Auditing last days of life in hospitals

We understand the loss of a loved one is a very difficult time and we want to extend our deepest condolences to you. When you feel able, we would like to invite you to take part in the bereavement survey as part of The National Audit of Care at the end of life (NACEL). NACEL is a national audit which looks at the care experienced by the dying person and those important to them during their last hospital admission. The bereavement survey is for friends and family of someone who died. The aim of the survey is to give those close to the person who died an opportunity to share their views on the care and support provided in hospital. We value your perspective and experiences, and we are grateful to you for sharing your personal experiences during this time. Your input can truly make a difference to the quality of end-of-life care across England, Wales and Jersey.

You can take part in the survey by scanning the QR code

However, if you wish to make a formal complaint regarding the care and experience you and your loved one received at London North West University Healthcare NHS Trust during their last hospital stay, please contact Patient Advice and Liaison Service (PALs). The contact information is documented below. PALs can be contacted either by email or telephone. (LNWH-tr.complaints@nhs.net or by telephone on 020 8869 2026)
