What is different about an annual visit after a woman turns 40?

We have an enhanced emphasis on wellness and prevention, providing individualized counsel on diet as well as supplements to maintain bone density and manage menopausal symptoms. Of course, we start ordering mammograms at age 40 and bone density screenings at 50. We can take care of everything— blood work, sonograms, urodynamic testing for incontinence, mammograms and bone density screenings—in one visit, under one roof.

What preventive treatment do you recommend for a woman in her 40s?

Many women are deficient in vitamin D and that’s very important to stay ahead of in your 40s if you want to maintain bone density. As you experience menopause and its associated lower estrogen support, you can lose over 20 percent of your bone density. If you are deficient in vitamin D and calcium in your 40s, you increase the risk of developing osteoporosis in your 50s. Don’t wait until you are in a menopausal place to increase your intake of these essential vitamins.

Why does an OB/GYN track weight, BMI and dietary habits?

Because of hormonal changes in midlife, women can gain weight around the midsection that may contribute to chronic health problems like diabetes, 6 Guide to Good Health 2 0 1 6

heart disease and cancer. Weight gain from thyroid imbalances can happen suddenly and medications can help you get back to normal. We will also watch your testosterone levels. Adding a supplement can help you retain lean muscle mass.

When should a woman begin hormone testing?

I tend to begin testing in a woman’s early 40s so she has the tools to regulate symptoms before they become bothersome. A soy or herbal supplement can help relieve hot flashes and melatonin may relieve sleep disturbances, but if a woman decides to incorporate hormones, we talk about a patch, pills, cream or bio-identical hormones. Insurance benefits may cover hormone therapy, including bioidenticals.

How do I start a conversation with my OB/GYN about painful intercourse?

Actually, your OB/GYN should be the one to initiate a conversation about sexual health and function. I always ask before the exam so that I can look for physical causes. We may discuss options to treat painful intercourse. For decreased libido, common in a woman’s 40s and 50s, I am certified to prescribe the new female arousal medication Addyi, and we can also discuss androgen therapy.

Speaking of uncomfortable topics…what about incontinence or prolapse?

As many as 50 percent of women will

experience prolapse, the relaxation of the bladder, uterus or bowel, and nearly 45 million Americans have incontinence issues. It’s common, but not normal to leak urine, so talk to your OB/GYN. You can restore your quality of life with physical therapy, medications and, as a last resort, minimally invasive surgery.

What can I do about heavy and unpredictable periods in my 40s?

The median age for menopause is 51, and every woman is unique in the transition, but periods do tend to become problematic in a woman’s 40s. Focusing on gynecology helps me provide the most up-to-date treatments for bothersome midlife menstrual issues. I can offer permanent solutions such as in-office ablation and single incision hysterectomy utilizing the da Vinci robot.

Minimally invasive surgery results in less downtime and discomfort. I have personally performed over 400 robotic surgeries; in my role as a certified proctor training other surgeons, I have assisted on many more.

How often should an over-40 woman see her OB/GYN? If your paps are normal, you may space out testing for up to five years. However, there is still a need to come in every year for a pelvic exam. The idea that a pap smear is the sole reason for a well woman visit is a misconception, so don’t forget to schedule an annual exam.

Without a pap smear, it just gives us more time to talk about other issues.

How Can I Benefit From Chiropractic?

Physiological Therapeutics, chiropractic is one of the most cost-efficient forms of healthcare. Research found that people under chiropractic care spent 85 percent less on pharmaceuticals, had 62 percent fewer outpatient surgeries, 60.2 percent fewer hospital stays, and shortened their hospital stay by 59 percent when they did have to check in. These numbers are significant. It has been estimated that up to ninety-percent of world-class athletes take advantage of chiropractic care to prevent injuries and increase their performance potential. You may not be a world-class athlete, but you might be a world-class working professional, parent or spouse, and your body is every bit as valuable and important to the task. Take care of it!

How Quickly Should I Expect Results?

