The back to school blues can be difficult and stressful. And, no, were not talking about your children having anxiety as the first day of school approaches.

Parents can have the back to school blues, too, knowing their children will be missing the leisurely days of summer, instead learning to navigate school hallways and making new friends.

It’s normal for parents of young children to get the back to school blues, even feeling a little loss of identity as the child ventures away from home. If you’re dreading a new school year, there are simple ways to cope with the separation anxiety.

Be a school volunteer, pursue other interests, such as yoga, music or exercise and, most of all, don’t be afraid to cry when they go.

Oh, and one last thing: Deep breaths, deep breaths. You can do it.

Chef Peter X. Kelly Teaching Kitchen opens at HVHC The kitchen is open!

Hudson Valley Hospital Center’s Chef Peter X. Kelly Teaching Kitchen recently debuted with a visit from Congresswoman Nita Lowey and an announcement of a Signature Chefs Series that will help to fund the kitchen in future years.

Children from the Peekskill School District worked with Chef Kelly to prepare breakfast for Congresswoman Lowey and the more than 50 guests who turned out to christen the new kitchen.

“With the opening of the Chef Peter X. Kelly Teaching Kitchen we have the opportunity to teach people of all ages a healthier approach to eating and to help them understand why their diet is so very important to their health. We thank Chef Peter Kelly and others who have supported this kitchen as well as our Congresswoman Nita Lowey for advocating for preventive health initiatives like this one,’’ said Hudson Valley Hospital Center President John C. Federspiel.

The Chef Peter X. Kelly Teaching Kitchen – named after the “Iron Chef” winner and owner of Xaviars Restaurant Group – is part of a larger initiative at the Hospital called Harvest for Health which includes an organic garden, twice -monthly farmers’ markets and a locally sourced food service for patients and employees.

Through this initiative the Hospital is working with patients and the general community to teach them to better manage their health through diet and lifestyle changes. Classes range from general cooking instruction to very specific curriculum for diabetics, cancer and heart patients, breastfeeding moms and those with gluten sensitivities.

A program designed especially for children - The Young Chefs of the Hudson Valley- works with children 11-13 in Peekskill and other local schools to prevent childhood obesity.

Chef Kelly said supporting the kitchen and, in particular, the Young Chefs program was important to him. To achieve this goal, Chef Kelly announced a Signature Chefs Series. The series will feature noted chefs of the Hudson Valley such as Waldy Malauf, Ethan Kostbar, David Dibari, Eric Gabrinowitz and Anthony Goncalves among others, and will host seasonal dinners in the kitchen showcasing locally sourced foods of the Hudson Valley. The meals will be auctioned and prepared privately for the highest bidders. All proceeds from the Signature Chefs Series will go to support the Young Chefs program for at- risk middle school children.

The Young Chefs program strives to teach children about the importance of healthy eating through cooking classes that are fun and interactive.

The goal is to break bad eating habits that can result in chronic diseases like diabetes, heart disease and high blood pressure later in life.

With ingredients donated by local farms including Coach Farms DoReMe Farms and Homestead Farms, children in the program prepared a goat milk yogurt parfait with homemade granola and a watermelon drink from guests at the opening.

Chef Kelly said unlike other programs that work to prevent childhood obesity, the Young Chefs program doesn’t stop at teaching kids about the healthy plate or five-a-day philosophy. It works to help children develop a love and appreciation of food from the garden to plate.

“When kids go into the garden and pick a carrot and then prepare it, they have a better understanding of where their food comes from,’’ said Kelly. “Hopefully, making this connection will completely change their relationship with food and help them to develop better eating habits.’’

school healthy eating guide

Kids are back to school and you’re back to business after a relaxing summer, but you don’t have to sacrifice eating healthy just because you’re all busy. Check out these ideas healthy options for you’re the school lunch box.

Fruits and veggies: Make your weekly farmers market trip a family outing and explain to kids what it means for something to be in season.

Show them how sweet and flavorful foods can taste when they’re at their peak and let them pick their favorites.

Cheese: In reasonable quantities cheese can be a satisfying snack.

Just stay away from the really processed stuff at the grocery store– check the ingredients label and avoid long, scientific sounding words.

Homemade granola: Store bought granola is usually more like dessert than a healthy snack, but you can make your own with less sugar and it is still delicious.

Peanut butter: Like hummus, peanut (or any nut) butter can be a wonderful dip for fruits and veggies. Natural nut butters that have no added sugar, no added trans fat, are full of healthy fats (this is good).

