Welcome and

introduction

This guide is intended to help you, your carer and relatives understand the processes and assessments that may take place while in hospital. It also gives you information to help you live independently and access support services in your area.

Our hospital team Rotherham Council has a Supported Discharge Pathways Team based in Rotherham Hospital. This team is made up of Team Mangers, Social Workers, Wellbeing Advisers, Independent Living Officers, Team Assistant and Support Planners.

Do I need to be referred to the Supported Discharge Pathways Team?

Planning for discharge normally begins at the point, or even before, you are admitted to hospital. The discharge process will be coordinated by a member of the nursing team on the ward. Following an initial assessment by nursing and medical staff, you will be referred, with your consent, to the hospital social work team if you appear to have social care needs

that require assessment prior to your discharge. This is done by the ward sending a request for assessment notification. You can also give your consent to your carer who can ask for you to be referred to the Supported Discharge Pathways Team. If you have any queries or want any general advice you can contact a Wellbeing Adviser on 01709 424650.

The assessment process If you agree to an assessment a member of the Supported Discharge Pathways Team allocated to your particular ward will contact you. The assessment is not a test, but a way of working out what your needs are and which ones might be putting you at risk.

The assessment process will start with discussions with the ward staff, with you and your family/ carers. This process of information gathering and clarification of the situation will enable the member of the Supported Discharge Pathways Team to work alongside you to: • identify your needs • establish your eligibility • give you information about services that could immediately support you to be discharged from

hospital or support you in the longer term.

Depending on how complex your care needs are, other specialist assessments may also be required.

The referral process and time scales for assessments are outlined in the Care Act 2014. The length of time required to complete the process can vary depending on how complex your needs are.

In Adult Social Care, we aim to do what we can to help people to live independently and safely. We ensure that we prioritise the people who are in greatest need of help.

This booklet tells you more about the assessments and eligibility criteria. It will also tell you about what will happen after you have had an assessment.

We know that there are some things which can be difficult or stressful to talk about, but please don't be embarrassed. We will understand your feelings and respect them.

We can provide access to an advocate to assist you with the process.

If your first language is not English, or if you use sign language, we can arrange for an interpreter. Please let us know what you need.

More about the assessment Once the ward has asked for an assessment, one of our staff will come and visit you on the ward. You can have a friend or relative with you, if you wish, at the assessment.

The assessment is free and will involve discussions about; • What you are able to do for yourself • What you would like to achieve • What type of help you feel is needed • What difficulties you are having at the moment • How stressful and urgent you feel your situation is Some of the topics of discussion will be; 1) How much control you have over your own life, and how easy or difficult you find it to make decisions about the way you live

2) Your health and safety, including any risks to your mental health or wellbeing 3) Your daily routines, such as personal care and domestic chores, and how well you are able to manage them 4) How well you are able to involve yourself in family responsibilities, community life, work or study.

We may want to get information from other people, such as the ward staff, Doctor, Therapists and family etc. If you have a Carer - a relative or friend who provides you with unpaid support - we may want to talk to them about what support they are able and willing to give.

The Care Act 2014 means carers may also be entitled to support with their caring role. If you have an identified Carer, this would also be an appropriate time for them to receive a carer's assessment. We will only share information about you with your consent.

On the assessment form we write down everything we have discussed and agreed, and also anything about which we might have disagreed. You will get a copy of the completed assessment form to keep. This will contain any contact numbers you may need following your discharge and details of any agreed contingency plans. If you have additional communication needs, i.e. large print, we can ensure you get any information from us in the correct format.

Following the assessment, we will give you information about support that may be available, and any charges involved.

iAgeWell - helping Rotherham to age well Rotherham Council has launched the iAgeWell Rotherham online tool to help residents improve their chances of living well for longer.

Developed by experts at Newcastle University and ADL Smartcare Limited, the free tool gives practical advice how to deal with the 15 stages of later life decline, which include the ability to cook a hot meal, getting dressed and walking without difficulty. The tool gives access to expert advice and suggests products, activities and services suitable for an individual’s specific needs.

