Closer to a cure by Dawn Gresko
S
ometimes clinical trials get a bad a rep: we hear horror stories of medical malpractice and misuse, as well as mistreatment of patients, and we’re quick to assume the worst. But, do these negative examples give a clear picture of what clinical trials aim to accomplish? To gather up the facts, Pensacola Magazine reached out to local medical facilities engaged in clinical trial research to find out more on protocols for open trials and procedures for signing up. While there are several open trials at Andrews Institute, Baptist and Sacred Heart hospitals, we honed in on a select few that focus on lowering bad LDL cholesterol, finding new treatments for Chronic Lymphocytic Leukemia (CLL), and a non-invasive treatment for helping those with osteoarthritis, all aimed at bringing us closer to a cure. In a clinical trial, volunteers receive a specific intervention, such as a drug or device, according to the research protocol, or plan, created by the investigators. A trial is typically headed by a principal investigator, often a medical doctor, who may oversee a research team including other doctors, nurses and other healthcare professionals. These trials can be sponsored (or funded) by pharmaceutical companies, academic medical centers, voluntary groups, and other organizations. Although an informed consent document, which contains all pertinent details of the trial, must be signed before a participant enters a trial, the document is not a contract; a participant may withdraw from a trial at any time regardless of whether or not it has concluded. Additionally, to ensure the health and best interests of all patients are a priority, all trials must be
reviewed and approved by the IRB (institutional review board) and the FDA, who are actively involved in adequately protecting all research participants. The Andrews Research & Education Institute (AREI) of Gulf Breeze is a special, nonprofit division of the Andrews Institute, which was established to advance the science of orthopaedic care and sports medicine to educate not only the professional community but the general public on issues affecting daily health and well being. Currently, there are as many as ten on-going clinical trials at AREI, including OrthoCor—a prospective, double blind, and placebo-controlled study on patient satisfaction outcomes with the use of an OrthoCor PEMF treatment for osteoarthritis, especially in regards to the knee. The goal of the OrthoCor clinical trial is to significantly decrease a patient’s pain due to knee osteoarthritis, especially if the patient has tried other treatments, such as intra-articular corticoseteroid injections, which have failed to reduce pain. “We are always looking for less invasive ways to treat our patients,” said Dr. Joshua G. Hackel and Dr. G. Daxton Steele of the Andrews Institute. “The OrthoCor brace is painless and will hopefully prove to be an additional treatment method for our patients suffering from osteoarthritis. We know the device works; it’s already been proven in previous trials. duce edema. PEMF is a low-level, time-varying electromagnetic field that penetrates superficial soft tissue, helping to accelerate the body’s natural anti-inflammatory and recovery responses. Those who are eligible for the trial include patients ages 45 to 75 who have on-going pain after receiving traditional therapies, and Andrews Institute uses a Kellergren Lawrence scale to see if patients rank high enough (between Grades 3 to 4) to be included in the study. The OrthoCor trial lasts six weeks and requires that patients wear the brace at least two hours a day starting in the morning. Laura Schubert is the research nurse for OrthoCor under AREI, and she attends to the patient’s care during the six-week trial. There are a total of four visits to Andrews Institute during the study: two visits with the doctors and two with the research nurse. Other trials at AREI include prospective treatments for plantar fascilitis, rotator cuff tears, chronic diabetic ulcers, rheumatoid arthritis, and more. You may not know it, but Sacred Heart Hospital has a full-fledge Cancer Center that participates in clinical trials by the National Cancer Institute. Sacred Heart’s current cancer research studies include several prospective treatments for breast and lung cancers, multiple myeloma, and many others including supportive treatment trials to address symptoms, such as mouth sores, caused by traditional radiation therapy for cancer affecting the neck and head. Clinical coordinators and research nurses Betsy Brou and Toni Hyatt are overseeing as many as 20 trials at the Sacred Heart Cancer Research Center, including three trials on CLL (Chronic Lymphocytic Leukemia). All three are randomized studies with a similar goal: keeping CLL patients in remission for as long as possible. Here’s a breakdown of each study: the first trial is for all CLL risk types that are either newly diagnosed or failed other lines of therapy, the second is for CLL high risk types that failed other lines of therapy, and the third is an observational rather than interventional trial that focuses on collecting data on CLL rather than administering experimental treatment.
