Did you know that 40 million men and 20 million women suffer from hair loss? A full head of hair is one of a man or woman’s most important physical features. But what happens when we begin losing our hair through thinning or balding? Anyone who has had the misfortune of experiencing this will tell you about the consequences that they suffer in terms of self-esteem.

Austin Hair Graft by Hall Plastic Surgery is the first to bring revolutionary hair enhancement programs to the area. We offer a no scalpel, no staple, no stitches, and no scars hair transplantation (this is not the old fashioned hair transplant), low level laser technology, and at-home care therapies to provide options for every patient with every type of hair loss! Our highly trained medical staff recognizes the psychological pain caused by hair loss and has turned their talents to helping men and women regain their selfesteem by utilizing our hair enhancement program. Jeffery Hall, M.D., board certified plastic surgeon, and former chief of plastic surgery for Seton Medical Group says, “I am committed to helping restore the appearance of a fuller, thicker, healthier head of hair to patients. There is nothing more rewarding than to see a patient’s face at the end of their program. We see an entire transformation through the process of their hair restoration.” For many men, hair loss is inevitable. It is a hereditary condition that increases with age. For women, hereditary factors can be involved, but hormonal changes, stress, illness, and poor nutrition can all play a role. Hair transplantation and low-level laser therapy can help reverse and prevent continued hair loss from these factors.

With the newest technology now available at Austin Hair Graft, hair transplantation and lowlevel lasers are discrete, completely natural, and permanent solutions. Dr. Hall says, “The majority of hair loss is called androgenetic alopecia. Testosterone is converted to dihydrotestosterone (DHT) which damages hair follicles. Those follicles will eventually lose their ability to grow.” Hair roots receive nutrients from vessels containing blood cells and the hormone testosterone. (Both men and women have this hormone). An enzyme present in the body called 5 alpha reductase can convert testosterone to another hormone called DHT. When DHT is present 6   Guide to Good Health 2 0 1 2

in the scalp tissue, men and women genetically predisposed to hair loss may experience reduced growth phase in their hair leading to extreme thinning and baldness. Estimates show that about 40% of men between the ages of 25 and 35 will experience hair loss, and 40% of women will have noticeable hair loss by age 40. At Austin Hair Graft our options are very simple. Our hair transplantation procedure using Follicular Unit Extraction (FUE) is the most effective transplantation technique with little to NO DOWNTIME, NO SCALPEL, NO STITCHES, NO STAPLES, AND NO SCARS or extensive surgery required. For those seeking an alternative to transplantation, or for those who are not candidates for transplant, our low level laser therapy is an excellent choice. The complete program takes 12 months with 48 or more treatments throughout the year. For convenience, mobile devices are also available. Austin Hair Graft also offers a line of home hair care products including shampoos, supplements, and prescription therapy. “Our program is comprehensive and works for most people. We see women respond just as quickly as men.” says Dr. Hall. Austin Hair Graft offers a complimentary scalp evaluation and consultation. During the consultation patients receive valuable information on their personal hair status and recommendations about what option(s) will be the most effective course of action. “We believe in a customized approach to hair restoration. We will individualize your program to give you the best results possible.”

COSMETIC DENTISTRY By Ashley Erikson

The staff at Austin Cosmetic Dentistry brings a total of over 40 years of combined experience in the industry and has worked on thousands of cosmetic cases. If you have ever wondered if you are a candidate for cosmetic dentistry, Dr. John Schiro shares how the procedure works and what you can expect as a patient. WHAT IS COSMETIC DENTISTRY? Primarily, it is the use of all-porcelain veneers and crowns to create a stunning, straight, white smile.

HOW DO I KNOW IF I AM A GOOD CANDIDATE? If your teeth are crooked, have gaps or spaces, or are chipped or stained, you can benefit from this approach. We can even help people who have missing teeth.

HOW DOES THE PROCEDURE WORK? We build a beautiful new smile, tooth by tooth, using all-porcelain veneers and usually involve all the teeth that show when you smile, or normally the front ten teeth on top and/or bottom. The veneers are as thin as a fingernail and look just like natural teeth. When bonded to the supporting tooth they become super-strong. The veneers and crowns we make are almost indistinguishable from natural teeth, and there’s no need to wait for up to a year as with braces. Most cases are completed in just a few office visits.

