Montefiore and Burke to establish Center of Excellence for Alzheimer’s Disease PAGE 4

In an ongoing commitment to delivering quality, compassionate care to those experiencing the effects of Alzheimer’s disease and other forms of dementia, Montefiore, which combines the expertise and best-in-class practices of the Montefiore Einstein Center for the Aging Brain (CAB) and Burke Rehabilitation's Memory Evaluation and Treatment Services (METS), has been designated as the exclusive Center of Excellence for Alzheimer’s Disease serving the seven county region of the Hudson Valley.

The 5-year, $2.1M grant from the New York State Department of Health will create a new clinical partnership between CAB and METS that will expand cognitive screening, diagnosis and care planning efforts. It will also provide training of primary care providers throughout the region who can help screen for and diagnose cognitive impairment and make referrals to specialists and community-based organizations that support those impacted by Alzheimer’s. Additionally, the availability of clinical trials will increase for patients with Alzheimer’s disease and related dementias, and telephone and video consultation will be provided to area healthcare providers.

The Center of Excellence for Alzheimer’s Disease (CEAD) will capitalize on the research and the training capabilities of Albert Einstein College of Medicine, of which Montefiore is the University Hospital for, as well as an expanding network of hospitals and physician groups in the Hudson Valley.

Launched in June 2014, Montefiore Einstein’s Center for the Aging Brain provides evaluation and treatment for illnesses and quality-of-life issues associated with aging. The Center serves both older individuals and people who fall outside the traditional "geriatric" category, with assessment for cognition, gait and function, psychological state, complex disease management, end-of-life issues and longterm care planning. It also provides comprehensive resources to help support caregivers.

Opened in 1979, Burke’s Memory Evaluation and Treatment Services (METS) program provides comprehensive outpatient assessment and treatment of memory disorders. While some types of problems with memory are “normal” as one grows older,

others are the first signs of disease, such as dementia, stroke, Parkinson’s disease or Alzheimer’s disease. An evaluation, which can be done through the METS program, is necessary whenever memory problems interfere with relationships, work, finances, and other routine activities of daily living. Memory may be improved in some cases or the progression of memory loss slowed with early treatment of disorders such as dementia and Alzheimer’s disease.

Co-director of the planned Center of Excellence for Alzheimer’s Disease, Barry D. Jordan, M.D., who is assistant medical director and director of the METS program at Burke Rehabilitation Hospital believes, “The new grant will allow Burke to continue its longstanding commitment to providing services to patients with dementia and Alzheimer’s disease. We will be working collaboratively with Montefiore to build upon the solid foundation both organizations have established in this area over the years.”

Neurologists find that moderate exercise may slow cognitive decline by 10 years

Researchers from NewYorkPresbyterian/Columbia University Medical Center (CUMC) and University of Miami, with support from the National Institutes of Health and National Institute of Neurological Disorders and Stroke, co-authored a study that may help older people slow their rate of cognitive decline.

The population-based observational study, published on March 23, 2016 in the online issue of Neurology, the medical journal of the American Academy of Neurology, found that moderate to intense exercise in people over the age of 65 was associated with a slower rate of decline in memory and cognitive function.

Researchers studied data on 876 people enrolled in the Northern Manhattan Study (NOMAS), a research study of stroke and stroke risk factors in the Northern Manhattan community conducted at the Neurological Institute, Columbia University, Division of Stroke and Critical Care. Using an in-person questionnaire, participants were asked how long and how often they

exercised during the two weeks prior to that date. Exercise was defined as the maximum intensity of any activity performed and was placed into two classification levels: no exercise to light intensity and moderate to heavy intensity.

Participants also received a brain MRI and a standard neuropsychological examination (NPE). The NPE tested a variety of different cognitive abilities, including processing speed, semantic memory, episodic memory and executive function. Five years later, participants received the examination again.

“The study found that people who did not exercise or exercised with only light intensity experienced cognitive decline that was equal to 10 more years of aging than people who reported exercise with moderate to heavy intensity,” says Dr. Mitchell S.V. Elkind, professor of neurology at Columbia University College of Physicians and Surgeons, attending neurologist on the Stroke Service at NewYorkPresbyterian and an author on the study.

