Final Footprints is a compassionate end-of-life care coordination service supporting people living with a non-malignant, life-limiting illness. Our focus is to enhance wellbeing, dignity, and quality of life during this important stage.
Our Care Coordinators walk alongside you and your loved ones, ensuring your wishes are heard, respected, and supported every step of the way.
Some examples of non-malignant life-limiting illnesses we help people with include COPD, heart failure, dementia, and Parkinson’s disease.

We provide personalised, holistic support to individuals and their carers by:
Promoting end-of-life planning and personal goals
Helping navigate Aged Care systems and access to Home Care services
Coordinating care across health and community providers
Assisting with Advance Care Planning and directives, including legal information
Advocating for your dignity, needs and providing emotional support Facilitating meaningful legacy projects (e.g. letters, memory books)
Providing care that is respectful of cultural, spiritual, and personal values by linking you with appropriate services and supports
This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.
The Final Footprints Care Coordination Service is available to anyone over 55, with a non-malignant life-limiting illness, living in Newcastle, Lake Macquarie and the Central Coast.

GP’s, nursing and allied health professionals
Local palliative care services.