WILL I EXPERIENCE DISCOMFORT OR PAIN DURING THE PROCEDURE? The vast majority of our patients are completely comfortable with only the use of Nitrous Oxide (laughing gas). For those guests who are extremely anxious about having dental work, we offer options such as oral sedation. With this option you are almost asleep during the procedure, and most patients don’t recall having their work done.

“Stacey was given treatment plans by two separate dentists that included jaw surgery and orthodontics and was going to take years to complete. We completed her case in under three weeks and only a couple of office visits!” - Dr. John Christian Schiro

ONCE COMPLETED, HOW LONG DO ALL-PORCELAIN VENEERS LAST? With proper care, there is no limit to how long your smile can last. Advantages to porcelain veneers are the protective layer they provide over the front of the natural tooth and the fact that they never stain.

WHAT IF MY NATURAL TEETH ARE IN TOO POOR OF SHAPE FOR THIS PROCEDURE? Actually, we see this in about twenty-five percent of our guests. For those who have been neglectful of their teeth and don’t brush or floss each day or maybe have not seen a dentist in their adult lives, we first have to

“With proper care, there is no limit to how long your new smile can last.” 8   Guide to Good Health 2 0 1 2

restore the teeth to health in order to have a strong foundation to build a new smile. In cases like these, it’s not unusual for us to perform a full-mouth restoration.

WHAT DOES A FULL-MOUTH RESTORATION INVOLVE? This is the term dentist’s use when they restore every single tooth in a patient’s mouth. Some very technical issues come into play in these cases that require getting the person’s bite to settle in just the right position. To achieve that we must have the dimensions of the back teeth precisely right. It can lead to relief from jaw pain, neck pain, and chronic headaches that were caused by their deteriorated dental condition.

John Christian Schiro, DDS, FAGD Austin Cosmetic Dentisty 512-382-5888 www.AustinCosmeticDentistry.com Dr. John C. Schiro has over 20 years experience and has completed thousands of cosmetic cases.

Varicose Vein Treatment By Ashley Erikson

Varicose veins are known for being a cosmetic nuisance. But what many people don’t know is that left untreated, they cause health problems. Dr. Chris Brennig, board certified vascular surgeon and founder of the Austin Vein Institute, shares what those risks are and how he treats varicose veins so that future health problems can be avoided while restoring the appearance of his patients’ legs. WHEN SHOULD A PATIENT COME IN TO SEE YOU? I see and treat a variety of patients from those with spider veins that are cosmetically displeasing, to complicated varicose veins, and everything in between. If someone has a question about the way their legs look; if they notice spider veins or bulging veins that are associated with symptoms of discomfort or heaviness, it’s probably an indication that they should be evaluated. The earlier a vein problem is treated, the simpler the treatment. This is because complications from varicose veins worsen with time.

WHAT ARE SOME OF THE HEALTH RISKS OF UNTREATED VARICOSE VEINS? The damage worsens with time but changes are often slow to appear. Health risks include swelling, pain and skin changes in the lower legs, as well as permanent skin discoloration and sores - known as venous or stasis ulcers. Serious problems are insidious and treatment before they develop is important for the long term health of ones legs.

WILL INSURANCE COVER SOME OR ALL OF A PATIENT’S TREATMENT? Treatments for clinically significant vein problems will often be covered by health insurance plans, so we always try to work with our patients and their insurance provider to obtain the appropriate treatment coverage.

WHAT CAN A PATIENT EXPECT WHEN GOING IN FOR AN EVALUATION AT THE AUSTIN VEIN INSTITUTE? All patients receive a comprehensive initial evaluation from me. As a board certified vascular surgeon with many years of experience successfully treating all types of vein problems, patients can be assured they are receiving an exceptionally high level of care. During the initial evaluation I take a thorough health history, perform an examination of the affected veins, and then perform a thorough diagnostic venous ultrasound. The ultrasound allows me to see what is going on below the surface of the skin and determine how well each of the veins is functioning. The treatment plan is then developed and discussed in person with the patient.

