Breast Cancer: a Fire Chief’s Journey from Discovery to Diagnosis to Treatment The following story is proba­ bly more personal information than I would normally share, but if my experience could help any of my brother & sister firefighters, it’ s all worthwhile. Products of combustion, chemicals and the other contaminants we're exposed to are the enemy. Wear your PPE, decon your gear & equipment, and wash your dirty hands!

In February of 2019,1 was di­ agnosed with breast cancer. Yes, men can get breast cancer but it is very rare. We were told that men only make up 1% of all cases and that the specific type I had, “mu­ cinous colloid carcinoma”, was rare as well. About six weeks later I underwent a mastectomy, where they surgically removed my entire right breast. As scary as my diag­ nosis and treatment was, they pale in comparison to the frightening reality that my cancer almost was­ n’ t discovered, or it may have been found much later than it was, resulting in a more dire outcome.

The reason for the delay would have been my ignorance and fear.

My whole story is considerably longer than could be published here, but there are a few important components that I believe need to be shared.

I’ m a guy in my 50’ s, and of course I have lumps and bumps, that’ s part of getting older. So when I felt a small lump under my right nipple, I didn’ t think much of it. I can’ t tell you how long I ig ­ nored it, but it had to be several months, or even a year. Eventually I asked my doctor about it during my annual CDL physical; he took a look and decided to write me a script for a mammogram and I re­ member thinking to myself “yeah right, like I ’ m gonna get a mam­ m ogram ”. So I folded it up, put it in my wallet and out of my mind.

Probably the luckiest thing that has ever happened in my life is that several weeks later, I in­ jured my back. I still don’ t know exactly what the cause was, but it was bad, real bad. It resulted in a chiropractor sending me for an xray and while I was at the radiol­ ogy office, I decided to pull out the script for the mammogram that I’ d been carrying around. After

admonishing me for not acting on the script sooner, they scheduled me for a mammogram, and thank God they did. During the test, the radiologist was immediately con­ cerned and had the technician do an ultrasound. After looking at those results he came in and told me the “mass”had characteristics of a “cancer”, and I need to con­ tact a surgical oncologist immedi­ ately for a consultation. He basically finished off by saying I should “keep my spirits up and consider this the beginning o f a journey and not the end of the road”. To say I was floored would be an understatement.

After some research and con­ sultation we decided to go with the Rutgers Cancer Institute of New Jersey (RCINJ), which is part of the Robert Wood Johnson system. My wife and I went to meet the surgeon for the first time and we both immediately liked her. She had a great bedside man­ ner and was very informative. She decided to waste no time and took several biopsies o f the mass right there in the office. She was also very honest and held nothing back. Based on the radiology re­ sults and her exam, the most likely course of action would be a mas­ tectomy (total removal) of my right breast. Once the biopsy re­ sults confirmed the cancer diagno­ sis, she wanted to waste no time and “get it out”. I was scheduled for surgery on March 25th, 2019.

The weeks leading up to the procedure were full o f tests, scans and interviews. The actual surgery took about 3.5 hours and I was on my way home the next day. From what I was told, everything went well and I actually felt pretty good. My chest was bound in a giant ace bandage and it took me a few days to build up the courage to actually look at the site. I also had two drains in place that I was told could com e out in two or three weeks. The next step was to determine if chemo or radiation treatment would be required, based on tests of everything they

removed from me. The results came back, I did not need chemo but would have to go for 25 ses­ sions of radiation therapy. I will say the radiation sucked, but it was over in 5 weeks and I moved on. The final part of the treatment plan was the need to take Tamox­ ifen for the next five to 10 years to help prevent any remaining cancer cells from finding a new home.

And that’ s where I am today, doing pretty good, going for my six month checks.