It depends on your goals and your commitment to achieve them. Every patient is different, but typically it depends on the amount of trauma a patient has been subjected to and how long it has gone untreated. Untreated trauma builds up, causing accelerated aging, and it takes time to reverse. Some patients realize dramatic, early results, but for most, the process takes time.

Exercise, core and muscle strengthening and yoga and massage therapy can support effective spine care, and patients who utilize these modalities usually see faster results.

How Should I Choose a Doctor of Chiropractic?

The choice is indeed a personal one, but here are some things to look for: First and foremost, look for a doctor who will listen to your goals, do a thorough history and exam and design a custom treatment plan for you. Bedside manner is important: does the doctor display genuine care and concern for you and your family? Results, however, are the key. There are many different chiropractic techniques, and your results could vary from one technique to another. If you don’t get the results you’re looking for, don’t give up on chiropractic; look for a doctor whose technique matches your goals.

When Should I See a Chiropractor?

It’s never too early to seek chiropractic care. I have a patient who started seeing a chiropractor when he was a teenager, and we found, at age 50, that he has Bertolotti’s Syndrome, a condition that typically causes pain and immobility in a person’s twenties. He credits chiropractic care for allowing him to live freely and without pain for thirty additional years, and he credits chiropractic care for keeping him mobile today. If you have pain, see a chiropractor. If you’re active and want to perform at the highest level, see a chiropractor. If you’re healthy and want to live a wellness lifestyle, see a chiropractor.

CommUnityCare practices street medicine Ask an Expert “Street Medicine is the first essential step in achieving higher levels of care.”

The Street Medicine program is led by Patricia Barrera, MA, LPC, who currently serves as CommUnityCare's Health Center practice administrator at the ARCH (Austin Resource Center for the Homeless) and DSRIP (Delivery System Reform Incentive Payment Program) Mobile Health Teams. The Street Medicine program is an important step toward implementing viable solutions for the homeless population of Austin. A well received addition to the programs in place for assisting Austin’s homeless, Street Medicine is laying a foundation for better health and wellness in our city by reaching out to those who are in desperate need of a helping hand.

What served as the inspiration for the creation of the Street Medicine Program?

How many times have you crossed over a roadway bridge, driven by an underpass or gone down a busy road in Austin? Probably more times than you can count. Many of us go along with our daily lives in a rush to get to our next appointment without a thought ever crossing our minds that there is a whole other world of people living under those bridges, underpasses and even tucked away in heavy brush throughout some of our busiest parts of town. This world is made up of individuals experiencing homelessness that have no other place to go. Many have exhausted their resources and networks, others may not have the mental or physical capacity to access a safe residence and many others just may feel this is their best option.

How is CommUnityCare addressing the need for health care in the Austin Homeless community?

In an effort to reach the most vulnerable and disenfranchised in our community with health care, CommUnityCare launched a Street Medicine Program. It is a component of the CommUnityCare

Mobile Health Teams. Street Medicine is the provision of medical care directly to those living and sleeping on the streets through mobile services such as walking teams, medical vans and outdoor clinics.

In our case, we are using walking teams.

For CommUnityCare, Street Medicine is the first essential step in achieving higher levels of care through assertive, coordinated and collaborative medical management. This is intended to reach out to those individuals who are experiencing homelessness who would otherwise forgo health care. The main goal for the Street Medicine team is to reach out to these individuals and provide needed health services.

Who works in the Street Medicine program and what services do they provide?

The Street Medicine team is comprised of a medical provider, two nursing staff members and a case manager. These team members have significant experience in working with this population either by having worked or currently working at our clinic at the ARCH. Additionally, we are partnering with the PATH (Projects for Assistance in Transition from Homelessness) team from ATCIC (Austin Travis County Integral Care), behavioral and mental health specialists, and the Community Health

Paramedics with the City of Austin EMS. Working collaboratively we are able to not only identify the encampments and areas where the individuals are living, but we’re also able to address the whole person’s emergency medical and mental health needs at once.

Has the Homeless Community been receptive of the Street Medicine Program?

