The contract prior to commencement of service states that at the end of first month of your service being provided we will jointly review the care and support service, to ensure that you are satisfied that you have made the right decision.

We will consult with you and, if appropriate and with your permission, your family, and seek your views as to whether you wish to revise the service in any way.

Home from hospital service After a stay in hospital, the first thing you want to do is get home and settled.

With our Home from Hospital service, we can help you rest and recuperate in the comfort of your own home.

Whether you have been discharged from hospital or respite care, we provide the support you need to ensure you settle back in to life at home.

Our home from hospital service include:

Post-Operative Care

Your personal carers can help you with: getting in and out of bed, dressing, bathing, continence care, household tasks, meal preparation, feeding and medication reminders.

Transport

Your carer can meet you at the hospital and escort you home. If you prefer, we can organise transport for you on your return. It’s just one of the ways we can help make coming home less of a worry.

Support

Equipment

Should you require short or long term care our team will give you the support you need to remain living independently in your own home. Our service can then be reviewed if and when any changes occur.

Our carers can liaise with the hospital discharge team to ensure all equipment you require, i.e. commodes, hoists etc. have been installed in your home. We will ensure a full risk assessment has taken place to ensure it is completely safe for your return.

Personal Care

Your personal carers can help you with: getting in and out of bed, dressing, bathing, continence care, household tasks, meal preparation, feeding and medication reminders.

Palliative Care

When suffering from a life threatening illness our personal carers can support you through all possible stages of your condition.

Social Companionship

Your personal carers can provide companionship, a nice chat, they can read to you or if you prefer, just sit with you. They can also help you with your daily routine including household activities and trips out, including anything from medical appointments to the hairdressers. www.avantacare.com

Personal Care Service How we can help you:

Dressing

Your carer can help you dress and undress, sort your clothes into the wardrobe ready to wear the next day, help with zips and trousers or just tying your shoes laces. No job is too small for your carers.

For many reasons, sometimes it can be a struggle to carry out everyday activities.

Meals Service

Our discreet personal care service is designed to support you with day to day living, providing you with the support you need to remain living independently.

Our carers can support you in all aspects of meal times including everything from preparing meals or cooking a full meal to just heating something in the microwave. We can also help with feeding, managing the food in your home, cleaning the fridge, helping with your shopping lists and assisting with your weekly shop.

Our personal carers can provide a range of services including getting in and out of bed, dressing, assisting you to bath or shower and continence care. We can provide these services at a time that suits you and promise your dignity and comfort will always come first.

Bathing

Bed Routine

Your carer will support you in your bathing requirements, from helping you in and out of the bath or shower safely to helping wash and dry your hair and body.

If you can bathe independently, your carer can sit with you and offer support if you should need it.

Let us know when you like to get up in the morning and our carers can help you in and out of bed.

All our carers receive training in moving and handling techniques so if you need hoisting we can arrange carers to assist you, making sure you feel safe and secure.

Household tasks

Continence Care

If you need a little help with household duties, your carer can lend a hand.

They can help with the laundry, vacuuming or washing up, it’s all part of our personal care service.

Your carer can help you with personal hygiene, assisting you to and from the toilet, incontinence pads and sheets as well as help you with bathing and changing your clothes.

Medication Assistance/Prompt

Sometimes it’s difficult to remember to take you prescribed medication or to take it in the correct way.

There’s no need to worry as our carers can help you take your medication and ensure you take the correct dosage.