I’ m lucky and my final out­ com e could have been much worse. To my brother and sister firefighters: while there is no de­ finitive proof that 30+ years of firefighting has anything to do with my cancer, it is also im possi­ t. There is ble to prove that it didn’ no history o f this disease in my family and I don’ t have any of the known genetic markers. There is an ever-growing list o f cancers that firefighters contract at a higher rate than the general public and our industry is paying much more attention to this issue. Prod­ ucts of combustion and other con­ taminants that we are exposed to are the enemy; wearing your PPE and performing decon are the first steps to protecting ourselves. R o­ tate hoods, clean your gear and wash your hands after every run or touching any equipment. Please take this issue seriously and take the steps to protect you and your family. If something doesn't look or feel right, get it checked. D on’ t let fear or arrogance stop you from doing the right thing, and don ’ t make the mistake I made and delay treatment. Never forget...

PROVIDENCE, RI - Around 11:30 A.M. on the morning of Thursday, September 6th, the Providence Fire Department re­ sponded to Hill side Village at 40 Leander Street for a box alarm.

Companies arrived to find nothing showing from the sixstory apartment building. Engine6 found the alarm panel showed floor-4. As they arrived to the fourth floor to investigate, they encountered smoke and sprinkler activation in Apartment 4l6.

A kitchen fire was discovered in Apartment 416 on the fourth floor. As a precaution, Battalion-2 struck a second-alarm. Crews

*4 39 worked to knock down the fire and evacuate all o f the remaining occupants o f the building. The heavy fire was quickly knocked down with the use of a hand line and the sprinkler system.

One victim on the third floor required EMS due to a medical problem, but it was unsure if it was related to the fire. The cause of the fire is under investigation.

Laconia Strikes Second-Alarm on Arrival LACONIA, NH - On September 4th, the Laconia, Gilford and Belmont Fire Departments were dispatched to a reported fire on St. Catherine St. in Laconia. Crews arrived to find heavy fire showing on the 'A' side of the building and struck a second-alarm. This brought crews from Meredith and Tilton-Northfield to the scene. The fire was knocked down in about 30 minutes.

Walpole recently placed a 2021 Pierce Saber in to service. The new Engine 1 has a 2000-GPM pump with a 750 tank. It replaces the 1999 Freightliner LDH reel truck that previously ran as Engine 1. This new apparatus, like some of their other trucks, has unique cartoon "firefighter" graphics. The minions were chosen for this unit.

NASHUA, NH - Nashua fire alarm began receiving calls for a fire in the area of East Otterson and Harbor Ave. just after 10:00 A.M. on September 3rd. C4 arrived first due and reported heavy smoke showing and struck the second-alarm. E4 arrived on scene and stretched lines into the auto body shop to gain access to the fire. Due to heavy fire conditions and the possibility of an exposure issue on side 4, a third-alarm was transmitted. Nashua Cl arrived on scene and trans­ mitted the fourth-alarm, bringing in mutual aid to the scene and to cover. Crews made an ag­ gressive interior attack, but were pulled out momentarily to a defensive position. Crews reentered the building to knock down the remaining fire and conduct overhaul.

Fire Found in Second Floor C eiling and A ttic o f K ingston Home KINGSTON, NH - On the afternoon of September 24th, Kingston Fire was toned out for a reported fire in a single­ family home at 25 Great Pond Road. Kingston Police were first on scene and reported heavy smoke and fire through the roof. Kingston's fire chief arrived on scene and re­ quested a working fire, bringing in mutual aid from several towns with tankers, as they were out of the hydrant area.

Firefighters were able to bring the fire under control in about 30 minutes. No injuries were reported.

SUV Crashes Into Second Floor of Home in Norwich NORWICH, CT - East Great Plain Vol. Fire Co. was dispatched on September 25th at 1:30 A.M. for a reported MVA at 665 West Thames St. involving a car into the second-story of the house.