Drawing from the experience of each of the organizations represented is critical to serving the needs of this population that is often faced with a lack of resources to care for multiple chronic conditions. To date our street medicine team has been working for about month.

In this month, we have accrued a wealth of knowledge on how best to serve these patients and provide them with access to health care they otherwise wouldn’t receive. The Street Medicine team has been warmly received and to quote one of our new patients who expressed his sincere gratitude. “Thank you for taking care of us, I didn’t think anyone cared”.

How can I prepare my body to boost preconception health?

We live in a health-conscious community, so it’s a way of life that will pay off when it’s time to get pregnant.

Now more than ever, embrace wellness by eating a balanced diet, drinking plenty of water and adding a prenatal vitamin each day to get the recommended dose of 400-800 micrograms of folic acid. Not all pregnancies are planned, but if you are able to talk with us three months prior to conception, your OB-GYN will provide a preconception checklist to help you follow a clear path to a healthy pregnancy. This includes assessing your vaccinations to ensure that you are up to date before getting pregnant.

Are there habits that I absolutely have to break?

Yes. Your OB-GYN will provide counsel and resources to help you identify and quit any practice that will harm your baby such as having unprotected sex, smoking cigarettes, drinking excessive amounts of alcohol and taking some forms of prescription drugs and any illegal drug. Ask your physician about any areas of concern.

How do I know that I am ovulating?

Your body is sending you clues each month that it is primed for pregnancy.

Pay attention to your menstrual cycles now and track your ovulation with an over-the-counter kit. Having regular periods is a good sign that you are releasing an egg each month, so see your OB-GYN if you experience irregular, painful or heavy periods. Also, calculate your body mass index (BMI), as fat cells produce estrogen that can cause problems with ovulation in women who are underweight or overweight.

It’s taking me longer than expected to get pregnant.

Should I worry?

Are you over age 35? In addition to ovulation problems, maternal age is one of the leading causes of infertility. As a general rule, we ask women under 35 to consult with a physician after 12 months of trying to get pregnant. Women over 35 should raise the red flag after six months. Ideally, you have already set a preconception health appointment so we will develop a timeline together.

What is on my checklist when deciding on an obstetrician?

A good rapport is essential, of course, but we also advise women to consider experience and expertise as must-haves for an OB-GYN. The majority of pregnancies proceed along as planned— from first sonogram through labor and delivery. If you do have a complication arise, however, you will benefit from partnering with a physician experienced in high-risk OB care. The RWG team has

privileges at St. David’s Women’s Center of Texas at North Austin Medical Center, with immediate access to Level III Neonatal Intensive Care if needed.

What’s more, all of our nurses and sonographers are trained in providing the very latest in prenatal diagnostic services.

I’ve heard about genetic screening. Who needs this test?

Advances in DNA mapping now make it easier and more affordable than ever to learn essential facts—prior to conception—that can safeguard the health of your baby. Many people are symptomless carriers of inheritable genetic disease. When two people who are carriers have a baby, there may be as much as a 1 in 4 chance of passing along the active form of a disease like cystic fibrosis. Some ethnicities are at greater risk, so ask your OB-GYN about the benefits of preconception carrier screening.

Which is better for my baby: natural childbirth or an epidural?

What does it mean to receive emergency medical care by a Board Certified Physician?

When you need immediate medical care that does not require the use of 911, the first line of advice is to find emergency medical care. These options range from traditional hospital emergency rooms, freestanding emergency rooms, and urgent care centers. However, all three of these options range in their availability, wait times, and locations.

Urgent care facilities are limited in the type of care that they can provide. If you need immediate medical attention, but not on a level that prompts an ER visit, you can visit urgent care. However, unlike an ER, you will not be able to visit urgent care centers around the clock. Most urgent care centers are open anywhere from 8 hours a day to 16 hours a day. If you do visit an urgent care center, and your medical situation is deemed an emergency, you will be referred to a local ER.