How do we decide what help you receive?

National Eligibility Criteria Social care is not free for everyone. We have to balance the needs of adults in Rotherham with the limited amount of money we have.

We do this by giving help and support to the people who are most at risk.

We use guidance to decide who needs the most help, and what help and support we can give. The guidelines are called 'Eligibilty Criteria', and are set out in the Care Act 2014.

In the Eligibility Criteria guidelines, the areas of risk are:(a) managing and maintaining nutrition; (b) maintaining personal hygiene; (c) managing toilet needs; (d) being appropriately clothed;

(e) being able to safely make use of the adult's home; (f) maintaining a habitable home environment; (g) developing and maintaining family or other personal relationships; (h) accessing and engaging in work, training, education or volunteering; (i) making use of necessary facilities or services in the local community including public transport, and recreational facilities or services; and (j) carrying out any caring responsibilities the adult has for a child.

Once we have completed your assessment we will look at how we can utilise support from friends, family and community services, prior to loking at commissioning Council services.

Paying for my care and support Social care, unlike health care, is not free for everyone. The financial assessment process helps us to work out how much the Council can pay towards your care and support needs, and for what costs you will be responsible.

If you are an adult with eligible needs and have assets above the capital threshold of ÂŁ23,250, you will be responsible for meeting the full cost of your care and support. However, if you need help arranging services, we can do this with you and offer you advice and information.

If you do not have assets above the capital threshold, you may still need to pay towards your care and support depending on your financial circumstances.

From April 2017 to April 2018, carers will not be charged for services meeting their eligible support needs. If the person being cared for has agreed to receive care services to meet the needs of their carer, they may need to complete a financial assessment to determine how much, if anything,

they will contribute towards those services.

How we work out what you must pay for your care and support Unless you are a carer, you will need to complete a financial assessment form. This will tell us about your financial situation so that we can work out if you will need to pay and if so, how much you pay towards your care and support services.

If it is clear that you have significant financial resources to use to pay for your care and support, and you do not wish to undergo a full financial assessment, we may offer a ‘light touch’ financial assessment.

If you do not agree with the light touch assessment, you can ask for a full financial assessment.

How much you will pay will depend on whether you are receiving care in a care home, your own home, or another setting.

In the following circumstances, the value of your main or only home will not be taken into account in the financial assessment:

• if you are receiving care in a setting that is not a care home.

• if your stay in a care home is temporary.

• where you no longer occupy the property (e.g. you live in a care home) but: - your partner continues to live in the property.

- or a family member who is over 60 continues to live in the property.

- or a family member who is under 60 and incapacitated continues to live in the property.

- your child under the age of 18 continues to live in the property.

If you have savings between £14,250 and under the self-funding threshold of £23,250 you will be deemed to have an income of £1 per week for every £250 (or part thereof) you have above £14,250.

This is called Tariff Income.

Any money you receive as regular payments such as state or private pensions and benefits will be taken into account as income. However, any money you

receive from employment is not taken into account.

If you live in a care home, you will contribute most of your income towards the cost of your care home fees. However, you will always have at least £24.90 per week for personal expenses.

If you live in your own home, you will be left with at least the minimum income guarantee amount for your age (equivalent to Income Support or Pension Credit Guarantee Credit) plus a 25% buffer.

These capital thresholds, personal expenses allowances and minimum income levels are subject to change based on annual review and Government policy.

We will give you a copy of your financial assessment once it has been completed.

Carers Carers’ Corner is a resource centre for Carers, former Carers, and professionals working with Carers.

This service is a central point of contact to signpost Carers to available support services in the community, whilst referring those with complex needs to statutory services. Carers groups meet regularly at the Carers Corner. Carers are able to access a number of training courses which are free for Carers. Please ask for details.

At Carers Corner you can get face to face advice – such as how to access benefits, advocacy, legal advice, Carers Emergency Scheme and Carers Assessments.