Car 5 (Chief) arrived and found significant damage to the house after the vehicle had crashed into the second floor. The front end o f the SUV was inside the house and the back end was rest­ ing on the bed of a pickup truck parked in the driveway. The driver of the SUV was still inside. All oc­ cupants o f the three-family home were outside and accounted for.

The SUV crashed into the 'A/D' corner o f the house, into a bedroom where a husband and wife were sleeping. Luckily, the wife only sustained a very minor injury. The power service to the home was resting on top of the ve­ hicle and needed to be discon­ nected. Utilities were notified of the incident and responded.

After assessing the scene. Car 5 called for additional assistance from the City of Norwich Fire Dept, which brought in BC1, E3, and Truckl. EGP Rescue 5 arrived and a plan was developed on how to get the driver out of the vehicle.

E52 arrived and secured a water supply. BC1, E3 and Truck 1 ar­ rived shortly after. The plan was communicated to all arriving de-

partments and members, and once the power was disconnected to the residence, it went into effect.

R5 used their rescue struts to stabilize the vehicle and truck parked in the driveway. Truck 1 was set up in front of the house and used their bucket to asses and gain access to the driver. Crews used R5 hydraulic tools to remove the rear driver's side door to gain access. The driver was removed and turned over to EMS. Teamwork was crucial at this incident due to the vehicle hanging out of the house and the structural damage the house sustained. This incident went flawlessly due to the coordinated effort of all agencies involved. There were no injuries to firefighters. After the driver was extricated, a rotator wrecker was brought in to remove the vehicle.

The accident remains under in­ vestigation by the Norwich Police Department. Agencies involved with this incident were East Great Plain Vol. Fire Co., Norwich Fire Dept., Norwich Police, American Ambulance, Norwich Public Util­ ities, and Norwich Building Offi­ cial.

Warehouse Point Fire Department Holds Station Dedication E WINDSOR, CT - On Sep­ tember 18th, the Warehouse Point Fire Department dedicated their Bridge Street station in honor of past member, friend and mentor, Anthony DiMastrantonio.

DiMastrantonio, who was known to all as "Tony D", passed away in 2019. Tony was remem­ bered as a genuine and caring per­ son. He was a mentor to many and made an impression on everyone he met. Stories that were shared

Quoted from a memorial write up when Tony passed, "Tony was a 56-year member o f the Ware­ house Point Fire Department and retired at the rank of Captain. In 1968, when the Junior Division was created, he became advisor and drillmaster, a position he held

for over 40 years. Many o f the youths he trained went on to be­ come members, officers and chiefs of their fire departments. He was a past president of the local depart­ ment and also served as a Fire Commissioner of the Warehouse Point Fire District for many years."

Working Structure Fire in Meriden MERIDEN, CT - Meriden units responded to a structure fire on Lambert Ave. in Meriden on September 6th. So. Meriden Engine 6, Wallingford Engine 2, Southington Squad 5, Berlin Truck 9 for mutual aid, Meriden PD and Hunters Am­ bulance were also on scene. No one was home at the time of the fire except for a dog that was able to get out safely.

No injuries were reported.

The Town of Windham operates four separate fire departments.

"The City" of Willimantic, now a census-designated place within the town, has paid personnel that handle fire responses within their district and EMS town-wide. The remaining three departments are volunteer with standing, auto-aid agreements between them and neighboring assets.

In the lower end of town, the South Windsor VFD operates out of a 4-bay firehouse. Four apparatus are crammed into these tight quarters. Two of the bay doors here were recently modi­ fied for higher profile vehicles. None of the bays are deep which required their new engine, and future purchases, to be higher on a shortened wheelbase.

The SWFD fleet consists of two engines, a light duty rescue and a service/forestry unit. The department also maintains a Zodiac and a John Deer Gator that is also outfitted for brush fires. All apparatus carry the -04 Suffix (Department 4).

Crews Face Challenges at Train Fire in Monson MONSON, MA - On September 7th, the Monson FD was toned for a train fire on Stafford Road, in the area of Monson Sand and Gravel.