For any doctor to practice medicine in the United States, they must be licensed by the state that they work in. However, just because a doctor is licensed does not mean that they are qualified to practice a medical speciality. Examples of specialties are surgery, family medicine or dermatology. Board certified doctors voluntarily meet extra standards that go beyond basic licensing. This demonstrates their expertise and requires additional education including the step of passing an exam that tests medical knowledge and clinical judgment. In addition, to remain board certified physicians must actively stay updated with the latest advances and demonstrate best practices.

What is the difference between a hospital ER and a freestanding emergency room?

Hospitals and freestanding emergency rooms will provide you the same level of care in emergency. Freestanding ERs, like hospitals, operate 24/7 with the requirement of being open 24 hours with ER physicians on site. Freestanding emergency centers also must have the proper emergency equipment found in hospital ERs. One of the main differences you will find between hospital ERs and freestanding emergency rooms is the location. Traditional ERs are located on hospital grounds, while freestanding ERs operate off-site. They also normally operate independently of hospitals.

Because freestanding emergency rooms are not limited to being in hospital locations, they are typically closer to patients, and provide access that is much more convenient. In addition, wait times are normally much shorter with nursing staff accessing patients quickly.

Dr. Devin Garza was one of the world’s first surgeons to use a single-site robotic approach for benign gynecology, performing over 1,000 procedures and given the distinction of da Vinci Mentor/Epicenter surgeon by Intuitive Surgical, Inc. He champions the use of minimally invasive surgery (MIS), training surgeons, and lecturing around the world on robotic techniques.

Last month, Dr. Garza, a board certified obgyn, co-chaired a post-graduate course in robotics at the American Association of Laparoscopic Gynecologists (AAGL) 2015 Global Congress, and addressed the pressing issues and exciting breakthroughs on the horizon for today’s gynecologic surgeons and the women they serve.

What is the #1 challenge that minimally invasive gynecologic surgeons face today?

A surgical technique called mechanical morcellation* has disappeared from the surgeon’s toolbox, and the conversation is now focused on alternatives. A morcellator would break up tissue into small pieces for removal through small incisions. Problems arose when the devise dispersed existing unknown cancer cells in the specimen throughout the body. Rather than resorting to open surgery, I teach surgeons how to safely remove tissue using a preferred minimally invasive approach, containing tissue within a pouch, and removing often very large specimens through a one-inch incision. This gives surgeons a tool with which to maintain MIS management of a large uterus for hysterectomy and myomectomy. *Renaissance Women’s Group does NOT use morcellators for gynecologic surgery.

Who is an ideal candidate for minimally invasive surgery?

People of all ages, stages and conditions benefit from MIS, but your physician will determine whether you are a high-risk or a low-risk patient for any surgery, and should discuss all viable alternatives.

Increased risks include: medical conditions, previous abdominal surgeries, advanced endometriosis and large tumors.

The parameters for who is and who is not a candidate for MIS can also depend on the experience of your surgeon.

Are all surgeons trained in the minimally invasive approach?

I am considering surgery. Are there questions that I should ask my physician?

Most are, but some surgeons are more experienced than others. You would expect that the pilot of your airplane has logged many hours in the air and in flight simulation and will continue to sharpen his or her skills. It’s the same with surgeons.

The type of hands-on training that I teach across the U.S. and other countries as an Epicenter/Mentor Surgeon prepares experienced physicians to perform the latest robotic techniques and safe tissue removal—often the very next day. This hopefully gives a patient who would have been offered open surgery as the only option the opportunity to have small incisions and a greatly reduced recovery time. Like the illustration of a rising tide that lifts all boats, when we further the knowledge of MIS, patients benefit.

You will, of course, have discussed all of the options to surgery, and the pros and cons of an MIS approach. If your physician does not recommend an MIS approach, due to the size of your uterus, especially with the morcellator unavailable, then I would ask: 1. Why is this not going to be a minimally invasive surgery? At Renaissance Women’s Group, our MIS rate approaches 99 percent.