Carers' Corner works in partnership with the voluntary and community sector (Cross Roads Care, Carer Forum, Carers4Carers, Active Independence, Alzheimer's Society, Age UK, Unity Centre, United Multicultural Centre and others) to develop flexible enabling approaches that works with individual Carers and their families to find personalised solutions to their needs.

Carers Emergency scheme This service is free and is for all adults. If you are

faced with an emergency of your own we can help you make the arrangements, so that the person you look after still receives the right level of care. We will discuss with you and record your emergency arrangements. This will include the names of two people of your choice (friends or relatives, known to the cared for person) who we can contact if there is an emergency. If there is no one who can assist then we will meet, with as little disruption as possible, the cared for person’s needs. We will hold the information about you and the person for whom you care.

This information is completely confidential.

You will then be issued with a credit card sized card and key ring attachment containing our telephone number. These should be carried with you at all the times.

Assistive Technology – What is it?

Assistive Technology is a range of preventative equipment or sensors which are specifically designed to help support you to live independently.

These sensors link to the Council's Rothercare pendant alarm system which provides an emergency call handling and mobile response service 24 hours per day for 365 days of the year.

Some of the equipment can be linked to a Carers pager rather than Rothercare if they live in the same property.

The sensors are either worn or placed around the home and will self-alert in the event of an incident e.g. in the event of a fall, epileptic seizure, the person leaving the property at an unusual time of day, the detection of smoke, fire, gas or floods, or even to bogus callers.

We also provide a range of preventative equipment which does not require Rothercare or Carers pagers e.g. Automated medication dispensers, clocks which orientate to the time and day, easy to use telephones, as well as bespoke equipment which can improve the individuals independence without the requirement for care interventions.

Reablement Service Rotherham Council works with local residents to ensure that they are able to exercise as much independence as possible in their own homes. Integral to this is the Reablement Service. When someone has experienced a deterioration in their abilities, and needs support to regain or re-learn skills, this is where the reablement service can help. It can help with tasks such as getting washed and dressed, making a meal or getting a drink, as well as promoting social inclusion in the local area.

Through providing care and support, free to the customer for up to six weeks, we help people to regain their independence. It may be that this support includes an introduction of Assistive Technology, direct care and support to help the customer to help themselves, or

an introduction to resources in their local community. Where needed, we will help the person to engage with a range of organisations (both in the statutory and voluntary sectors) to accommodate any concerns and needs that they may present with, always focusing on the long term wishes of the customer and including their carers wherever possible and desired.

Through the inclusion of Occupational Therapy support and aids we endeavour to ensure that someone has clear and achievable goals and they are supported to meet these. The wellbeing and happiness of the customer is central to what we do, making sure that any care plans are personcentred and directly relate to what the person wants to achieve.

Intermediate care Intermediate Care is the name for support to help you regain your skills and enable you to live at home as independently as possible. In some cases hospital staff may recommend a period of rehabilitation in order to maximise your independence. A member of the Therapy Team

would assess you for this. An intermediate care service may be provided by community health and social care services to support you during this period. This service will not require input from the Supported Discharge Pathways Team, the ward will advise you about this.

Safeguarding adults at risk The Care Act places a legal requirement on all agencies to make sure that all adults (aged 18 and over) should be able to live without being harmed or be at risk of abuse/harm.

Adults who have care and support needs (visual impairment, hearing impairment, physical disability, physical ill health, learning disability, mental health issue etc) may be less able to protect themselves from harm and may struggle to share their worries.

Where a Local Authority has reasonable cause to suspect that an adult in its area (whether or not ordinarily resident there) – • has needs for care and support (whether or not the authority is meeting any of those needs) AND • is experiencing, or is at risk of, abuse or neglect, AND • as a result of those needs is unable to protect himself or herself against the abuse or neglect or the risk thereof.

Harm is defined in the Care Act as Physical – for example, hitting etc.

Sexual – for example, forcing adults to do sexual acts they don’t want to or can’t consent to (including rape, sexual assaults etc).