CAN SOMEONE SEE YOU IF THEY HAVE HAD PRIOR VEIN TREATMENTS BUT ARE NOTICING A RECURRENCE? We have been very successful at the Austin Vein Institute in treating patients with recurrent varicose veins. For those who have developed recurrence, we perform what is known as ‘rescue therapy.’ We perform our

“The earlier a vein problem is treated, the simpler the treatment.” 1 6   Guide to Good Health 2 0 1 2

comprehensive evaluation to determine what factors are present that have caused the recurrence. Then we put together our treatment plan to correct the problem in order to obtain the most durable results.

WHAT CAN A PATIENT EXPECT FROM THE PROCEDURE? I perform all spider vein and varicose vein treatments myself. These include sclerotherapy, microphlebectomy and a procedure called endovenous laser therapy (EVLT). EVLT lets me treat many failing veins in the quickest and most effective way possible. EVLT uses laser light to essentially cause the faulty vein to close down and later dissolve as part of the body’s natural healing process. We use one of the gentlest yet most effective lasers available. The procedure is done in our private and comfortable medical office and takes less than an hour. Patients can walk out after the procedure and resume most of their regular activities right away. For patients with more advanced or complicated varicose veins, subsequent treatments may be necessary to complete the therapy and prevent recurrence.

CENTRAL HEALTH’S SERVICES By Ashley Erikson

For those in Travis County who need health services and are uninsured or need somewhere to turn, Central Health offers resources to help you get access to the healthcare you need. Central Health’s Board of Managers Chairperson Rosie Mendoza gives insight on how Central Health accomplishes this and shares its efforts to expand service. WHAT SERVICES DOES CENTRAL HEALTH PROVIDE? Central Health’s mission is to create access to healthcare for those who need it most, such as the uninsured or underserved. One of the things that make us unique among Texas hospital districts is that we contract with various healthcare providers rather than directly providing those services. This is done through many different avenues such as a call center, partnering with Seton Healthcare Family to operate Central Texas’ only Level 1 trauma center at University Medical Center Brackenridge, and by expanding infrastructure in the form of community clinics. Central Health also partners with CommUnityCare, a network of community-based health centers, and Sendero Health Plans, a community-based Health Maintenance Organization (HMO) that serves Medicaid and CHIP enrollees.

CAN YOU TELL ME MORE ABOUT THE CALL CENTER? HOW IS THAT OPERATED? The call center has been in operation since October 2009. We have a partnership with United Way for Greater Austin to provide the service, which includes screening for Medical Access Program, Medicaid and CHIP eligibility, scheduling of eligibility appointments, referrals to providers, and providing information to MAP enrollees regarding their benefits. It’s been a huge success, averaging about 91,000 calls each month. Through this partnership with United Way, a seamless system has been established to send or receive calls from the 211 system, which they also run for this region.

HOW DOES CENTRAL HEALTH IMPROVE THE QUALITY OF HEALTHCARE FOR THE COMMUNITY? We provide care when you need it most. Central Health owns University Medical Center Brackenridge, home to the highest level of trauma care available in Central Texas. Together with our partners, the Seton Healthcare Family, we’re making sure that trauma care is here for everyone. We match people with care. Central Health’s call center connects people to the care they need. This one-stop-shop for service and information helps the entire Travis County healthcare system function more efficiently. We fill gaps in care. Central Health funds essential behavioral health and emergency psychiatric services needed by thousands in Travis County. When we saw a gap in services, we stepped up to make sure help is available to our community members during their most difficult times. We provide care where it belongs. Central Health has expanded primary care services for thousands of people in Travis County. We focus on prevention, wellness and early treatment — reducing the need for expensive care in emergency rooms.

“Central Health offers resources to help you get access to the healthcare you need.” 2 2   Guide to Good Health 2 0 1 2

We keep all of Travis County healthy. Central Health’s mission is to create access to primary and preventive care. These services reduce the human and financial cost of chronic and contagious disease where we live, work, play, and pray.

WHAT IS MOST IMPORTANT FOR PEOPLE TO KNOW ABOUT CENTRAL HEALTH? With our progress in expanding access to health care for the uninsured, Central Health provides a vital public health service to our community. We believe that a healthy community is essential to the success of our schools, our workplace and all institutions that contribute to our economy and our quality of life. The work we do makes it possible for many more people in Travis County to participate and contribute to our community.