2. Is there a surgeon you trust in the local medical community that can perform this particular surgery using MIS? Collaboration among surgeons is key to maintaining a high MIS rate for our patients.

3. Is there a scenario that always warrants an open surgical approach? The answer should be: Rarely. Even with cancer present, surgeons perform MIS 80 percent of the time.

How do you teach physicians to perform surgery?

At my advanced and masters level courses, we take a systematic approach— with reproducible steps—so that surgeons learn how to manage complex cases.

Sometimes I narrate as I perform the procedure live or via teleconference, or most often, refer to video from prior surgery. At the 2015 AAGL Global Congress course, for example, we used cadavers, robotic simulators and taught tissue extraction using plastic models, extracting beef tongue through a one-inch opening.

What’s on the horizon in MIS? There is international buzz around the new da Vinci Xi robotic platform, and we were among the first to use it. The new platform will further improve surgical outcomes in gynecologic surgeries, as well as other specialties. Technological advancements, including improved vision, increased range of motion and access to complex pathology, all help maintain MIS.

Most women experience discomfort when Aunt Flo comes to visit. However, there are varying degrees of pain associated with menstruating, as it affects women differently depending on the symptoms and causes. Heavy menstruation and severe cramps not alleviated by healthy lifestyle and use of over-the-counter pain relievers should be evaluated further by a specialist. When your body doesn’t respond to these tactics, it could potentially be more serious and even require surgery. Endometriosis, adenomyosis, uterine fibroid tumors, ovarian cysts and prolapse are some of the common diagnoses for women suffering from pelvic pain and/or abnormal bleeding.

WHAT ARE THE SIGNS OF AN ABNORMAL PERIOD? One sign is pain that doesn’t respond to Motrin, or [the patient] has to dose frequently and take several Motrin to be able to get relief and go about their normal activity. As far as unusually heavy bleeding (i.e. Changing a Kotex every hour or two.) that would be considered not normal. Staining bed clothes at night or having to get up a couple times in the night to change a Kotex would also be considered not normal. If your pain does not respond to various therapies (healthy diet, exercise, pain relievers, birth control), if you are bleeding between periods, experiencing a heavy flow or if your menstrual cycle is interfering with your quality of life, preventing you from going to work or leaving the house, then it is time to seek help from a women’s health professional.

WHY DO HEAVY MENSTRUATION AND CRAMPS OCCUR? Painful menstrual cycles can occur as young as the early teenage years, but tend to peak in a woman’s 20s and 30s.

The painful symptoms typically go away with pregnancy and then oftentimes return for women in their 40s.

symptoms from endometriosis and pelvic pain for several years.

Endometriosis is a condition in which endometrial type cells have implanted outside the uterus, causing severe cramps, abnormal bleeding and other pain, including pain with intercourse.

Endometriosis can affect at least 25 percent of women. It can be genetic or even associated with congenital uterine anomalies. There’s a condition related to endometriosis called adenomyosis and that’s a common reason for severe menstrual cramps as well as heavy bleeding. It’s a condition where the endometrial lining has implanted in the muscle wall of the uterus.

Pain can also be caused by uterine fibroids and can cause bleeding problems if the fibroid is affecting the lining of the uterus. Fibroids are benign, non-cancerous, fibrous-like tumors in the myometrium (muscle layer of the uterus). Pelvic pain should be further investigated to assure that it is not related to a gastrointestinal, urologic or musculoskeletal disorder. Heavy bleeding can even start with a young woman’s first few menses. Up to 25 percent of these adolescents may have a hereditary bleeding disorder, which can be determined by simple blood work.

Later on, heavy bleeding, bleeding between cycles or with intercourse can be secondary to overgrowth of the

endometrium, uterine polyps, fibroids or adenomyosis.

WHAT ARE THE TREATMENT OPTIONS? The first step would be to see how the body responds to pain relievers and possibly hormonal contraceptives. If the body is not responding, the next step is evaluation and possible ultrasound in order to determine the etiology.