The new treatment being tested is Ublituximab, compared to the approved treatment Ibrutinib. Patient visits to the cancer center for treatment vary anywhere from two weeks to a month. “We encourage more to participate in clinical trials,” said Toni Hyatt, research nurse at Sacred Heart Cancer Center. “Without research we don’t get the answers to the burning questions we’re asking about cancer. We continue to make advances in cancer treatment because of research and we need participants from all walks of life to consider a clinical trial.” Trial eligibility depends on factors like whether or not you’ve been treated for CLL before, or whether or not you are considered high risk. Screening tests to determine risk status and eligibility are created by a clinical trial sponsor, who outlines what defines a patient’s eligibility to participate in the clinical trial. However, determining risk status also helps determine treatment modality, or the way a treatment is administered according to a patient’s individual needs. Although the research nurses do not administer any of the treatment, they are still very much involved in patient care and well-being on an active basis. “We’re there every step of the way,” said Betsy Brou, research nurse at Sacred Heart Cancer Center. “From walking patients to meet with doctors to making sure they get to appointments, we’re there so each patient receives exceptional care.” Patients may remain on the clinical trial for as long as the treatment is working. Signs that the treatment is working includes measuring patient blood counts, as well as patient accounts of feeling better and visible reduction in lymphnode swelling within 15 days of starting treatment. Baptist Clinical Research conducts research in all areas of medicine and participates in drug, device and psychosocial clinical research projects. Research studies coordinated by Baptist Clinical Research come from a variety of sponsors in-
cluding the pharmaceutical industry, the federal government and academia. One such study is a clinical trial for lowering LDL cholesterol— better known as the “bad” cholesterol. The study is examining a treatment that can inhibit PCSK9 receptors in the liver from producing LDL, which would effectively lower LDL cholesterol. The treatment contains an antibody that attaches to LDL receptors and essentially deactivates them. “If you get a flu shot, it stimulates your body to make antibodies against influenza,” said Dr. Doty of Baptist Clinical Research. “The medication we’re testing is very advanced. It contains antibodies selectively made to intervene with LDL cholesterol receptors in the liver.” So far, the PCSK9 receptorblocking treatment has helped reduce LDL cholesterol production by 50 to 70 percent. The drug is injected every two weeks and it can be used in conjunction with statins like Lipitor and Crestor, which are drugs that can also lower your cholesterol by blocking HMG-CoA, an enzyme your body needs to make cholesterol. Volunteer eligibility depends on your level of LDL cholesterol. For example, one trial looks for participants with LDL over 100, while others look for those with LDL greater than 70. In addition to testing the efficacy of the new treatment to lower LDL cholesterol, the newer LDL cholesterol lowering trials at Baptist are geared toward looking at outcomes of whether or not a lower LDL will also reduce risk of cardiovascular events like stroke, heart attack, tachycardia and so on. Patient follow up for LDL cholesterol lowering trials on average is five years after the trial ends. However, Baptist’s clinical research stretches beyond LDL cholesterol. Other trials include those on finding new ways to treat Chronic Obstructive Pulmonary Disease (COPD), diabetic foot ulcers, hypertension, diabetic neuropathy, and using Dermacell to treat chronic wounds in the lower extremities of diabetic patients. In what could be one of the state’s most hotly contested congressional primary battles, state Sen. Greg Evers said Monday he is running for an open Northwest Florida seat being vacated by veteran U.S. Rep. Jeff Miller. Evers, a Baker Republican, enters an alreadycrowded GOP field, which includes state Rep. Matt Gaetz and James Zumwalt, an Iraq war veteran who also served as an aide to Miller. After toying with the possibility of running for weeks, Evers on Saturday filed a statement of candidacy for Congressional District 1 with the Federal Elections Commission, setting up what could be a grudge match with fellow state legislator Gaetz, who announced his candidacy for the seat almost a month ago. “After prayerful consideration, I’m humbled and honored to announce my candidacy for CD 1. FLORIDA’S JOBLESS RATE AT 4.9 PERCENT