Cellulaze By Ashley Erikson

Women all over the country deal with cellulite, and until recently, haven’t had a way to seek treatment. Dr. Jeffrey Hall is the first in Austin to perform a revolutionary treatment that rids women of cellulite, for good. He explains how we get cellulite and what patients can expect when undergoing the procedure to remove it. THE ANATOMY OF CELLULITE Cellulite is not a volume or shape issue in the skin, but is one of texture, that usually involves the buttocks or thighs. Over time, collagen in the skin worsens and causes the skin to sag, which magnifies cellulite. An anatomical issue, cellulite is made up of ‘bulges’ which are fat that herniates out, and ‘valleys’ caused by connections of vessels under the skin that are too tight. Since the anatomy of men and women differ, cellulite is typically only found in women.

TREATMENT OF CELLULITE With Cellulaze, doctors can finally offer a treatment for cellulite, which wasn’t a reality before. “In the past there were a lot of non-surgical treatments that would help cellulite, but once you stopped those methods it would come back,” shares Dr. Hall. “This is because it wasn’t addressing the underlying cause or anatomy of cellulite, but was simply covering it up.” Now, a change in the anatomy can occur by using a laser underneath the skin to address two things. Its perfected wavelength is used to destroy tight spots, or ‘valleys’ in the skin, so that they relax. Then, laser energy is used to make bulges smaller. The procedure can be performed in-office using an injectable numbing agent to the treated area and oral medication. There is very little downtime or

recovery with the procedure and patients can resume normal activities within a couple of days post-procedure.

WHAT TO EXPECT An initial evaluation helps Dr. Hall and his staff to determine who is a good candidate for Cellulaze. Those with simple rippling of the skin are the best candidates. There are instances when a patient also requires volume reduction and liposuction is performed first to solve contour or shape issues prior to proceeding with textural cellulite reduction. Once a patient undergoes the Cellulaze procedure, some discomfort can be expected. However,

“Cellulite is not a volume or shape issue in the skin, but is one of texture, that usually involves buttocks or thighs. ” 2 0   Guide to Good Health 2 0 1 2

patients with only a small area of cellulite treated can return to work in many cases by the next day. After the procedure, swelling and bruising for a couple of weeks is typical, but patients can expect to see improvement in appearance at one month, with full results achieved at three months. “What is most exciting during the treatment process is that we can now offer someone a solution to a problem, which couldn’t be done before,” shares Dr. Hall. “Now that we are able to treat cellulite at the anatomical level, we can offer a treatment that truly resolves its underlying causes.”

BREAST HEALTH & WOMEN’S IMAGING

By Ashley Erikson

When it comes to women’s health, awareness is an important piece of the puzzle as your body constantly changes. Dr. Sarah Avery of Austin Radiological Association wants to make sure every woman not only understands the importance of women’s health and early detection, but also knows what type of imaging care is available at ARA. Below, she shares information on when you should consider imaging studies and what tests are available to you. WHAT IS MOST IMPORTANT FOR A WOMAN TO KNOW ABOUT BREAST HEALTH AND WOMEN’S IMAGING? Women should know that there are several screening tests that can be done to help identify medical problems or cancer in the earliest stages when they are most treatable. Most of these tests take only a short amount of time and are relatively easy and painless.

WHEN SHOULD A WOMAN COME IN FOR IMAGING STUDIES? There are several recognized organizations that have recommendations for when women should have screening tests. The American Cancer Society recommends women have an annual screening mammogram beginning at age 40. The American College of Gynecology recommends that women over 65 have a Bone Densitometry test every 2 years to check for osteoporosis, earlier if requested by your physician. Most screening tests do not require a referral form from your physician.

WHAT ARE THE MOST COMMON IMAGING STUDIES? Breast Imaging Services • Digital Mammography-Screening and Diagnostic • Breast MRI • MBI (Molecular Breast Imaging) • Breast Ultrasound • Breast Biopsy (stereotactic biopsy, ultrasound-guided breast biopsy or

aspiration, and MR-guided breast biopsy) • Ductogram/ Galactogram Other Services • Bone Densitometry • Ultrasound • General Radiolog-ARA offers additional radiology services, including general x-ray and CT, as well as nuclear radiology, interventional radiology, and neurointerventional surgery.

