Stuck mulling over the Medicare muddle? Crucial tips for understanding Medicare By Karen Telleen-Lawton

through December 7), it’s a good idea to determine which plan is least expensive and most convenient for your menu of prescriptions. You can choose a new drug plan each year with no penalty; however, if you require a new prescription during that year, you’ll be at the mercy of your current drug plan. When choosing your Medicare plan, start by asking your doctors and your friends what plan(s) they recommend. Perhaps the clinic or hospital your doctor is associated with only accepts one or two plans. If your friends have had good experiences with their plans, consider those, but understand that what works for your friends may not always work for you.

’ve been writing about Medicare for years, so I wasn’t particularly affronted when, not long after my 64th birthday, I started receiving advertising circulars about Medicare, Medicare Advantage and supplemental plans. Whether you are a newly enrolling senior or have been enrolled in Medicare for years, you do need to understand the system and the changes that happen nearly every year.

AGING RESOURCES

WHERE TO START Confusion about Medicare stems from it being cobbled together over the last 55 years by many political administrations. When approved by President Lyndon Johnson in 1965, it included only Part A (hospital) and Part B (doctor visits) coverage. In the 1980s, supplemental private insurance in the form of Medigap plans were offered to cover some of the deductibles and copays. The Medigap plans currently available in most states are A, B, D, G, K, L, M and N. Each plan offers a different level of coverage. Two other plans, C and F, are no longer available to new enrollees. In the 1990s, Medicare Advantage (MA, or Part C) was offered as an entirely different way to access the Medicare system. MA providers offer more services than original Medicare with the tradeoff of limiting your choice of medical providers. Whereas the focus of Medigap is covering the “gap” of deductibles and copays, MA’s focus is providing additional services such as vision and dental. Your first decision, and the most important, is deciding whether to expand your upcoming Medicare coverage either through Medicare Advantage (Part C) or Medigap (supplemental Medicare). A good place to start is by reading “Medicare & You: The Official U.S. Government Handbook,” available at www.medicare.gov.

DRUG COVERAGE Part D Drug coverage is different in an important way. Since going into effect in 2006, insurance companies tinker with their list of covered drugs. Because of this, each year during Medicare Open Enrollment (October 15

You can also contact a local Medicare insurance sales advisor.

ADDITIONAL THINGS TO KNOW: • Your initial Medicare enrollment period begins the third month before your 65th birthday month and finishes at the end of the third month after your birthday month. That gives you seven months to make some pretty momentous decisions. • You can sign up later, at retirement, if your current coverage through an employer is at least as comprehensive as Medicare (“creditable coverage”). Otherwise, you may face a permanent financial penalty. • Open Enrollment—when current insureds can choose a new MA, drug, or supplemental plan—is October 15 through December 7 each year. Coverage starts January 1, 2022. If you’re approaching your 65th birthday, make some time for Medicare decisions. Despite my background in financial planning, I still find the choices complex and confusing. ■

How to refurbish your kitchen on a budget AFTER

only wanted new blinds for the kitchen. However, the best-laid plans, etcetera. Wandering through the showroom of a local window and blinds dealer, I fell in love with pale gray, custom vertical honeycomb insulating blinds. The clincher? They were half price. I ordered one for the slider, one for the small window and settled back to wait for delivery. What could possibly go wrong? A lot. When I removed the one-year-from-ancient traverse drapes, I found a doggy door, long forgotten because the drapes had hidden it for years. My nephew, a carpenter, tore out the eyesore and patched the hole. He and I installed the new blinds. That wasn’t the end of it, though. The creamy walls looked dirty, and they clashed with the blinds. So off to buy the palest gray and stark white paints available. Fortunately, I like painting, because I would spend the next two weeks painting the walls and refreshing the cabinet paint. Years ago, I’d painted the doors and drawer fronts spring-green. They really didn’t work with the new color scheme. Off came the doors (all 22 of them) and seven drawer fronts. The original brass hardware, discolored and dirty, would have been an insult to a second-hand store. My neighbor Bob and I spent the next two weeks stripping green paint, sanding surfaces and priming doors in batches. The doors came home to my studio to be given three coats of pure white paint. Replacement hinges and knobs fit the clean lines I envisioned. Mounting the lower doors was easy. The high ones, not so much. Again, Bob arrived and had the tall upper doors in place (with me holding them) in half an hour.