THE NEWS SERVICE OF FLORIDA Florida’s unemployment rate stood at 4.9 percent in March, down from a revised February rate of 5 percent. The March jobless rate represented 484,000 Floridians out of work from a labor force that has grown to 9.8 million, the state Department of Economic Opportunity said Friday. The preliminary numbers indicate

As a lifelong resident of the Panhandle, I’m ready to carry our message to D.C., just as Jeff Miller did,” Evers told The News Service of Florida in a telephone interview Monday. Evers, a 60-year-old farmer, said he intends to formally announce his entrance into the race at a press conference Tuesday morning in Milton. Evers in 2001 won a special election for the state House, where he served until his election to the Senate in 2010. The Northwest Florida district --- one of the state’s most conservative --- abuts the Alabama border and spans Escambia, Okaloosa, Santa Rosa and Walton counties and includes most of Holmes County. The winner of the Aug. 30 GOP primary is almost certain to go to Washington, and a match-up between Gaetz and Evers could result in the type of political slugfest that the Panhandle
is known for. Gaetz has already raised more than $350,000, including putting more than $100,000 of his own money into the race. But Evers, known among reporters for a droll wit delivered in a thick Southern drawl, didn’t flinch when asked about Gaetz’s campaign war chest. “The last time I checked, it’s the voters that elect, not dollars,” he said. The district includes five military installations, which could give Zumwalt a leg up in a region heavily populated by retired and active-duty military voters. According to his website, Zumwalt served two tours in Iraq and was awarded a Bronze Star and later served as an analyst at the Pentagon before becoming an adviser to Miller Evers stressed his commitment to veterans on Monday. “I want to go to Washington and I want to fight for the veterans. So many have given so much, and some gave all. It’s something that I’m extremely proud
of, the veterans that I have represented over the years. I want to be their voice in Washington, as I was on the state level,” he said. Evers also said he wants to “fight for every citizen of the Panhandle,” especially small business owners like himself. “These are serious times and serious problems that we have, and it’s not a time for political gamesmanship. It’s a time for statesmanship, and a statesman to stand up and carry the light to Washington D.C,” he said. After representing the district for 15 years, Miller took the Florida political world by surprise in March when he announced he would not seek re-election in the fall. His retirement also set off a scramble for legislative seats, as Gaetz and Evers shifted from running state Senate campaigns to looking at runs for Congress. Matt Gaetz had planned to run for a Senate seat that his father, former Senate President Don Gaetz, R-Niceville, will leave this fall because

Two Republican state representatives --- Mike Hill of Pensacola Beach and Doug Broxson of Gulf Breeze --- are poised to run for Evers’s Senate District 1 seat, where, again, the August primary will almost certainly determine the general election winner. Broxson said he intends to file his campaign paperwork for the Senate seat shortly after Evers makes a formal announcement Tuesday morning. “We think we have a great chance of telling our story of why we should be (in the Senate) and we’re looking forward to the process,” Broxson, elected to the House in 2010, told the News Service in a telephone interview Monday. Hill, the victor of a special election in 2013, said he intends to announce his candidacy for Senate “very soon.” Unlike what could be a brutal primary battle for the congressional seat, both men said they anticipate the race to replace
Evers will be tamer. “I think we’re going to see very civil race. It’s going to be highly contested. But Rep. Broxson and I are friends,” Hill said in a telephone interview. Hill, meanwhile, said that Zumwalt and other candidates’ military experience could give them an advantage over the state legislators in the congressional match-up. Gaetz could have a different edge, according to Broxson. “The Gaetzes have a reputation of being able to raise massive amounts of dollars and I’m curious to see how Sen. Evers is going to match up the money campaign. I think he has pretty good polling information that says he should run. I’m going to be a spectator and see what they do and how they’re going to handle their campaigns,” he said. “Hopefully everyone would hope that it would be positive and they’ll concentrate on their legislative successes, but my guess is it could be a pretty bloody battle.”