ARE THERE ANY NEW STUDIES THAT WOMEN SHOULD KNOW ABOUT? Molecular Breast Imaging (MBI) is a new technology that can detect breast cancers that may not be identified on other tests. In Central Texas, this new technology is only available at ARA. MBI uses an injection of a low dose radionuclide, similar to those used in other types of molecular imaging, and a specialized camera to create an image of the breast. The image identifies areas within the breast that have a higher metabolic rate, which is often associated with cancer. Women who are at higher risk of breast cancer can especially benefit from this new technology as a supplement to their annual mammogram.

CAN YOU TELL ME A BIT ABOUT YOUR IMAGING CENTERS? ARA gives women a complete health picture with an array of diagnostic exams and screening services. ARA has multiple locations and offers many options for scheduling an appointment at a location and time which

works with women’s busy schedules. Women may be in and out of our office in less than 30 minutes for some exams. We use accredited technologists and radiologists who are board certified in radiology, including many with breast imaging fellowships. At our Women’s Imaging Centers, we strive to deliver a pleasant imaging experience with comfortable waiting rooms, private dressing rooms, easy appointment scheduling, and convenient parking. For your safety, ARA adheres to the latest national guidelines to limit radiation exposure, and we have medical assistance available onsite if needed. ARA is accredited by the American College of Radiology (ACR) in all forms of advanced imaging. Our Women’s Imaging Center in Central Austin has been designated a Breast Imaging Center of Excellence by ACR.

Snoring By Ashley Erikson

You may have spent a sleepless night – or many – tossing and turning because of your significant other’s little problem: snoring. You may even be the one causing the restless night. Either way, snoring is something that is completely controllable, and Dr. Zachary Wassmuth from Capital Otolaryngology has some advice for those who need relief, for themselves or a loved one, so sleepless nights can be a thing of the past. THE CAUSES OF SNORING About forty-five percent of adults snore. Snoring can be caused by a number of factors, and sometimes, multiple factors at once. Nasal obstruction caused by allergies, chronic sinus disease or nasal septal deviation can increase snoring. Enlarged tonsils, weight gain, sleep apnea and elongated soft palate or uvula can also be causes. “The most common cause for snoring is the soft palate,” states Dr. Wassmuth. “That is what vibrates in the back of the throat and results in the noise you hear.”

WHEN TO SEE A DOCTOR “One of the biggest problems to bring patients into the clinic is when the significant other leaves the bedroom secondary to snoring,” states Dr. Wassmuth. There are things to try to help improve snoring at home, but when those methods fail, it is best to see a doctor. Snoring is exacerbated by weight gain, allergies, muscle relaxers, alcohol and sleeping on your back. “If you fix these issues, you will get improvement in snoring as a side effect,” shares Dr. Wassmuth. Another time to go in to see a physician is if you think you may have sleep apnea. This sleep disorder causes irregular breathing during sleep and can also exacerbate snoring. If there are signs a patient is tired or having headaches – or if others have witnessed the patient holding their breath

during sleep – doctors will order a sleep study to determine the diagnosis. If snoring is a symptom of sleep apnea, once the apnea is treated, the snoring will go away. Because sleep apnea increases the risk for hypertension, heart disease, stroke, and depression, those who are at risk should consult a doctor.

HOW SNORING IS TREATED Once a patient has decided to seek medical treatment for their snoring and visit an office, they can rest assured that a snoring workup, though quick and simple, is also thorough. “At Snoring Austin, we evaluate all potential areas that can contribute to snoring, instead of immediately zeroing in on the soft palate,” shares Dr. Wassmuth. “We want to make sure we know what all of the areas are doing so we can fix all contributing factors.” The workup includes checking for nasal obstructions, looking at the tonsils and evaluating the soft palate. If sleep apnea is suspected, a sleep study will be performed in the sleep lab located on-site. Treatment is then determined based on the problem, whether singular or multi-factorial.