Upon arrival, 14 Chief 1 reported that the train was inac­ cessible to fire crews.

An additional locomo­ tive was brought in to push the train approx­ imately 2.5 miles to the nearest rail cross­ ing. Crews found smoke coming from the engineer compart­ ment on a locomotive and were advised not to initially use water due to the potential of it still being electrically charged.

The locomotive was separated from the rest of the train and crews at­ tempted to knock the fire down using 201b. dry chem extinguishers until the electrical threat was mitigated, at which time tactics were changed and hand lines were deployed. Hidden pockets of fire made extinguishment difficult. There were several re-igni­ tions due to the amount of heat re­ tained and put off by the metal in the locomotive. Mutual Aid was provided by the Stafford FD.

LAWRENCE, MA- On September 23rd, Lawrence firefighters arrived to find fire showing from a two-story wood-frame multi-family dwelling and quickly struck a second-alarm.

Crews encountered heavy fire conditions in the attic and had to be evacuated from the building. Several master streams were used as well as outside hand lines to bring the fire under control.

Whitman Fire Department Swears-ln Lieutenant Bryan Smith WHITMAN, MA - On the evening of Tuesday, August 24th, the Whitman Fire Department held a swearing-in ceremony for newly appointed Lieutenant Bryan Smith.

The ceremony was held at the be­ ginning of the Board of Selectmen's meeting at the Whitman Town Hall.

Whitman Fire Chief Timothy Clancy introduced Lieutenant Smith prior to the swearing-in.

Chief Clancy noted that Lt. Smith was one of Whitman Fire's first high school interns. Lt. Smith is a 2001 graduate of Whitman-Hanson

He was a deputy chief at the time of an urban fire department that covered five mu­ nicipalities in Hudson County, New Jersey, the smallest but most congested county in the state and possibly the country. The class is made up of firefighters from the Heartland. Although these two ju ­ risdictions are not identical in fire load, demographics and hazards, etc., the knowledge presented is common and that is safety and

discipline on and off the fireground. The goal is to accomplish the mission with effective strate­ gies while keeping firefighters safe from injury and death.

The disk is 115 minutes in length, but I can assure you that unless you are fatigued when you view it, you will not fall asleep.

The content is presented in a down to earth manner supported by photos, videos and tables of statistics. The photos are both on the fire ground as well as pre-fire assessments of structures and what hazards they can present.

Personal safety is stressed and ex­ amples are shown on how fire­ fighters become their own enemies by lack of discipline and laziness, as well as perhaps by a lack of training. Self discipline is the primary requirement for offi­ cers to effectively lead their crews and work in unison with other re­ sponding companies. It starts at the station with rules and regula­ tions being stressed. As a fire­ fighter serves his or her time in their career, knowledge of the job and the types of construction in their jurisdiction benefits them to live to retire.

This is a presentation that is highly recommended.

Truck A c c id e n t Dam ages B u ild in g in W altham WALTHAM, MA - Waltham’s 9-1-1 center received a call on September 24th for a trailer truck into a building at 580 Winter Street. Engine 2, Rescue 1, Tower 1 and Medic 1 were dis­ patched. Tower 1 reported that a truck was stuck under a de­ livery bay. The driver was uninjured but the truck, which could not be removed, had done a great deal of structural damage. The Waltham Building Department and the facility manager of the building were assessing the damage to de­ termine how best to remove the truck.

On arrival one patient was found with life-threatening traum atic in­ juries. Lifeflight responded and transported the patient to a Boston Trauma Center.

Car Vs. Building in Townsend

Originally built as Odd Fellows Hall around 1915, the town of Groton purchased this building in 1940 and converted it to serve as the Central Fire Station. Years later, the two bays on the right were added to house additional apparatus. GFD remained here for 74 years, moving to their new firehouse in 2014. The property was purchased by the McElroy Family a short time later. It was fully renovated and converted over to "the Groton Station House", an upscale restaurant featuring a craft cocktail lounge and function hall.