Financial or Material – for example taking money or anything of value from adults.

Neglect and Acts of Omission – any action that restricts choice, causes harm or isolates people, for example, not supporting them to get washed/dressed etc.

Discriminatory – It is a criminal offence to bully someone who has a disability or is “different”, or other associated hate crimes.

Psychological or Emotional – for example, threatening to leave them alone or intimidating them etc.

Self Neglect - is any failure of an adult to take care of himself or herself that causes, or is reasonably likely to cause within a short period of time, serious physical, mental or emotional harm or substantial damage to or loss of assets.

Domestic Abuse – Any incident of threatening behaviour, violence or abuse (psychological, physical, sexual, financial or emotional) between adults who are or have been intimate partners or family members, regardless of gender or sexuality.

Modern Slavery/Human Trafficking – The movement, recruitment, transportation, transfer, harbouring or receipt of people.

Organisational Abuse – Organisational abuse can typically occur in a care home, nursing home, acute hospital or in patients home setting. It can be caused by the regimentation of residents or of the individuals that are supported and the rules and regimes can be oppressive and abusive.

Organisational abuse can also be in the forms of lack of choice, lack of privacy, lack of respect or lack of dignity.

What should I do if I think something is wrong?

• If you feel a crime has been committed, contact the police on 101, or in an emergency situation, call 999 immediately.

• You can share your concerns without giving your name.

• You can complete an online form at www.rotherham.gov.uk.

• You can ring Single Point of Access Team on 01709 822330.

• You can Text2Tell on 07748 142816.

• You can also report concerns about hospitals, home care agencies and care homes to the Care Quality Commission (CQC) on 0300 616161.

Direct payments Every local Council must now offer people who need help to stay in their own home, money instead of arranging services for them. This is called direct payments.

These direct payments enable you to choose how you organise the help you need. Giving you money instead of services will give you greater control over your life, because you make the decisions about how your care is provided.

The vast majority of people getting care from the council have a right to receive a direct payment.

The member of the Supported Discharge Pathways Team who undertakes your assessment will discuss direct payments with you when they assess your care needs.

24 hour care homes If it is felt that you can no longer manage at home even with support, the decision may be for you to move to a care home. You and your family will be involved in this decision. The assessment will identify your care needs. This will help you and your family to select a registered care home which will be able to meet your needs.

There are three categories of care homes.

• Residential care – provides accommodation meals care and support throughout the day and night.

• Nursing care – offers the same as residential care with the addition of 24 hour care by a qualified nurse.

This will require a CHC assessment (see page 16).

• Dementia care or EMI (Elderly Mentally Ill) care – which provide specialist extra care for people often due to dementia or other mental health diagnosis.

You and your family have the right to choose the care home to which you will move.

During the care planning process you should be supported to identify a suitable care home which is registered to meet your needs and has a vacancy.

Your social worker/social services officer can help you with this process. However if the home of your choice has no vacancy, you may have to accept a place in an alternative care home until a place in your home of choice is available.

What the Council will pay towards care home costs Following your assessment if a decision is made for you to move into residential care you will always have to contribute something towards the cost of your care.

Each year, the Council sets a maximum figure that we will pay towards your weekly care home costs. The amounts vary depending on the type of care you receive.

The current maximum weekly Council contribution is:

Private care home £432 for residential care £436 for nursing care £467 for residential dementia care £518 for nursing dementia care

Rotherham Council care home £564 for residential care The amount you pay is worked out using a national set of rules. The Council will pay the difference between your contribution and the fee for your place in the home, so long as the home doesn't charge more than we expect to pay.

The value of your former home will be included as part of the financial assessment, however it should be disregarded if your spouse or partner lives there, or another relative lives there who is either over 60 years of age or receiving a disability benefit. Your home is also

ignored if you enter a care home for a temporary stay. If your property is taken into account, you will usually have more than the limit for getting local authority assistance.

The local authority may assist you under the ‘deferred payments’ scheme agreement.