The room looked great except for the 1980 woodgrained Formica countertop with loose edge strips that caught on anyone passing. Replacing the counter would mean big bucks. There had to be a less expensive way. Sunday morning I went out to get the paper and lo and behold! There was an ad for a company that refinishes counters—with a $70 off coupon! Monday, I went to the showroom to find out when I could get an estimate. The young lady left, returned, and said, “How about today?” I barely beat the installer home! Later that week, the fridge took up residence on one side of the dining table and the stove on the other. Plastic sheets draped cabinets, sink and floor. The installer had filled all the dings and cracks. By 5 p.m., the Formica had been transformed into what looked like a granite slab. Even though I’d been warned not to touch, I couldn’t help gently tapping the “stone” with an inquisitive finger. I was in love. On Friday, the installer sprayed her little heart out, then cautioned me, “Give it 48 hours.” When the stove and fridge were back in place and my usual countertop clutter restored, my heart felt as if it would burst. Friends and strangers alike had created a modern space decades away from the 40-year-old kitchen I’d worked in for 18 years. The cost was affordable, but the best part of the refurbishing cost nothing. Relining the cabinets and drawers made me revisit my stuff. Did I actually need 23 coffee cups? Or three partial jars of thyme? I had gotten into the habit of leaving dirty dishes in the sink and boxes of cereal and other clutter on the counter. Now I’m eager to keep my kitchen clean and neat. I sit in the morning sun, serene and joyful, write in my journal, and smile as I look at my brand-new kitchen. ■

hen a person is strong mentally and emotionally, they are happier, healthier and more resilient. Loneliness, change, tragedy and uncertainty can present challenges to mental health, particularly in older adults. This leaves them at risk for depression, anxiety and feelings of isolation, which can dramatically impact one’s quality of life. Maintaining positive mental health can offset the effects of these feelings. Here are 10 tips on how to keep your mind engaged and prepared for positive thinking.

Talk to your doctor. Be honest with your doctor about how you’re feeling mentally and emotionally. Tell him or her about symptoms you’re experiencing as well as major life changes, such as divorce, losing a loved one or financial problems.

Stay physically active. Enjoy Colorado’s breathtaking scenery and abundant sunshine by taking a brisk walk or bike ride. Join a gym, take up yoga or line dancing lessons at the recreation center, or swim laps in the pool.

Avoid alcohol and other substances. Regular drinking or substance use interferes with brain chemicals and processes that are vital for mental health. Disruption to this delicate balance can affect our feelings, thoughts, actions and sometimes our long-term mental health. Because alcohol is a depressant, drinking it can lead to negative emotions like depression, anxiety, anger and stress. Eat healthy foods. Healthy diets can help with symptoms of anxiety and depression, while unhealthy diets can cause an increased risk of dementia or stroke. Eating fresh fruits and vegetables, whole grains, beans and lean meats positively affects production of neurotransmitters—chemicals that carry messages from the gut to the brain. When you eat healthily, your brain receives positive messages, leading you to feel positive emotions. Conversely, eating processed foods high in refined sugars and fats inhibits neurotransmitter production. A healthy diet results in fewer mood fluctuations, improved focus and an overall happier outlook.

Become a pen pal. Studies show that writing and exchanging letters benefits both the sender and receiver. It helps improve memory, maintain family ties and leads to greater feelings of happiness. Chat on the phone or video call. It’s important to feel connected, especially when you’re feeling down or depressed. Reconnect with loved ones by visiting with them over the phone. Use a video app such as FaceTime, Skype or Zoom to see them in real time. FaceTime comes pre-installed on Apple devices, and Skype and Zoom can be easily downloaded.