TOWNSEND, MA- On September 23rd, Townsend firefighters were called to Harbor Auto on Main St. for a vehicle into the building. Upon arrival, a minivan was found to be partially through the front of the building. Significant structural dam­ age was visible. The occupant had self-extricated from the vehicle and was evaluated by EMS. The building inspector was called to the scene.

Pedestrian Accident at Costco in Waltham CHUCK LOWE

Brush 1 in Buckland was built by V-Tec on a 2013 Ford F-450 4x4 chassis. This apparatus has a 250-gallon tank and a 250-GPM pump.

WALTHAM, MA - On September 24th, a report of a pedestrian pinned under a car at Costco Wholesale parking lot in Waltham sent Engine 2, Rescue 1 and Medic 1 responding. Rescue 1 arrived to find an elderly man under the car and bleeding from the head. He was easily re­ moved from the car and transported to Lahey Hospital in Burlington with unknown injuries.

Outside Burning Complaint in Whitman May Have Prevented House Fire WHITMAN, MA - Around 4:50 P.M. on the afternoon of Wednesday, September 15th, the Whitman Fire Department re­ sponded to the area o f Washington Street and Old Coach Road for a report of illegal burning. Engine243 and Car-3 responded. Compa­ nies arrived to find illegal burning at 82 Washington Street and ad­ vised the resident to extinguish the fire.

As companies were picking up to return, they noticed heavy smoke across the street. Com pa­ nies relocated to 101 Washington Street and found mulch and bushes burning in the front of the building, that had extended to the

house. One line was stretched off o f Engine-243 and the fire was quickly knocked down. Firefight­ ers removed the siding from the house and checked for extension on the first and second floors.

An initial investigation shows the fire is likely to have started in the mulch bed in the front of the house and extended to the bushes, which then caught fire. Damage is estimated at $20,000.

Well, no matter how difficult the situation or how tumultuous the season may be, there’ s always something to be thankful for. This month our entire nation will cele­ brate Thanksgiving.

Supermarkets will sell more turkeys in a single month than all the combined turkeys the whole year. Turkeys will be carved, din­ ners planned, drinks will be served, family gathered together.

The irony o f it is that very little thanks to God during Thanksgiv­ ing. We are so busy preparing, cooking, entertaining, watching football and eating, that about the only real thanks that gets done on Thanksgiving is done in a single prayer before the big meal. And it’ s usually done by one person.

Truth be told, millions will miss the whole point and original in­ tent o f this holiday - to give

thanks to God for what H e’ s done in our lives.

The Bible tells us: "Everything has come from You and everything belongs to You" (1 Chronicles 29:16).

It all starts with recognizing and affirming that God is the source of all good! Everything that we enjoy in life is a gift from God. t seem to go Even when things don’ our way God tells us to thank Him. In everything give thanks; for this is the will o f G od in Christ Jesus fo r you" (1 Thessalonians 5:18).

I want to emphasize the open­ ing word in this scripture - “IN” everything. In the middle of your challenge, disappointment, dis­ couragement, hardship, trial... give thanks to ‘ God. Why? B e­ cause there’ s always more in life to be thankful for, than to gripe about.

In spite o f any difficulty you may be experiencing, here’ s a list of things every Believer should thank God for.

He forgives all my sins.

He heals me.

He ransoms me from hell.

He surrounds me with love.

He fills my life with g ood things.

He is merciful and tender to those who don't deserve it (grace).

He is slow to get angry.

He never bears a grudge.

He has not punished us as we deserve fo r our sin s" (Psalms 103:2-11 (LB) Just to name a few.

So, when you carve that turkey, slice the pumpkin pie, watch the game, share good times with your family - remember to thank God IN everything. God has blessed us and “...is the giver o f all good things. ”
