Stop mulling over the Medicare muddle Crucial advice for understanding Medicare

least expensive and most convenient for your menu of prescriptions. You can choose a new drug plan each year with no penalty; however, if you require a new prescription during that year, you’ll be at the mercy of your current drug plan.

When choosing your Medicare plan, start by asking your doctors and friends what plan(s) they recommend. Perhaps the clinic or hospital your doctor is associated with only accepts one or two plans. If your friends have had good experiences with their plans, consider those, but understand that what works for your friends may not always work for you.

I

’ve been writing about Medicare for years, so I wasn’t particularly affronted when, not long after my 64th birthday, I started receiving advertising circulars about Medicare Advantage and supplemental plans. Whether you’re new to Medicare or have been enrolled for years, it’s crucial to understand the system and familiarize yourself with the changes that happen nearly every year.

AGING RESOURCES

WHERE TO START Confusion about Medicare stems from it being cobbled together over the last 57 years by numerous political administrations. When it was approved by President Lyndon Johnson in 1965, it included only Part A (hospital) and Part B (doctor visits) coverage. In the 1980s, supplemental private insurance in the form of Medigap plans were offered to cover some of the deductibles and copays. The Medigap plans available in most states are A, B, D, G, K, L, M and N. Each plan offers a different level of coverage. Two other plans, C and F, are no longer available to new enrollees.

In the 1990s, Medicare Advantage (Part C) was offered as an entirely different way to access the Medicare system.

Part C providers offer more services than original Medicare with the trade off of limiting your choice of medical providers. Whereas the focus of Medigap is covering the “gap” of deductibles and copays, Part C’s focus is providing additional services such as vision and dental.

Your first decision, and the most important, is deciding whether to expand your upcoming Medicare coverage either through Part C or Medigap. A good place to start is by reading “Medicare & You: The Official U.S. Government Handbook,” available at www.medicare.gov.

For more information, contact your local State Health Insurance Assistance Programs (SHIP) for free, in-depth, one-on-one insurance counseling and assistance. You can also contact a local Medicare insurance sales advisor.

ADDITIONAL THINGS TO REMEMBER If you’re approaching your 65th birthday, make some time for Medicare decisions.

• Your initial Medicare enrollment period begins the third month before your 65th birthday month and finishes at the end of the third month after your birthday month.

That gives you seven months to make some pretty momentous decisions.

DRUG COVERAGE

• You can sign up later, at retirement, if your current coverage through an employer is at least as comprehensive as Medicare (“creditable coverage”). Otherwise, you may face a permanent financial penalty.

Part D Drug coverage is different in an important way.

Since going into effect in 2006, insurance companies tinker with their list of covered drugs. Because of this, each year during Medicare Open Enrollment (October 15 through December 7), it’s a good idea to determine which plan is

Reinvent yourself over 50

was recently asked to give a talk about the wonder and excitement of reinventing yourself after 50. The person who asked me said she viewed me as being a “liver” (as opposed to a kidney?) and thought my words might inspire others.

After considerable thought, I decided there are four components that influence the quality of one’s life after 50: attitude, humor, stress and purpose.

ATTITUDE. Your attitude colors your entire life, shaping how you perceive the world around you. A positive attitude can find the remarkable in the mundane, tolerance in the intolerable and humor in the midst of sorrow. It reminds you to embrace the person you’ve become and not give a damn about wrinkles or what others think.

Dr. Phil summed it up when he said, “You wouldn’t worry so much about what people think of you if you knew how seldom they did.” So stop yielding to yesterday’s belief of what seniors are supposed to look like. We walk around like we’ve got expiration dates stamped on our foreheads, but

STOP YIELDING TO YESTERDAY’S BELIEF OF WHAT SENIORS ARE SUPPOSED TO LOOK LIKE. INSTEAD OF PINING OVER YESTERDAY, GET INTERESTED AND INVOLVED IN TODAY. like our celebrity peers George Clooney, Meryl Streep and Kevin Costner, we’re living longer and are no longer behind the scenes.

We’re a force to be reckoned with. Instead of pining over yesterday, get interested and involved in today. Reconnect with your childhood by making a snow angel, getting a squirt gun to ambush your husband, or jumping up and down on your bed. (And, don’t forget to have your medical alert button with you at all times.)

HUMOR. The more things you can find to laugh at, the happier you’ll be. Plus, laughing has many health benefits such as lowered blood pressure, reduced stress, diminished depression and reduced aggression. Studies show that laughing more may be as important as exercising, not smoking, and eating a low-fat diet. Laughter also produces an overall sense of well-being and makes your heart healthier. It’s the pressure valve that gets us through daily obstacles and challenges.

Funny things happen around you all the time if you’re tuned in. Like the time I waited for over half an hour in a crowded deli store before realizing I’d forgotten to take a number. Even though life isn’t fair, when you laugh at yourself or at your situation, you’re in control of your life.

REDUCE STRESS. We can’t avoid it but we can handle it.

Playing board games regularly offers almost as much stress relief and prolonged life expectancy for people over 65 as for those who exercise regularly. Playing music can lower your blood pressure and heart rate.

Having friendships is vital. Loneliness has been linked to making people more susceptible to stress, depression and other illnesses. We all need someone with whom to discuss our problems and share our good times.

