How to access home care

Love living at home? Home care is about so much more than a helping hand. It’s about staying connected to the life you love, in the place that matters most. If you’d like some support to keep living independently at home, you may be eligible for Government-funded Home Care services. We can help you work out what’s available and make the process as simple as possible.

How to access home care Learning how to access government-subsidised home care services can initially be a little overwhelming. We’re here to help by breaking the process down into five simple steps.

STEP 1

Register The first step is registering with My Aged Care, the Australian Government’s aged care service. You can do this by calling on 1800 200 422 or visiting www.myagedcare.gov.au If you’re unsure where to start, our friendly team can answer questions, explain the process and help you feel more confident navigating the aged care system.

STEP 2

Be assessed Next, a My Aged Care assessor will take time to understand the level of support you need, either over the phone or by visiting you in your home. If you’re looking for some basic or short-term help with things like housework, personal care or shopping, you may be approved for funded support through the Commonwealth Home Support Program (CHSP). If your needs are more complex or would benefit from coordinated care, you may be approved for a Support at Home (SaH) budget, with funding classifications ranging from 1 to 8, depending on the level of support you require. Additional short-term funding pathways may also be available for things like recovering after illness or making changes to your home. STEP 3

Wait for funding Your funding can take some time to be finalised. You may be offered interim Support at Home funding, allowing you to begin receiving some services before your full budget is allocated. While you’re waiting for funding, we can also support you with a wide range of self-funded home care services.

STEP 4

Choose a provider Once your Support at Home budget has been approved and assigned, you’ll receive a letter from My Aged Care confirming your funding. From there you can choose a provider such as BaptistCare to deliver your services and support at home. STEP 5

Choose how to use your Support at Home budget Working with your dedicated Care Manager, you can take your time and choose the home care services that are most important to you. This will likely involve a multi-skilled care team, such as physiotherapists, registered nurses, or gardeners. Your Care Manager will then assist with the set-up and ongoing management of your services. Your needs and services will be reviewed at least annually and if your needs change. Our Care Managers will support you and your family to access additional support, so you can be confident you’re always receiving the best possible care to help you live life on your terms.
