives in their own homes.

If you employ a care worker privately, you will not be obliged to use the DBS scheme, but you can use it if you choose to. You need to ask social services or the police to make the checks on your behalf. The care worker must have already applied to be vetted, and must consent to the check.

If you have concerns about the suitability of someone you employ privately to work with a vulnerable adult or child, you can ask social services to investigate the matter. They can refer the worker to the ISA on your behalf.

The Friends and Family Test What is the Friends and Family Test?

The NHS wants to ensure that you have the best possible experience of care. The Friends and Family Test is a way of gathering your feedback about this experience and helping to drive improvement in hospital services.

When you receive care as an inpatient or in an Accident and Emergency (A&E) department, you will be given the opportunity to give your feedback by answering a simple question about your experience.

The results will provide a way for you to easily compare NHS hospitals so that you know where you and your family can get the best possible care.

The information will also give the NHS invaluable information on what patients think of services, which can be used to help make improvements if required.

How will it work?

When you are discharged, or within the 48 hours that follow, you will be asked to answer the following question: “How likely are you to recommend our ward/A&E department to friends and family if they needed similar care or treatment?”

You will be invited to respond to the question by choosing one of six options, ranging from ‘extremely likely’ to ‘extremely unlikely’.

It is really important to us that you tell us why you gave your answer, so please answer any follow-up questions.

Your answer will not be traced back to you, and your details will not be passed on to anyone, so please tell us exactly what you think. A member of your family or a friend is welcome to help you give your feedback to the question if you are unable to.

We publish independent inspection reports and ratings about services – information you can use when you’re choosing care for yourself, or a loved one.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

CQC’s Chief Inspector for Adult Social Care, Andrea Sutcliffe always uses ‘The Mum Test’: is a care home safe, caring, effective, responsive to people’s needs and well-led? In other words, is it good enough for my Mum (or anyone else I love and care for)?

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive.

They should also be capable and confident in dealing with your particular needs. You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

If you or a loved one needs help with day-today care, you can contact your local council’s social services department. They will ‘make an assessment of your needs’ and depending on circumstances, may be able to help you access financial help. For more advice visit Age UK’s website www.ageuk.org.uk /home-and-care.

If you would like to organise your care yourself, you can find a care worker or personal assistant through an agency. Your local social services department should be able to provide details of approved agencies.

Safeguarding adults who receive social care is everybody’s business. If you are concerned about the safety of a loved one receiving care, contact the service provider in the first instance. You can also contact social services at your local council. If you feel a crime has been committed, contact the police.