“Sleep apnea increases the risk for hypertension, heart disease, stroke, and depression, those who are at risk should consult a doctor.” 2 8   Guide to Good Health 2 0 1 2

Typically, if sleep apnea is not involved, snoring treatments are performed in the clinic. If the soft palate is involved, patients can get palate implants to make it less ‘floppy’ or injections that produce scar tissue to tighten the region. These in-office treatments are eighty-to-ninety percent successful in resolving snoring. “It is important patients know there is always something that can be done,” shares Dr. Wassmuth. For more information or to schedule a consultation, visit www.capoto.com.

SNORING FACTS: Forty-five percent of adults snore occasionally, while twenty-five percent of adults snore nightly. Over 12 million Americans have sleep apnea, which can be a factor in snoring. Snoring is usually worse in males and those who are overweight, and if not treated, can worsen with age.

TEXAS INSTITUTE FOR ROBOTIC SURGERY WOMEN’S DAVINCI® SURGERY By Ashley Erikson

WHAT IS DAVINCI® SURGERY (ROBOTIC ASSISTED SURGERY)? Approximately 25 years ago surgeons began practicing minimally invasive surgery. During this era, tiny cameras in instruments called endoscopes or laparoscopes were inserted into the patient’s body through small ports. In 2005, a further evolutionary stage was achieved in gynecology with the application of robotic assistance in female surgery. Through tiny one-to-two centimeter incisions, surgeons using the daVinci® system (robotic assistance) can operate with greater precision and control, minimizing the pain and risk associated with large incisions, while increasing the likelihood of a fast recovery and excellent clinical outcomes.

WHAT SYMPTOMS AND CONDITIONS ARE TREATED? Some of the most common gynecologic symptoms that women experience are pain, excessive or irregular bleeding and involuntary loss of urine. Pain can be described in many ways and pain such as pelvic discomfort, abdominal cramps, backache, painful urination/bowel movements and pain during intercourse may indicate a different gynecologic condition. As for bleeding, monthly menstrual bleeding is different for each woman, so it is important to describe what is normal for you. Loss of bladder control in women can affect up to 50% of the female population. In general,

incontinence is defined as stress, urge, or mixed. The origin can be localized to the nervous system, bladder musculature, or change in female anatomy (prolapse). There are common types of gynecologic conditions that cause chronic pain, heavy bleeding and loss of bladder control. Endometriosis can cause pain and is present when tissue that is normally bled from inside of the uterus, instead bleeds on the outside of the uterus. Another condition, fibroids, can cause pain and heavy bleeding and are benign tumors that grow underneath the uterine lining, inside the uterine wall or outside of the uterus. Finally, pelvic prolapse is a condition that is defined as female anatomy slipping out of position. Prolapse happens when the pelvic muscles and connective tissues weaken with time and child birth.

WHAT ARE THE TYPES OF DAVINCI® SURGERY FOR WOMEN? Types of da Vinci® surgery include: Endometriosis Resection - the precise excision and removal of endometriotic implants; Myomectomy - the removal of fibroids from the uterus while preserving the uterus; Hysterectomy - the removal of the uterus only, uterus plus cervix, or uterus plus cervix and ovaries; Prolapse surgery – the correction of uterine, vaginal, bladder or rectal prolapse.

WHAT ARE THE BENEFITS OF DAVINCI® SURGERY? In comparison to traditional open surgery, the potential benefits include: significantly less pain, less blood loss, fewer complications, less scarring, shorter hospital stay, faster return to normal daily activities and faster return to work.

3 0   Guide to Good Health 2 0 1 2

WHAT IS THE FUTURE OF DAVINCI® SURGERY? As high tech moves into the surgical sciences, the future is very promising. Locally, through collaborative efforts, gynecologic surgeons at the Texas Institute for Robotic Surgery are having an international impact teaching colleagues, performing research and contributing to advancements in female surgery. SINGLE-SITE GALLBLADDER REMOVAL By Ashley Erikson

Robotics surgery for gallbladder removal used to require three-to-four incisions, but with current advancements, now only requires a single incision. Dr. Buzad and his partner, Dr. Corne, the only two surgeons in Austin qualified to perform this procedure, use Single-Site™ da Vinci® surgical tools in these procedures. Dr. Buzad explains the benefits of single-site robotic assisted gallbladder removal and what you, as a patient, should know. HOW COMMON IS GALLBLADDER REMOVAL? From the general surgery perspective, it is one of the most commonly performed operations and most general surgeons, on average, can perform the procedure an average of two times per week.

