New to Medicare? Here’s an overview

f your 65th birthday is around the corner or you’re anxious about the Medicare open enrollment period, it’s a good time to start focusing on your options.

Health care choice has become a bigger factor in the lives of pre-retirees since the Affordable Care Act. Though a separate federal health insurance system with no connection to Obamacare or its online marketplaces, Medicare is going through its own evolution in terms of plan offerings and customer access.

Here’s a basic primer for future Medicare enrollees: What is Medicare? Medicare is a government-provided

health and hospitalization insurance program for people 65 and older and for some people under age 65, based on disability or particular forms of illness.

What does it cost? Though you’ve likely paid taxes into the

Medicare system your entire career, Medicare isn’t a completely free program. You’ll pay premiums deducted from your Social Security checks for some portions of your benefits.

There may be copays and deductibles for certain services.

When do I get to join? You can apply for Medicare

through your nearest Social Security office up to three months before your 65th birthday. If you wait longer than three months after your 65th birthday to apply, you may face a late enrollment penalty.

WHAT DOES COVERAGE LOOK LIKE? • Parts A (Hospital) and B (Outpatient). Part A

covers inpatient hospital care and Part B covers outpatient medical care and procedures. You’ll hear Parts A and B referred to as “Original Medicare” because if you choose

to take this option, you’ll sign up for them as a pair. You’ll get to go to any doctor, hospital or facility that’s enrolled in Medicare and accepting new Medicare patients. You’ll end up paying a premium for Part B, which can change from year to year, while Part A is usually free.

• Part C (Medicare Advantage). Call this one-stop shop-

ping for your hospital, outpatient (Part A and B) and usually your prescription coverage (see Part D below). Medicare Advantage policies are sold by private insurers. Medicare pays their share and you pay your premium. They may include different assortments of services, including vision, prescription drug, dental or hearing. But keep in mind, different Medicare Advantage plans have different rules for services and can charge different out-of-network costs.

• Part D (Prescription drug coverage). Part D added

prescription drug coverage to Original Medicare but may be folded into Medicare Advantage coverage if you elect to take it.

What’s Medigap insurance? If you go with Original Medicare (Part A and B), you’ll find some gaps in your Medicare coverage. Medigap is the nickname for Medicare Supplement Insurance, which you purchase privately. If you’re on Medicare Advantage, it’s unlikely you’ll need separate Medigap coverage but it’s important to check.

What if you plan to keep working with company health benefits after you turn 65? Confer with your

financial advisor and Medicare before you make any decisions, but you may qualify for a special enrollment period. People who continue to work past 65 may enroll in Part A but bypass Part B and D if they’re on a company plan.

Are your doctors on the plan? You’ve probably had

some experience with this, but continue to check to see whether your physicians are in network on Medicare Advantage.

What can your state do for you? If you find

you’re not getting answers on specific Medicare subjects from Medicare itself, then contact SHIP (State Health Insurance Assistance Program). SHIP offers one-on-one counseling on Medicare. To contact the Mesa County SHIP office, call 243-9839.

Is your retirement plan doable?

By Teresa Ambord re you approaching retirement? If so, it’s time to review your plan and figure out if it will work for you or not. The Wall Street Journal (WSJ) recently offered some tips for those hoping to retire in the next five years. While there’s time, ask yourself the following questions.

A

Do you have prepaid or discounted legal services through your job?

According to the WSJ, one of the biggest regrets clients have in retirement is that they did not take advantage of this type of plan when they had the chance. If it’s available, use it to have a will and trust drawn up.

How much income will you need to retire the way you want to?

Calculate what you spend now. This might mean keeping a record of every dime you spend for a few months so you’ll have a baseline. Then make a rough retirement budget and adjust your current spending to what you expect to have at retirement.

Do you plan to travel? Are you going to work part time or pursue a second career?

By making a retirement budget and comparing it to what you spend now, you can know what lifestyle changes you’ll have to make after you stop working full time.

Where do you want to live?

Do you plan to stay put, downsize or relocate?

Factor in the costs to move and the closing costs of buying a new residence. If you plan to move to a condo, what fees should you expect to pay?

If you expect to move to another state, what will the differences be in state personal income tax, property tax and sales tax?

State and local taxes can vary widely. Some states do not charge personal income tax at all, and in other states, property taxes are oppressive. On the other hand, some states have such affordable property tax that you’ll find you can afford a bigger house than you expected.

Will you have a mortgage?

It might be a good idea to refinance your mortgage while you are still working. Interest rates are low, but may head higher. If you can’t pay off your home before you retire, consider refinancing now to reduce your payments to a level that will be easier to manage on retirement income.

Remember, you’ll have your mortgage interest tax deduction to offset your taxable interest income.

What car or cars will you need?

You’ll want your overhead to be as low as possible once you retire, so it’s best to avoid car loans. If you hope to upgrade your vehicle in retirement, it might be best to do it now. Also consider whether you’ll need two cars. If one is enough, you can sell the other and tuck away the money or upgrade the vehicle you keep.

What will your taxes be in retirement?