Of the participants, 90 percent reported light exercise or no exercise.

Light exercise included activities such as walking and yoga. They were placed in the low activity group. The remaining 10 percent reported moderate to high intensity exercise, which included activities such as running, aerobics or calisthenics. They were placed in the high activity group.

“The study found that people who did not exercise or exercised with only light intensity experienced cognitive decline that was equal to 10 more years of aging than people who reported exercise with moderate to heavy intensity.”

“This research presents us with valuable information that may help people 65 and older retain their memory and thinking skills,” says Dr. Joshua Z. Willey, assistant professor of neurology at Columbia University College of Physicians

and Surgeons, assistant attending neurologist on the Stroke Service at NewYork-Presbyterian and lead author of the study. “The population of people 65 and older in the United States is increasing, and so should our efforts in helping these individuals do everything they can to live long lives with their memories intact.”

For those individuals who showed no signs of memory and thinking problems at the beginning of the study, researchers found that those reporting low to no activity levels showed a greater decline over five years than those with high activity levels. Participants were tested on how fast they could perform simple tasks and how many words they could remember from a list. The difference was equal to that of 10 years of aging.

Researchers adjusted for factors that could affect brain health, such as alcohol use, smoking, high blood pressure and body mass index, and the 10 years difference remained.

“It’s remarkable how much has changed in hearing aids over the years,” says June Holaday, director of marketing for New York Beltone Hearing Aid Centers.

“But for some people, a stigma remains that hearing aids are big, clunky devices, and that wearing them is a sign of old age.”

The truth is, Holaday says, being able to hear well actually helps a person feel younger.

“You can play in the yard with your grandchildren, or go out to eat with friends, and not have to keep yelling, ‘What did you say?’ when others talk to you,” she says. “You feel less isolated, more confident, and more involved with life.”

Hearing loss is widespread, notes Beltone audiologist Stephanie Robinson, MA, CCCA. “As many as 48 million Americans have significant hearing loss, and their family and friends are also impacted,” Robinson says.

“Hearing loss can negatively affect emotional and mental health, social skills, academics and work.”

Holaday says studies have shown links between hearing loss and a variety of health problems, from depression and heart disease to diabetes. “Untreated hearing loss has even been found to have links to dementia,” she says.

Mini-computers Today’s hearing aids are a far cry from bulky, one-size-fits-all devices of the past.

“They’re no longer like a giant amplifier; they’re more like a tiny mini-computer,” Holaday adds.

The company was the first to bring this new technology to the marketplace, Holaday says.

“And we continue to introduce updated equipment about every 18 to 24 months.”

Beltone, which marked its 75th anniversary last year, has approximately 1,600 locations in the U.S., including local centers in Ossining, Yorktown Heights, the Bronx and Stamford.

“Before this, a hearing aid in one ear wasn’t able to ‘talk’ to a device in the other ear, so you had to adjust them separately. Now, you can have immediate sound adjustment in both ears,” she says.

“If, for instance, you’re going out to play golf on a windy day, you can program the Legend ahead of time with a special smartphone app, so it automatically prevents the buzzing noise sometimes caused by the wind.

“Later, if say, you’re at an event or meeting with someone speaking at the front of the room, you can reset it for that. And when you go to a restaurant, you can set it to block out background noise.”

All settings can be adjusted with a smartphone app. “You just touch the app; you don’t have to fiddle with your hearing aid, which embarrasses some people when they’re in public,” says Holaday. For folks who don’t use a smartphone, a small control device is available that works the same way.

Thanks to the Baby Boom generation, a tsunami of aging adults virtually guarantees more Americans over age 65 in coming years than at any point in history. But here's what else it guarantees: more people with aging spines who suffer from back pain, according to Kaliq Chang, MD, of Atlantic Spine Center.

Rates of back pain - experienced by 8 in 10 American adults regardless of age - only increase with advancing years because of the gradual breakdown of bone, joints and muscles. But all is not lost with the passage of time: Older adults can take many effective steps to build their back strength and prevent spine-related pain, explains Dr. Chang, an interventional pain management specialist.

“An aging body means the spine is aging, too,” Dr. Chang says. “But while certain back conditions are more common among older adults, there are also more ways than ever to combat the effects of getting older on the spine.”