Hummus: A small tupperware of hummus is a perfect accompaniment to cut up vegetables, whole grain breads and crackers.

Trail mix: If your child can tolerate nuts then trail mix is a fun and nutritious snack. Try different combinations of nuts and dried fruits.

Healthy sandwich choices include: hummus, avocado, peanut/almond butter, soft fruit, canned Alaskan salmon, cheese, roasted chicken or turkey, egg salad, mixed veggies, etc. Try not to choose deli meats as your standard choice, since they are highly processed and have been tied to all sorts of health

problems. Likewise, limit canned tuna to once per month because of the high mercury content.

Sandwiches: When choosing bread, look for artisan brands with few ingredients and no preservatives. This kind of bread is often found in paper bags and costs less than the fake-healthy soft stuff in plastic.

True Spirit of a Champion When Sophia Mercado’s mother saw her through the school bus window, she knew something wasn’t right with her youngest daughter. Sophia had trouble descending the school bus steps, and by the time mom and daughter crossed the street and entered their home, Sophia’s face was drooping and she’d lost feeling and function on the left side of her body.

Young Sophia was whisked to Maria Fareri Children’s Hospital at Westchester Medical Center, where she was diagnosed with an abnormal tangle of arteries and veins.

Her brain was hemorrhaging and she needed emergency neurosurgery to save her life.

Since that fateful autumn day in 2011, Sophia has undergone a second neurosurgery to care for another malformation.

Despite these challenges, today she is doing well. She uses her own medical journey as inspiration for other children and families facing similar circumstances. The way Sophia sees the world, there’s always reason for hope and optimism.

It’s no wonder that Mercado, 11, of Walden in Orange County has been named the state champion for the Children’s Miracle Network Hospitals (CMNH). Nominated by the Maria Fareri Children’s Hospital at Westchester Medical Center, Sophia was chosen because she embodies the true spirit of a champion after bravely facing three brain surgeries (from 2011 to 2013) due to an Arteriovenous Malformation (AVM) hemorrhage.

“We are thrilled Children’s Miracle Network Hospitals one again chose a patient from our hospital to be the Champions ambassador for all of New York State,” stated Michael Gewitz, M.D., Physician-in-Chief, Maria Fareri Children’s Hospital at Westchester Medical Center. “This honor along with Sophia’s story of courage and determination symbolize the exceptional, advanced pediatric care provided by Maria Fareri Children’s Hospital, and the inspirational spirit of the children we care for each and every day.”

Each year, the CMNH champions program chooses an exceptional child from each state with an amazing medical story.

This year, 52 champions were chosen to serve as ambassadors for the 17 million children treated at Children’s Miracle Network Hospitals annually. Burke Rehabilitation Hospital to Host 3rd annual Wound Healing Teaching Day Burke Rehabilitation Hospital, in collaboration with Montefiore Department of Family & Social Medicine, will host the BronxWestchester Academy of Family Physician’s 3rd annual Would Healing Teaching Day on Saturday, September 6.

Chronic wounds, such as pressure ulcers, leg ulcers, and diabetic foot ulcers are commonly encountered by physicians in both outpatient and inpatient settings. Family physicians are critical when it comes to prevention, early recognition, treatment, and comprehensive management of patients with chronic wounds.

At this one-day conference, attendees will learn how to optimize diabetic foot ulcer outcomes and pressure ulcer outcomes, learn evaluation and management of leg ulcers, and learn about hyperbaric therapy for wound management.

The event, which is geared towards family medicine physicians and residents, nurses, nurse educators, and nurse managers, will feature discussions by leading wound healing experts, including Anna Flattau, M.D., Director of the Wound Healing Program at Montefiore Medical Center.

“Now in its third year, the Wound Healing Teaching Day is an important seminar that offers physicians, nurses, and others in the medical field the latest information on how to effectively treat wounds,” said Sudhir Vaidya, M.D., FRCS(G), FAAFP, Director of Sports Medicine and Pain Management at Burke, Chairman of the Wound Management Committee and Course Director for this Continuing Medical Education event. “This will enable them to provide even better care to their patients and help in both preventing wounds in the first place and managing them if they do occur.”

The Putnam county wine & Food Fest was proud of their 4th annual event.