7,000 fewer Floridians qualified as unemployed in March than in February. Gov. Rick Scott, in a prepared statement, focused on the private sector having created 36,000 jobs since the start of the year, the same number his office announced had been created by the private sector in January. “I am proud to announce today that Florida businesses have created more than 36,000 new jobs so far this year,” Scott said Friday. “Florida continues to be a leader in job creation, and our annual
private-sector job growth rate has exceeded the nation’s for four straight years.” The current figures place Florida just ahead of the national unemployment rate, which stands at 5 percent. Scott announced the monthly figures during an appearance Friday at Beneficial Blends, a manufacturer in Tampa. The state initially announced the February unemployment rate at 4.9 percent before revising it to 5 percent. The state has now revised up its initial monthly projection for five consecutive months.
CALDWELL, INGRAM DRAW DEMOCRATIC CHALLENGERS
Scott opened a campaign account last week to run against Caldwell in Lee County’s House District 79, according to the state Division of Elections website. Caldwell, chairman of the House State Affairs Committee, had raised $178,280 for his campaign account as of March 31 and had nearly $82,000 in cash on hand, a finance report shows. Meanwhile, in Northwest Florida, Pensacola Democrat Gloria
Robertson-Wiggins opened an account to run against Ingram in Escambia County’s House District 1. In medium nonstick skillet, brown ground beef and garlic over medium heat, breaking beef mixture into smaller chunks with spoon. Drain fat and stir in 1/2 cup salsa; set aside. Spread 1/3 cup rice in center of tortilla, leaving a 1/2-inch border. Scatter about 2 tablespoons of beans and 11/2 tablespoons corn over rice. Spread 1/3 cup beef mixture and 1/4 cup cheese over corn. Top with 2 teaspoons salsa and a few pieces of green onion. Fold in two opposite edges of tortilla 1 inch each and roll up. Place, seam side down, on microwave-safe dish. Repeat with remaining tortillas. Place burritos in microwave and heat 1 minute, or until heated through. Serve with remaining salsa.

Peach Jalapeno Jam 3 tablespoons peach preserves 1/2 fresh jalapeno, seeded and finely chopped (about 2 teaspoons) Grilled Cheese 1 1/2 tablespoons unsalted butter softened 4 slices 12-grain bread, divided 2 slices white cheddar cheese, divided 2 slices pepper jack cheese, divided 2 slices smoked Gouda cheese, divided 1/2 small avocado, thinly sliced, divided To make peach jalapeno jam: in small saucepan over medium heat, combine peach preserves and jalapeno. Stir mixture constantly until preserves are melted. Remove from heat and set aside. To make sandwiches; heat large nonstick or cast iron skillet over medium heat for about 2 minutes. Butter one side each of 2 slices of bread. Turn buttered slices over, buttered side down and spread peach jalapeno jam over second side of each bread slice. On top of jam side of each bread slice, layer 1 slice cheddar cheese, 1 slice pepper jack cheese, half the slices of avocado and 1 slice of Gouda. Spread jam on remaining slices of bread and place jam side on cheese. Spread remainder of butter on bread on top of sandwiches. Place sandwiches buttered side down in pan. Partially cover with lid, allowing steam to escape, and cook for 1 1/2-2 minutes, or until bread is toasted and browned. Flip sandwiches over with spatula. Partially cover with lid and cook for additional 1 1/2-2 minutes, watching carefully for bread to brown and cheese to melt. Remove lid and check for doneness. Remove sandwiches from heat and cut in half. Serve warm.