Find a hobby. Invite friends over for games or play with them online. Join a book club, watch concerts and theater productions, or head to a local museum or art gallery. Check out books from the library or watch YouTube videos to learn a new skill like crocheting, fly-fishing or watercolor painting.

Volunteer. Find purpose in volunteering, and meet like-minded individuals. Turn to page 46 of the BEACON Guide for places to volunteer in your area. Reach out to the organizations that would best benefit from your time and expertise. Make sure it’s a good fit for you as well. Limit your news intake. It’s important to be well informed, but if watching the news causes you anxiety or distress, limit your intake. Catch up in the morning and at night, limiting yourself to 30 minutes each time. Seek help. The Center for Mental Health Support Line is a free, 24-hour line open to the community. If you’re feeling stressed, anxious or just need to talk with someone, call 252-6220. Determine what level of care is right for you: Are you independent? Need a little assistance—or a nurse? By Teresa Ambord

lder housing isn’t what it once was, and thank goodness for that. I worked part-time in a real convalescent hospital when I was in college. I remember walking in the front door of that nursing home, and the first thing that hit you was the smell of urine. Too many patients and too few staff to adequately care for them meant the residents were miserable much of the time. These days, elder care has become a booming industry and most senior facilities have stepped up to meet the demand by providing excellent care. The older I get, the more friends and family members I have that need these various levels of care, from temporary stays in rehabilitation hospitals to independent living to full nursing care. If you foresee that you or a loved one will soon need some degree of long-term care, it might be a good idea to start looking at the options. Here’s a breakdown of the major types of facilities. Independent living. These are best for people who want convenience, don’t have specific medical care needs and can live on their own. Independent living communities offer meal preparation, housekeeping and security, and may have on-site beauty parlors and barbers, church services, buses to take residents shopping and transportation to appointments. Assisted living. These are for people who need some medical care. They include everything offered by an independent living facility, but can also provide help with managing medications when needed, and with activities of daily living, such as bathing, dressing and grooming. Some assisted living homes are apartment-like settings and others are more institutional. Nursing facilities. Nursing homes are for people who need round-the-clock nursing care and other support services. If this is the level of home you are considering, it’s important to inquire about how emergency care is provided. How available are physicians? Are staff members certified in CPR? How does the nursing home deal with family visits? How are patients with Alzheimer’s or Parkinson’s cared for? Your local Area Agency on Aging may have some of this information available. News of substandard care travels fast in the medical community, so ask the patient’s doctor, nurses and thera-

WHAT TO CONSIDER

If possible, let the patients participate in the decision of where they will live even if their needs for care are far into the future. If you go facility shopping, here are a few points to pay special attention to: • Cleanliness. Observe hygiene. It shouldn’t be hard to notice if residents are being neglected. • Staff. Does the number of staff seem adequate to manage the number of patients? That’s not to say that employees aren’t hopping busy, but the staff-to-patient ratio should be such that all patients get bathed and receive other personal attention they need. The same is true not just for nursing care, but also general maintenance and kitchen staff. • Services. What are the patient’s special needs? Be sure the facility provides for those needs. For example, a patient may require the use of a lift machine to get in and out of bed. Many will need physical therapy and/or a gym where they can go to strengthen their limbs. How are supplies paid for? If supply costs are added to the patient’s bill, are the costs reasonable? Are there regular increases in costs? Hidden charges? Obviously, meals are a key service. Visit facilities during meal times and observe if meals are served in a relaxed manner and if there’s plenty of food available.

GET THE UNSEEN STORY Don’t accept any facility without first checking its financial stability, record of legal compliance and quality of management. Facilities aren’t likely to be perfect, but significant problems in any of these areas may lead to trouble finding a sufficient staff of qualified health professionals. Find out if the facility has current valid state and local licenses, and liability and malpractice insurance. Ask to review state inspection reports and investigate whether they have lawsuits pending. ■