Common conditions in aging spines The incidence of certain spine conditions increases in seniors, Dr. Chang notes.

These include:

Arthritis: The accumulation of wear and tear on the joints, arthritis is likely the most common spine problem of age simply because the spine is comprised of three joints between each two vertebrae - with a total of 25 joint levels from top to bottom of the spine, Dr. Chang says.

Herniated discs: The gel-like discs between each set of spinal vertebrae naturally lose water content over time, making them more likely to flatten under the vertebrae's weight and become herniated. This outward protrusion can place pressure on spinal nerves, causing pain, Dr. Chang says.

Spinal stenosis: A narrowing of the spinal column that often results from arthritis or injuries, spinal stenosis places pressure on the highly sensitive nerves of the spinal cord, he says.

Spondylolisthesis: This problem occurs when a single vertebra slips out of place, causing the entire spinal column to become unstable and increasing the chances of a back injury. Since ligaments and other connective tissues lose strength and elasticity in older adults over time, spondylolisthesis becomes more common in this group, Dr. Chang says.

Frail vertebrae from osteoporosis: Bone density typically decreases in seniors, with many developing the bone-weakening condition osteoporosis. This condition leads to a heightened risk of fractures.

Tips to build strength and prevent back pain

Exercise: Unquestionably, regular physical activity can not only ease muscle tension and inflammation, but strengthen back muscles.

“This helps your core to be stronger and more supportive of your spinal column,” Dr. Chang explains, “making injuries less likely.”

Maintain a healthy weight: Extra pounds, particularly around the middle, can shift your center of gravity and strain your lower back. “Staying within 10 pounds of your ideal weight is the goal, and may help control back pain,” he says.

Practice good posture: Keep knees a little higher than your hips while seated, and look for chairs with a straight back or lower-back support. Also, when walking, keep your head up and your abdomen muscles pulled in.

Lift carefully: Always lift heavy or bulky objects by bending at the knees, not at the waist. “Don't twist while lifting,” Dr. Chang advises, “and if it's possible, push rather than pull heavy objects.”

If, despite all efforts, you're dealing with back problems, consider visiting a spine specialist for a firm diagnosis and course of action, Dr. Chang says. Treatment may include prescription or over-the-counter medications, steroid injections, physical therapy or surgery.

How elders and caregivers can take advantage of summer weather Boomers & Beyond - Westchester

Enjoying a breezy spring day or the warm summer temperatures don’t have to be a distant memory for elders and caregivers. After being cooped up in the house for possibly months at a time, senior adults can breathe in the fresh air, even if they are experiencing mobility problems.

1. Catch a sporting event.

The events could be watching a grandchild's soccer game, which are not all-day events, or attending a professional game, like baseball.

6. Be a bird lover. If you have a birdhouse, bird feeder or bird bath in your yard, checking on the those daily give elderly individuals a reason to go outside.

3. Be a tourist. If you live in a city, take an open-air bus or trolley tour to see the local sights. Another option could be a boat tour, depending on what type of equipment an elder needs to take with them.

2. Fish for fun. For folks who enjoy fishing, you can cast a rod from a pier or other location, even if someone is wheelchair bound.

5. Stroll around. If a walk is possible, start slow. Try three or four minutes in one direction, turning around and coming back.

It takes some advance planning and choosing an activity that won't seem like a chore, but it's worth getting out of the house, for you and your elderly parent. Here are 10 outdoor activities for caregivers to enjoy with ther elders.

8. Go fly a kite. Head to a park or beach and get a kite soaring.

Let an elderly individual take control, which they can do while sitting down. If children are around, they can get involved by trying to keep the kite in the air.

10. Celebrate the season. From outdoor concerts to fireworks, there are plenty of community events this summer with opportunities for elders to get out and be part of the crowd.

7. Pedal around. Rent a threewheeled bicycle, which are easier to mount and ride, and also could offer back support.

4. Take a dip. For some folks, it may just be putting a foot in the pool, while others may be able to handle low-impact water aerobics.

9. Picnic outdoors. Picnics are another park or playground activity. Elderly individuals enjoy the buzz of outdoor activity.