You will always be left with a weekly amount for your own use called a Personal Allowance. This amounts to £24.90 per week.

If you need nursing care the NHS will pay a contribution towards your nursing care, which is called Funded

Nursing Care or Continuing Healthcare. The NHS will pay this money directly to the care home to cover the costs of providing nursing cover on a 24 hour, 7 days a week basis.

If you choose a care home that is more expensive than what the Council will pay, you will need to make arrangements for someone else to pay the difference.

Discharge from Hospital Once all assessments are completed the member of the Supported Discharge Pathways Team will discuss with you what help may be available and decide with you how your care needs can best be met.

Your discharge should take place in a timely fashion.

We will endeavour to give you a copy of your Care Act Assessment and support plan before leaving hospital.

If this is not possible we will give you a discharge letter giving you an overview of your planned care and your Care Act Assessment will follow in the post.

Problems or queries If you have queries or are experiencing difficulties relating to your discharge you can either contact the Supported Discharge Pathways worker working with you or speak to the nurse caring for you on the ward.

If a Supported Discharge Pathways worker has not been allocated to you, you can contact the team to

speak to a Wellbeing Adviser who will assist to resolve any problems or issues which are the responsibility of Adult Social Care.

Rotherham Intermediate Care Centre (RICC) – 01709 514003 The Rotherham Intermediate Care Centre, located on Badsley Moor Lane, is a joint partnership between Rotherham Council and Rotherham Foundation Trust. The centre is set to provide rehabilitation facilities within a day setting for the people of Rotherham.

The centre works as an intermediate care hub, bringing together therapists, specialist mental health workers and support workers who are providing services to promote and maximise independence following a patient's recent change in health and change in functional abilities or a worsening of their long-term condition. The service will provide activities which are tailormade to a patient's own interests and abilities in relation to facilitating their community goals.

Community Occupational Therapist Service – 01709 822330 The Community Occupational Therapy Team helps anyone who is elderly or disabled and is

having problems with the activities of daily living.

You can refer yourself for help, or someone else can contact us on your behalf. All contacts to the Community Occupational Therapy Service are taken by the Single Point of Access Team. They may be able to respond to your problem at the point of contact or they may pass your request to another agency, such as the Home Improvement Agency. If they are unable to do this, your request will be passed to the Community Occupational Therapy Team.

NHS continuing healthcare Some people with long-term complex health needs qualify for free social care arranged and funded solely by the NHS. This is known as NHS continuing healthcare.

Where can NHS continuing healthcare be provided?

NHS continuing healthcare can be provided in a variety of settings outside hospital, such as in your home or in a registered care home.

Am I eligible for NHS continuing healthcare?

NHS continuing healthcare is for adults. Children and young people may receive a “continuing care package” if they have needs arising from disability, accident or illness that can’t be met by existing universal or specialist services alone.

To be eligible for NHS continuing healthcare, you must be assessed by a team of healthcare professionals (a “multidisciplinary team”). The team will look at all your care needs and relate them to: • what help you need

• how complex your needs are • how intense or severe your needs can be •h ow unpredictable they are, including any risks to your health if the right care isn’t provided at the right time Your eligibility for NHS continuing healthcare depends on your assessed needs, and not on any particular diagnosis or condition. If your needs change then your eligibility for NHS continuing healthcare may change.

You should be fully involved in the assessment process and kept informed, and have your views about your needs and support taken into account.

Carers and family members should also be consulted where appropriate.

A decision about eligibility should usually be made within 28 days of it being decided that the person needs a full assessment for NHS continuing healthcare.

If you aren’t eligible for NHS continuing healthcare, you can be referred to your local authority who can discuss with you whether you may be eligible for support from them. If you still have some health needs then the NHS may pay for part of the package of support. This is sometimes known as a “joint package” of care.

NHS continuing healthcare assessments Clinical commissioning groups, known as CCGs (the NHS organisations that commission local health services), must assess you for NHS continuing healthcare if it seems that you may need it.

