What is home care?
Home care is for people who love their home and want to stay there – with a little help along the way. Maybe you’d like a hand with the heavier housework, help recovering from an illness or injury, or just someone to check in a few times a week. Whatever you need, home care brings support to you so you can keep living life on your terms.

Home care programs The Australian Government subsidises home care through the Commonwealth Home Support Program and Support at Home. If you are not eligible for government-funded services, or you would like support sooner, private home care is also available.
Who’s eligible? To access home care, you’ll generally need to be 65 or older (50 or older if you’re Aboriginal or Torres Strait Islander, or if you’re homeless or at risk of homelessness).
TYPE 1
Commonwealth Home Support Program (CHSP) If everyday tasks are getting a little trickier and you think some support would help, the Commonwealth Home Support Program could be a great fit. It’s designed for people who are still largely independent but could do with a hand here and there, whether that’s help around the house, getting to appointments, or some allied health support to help you feel strong and confident at home. The Government covers most of the cost, with a contribution from you if you’re able to.

TYPE 2
Support at Home (SaH) If your needs are a little more involved, Support at Home may be right for you. It provides Government funding for both ongoing and shortterm pathways, with eight classification levels and three short-term pathways, so support can be matched to where you’re at right now. If you’re currently on a Home Care Package, there are also four transition classifications to help you move across smoothly. SaH Classification

Palliative care at home When home is where you’re happiest, we’ll do everything we can to help you stay there. Palliative care is about living as actively and as comfortably as possible, whatever that looks like for you. Our team focuses on managing pain and symptoms, spending real time with families and providing the emotional support and respite that makes such a difference. And while people often think of palliative care as something that happens in hospital, it can also happen at home. In fact, getting started earlier often means better comfort and symptom management from the outset.

BaptistCare Home Care
Every person’s experience is different. By listening carefully to your needs and those of your family, we can create a personalised care plan that brings peace of mind to everyone.

Your Care Manager can help you understand what’s available and make sure everything is in place. If you’re at the beginning of a palliative care journey – yours or a loved one’s – our team is here to help. Please reach out.

Depending on your needs, you may be approved for the Commonwealth Home Support Program. For more complex needs, you will be approved for Support at Home with a classification between one and eight, with eight being the highest. You will receive a notification advising you of the outcome of your assessment.

Frequently asked questions Do I need to be assessed to access home care?
How can my Support at Home funds be used?
If you’re accessing Governmentfunded care, you’ll need to start with a conversation with My Aged Care, who will work out the right level of support for you. Depending on your situation, this assessment may be conducted over the phone or in person at your home.
Support at Home funds can only be used on goods and assessed services that are directly connected to enabling you to continue to live safely at home.
If you’re approved for Support at Home, a further financial assessment through Services Australia will determine what cocontribution, if any, you’ll need to make towards your services. If you’d prefer to arrange private home care without going through the Government system, no external assessment is required and BaptistCare can get you started straight away with our full range of services.

BaptistCare Home Care
Your Care Manager will work with you to develop a personalised Care Plan based on your goals and care needs. Your funding will be used based on a range of criteria including your assessment outcomes, safety, needs, scope of the Support at Home program, goals, available funds and value for money. Where appropriate, GPs or allied health professionals such as occupational therapists may be involved in recommending the right equipment or home modifications for your situation.