Use these tips to help you balance increased caregiver needs during the summer months:The summer months can be especially challenging for caregivers who are in the sandwich generation. These are the adults who are “sandwiched” between raising their children and taking care of their senior parent. During the summer season children who were in school during the fall, winter, and spring months are now home, and the temperatures are creeping up so high that it is important to keep both old and young safe and healthy by keeping them cool and out of the sun. Balancing these needs and making sure that everyone gets the care they need can be demanding, but with careful planning and the help of a home care provider, it can be done.

Do not overdo it.

Your first thought during these months may be to pack your schedule as full of activities and events as you can to make sure that your parent and your children are occupied. While the summer months offer fantastic opportunities for entertainment, leisure, and celebration, doing too much can quickly burn your family out and leave them unable to enjoy it.

Be cautious in your scheduling and be sure to give each activity or event plenty of time so that you do not feel rushed.

Keep up the structure.

Three months of relaxation and leisure may sound great, but the truth is that most people get bored when they have no structure in their life.

Avoid the cries of “I’m bored” within the first couple of weeks of summer vacation by keeping up the structure in your children’s lives. Look for day camp opportunities that give them something to look forward to each day or a few times a week. This will keep them getting up at a set time and stimulate their minds, reducing the stress that can come from bored, stir-crazy children.

Find “camp” for your parent.

Your children do not have to be the only ones who enjoy learning and doing new things in the summer months. Talk to your parent about activities that they enjoy or interests that they have and set up your own “summer camp” for them. Go on outings, do crafts or projects involving that interest, or do research together to learn more.

Share the time.

During the rest of the year your care tasks for your parent and your children are largely divided. This does not have to stay this way during the summer. Instead, find ways that you can combine care for both generations. This not only prevents you from having to spread yourself so thin, allowing you to devote more time and energy to all of your loved ones, but it also gives them the chance to benefit from one another. Consider joining a local water park or activity center where you can all go together and enjoy quality time that lets you experience things as a family or individually as you please.

Believe it or not, you have an estate. In fact, nearly everyone does. Your estate is comprised of everything you own— your car, home, other real estate, checking and savings accounts, investments, life insurance, furniture, personal possessions. No matter how large or how modest, everyone has an estate and something in common—you can’t take it with you when you die.

To ensure your wishes are carried out, you need to provide instructions stating whom you want to receive something of yours, what you want them to receive, and when they are to receive it. You will, of course, want this to happen with the least amount paid in taxes, legal fees, and court costs.

That is estate planning—making a plan in advance and naming whom you want to receive the things you own after you die.

Estate Planning Essentials

Estate planning is for everyone.

It is not just for “retired” people, although people do tend to think about it more as they get older. Unfortunately, we can’t successfully predict how long we will live, and illness and accidents happen to people of all ages.

Estate planning is not just for “the wealthy,” either, although people who have built some wealth do often think more about how to preserve it. Good estate planning often means more to families with modest assets, because they can afford to lose the least.

Too many people don’t plan.

Individuals put off estate planning because they think they don’t own enough, they’re not old enough, they’re busy, think they have plenty of time, they’re confused and don’t know who can help them, or they just don’t want to think it. Then, when something happens to them, their families have to pick up the pieces.

If you don’t have a plan, your state has one for you, but you probably won’t like it.

If you die without an intentional estate plan, your assets will be distributed according to the probate laws in your state. In many states, if you are married and have children, your spouse and children will each receive a share. That means your spouse could receive only a fraction of your estate, which may not be enough to live on. If you have minor children, the court will control their inheritance. If both parents die (i.e., in a car accident), the court will appoint a guardian without knowing whom you would have chosen.

An estate plan begins with a will or living trust.

A will provides your instructions, but it does not avoid probate. Any assets titled in your name or directed by your will must go through your state’s probate process before they can be

Educational forum led by George Oros, Chief of Staff, Westchester County, to enlighten leaders about the importance of decreasing housing costs through shared services. Participants became knowledgeable about the taxing districts in Westchester County and the value of municipalities and taxing districts sharing services.