This two day festival was held on August 2nd and 3rd at the grounds of the Patterson Flea Market (3161 Route 22, Patterson, NY) which showcased New York's best wine producers as well as, world class culinary professionals, breweries, food, arts and crafts, fresh produce, local and international artists and also features a flea market. Getting to the Wine Fest from New York City and other towns is a breeze too. Some customers took Metro-North's Harlem Line to the Patterson Station to connect with free shuttle buses to and from the Wine Fest during the fest hours. It was run 11am-6pm on Saturday and 11am-5pm on Sunday, guests did enjoy musical performances from pop to country to R&B to Reggae and other genres. Some guests brought blankets and chairs to enjoy the event. A portion of the event’s proceeds will be donated to Hillside Food Outreach, which exists to provide food to those in need throughout Westchester County, Putnam County and Western Connecticut.

Urgent Care Centers: The Emerging and Convenient Method for Healthcare

By Gina Cappelli, President of Forme Urgent Care and Wellness Center When you are sick or injured, an urgent care center can be a very convenient option as opposed to having to wait for an appointment with your physician or going to the dreaded emergency room. Urgent care centers are usually open for patients 7 days a week from the early morning, to the late evening, and they usually don’t require patients to make an appointment. The long flexible hours of urgent centers provide a convenient, and stress free environment for people to come in and get immediate medical attention.

Urgent care centers are often the best bet for patients who need medical attention for non-life threatening conditions.

The urgent care industry is growing. According to the American Academy of Urgent Care Medicine (AAUCM), there are approximately 9,300 urgent care centers in the country and anywhere from 50 to 100 clinics open up every year. There are currently 20,000 and counting physicians working at these urgent care facilities around the country. With the growing popularity of urgent care centers around the country, research has shown that by 2025 there will be a need for approximately 52,000 physicians working at urgent care centers.

There are many reasons why urgent care centers are becoming so popular, such as: Urgent care is regularly available. Urgent care is the most convenient and readily available healthcare option available for patients today. The majority of primary care doctor’s offices are only open Monday through Friday, and many offices close at 5pm. Few primary care doctors have after hours appointments available. Meanwhile, urgent care centers are usually open to patients 365 days a year, from the early morning to late evening and usually do not require patients to make an appointment.

Urgent care is NOT emergency care. There is a common misconception that urgent care is equivalent to emergency care, however, that is not true. Emergency rooms are designed for life threatening and/or limb saving care, while urgent care centers are for treating conditions that need to be taken care of immediately, but are not a major emergency. Additionally, the wait times at urgent care centers are normally much quicker than those in the emergency room.

Urgent care centers save both time and energy. Urgent care centers provide timely care to patients, all hours of the day. According to the AAUCM, the wait time to see a health care provider in an urgent care center is typically only half an hour or less, while an emergency room visit can require multiple hours of waiting. If a patient is suffering from an ailment that is not fit for the emergency room, it is more cost and time efficient to seek medical care at a local urgent care center that can see patients more quickly and less expensive than the emergency room.

While value is high, the price is not. A visit to an urgent care facility is likely to be about one-third to one-half the price of a standard emergency room visit.

According to a study conducted by RAND Corp, up to 27% of emergency room cases could have been treated by an urgent care center, which would have saved a total of $4.4 billion dollars per year in health costs. Research by CareFirst BlueCross BlueShield showed that if a patient were to seek medical care for a sore throat, choosing an urgent care center over an emergency room would provide this person with nearly 85% savings.

In most situations, you will save time and money by going to an urgent care center instead of an emergency room. If you have a minor injury such as a bad cut or ear infection, you may end up waiting for hours in the emergency room and paying hundreds or thousands of dollars for your care, while at an urgent care center you would be taken care of in a speedy, and personable way for a fraction of the emergency room price.

Additionally, most urgent care centers are open for extended hours, to cater to their patients’ needs. Also, you will usually end up paying much lower co-pays at an urgent care center as opposed to an emergency room.

In addition to his position at CDHV, Dr. Lee is an assistant attending physician at the New York-Presbyterian Hospital/ Columbia University Medical Center. Dr. Lee is board certified in cardiovascular disease, clinical cardiac electrophysiology, vascular ultrasound and internal medicine and is board eligible in comprehensive echocardiography and nuclear cardiology. He has also co-authored several peer-reviewed articles in medical journals and has given presentations at both national and local conferences.