Of course, nobody knows what the tax rates will be, but here are some points you can anticipate. If you’re contributing to a 401(k) plan, you are likely getting a healthy tax break for that. But once you stop contributing and start collecting, the tax situation flips. You no longer have a deduction, and the distributions you get will be taxable.

You might be able to avoid that harsh reality by moving some funds now into a Roth 401(k) or Roth IRA. You will pay the income due on those funds now, but your future earnings will be tax free. Plus, if you believe taxes will rise in the future, you can save by getting the tax bite over with now. Conversions can be complex, so don’t attempt them without the help of your tax adviser.

Also, if you currently have a mortgage but plan to sell your home or pay off the mortgage before retirement, keep in mind that you will lose a significant tax deduction.

What is your health insurance situation?

Will you keep the benefits you have through your employer?

Of course, you won’t be able to qualify for Medicare until you are 65, and even then you will need to cover significant out-of-pocket costs.

Sit down with your financial adviser to look at what you have and what employer policies, if any, you can keep.

How is your money currently invested?

If you’re five years from retirement, you may want to shift some of your money away from growth stocks, which carry more risk, and towards something more conservative, such as income investments. But don’t move all of your money.

If the market grows, you’ll want to have some skin in the game. Even this close to retirement, you still need some diversity in your investment portfolio.

What’s your game plan?

It’s easy to put off thinking about retirement until after you retire. Take some time to consider potential hobbies and lay the foundation for what you’d like to pursue in retirement, so when the time comes, you can step right into the next chapter.

Do you really need a Realtor?

Maybe you’ve considered hiring a Realtor to represent you, but then you think, “How hard can it be? Lots of people sell homes on their own, and I can save a bundle on commissions. So do I really need a Realtor?”

The answer is yes. Marie-Louise Hausermann found that out the hard way.

A year ago, she decided to list her former home as for sale by owner, cash sale, as is. It didn’t take long for a buyer to appear. The buyer was in a legal wrangle with an ex, but promised that the sale would be completed in a couple of months. Hausermann and the tenant signed a lease, not a sales contract, expecting that the sale money would be in the bank soon. After a year of promises, legal hassles and stress, she discovered the tenant had moved out two days before the closing date.

GET TOP VALUE FOR YOUR HOME

Although some who put their properties on the market themselves may manage to navigate the complex legal issues of selling a house, there are many more who wish they had just contacted a Realtor in the first place.

“Realtors know the market and we know the things that will really make a home sell and get the top price,” said Carol Gerber, a Realtor with Bray Real Estate.

Michelle Klippert, owner of The Real Estate Store in Montrose, said that in 2016, the average sales price of a home listed by its owner was $185,000. The average price for a home represented by an agent was $245,000—a $60,000 difference.

“Seniors can get a good idea of what homes are going for in their area but they usually do not have the data that is associated with the upswing and downturn of a market,” Klippert said. “This is associated with market value and not appraised value.”

KNOWLEDGE AND UNDERSTANDING

The benefits of using a Realtor far outweigh the fee charged. For instance, a broker can interpret the 35-40 pages of the contract, which is always couched in legalese.

Realtors also continuously update their education so they always know the industry’s latest rules and regulations.

What to look for in a Realtor Gerber and Klippert gave the following advice:

• Ask how long they’ve been in the business.

• Aim for Realtors with experience and who have a track record of sales.

• Ask what additional education they have done and verify their credentials.

• Interview them to see if they will be your trusted adviser.

• Seniors need more than a salesperson. They need advisors to help with all aspects of their transition, which may include information on movers, house cleaners, maintenance people and more.

Realtors market your home professionally, listing it on multiple websites and in the newspaper, using attractive signs and hosting open houses.

Realtors are a great help in the buying process as well. If you’re in the market to buy a home, the real estate professional who shows you properties does not charge a fee for showing them. He or she will be paid only when your offer is accepted, the contract is signed and the sale is complete.

Klippert said Realtors can provide a perspective that their clients may not have, viewing the house as it is and not as buyers and sellers want it to be.

“This includes viewing the deferred maintenance, dated elements of the home and establishing time frames to pack and move,” she said. “They can communicate with the sellers and sometimes even the extended families to create game plans for downsizing and correcting home issues that can help with the sale of the home.”

Realtors work with owners, offering suggestions on small changes that can make a big difference in the price and how quickly the property sells.

“I recently helped a client downsize from her family home of 40 years. I advised her to make a few changes, like placing tile in bathrooms and removing carpet,” said Gerber. “We were able to market her home and get it sold fairly quickly at a higher price than she thought she could get.”

Clients should be up front about their properties and what results they want and expect. Only you can decide if you want to hire a professional or buy or sell your home on your own. You owe it to yourself to know as much as you can about the process before making a decision that will affect your life—for better or for worse.

OT, PA, PT, MA—what’s the difference?

Navigating medical titles for the best health care experience

In our golden years, we visit the doctor more often. He or she sends you off to someone else for tests or rehabilitation, but you may have little or no clue who. Here’s a quick rundown of the duties behind each medical title.