For most people, there’s an initial checklist assessment, which is used to decide if you need a full assessment. However, if you need care urgently – for example, if you’re terminally ill – your assessment may be fast-tracked.

Initial assessment for NHS continuing healthcare The initial checklist assessment can be completed by a nurse, doctor, other healthcare professional or social worker. You should be told that you’re being assessed, and be asked for your consent.

Depending on the outcome of the checklist, you’ll either be told that you don’t meet the criteria for a full assessment of NHS continuing healthcare and are therefore not eligible, or you’ll be referred for a full assessment of eligibility.

Being referred for a full assessment doesn’t necessarily mean you’ll be eligible for NHS continuing healthcare. The purpose of the checklist is to enable anyone who might be eligible to have the opportunity for a full assessment.

The professional(s) completing the checklist should record in writing the reasons for their decision, and sign and date it. You should be given a copy of the completed checklist. You can download a blank copy of the NHS continuing healthcare checklist from GOV.UK.

Full assessment for NHS continuing healthcare Full assessments for NHS continuing healthcare are undertaken by a “multidisciplinary team” made up of a minimum of two health or care professionals who are already involved in your care. You should be informed who is co-ordinating the NHS continuing healthcare assessment.

The team’s assessment will consider your needs under the following headings: • behaviour • cognition (understanding) • communication • psychological/emotional needs • mobility • nutrition (food and drink) • continence • skin (including wounds and ulcers) • breathing • symptom control through drug therapies and medication • altered states of consciousness • other significant needs These needs are given a weighting marked “priority”, “severe”, “high”, “moderate”, “low” or “no needs”.

If you have at least one priority need, or severe

needs in at least two areas, you should be eligible for NHS continuing healthcare. You may also be eligible if you have a severe need in one area plus a number of other needs, or a number of high or moderate needs, depending on their nature, intensity, complexity or unpredictability.

In all cases, the overall need, and interactions between needs, will be taken into account, together with evidence from risk assessments, in deciding whether NHS continuing healthcare should be provided.

The assessment should take into account your views and the views of any carers you have. You should be given a copy of the decision documents, along with clear reasons for the decision.

Fast-track assessment for NHS continuing healthcare If your health is deteriorating quickly and you’re nearing the end of your life, you should be considered for the NHS continuing healthcare fast track pathway, so that an appropriate care and support package can be put in place as soon as possible – usually within 48 hours.

An organisation called Beacon gives free independent advice on NHS continuing healthcare.

Care and support planning If you’re eligible for NHS continuing healthcare, the next stage is to arrange a care and support package that meets your assessed needs.

Depending on your situation, different options could be suitable, including support in your own home and the option of a personal health budget. If it’s agreed that a care home is the best option for you, there could be more than one local care home that’s suitable.

Your CCG should work collaboratively with you and consider your views when agreeing your care and support package and the setting where it will be provided. However, they can also take other factors into account, such as the cost and value for money of different options.

NHS continuing healthcare reviews If you’re eligible for NHS continuing healthcare, your needs and support package will normally be

reviewed within three months and thereafter at least annually. This review will consider whether your existing care and support package meets your assessed needs. If your needs have changed, the review will also consider whether you’re still eligible for NHS continuing healthcare.

Refunds for delays in NHS continuing healthcare funding CCGs will normally make a decision about eligibility for NHS continuing healthcare within 28 days of getting a completed checklist or request for a full assessment, unless there are circumstances beyond its control.

If the CCG decides you’re eligible, but takes longer than 28 days to decide this and the delay is unjustifiable, they should refund any care costs from the 29th day until the date of their decision.

If your CCG decided that you weren’t eligible for NHS continuing healthcare, but then revised this decision after a dispute, it should refund your care costs for the period between their original decision and their revised decision.

If you’re not eligible for NHS continuing healthcare If you’re not eligible for NHS continuing healthcare, but you’re assessed as requiring nursing care in a care home (in other words, a care home that’s registered to provide nursing care) you’ll be eligible for NHS-funded nursing care. This means that the NHS will pay a contribution towards the cost of your registered nursing care. NHS-funded nursing care is available irrespective of who is funding the rest of the care home fees.