The treatment depends on what we find and whether that person wants to have children or has completed their family. If the findings are suggestive of endometriosis based on history and physical exam, then we typically proceed with a laparoscopy, which, in my experience, I prefer using da Vinci surgery. The da Vinci robotic system gives me 3D visualization, better magnification and use of instruments that have more precise movements.

Laparoscopy allows me to see into the abdomen through small incisions in order to diagnose what is causing the pain or bleeding. I am now able to perform more complicated, minimally invasive surgeries rather than making large abdominal incisions, including single-site umbilical laparoscopic surgery. With da Vinci, patients experience less pain, less risk of infection and scarring, and shorter recovery time, among many other benefits.

Many people suffer from chronic discomfort in their teeth. Some people can never drink ice water because the cold liquid causes the teeth to ache.

While sensitivity is a common problem, there are solutions to reduce and eliminate discomfort. Several different factors can cause sensitivity, including misalignment of the teeth that leads to a destructive bite, gum recession, nighttime grinding, cavities, or even using the wrong dental products at home. A simple change of toothpaste could help. Or you may need to discuss realigning the teeth for a better functioning bite to reduce pressure on specific teeth. It is always a good idea to ensure any active decay is removed to reduce sensitivity caused by cavities. Nighttime clenching and grinding could be the culprit, in which

case a custom-made splint can relieve sore jaw muscles and reduce pain in the teeth. We can now provide night guards that are streamlined and far less bulky than the traditional style.

There is rarely just one cause for tooth sensitivity, but it is treatable and at your next dental checkup, you should discuss it with your dentist.

If you know you are trying to get pregnant, discuss your dental health with your dentist and make a plan to be as healthy as possible before pregnancy. And if you are already pregnant, consider seeing your dental hygienist two to three times during the pregnancy to help treat or even prevent infection in the gums.

Good dental health is always important, but especially during pregnancy. To avoid prenatal issues caused by poor dental health, be sure your mouth is healthy before you are pregnant by having any decay and cavities removed, rebuild any teeth that show signs of fractures or cracks, and be sure your gums are healthy and free of infection. Pregnancy Gingivitis is a common occurrence, even for patients who normally have no dental problems. This condition occurs when your body is focused on the health of the fetus and infection begins to set in your gums. Prenatal issues can be linked to the mother’s poor dental health and can be as serious as low birth weight and even premature birth.

No one wants to be in pain, and dental pain has been described by some as being worse than child birth! The best way to prevent dental pain and emergency situations is to consult with your dentist to replace any old and failing fillings before the tooth is symptomatic. Many people seek dental treatment only when they have pain.

But did you know, if you remove any infection and decay before you feel it, the treatments will cost much less and can be done pain-free. By ensuring all of your teeth are healthy today, you can avoid costly and painful emergencies and thus keep your teeth longer.

What is 3D mammography?

This technologically advanced scan gives radiologists the ability to view inside the breast layer by layer. During this simple exam, multiple images, or “slices,” are taken of different angles of the breast, exposing fine details that might be obscured by overlapping tissue. Breast imaging radiologists can now view a mammogram in a brand new way.

What is it like getting a 3D mammogram?

Exactly like getting a standard mammogram except the scan takes a few seconds longer as the machine arm passes over the breast. There is no additional breast compression and the scanner is able to produce both a standard and 3D mammogram simultaneously.

What are the benefits of 3D mammography?

Who should have 3D mammography?

• 41 percent more cancer detected. By revealing more of the breast tissue, 3D mammography improves early detection of cancer. Studies have shown an up to 41 percent increase in the number of invasive breast cancers detected.

• Fewer callbacks. Because radiologists can have a closer look at features in the breast, they are better able to distinguish between benign and cancerous abnormalities. That means fewer women get called back for additional imaging, cutting down on anxiety and expense. With 3D mammography, the need for additional imaging has been reduced significantly.

Studies have shown that all patients can benefit from this revolutionary technology regardless of breast density or family history. Early detection is the best defense against breast cancer and 3D mammography has been shown to be more effective at detecting cancer than conventional mammography.