PURPOSE. Become involved in something you feel passionate about—something that makes you feel useful and necessary, gets your juices flowing, causes you to spring out of bed in the morning and keeps you riveted long past bedtime. Having a purpose reduces stress and the risk of depression.

The bottom line is to eat sensibly, laugh and play a lot, move your body and remember that age is only a number. ■

D

ecades ago, my primary care doctor referred me to a urologist for suspected kidney stones.

After straining my urine and collecting a little vial of stones, I phoned in the results to the urologist’s nurse. In my first meeting with the doctor, I placed the vial on his desk.

He leaned back in his chair, hands crossed behind his neck, and mused, “We’ll have to figure out what your pain was because young women like you don’t get kidney stones.”

“Then what are those?” I blurted out. He lurched forward in his seat to examine the stones. Thus began our “rocky” relationship.

At my next kidney stone attack, the nurse reported that the doctor was on the golf course and not answering his pager. I ended up in the emergency room, vomiting on the hallway floor. After that, I changed doctors.

As we age, we tend to spend less time jogging and more time with doctors. Our relationships with various medical professionals become increasingly important. What happens when we don’t see eye to eye with a doctor?

Complaints about doctors are more likely to be about communication than medical care.

A recent study revealed that 75 percent of doctors believe they communicate satisfactorily with their patients.

Only 21 percent of those patients agreed.

As medical knowledge about how the body works continues to grow, expectations increase for doctors and nurses.

At the same time, medical insurers push medical practices to accept larger patient loads. Health care may be at a breaking point, but when you are at a doctor’s appointment, your own health care is what rightly concerns you.

DO YOUR PART Before you give up on your current team, make sure you are contributing to good communication with your doctors.

Be firm but respectful of their professional knowledge.

Express your concerns honestly and graciously. If these attempts don’t resolve the issue, consider another doctor in the same practice, which would cause you less disruption in terms of medical records.

In my case, I never again met with that urologist, but that was partly because I’ve never had stones again. Each episode occurred within two months of childbirth, so I stuck with just two kids and skipped further stones.

In these difficult days for health care, you may feel like you have no choice but to keep your team together. In this case, address your concerns in the way most comfortable for you: in person, by phone, by mail or email. Don’t suffer in silence because, frankly, that’s not good for your health either. ■

lder housing isn’t what it once was, and thank goodness for that.

I worked part-time in a real convalescent hospital when I was in college. I remember walking in the front door of that nursing home, and the first thing that hit you was the smell of urine. Too many patients and too few staff to adequately care for them meant the residents were miserable much of the time.

These days, elder care has become a booming industry and most senior facilities have stepped up to meet the demand by providing excellent care. The older I get, the more friends and family members I have that need these various levels of care, from temporary stays in rehabilitation hospitals to independent living to full nursing care.

If you foresee that you or a loved one will soon need some degree of long-term care, it might be a good idea to start looking at the options. Here’s a breakdown of the major types of facilities.

Independent living. These are best for people who want convenience, don’t have specific medical care needs and can live on their own. Independent living communities offer meal preparation, housekeeping and security, and may have on-site beauty parlors and barbers, church services, buses to take residents shopping and transportation to appointments.

Assisted living. These are for people who need some medical care. They include everything offered by an independent living facility, but can also provide help with managing medications when needed, and with activities of daily living, such as bathing, dressing and grooming. Some assisted living homes are apartment-like settings and others are more institutional.

Nursing facilities. Nursing homes are for people who need round-the-clock nursing care and other support services. If this is the level of home you are considering, it’s important to inquire about how emergency care is provided. How available are physicians? Are staff members certified in CPR? How does the nursing home deal with family visits?

How are patients with Alzheimer’s or Parkinson’s cared for?

Your local Area Agency on Aging may have some of this information available.

WHAT TO CONSIDER If possible, let the patients participate in the decision of where they will live even if their needs for care are far into the future. If you go facility shopping, here are a few points to pay special attention to: • Cleanliness. Observe hygiene. It shouldn’t be hard to notice if residents are being neglected.

• Staff. Does the number of staff seem adequate to manage the number of patients? That’s not to say that employees aren’t hopping busy, but the staff-to-patient ratio should be such that all patients get bathed and receive other personal attention they need. The same is true not just for nursing care, but also general maintenance and kitchen staff.

• Services. What are the patient’s special needs? Be sure the facility provides for those needs. For example, a patient may require the use of a lift machine to get in and out of bed. Many will need physical therapy and/or a gym where they can go to strengthen their limbs. How are supplies paid for? If supply costs are added to the patient’s bill, are the costs reasonable? Are there regular increases in costs? Hidden charges? Obviously, meals are a key service. Visit facilities during meal times and observe if meals are served in a relaxed manner and if there’s plenty of food available.

GET THE UNSEEN STORY Don’t accept any facility without first checking its financial stability, record of legal compliance and quality of management. Facilities aren’t likely to be perfect, but significant problems in any of these areas may lead to trouble finding a sufficient staff of qualified health professionals.

Find out if the facility has current valid state and local licenses, and liability and malpractice insurance.

Ask to review state inspection reports and investigate whether they have lawsuits pending. ■