The issue: Westchester County has one of the highest median property taxes in the United States, and is ranked 1st of the 3143 counties in order of median property taxes. The average yearly property tax paid by Westchester County residents’ amounts to about 8.1% of their yearly income. Westchester County is ranked 5th of the 3143 counties for property taxes as a percentage of median income. In his 2016 annual State of the County address, County Executive Rob Astorino said an increase to the county property tax levy was out of the question. “It's a pledge (not raising the tax levy) I have kept for five years and I am making it again tonight, because county government has to be on the side of every young person, couple, senior and business in Westchester that is struggling to make ends meet so they can stay here.”

Gardening is America’s favorite leisure outdoor activity, enjoyed by more than 78 million people, according to the National Gardening Association. For most of us, gardening provides a welcome sojourn in the natural world, a sanctuary that promises relief from the challenges of life. Yet many feel the need to give it up as they become less physically able. Gardening can enrich the life of the senior gardener in many ways - physically, mentally, and spiritually.

The following tips for modifying tools and gardening techniques can make all the difference for the senior gardener.

• Paint your garden tools with a bright color. This will enable the senior gardener to easily locate tools.

• Purchase seed and seed tape for easier handling and planting.

• Grow plants that heighten the sense of touch or smell.

• Use a vertical garden or trellis. This allows the senior gardener to plant and weed without stooping or bending.

• Build and design raised beds that provide a place to sit and garden.

• Try using a stool, chair, or bench to avoid constant stooping or squatting.

• Use the right length tools. Long handled or curved handled tools provide better grips and more leverage.

• For safety sake, garden early in the morning or late in the day.

Avoid being out between 10:00 a.m. and 2:00 p.m.

• Drink plenty of decaffeinated fluids to prevent dehydration.

• Take time for breaks in the shade.

• Wear lightweight clothing, long sleeved shirt, eye protection, sunscreen, a big hat to shade face, and gardening gloves.

Summer Cucumber and Dill Salad

Straight from a loving grandmother’s kitchen comes this wonderful way to celebrate the bounty of garden cucumbers.

Mixed with onion and sea salt and then chilled, the cucumbers take on a tendercrisp texture. Plan ahead to make this simple salad, as it requires several hours of refrigeration before serving.

Ingredients: 3 medium cucumbers, peeled and very thinly sliced 1 small white onion, very thinly sliced 2 teaspoons fine sea salt, plus one pinch salt 2 tablespoons white wine vinegar 1 tablespoon finely chopped fresh dill or 1 teaspoon dried

Method: In a large colander, toss together cucumbers, onion and 2 teaspoons salt.

Place the colander over a bowl to catch draining liquid and refrigerate for 1 to 2 hours. Cucumbers will shrink considerably as salt releases their moisture.

Rinse cucumbers and onion under cold running water and let drain thoroughly, about 20 minutes, then transfer to a bowl. Add vinegar, dill and pinch of salt and toss to combine.

Cover and refrigerate for 1 to 2 hours before serving.

As the old adage says, with age comes wisdom. And as we all grow older, certain health concerns earn our full attention. One of those concerns is the importance of oral health for seniors.

We all only get one set of permanent teeth, so it's crucial to take care of them for our entire lives.

Gum disease, sensitive teeth, diabetes and dry mouth are just some of the conditions that stress the importance of oral health in older adults. Let's take a look at these in a bit more detail.

Here a few important tips on how to keep your mouth in good health:

Sensitive Teeth At some point, we've all tossed back a nice, cold glass of water only to grimace at that sharp, tingling sensation in our teeth. A number of factors cause tooth sensitivity, including cavities, gum disease and worn tooth enamel. Proper brushing and using a toothpaste for sensitive teeth will gradually appease that sensitivity.

Diabetes Diabetes occurs when your body's blood glucose exceeds normal levels, according to the American Diabetes Association. There are two types of diabetes – Type 1 and Type 2. The latter is far more common, and typically occurs in older adults due to a poor or worsening diet.

Type 1 is found in children and young adults.

Those with diabetes are at much greater risk for gum disease, thrush (resulting from a fungus that grows in the mouth) and dry mouth.

Even if you're a denture wearer, getting your gums checked is still important.

Don't forget to brush twice each day, and use toothpaste that contains fluoride.

After you brush, floss. Cleaning between your teeth ensures healthy gums and teeth.

If you wear full or partial dentures, remember to clean them on a daily basis. Take your dentures out of your mouth for at least four hours every day. It’s best to remove them at night.