Women should begin having yearly screening mammograms at age 40.

Is it safe?

Technological advances have reduced the radiation dose needed for mammography and the benefit of early diagnosis and treatment of breast cancer far outweighs the risk. Our mammography facilities are accredited by the American College of Radiology.

Is it covered by insurance?

3D mammography (also known as breast tomosynthesis) is covered by Medicare and by a number of other insurance companies, although not all.

What can women do to reduce their risk of heart disease?

Medical conditions strongly correlated with heart disease include high blood pressure, high cholesterol, diabetes and being overweight. As such, it is important to know the following numbers:

• Blood pressure should typically be less than 120 systolic and less than 80 diastolic for an adult age 20 or over.

Healthy numbers mean a healthy heart.

Women can significantly reduce their risk of heart disease by taking the following steps:

• Blood pressure – the force of blood against the arteries when the heart beats and rests

• A BMI range of 18.5 to 24.9 and a waistline smaller than 35 inches is considered healthy.

• Cholesterol – a fat-like substance that is found in all cells of the body

• A normal blood sugar level is less than 100 mg/dL after fasting, and less than 140 mg/dL two hours after eating.

When measuring these key indicators, be aware of where you stand—and also pay attention to where you are going. If your numbers are high, but trending down with each check, that’s a sign you’re on the right path to a healthier heart.

• Glucose levels – sugar stored in the blood as the body's main source of energy • Body mass index (BMI) – the measurement of body fat based on weight and height Most of these numbers are tested at annual checkups, so it’s important to “know your numbers” and understand what they mean. By keeping these numbers within the recommended healthy range, individuals can improve their heart health and reduce the risk of heart disease.

Also, it is just as important that people know their family history and risk of hereditary diseases. Patients should share this information with their healthcare providers to evaluate risks and determine steps toward prevention.

• A total cholesterol score of less than 180 mg/dL is considered optimal.

At what age should women “know their numbers”?

The earlier the better! Heart disease can occur at any age, and it can take years before any symptoms appear. According to the American Heart Association, prevention of a heart attack begins at age 20. Visit your doctor for regular checkups and become aware of your heart health numbers. The frequency of follow-up visits will depend on each person’s level of risk.

1. Eat a healthy, well-balanced diet with plenty of fruits and vegetables, whole grains, lean proteins and low-fat dairy products.

2. Exercise regularly. Try to walk, run, bike or dance at least 30 minutes, five days a week.

3. Quit smoking and limit alcohol.

4. Manage stress by getting enough sleep, taking time for yourself and using meditation or calming techniques.

5. Visit a physician regularly for annual health checks and determine high-risk medical conditions.

Disease prevention is a numbers game that can be controlled. Take steps to be aware and accountable. “Knowing your numbers”—and how they compare to healthy, normal levels—is a powerful way to take charge of your health.

They need not be literary masterpieces, just whatever comes to your head: “I need to grocery shop today,” or “Sarah was really weird at work yesterday.” Whatever is up there, put it on paper. It’s a way to clear the clutter and focus on the more important things in life.

How can I tell if I need a skin check with a dermatology provider?

Many things increase your risk of getting a skin cancer. The more you have the more important it becomes to get a skin check with a dermatology provider. Let's review a few: Age: Overall, 1 in 5 or 20 percent of Americans will get at least one skin cancer at some point in their lives. If you are over the age of 65 your risk of skin cancer increases to 1 in 2 (50 percent).

That's a huge number of people.

However, you should not wait until you are 65 years of age to get your first skin check if you have multiple other risk factors for skin cancer.

Gender: Men are more likely than women to get skin cancer, but both men and women are susceptible.

Family: If you have Northern European ancestors you have a higher risk of skin cancer. This is because the skin genes those ancestors passed on to you were built for climates that are much less sunny than our Texas sky. However, skin cancer rates for all skin types/ethnicities have been increasing. If you or anyone in your family has had skin cancer that also puts you at a higher risk.

Eyes: Having blue or green eyes is a risk factor.