Dr. Lee earned a Bachelor of Science in Chemistry and a Master of Science in Biological Engineering from the Massachusetts Institute of Technology. He went on to complete medical school at Temple University School of Medicine, where he was inducted into Alpha Omega Alpha National Medical Honor Society. Dr. Lee completed his residency and fellowships in internal medicine, cardiovascular disease, clinical cardiac electrophysiology and cardiovas-

cular imaging at New York-Presbyterian Hospital/ Weill Cornell Medical Center.

Why establish a Health Savings Account?

By Andrew J. Cavaliere, CLTC Given all the turmoil in the health insurance marketplace today, there are so many reasons why employers and employees should establish Health Savings Accounts (HSAs).

Here are just a few: 1- HSA eligible high deductible health plans save money for employers and employees.

2- HSAs contributions are vested day one by the accountholder.

4- HSA interest is tax-free.

5- HSA distributions for qualified medical expenses are tax-free.

6- HSA qualified medical expenses extend beyond major medical to include dental, vision, chiropractor, etc.

9- HSA funds that are not used roll over from year-to-year and after age 65 can be distributed for retirement.

Today, many accountholders continue to use their HSA the same way they used to use their flexible spending account (FSA). That is, they simply use their available funds to cover current medical expenses, leaving little or nothing to rollover from year-to-year. While that is an option with HSAs, it is not a requirement. More importantly, employees are missing out on the real benefit of HSAs which is namely growing their balances for retirement…! It is easy to understand this since many consumers often overlook the benefits due to lack of education and company communication.

There’s a great need for employers to help shift this mindset beyond just paying for this year’s medical bills. Employers should systematically encourage their employees to save for their future. Supplying orthotic appliances to in-patients: Introduction

When a physician orders brace for an in- patient, what steps are hospitals taking to ensure public safety? Hospitals tend to contact an outside vendor to fill the order for the brace. As you have probably have read elsewhere, orthotic appliance vendors are under tight scrutiny secondary to fraud and abuse. In the state of New Jersey, a person must be licensed as an Orthotist to fit a patient with an orthopedic device. Unfortunately, there are several vendors that are owned by an Orthotist but send a ‘fitter’ or ‘assistant’ to hospitals for fittings. Under this scenario, the hospital and ordering physician assume product liability for services provided by the vendor.

There is a clause in the state law that allows ‘fitters’ and licensed Orthotist assistants to fit products under the direct supervision of the ordering physician.

However, the ordering physician must be present at fitting and is now deemed responsible for the fitting and performance of device.

The hospital, via written contract, should mandate that all fittings within their facility must be performed by a licensed Orthotist, not assistant or fitter. Even a licensed assistant Orthotist is not allowed to fit products in a hospital setting unless the supervising Orthotist is present at fitting. If the licensed Orthotist assistant fits without the licensed Orthotist present, then responsibility falls to hospital and ordering physician again.

Who are these Orthotists?

A licensed Orthotist must have a bachelor’s degree, complete a postgraduate certification program from an accredited college or university, and then perform 1900 hours of clinical residency under the supervision of a licensed Orthotist.

This intense training is imperative to ensure public safety. Orthotic appliances can danger patients if not fabricated and fitted properly. Not just from creating

wounds and bed sores, but also causing muscular skeletal issues from improper positioning of orthotic appliance. Think of this, a typical leg fracture brace consists of very rigid plastic around thigh and calf connected with a metal joint on both sides of brace. Then the patient lies down in this contraption for 6 – 8 weeks. If this brace is not fabricated and fit properly, long and short term issues may arise.

When is a hospital financially liable to bring in an Orthotist for services/products?

When an in-patient is being covered under a government diagnosis-related group payment system, the hospital is financially responsible to provide orthotic appliances ordered. If the in-patient has private insurance, the vendor should be billing the insurer directly. Hospitals need to assist vendor with obtaining proper referrals and authorizations from time to time, but generally the vendors should be billing them directly. It may be necessary for hospitals to have more than one vendor in order to ensure that the patient is being treated by a vendor that has a contract with their insurer.

If the patient has no insurance at all, then there is a gray area on who is responsible. I think the patient is technically responsible for Orthotist service, but probably won’t be able to afford the services prescribed. Then the physician may not allow patient discharge without the product. So hospital may step in and pay for the product and service of the Orthotist. In this scenario, hospitals with good relations with their vendors of choice may request a reduced rate in order to obtain their service.

What do doctors do to treat their own pain?

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result.

Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead.

My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked.

Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life.