I

f you’ve had a lab test, you’ve already met your friendly neighborhood phlebotomist, who draws blood to be analyzed. You may believe they’re direct descendants of Dracula, but phlebotomists perform a vital function in health care. Blood analysis reveals developing problems or those already underway.

If you have asthma or emphysema, you’ve probably visited a respiratory therapist. Respiratory therapy also helps you regain your ability to breathe as you recover from a heart attack or stroke.

An occupational therapist (OT) teaches injured or disabled patients to use common movements throughout the day to get better sooner.

A physical therapist (PT) helps patients recover from illnesses or injuries using special equipment to work your muscles and build strength. If you’ve had rotator cuff surgery, joint replacement, a car accident, or if you’ve strained a knee in pickleball or golf, you’ve probably seen a PT.

A radiologic technologist (RT) dresses you in that cute little gown—you know, the one with the open back—so he can take x-rays or scans without interference from zippers and other closures. Radiologic techs are not to be confused with radiologists, which are medical doctors trained to read those images. That’s why you see two different charges for x-rays and scans on your insurance statement.

There’s also a new kid on the block: the medical assistant (MA). Back in the day, a nurse in a starched uniform and jaunty cap performed the job, but MAs are not nurses.

They’re trained to do administrative tasks such as measuring and entering blood pressure, temperature and symptoms into your chart before the doctor comes in. Most MAs are certified and have had job-specific training in the field.

You may have also been examined by a physician’s assistant (PA) or nurse practitioner (NP). Both practice under the supervision of a doctor. They often work in clinics, such as Marillac or in rural areas, but are becoming a familiar sight in doctors’ offices. NPs and PAs examine patients to diagnose problems and prescribe medication or therapy, but PAs have more training and may be working in a clinic while earning their medical degrees. Both are highly qualified.

If problems arise that they can’t handle, a doctor is called upon to provide more specialized care.

With luck, your experience with any of the above will be limited, but it’s comforting to know they’re around to help you get the best health care you could ask for.

Are you independent? Need a little assistance—or a nurse?

E

I worked part-time in a real convalescent hospital when I was in college. I remember walking in the front door of that nursing home, and the first thing that hit you was the smell of urine. Too many patients and too few staff to adequately care for them meant the residents were miserable much of the time.

These days, elder care has become a booming industry and most senior facilities have stepped up to meet the demand by providing excellent care. The older I get, the more friends and family members I have that need these various levels of care, from temporary stays in rehabilitation hospitals to independent living to full nursing care.

If you foresee that you or a loved one will soon need some degree of long-term care, it might be a good idea to start looking at the options. Here’s a breakdown of the major types of facilities.

Independent living. These are best for people who want convenience, don’t have specific medical care needs and can live on their own. Independent living communities offer meal preparation, housekeeping and security, and may have on-site beauty parlors and barbers, church services, buses to take residents shopping and transportation to appointments.

Assisted living. These are for people who need some medical care. They include everything offered by an independent living facility, but can also provide help with managing medications when needed, and with activities of daily living, such as bathing, dressing and grooming. Some assisted living homes are apartment-like settings and others are more institutional.

Nursing facilities. Nursing homes are for people who need round-the-clock nursing care and other support services. If this is the level of home you are considering, it’s important to inquire about how emergency care is provided.

How available are physicians? Are staff members certified in CPR? How does the nursing home deal with family visits?

How are patients with Alzheimer’s or Parkinson’s cared for?

Your local Area Agency on Aging may have some of this information available.

News of substandard care travels fast in the medical community, so ask the patient’s doctor, nurses and therapists what facilities they would recommend. Also ask social workers, relatives and friends.

WHAT TO CONSIDER

If possible, let the patients participate in the decision of where they will live even if their needs for care are far into the future. If you go facility shopping, here are a few points to pay special attention to: • Cleanliness. Observe hygiene. It shouldn’t be hard to notice if residents are being neglected.

• Staff. Does the number of staff seem adequate to manage the number of patients? That’s not to say that employees aren’t hopping busy, but the staff-to-patient ratio should be such that all patients get bathed and receive other personal attention they need. The same is true not just for nursing care, but also general maintenance and kitchen staff.

• Services. What are the patient’s special needs? Be sure the facility provides for those needs. For example, a patient may require the use of a lift machine to get in and out of bed. Many will need physical therapy and/or a gym where they can go to strengthen their limbs. How are supplies paid for? If supply costs are added to the patient’s bill, are the costs reasonable? Are there regular increases in costs? Hidden charges? Obviously, meals are a key service. Visit facilities during meal times and observe if meals are served in a relaxed manner and if there’s plenty of food available.

GET THE UNSEEN STORY

Don’t accept any facility without first checking its financial stability, record of legal compliance and quality of management. Facilities aren’t likely to be perfect, but significant problems in any of these areas may lead to trouble finding a sufficient staff of qualified health professionals.

Find out if the facility has current valid state and local licenses, and liability and malpractice insurance.

Ask to review state inspection reports and investigate whether they have lawsuits pending.