Spots: Having multiple freckles and/or multiple moles are risk factors.

Sunburns: If you sunburn easily, that's a risk factor. In fact, if you had a blistering sunburn during your childhood that doubles your risk of skin cancer.

Indoor Tanning: Everyone wants to look good and feel good. I can't tell you how many people tell me, "Tanned fat looks better than pale fat." Indoor tanning is the problem with this thinking because most of the UVA ultraviolet rays that tanning facilities use actually penetrate much deeper in your skin and do more damage than regular sunshine, which is mostly UVB. This is why people who are in the habit of indoor tanning have a higher risk of skin cancer.

and/or squamous skin cancers, every 3 months. Most patients who have either had a skin cancer in the past or who have multiple risk factors for skin cancer are seen every 6 months. Those with very few risk factors and no history of skin cancers are typically seen once a year. Of course these are just guidelines.

If a patient sees a new mole, a changing mole or a skin lesion that won't heal or is bleeding, we recommend they schedule an appointment as soon as possible.

How often should I get my skin checked by a dermatology provider?

Tru-Skin Dermatology is excited to be the first in central Texas to offer the newest innovations in skin cancer treatment, specifically SRT-100 and EBT (electronic brachytherapy). These non-invasive new technologies are revolutionary, because for the first time we can offer patients a pain-free option for treating skin cancers and keloid scars. Cataract surgery is an inevitable part of the aging process given our increase in life expectancy as well as the desire for an active lifestyle necessitating good vision. Cataracts affect more than half of Americans over age 65. Gone are the days when cataract surgery was an inpatient hospital ordeal. Today, this procedure has become streamlined and incredibly sophisticated. The latest of these technological advances is laser assisted cataract surgery.

WHAT IS A CATARACT? A cataract is clouding or yellowing of our own natural lens inside our eye. This occurs slowly as we age as a result of protein build-up in the lens. Cataracts prevent the passing of light to the inside of the eye and cause glare and loss of vision as a result. Common symptoms include difficulty with night vision and driving, glare from headlights and difficulty reading. Fortunately, this is a totally curable condition with outpatient surgery.

WHAT DOES THE SURGERY ENTAIL? Cataract surgery is an outpatient procedure that can be as short as 15 to 20 minutes long. The entire lens is removed through micro-incisions and replaced with an artificial lens implant made of acrylic that usually does not have to be replaced. Surgery is performed under IV sedation, one eye at a time, and once a cataract is removed, it never grows back.

WHAT IS LASER CATARACT SURGERY? Until recently, cataract surgery was always performed with a blade. Now we have the ability to use femtosecond lasers, similar to Lasik, to assist in cataract surgery. The laser measures the patient's eye shape prior to the procedure and customizes the incisions as a result. The incisions are then constructed with the help of the laser computer and without a blade. Usually, the entire laser treatment is finished under 30 seconds! In that time, the physician is able to create up to six different incisions to aid with the surgery.

WHY CHOOSE LASER? The laser procedure has many benefits, making it a great tool to use during surgery. Each treatment consists of an ultrasound, which helps customize incisions specifically to each patient. The incisions are then constructed in a very predictable fashion with great accuracy and seal incredibly well as a result. The

laser is also very effective in treating astigmatism. While blade surgery cannot reliably remove astigmatism because it is very difficult to place corrective incisions at the right depth, the laser is able to measure the patient's cornea accurately and place the incisions at the perfect depth. Astigmatism is managed quite effectively as a result. Finally, the laser can be safer in certain eye conditions as opposed to the blade.

WHICH LASER IS THE BEST? Westlake Eye has had great results with the LenSx laser. This particular laser creates excellent corneal incisions and has led the way in FDA approval and software upgrades. In addition, the support staff for this laser is outstanding and ensures that it is in perfect condition. Performing well over 500 laser surgeries to date, Westlake Eye strives to deliver cutting edge technology to patients and the laser has been a great addition to its practice, with patients reporting they are pleased with the results.