Can I refuse an assessment for NHS continuing healthcare? If I refuse, will I be able to get services from my local authority?

An assessment for NHS continuing healthcare can’t be carried out without your consent, so it’s possible to refuse. However, if you refuse, although you’ll still be entitled to an assessment by the local authority there’s no guarantee that you’ll be provided with services. There’s a legal limit on the type of services that a local authority can provide.

If you refuse to be assessed for NHS continuing healthcare, the CCG should explore your reasons for refusing, and try to address your concerns. If someone lacks the mental capacity to consent to or refuse an assessment, the principles of the Mental Capacity Act will apply and in most circumstances an assessment will be provided in the person’s best interest.

My relative is in a care home and has become eligible for NHS continuing healthcare. The CCG says the fees charged by this care home are more than they would usually pay, and has proposed a move to a different care home. I think a move will have a negative effect on my relative. What can we do?

If there’s evidence that a move is likely to have a detrimental effect on your relative’s health or wellbeing, discuss this with the CCG. It will take your concerns into account when considering the most appropriate arrangements.

If the CCG decides to arrange an alternative placement, they should provide a reasonable choice of homes.

Is it possible to pay top-up fees for NHS continuing healthcare?

No, it isn’t possible to top up NHS continuing healthcare packages, like you can with local authority care packages.

Frequently asked questions about NHS continuing healthcare

The only way that NHS continuing healthcare packages can be topped up privately is if you pay for additional private services on top of the services you get from the NHS. These private services should be provided by different staff and preferably in a different setting.

I have a local authority support package that works well. I’m now eligible for NHS continuing healthcare – will my support package change?

If you’re concerned about changes to your care package because of a move to NHS continuing healthcare, your CCG should talk to you about ways that it can give you as much choice and control as possible. This could include the use of a personal health budget, with one option being a “direct payment for healthcare”.

What is NHS-funded nursing care?

NHS-funded nursing care is care provided by a registered nurse for people who live in a care home. The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

Who is eligible for NHS-funded nursing care?

You may be eligible for NHS-funded nursing care if: • you are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse • you live in a care home registered to provide nursing care

How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Most people don’t need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you.

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home. Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility,

you can ask your CCG to review the decision.

What is the rate of payment for NHS-funded nursing care?

NHS-funded nursing care is paid at the same rate across England. In April 2017, the rate was set at £155.05 a week (standard rate).

Before October 1 2007, there were three different levels or bands of payment for NHS-funded nursing care – low, medium and high.

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. In April 2017, the higher rate was set at £213.32 a week. You’re entitled to continue on this rate unless: • you no longer have nursing needs • you no longer live in a care home that provides nursing • your nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £155.05 a week, or • you become entitled to NHS continuing healthcare instead.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive. They should also be capable and confident in dealing with your particular needs.

You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

If you or a loved one needs help with day-today care, you can contact your local council’s social services department. Your GP will assess you and offer appropriate advice or treatment. They can refer you to a psychological treatment service or a specialist mental health service for further advice or treatment.

Mental health services are free on the NHS but you will usually need a GP referral. If you don’t want to go to your GP you may also have the option of self-referral. This means you can go directly to a professional therapist. The NHS Choices website has a searchable directory of services which you can use to find a service near you.

An innovative project providing structured musical stimulation for people with dementia and their carers. Singing is an enjoyable activity and provides a way for people with dementia to express themselves and socialise with others in a fun, supportive group. Groups run once a month.

Carers Resilience is a new integrated service: Crossroads Care and Alzheimer’s Society will work together to provide Link Workers for all GP Practices across Rotherham. It works to build confidence and resilience by offering: a full assessment of the needs of the carers by a Dementia Adviser; fast track to immediate short term in-home respite care where necessary; support for carers for up to one month; signposting on to other services as required.