ARE THERE SPECIFIC SYMPTOMS PEOPLE SHOULD BE AWARE OF? Typical symptoms are usually in the form of upper abdominal pain in the right or mid-upper abdomen that will sometimes radiate toward the back. Also, nausea after eating is common – especially with a meal high in fat – as this can cause the gallbladder to contract, and when people have gallbladder disease, it intensifies their symptoms. Seeing the primary care doctor or gastroenterologist is the first step, and after tests are run, if gallbladder disease is detected, they will be referred to a surgeon for removal if necessary.

WHAT IS MOST IMPORTANT THAT THEY KNOW ABOUT THE PROCEDURE? As I stated, gallbladder removal is one of the most common procedures that I perform. The risks of not proceeding with surgery if required are higher than the risks of the surgery itself. With single-site robotic assisted gallbladder removal, I can now use a single incision along the belly button, which is not only cosmetically nicer than the old four-incision procedure, but also causes fewer problems for

“Due to the single incision instead of four incisions, there are fewer problems for the abdominal wall including reduced pain, reduced chance of wound infection and quicker recovery times.”

3 2   Guide to Good Health 2 0 1 2

the abdominal wall. This means quicker recovery times and less discomfort for the patient.

WHAT ARE THE RISKS THAT ARE ELIMINATED BY USING ROBOTICS IN THIS PROCEDURE? Due to the single incision instead of four incisions, there are fewer problems for the abdominal wall including reduced pain, reduced chance of wound infection and quicker recovery times. The accuracy and visualization that the Single-Site™ da Vinci® surgical tools provide during this procedure give me the chance to now see 3D views of the gallbladder (and surrounding anatomy) during the procedure, versus the 2D view that surgeons have when performing laparoscopic gallbladder removal. This precision also benefits patients who may not have been candidates for manual laparoscopic surgery. Once I started using the robotic system, I have rarely gone back to standard laparoscopic cases because it is such an improvement on what it offers both doctors and patients.

FRANCIS BUZARD, M.D.

FACTS & FIGURES: Approximately 500,000 gallbladder surgeries or cholecystectomies are performed each year in the US Gallstones are twice as likely to occur in women. Overweight people, older adults, Native Americans and Mexican Americans are also at greater risk. Once gallstone symptoms appear, they recur in the majority of patients. Symptomatic patients are 25 percent more likely to develop complications within 10 to 20 years. Upon initial release, Single-Site™ da Vinci® surgical tools were used by only 20 surgeons who were chosen around the country, including Dr. Buzad.

Assisted Reproductive Technologies By Ashley Erikson

Motherhood is an experience that no one has to miss out on, even if biological factors are interfering. At Austin Fertility & Reproductive Medicine, the physicians are able to offer services of a state-of-the-art in vitro fertilization (IVF) lab called “Westlake IVF”, used for assisted reproductive technologies (ART). Dr. Shahryar Kavoussi explains what those technologies are and what steps to take when considering treatment by a fertility specialist. WHEN SHOULD A COUPLE CONSIDER SEEING A FERTILITY SPECIALIST? Women who are under the age of thirty five who have been trying to get pregnant for one year and are still having a regular cycle should consider coming in to see a fertility specialist. For women who are over thirty five years of age, it is recommended that they come to see a fertility specialist if they have been trying to get pregnant for six months without success.

WHAT IS THE INITIAL EVALUATION THAT IS PERFORMED? A detailed medical and reproductive history is the first step. Next, we recommend a series of fertility tests. Blood tests and ultrasounds give information about a woman’s egg quality and number. A hysterosalpingogram, which is a dye test with x-ray, is performed to evaluate whether or not the fallopian tubes are open and if the uterus has a normal shape. A semen analysis for the husband/male partner is always recommended as well. These tests help us determine which method of treatment is best to proceed with in order to help the couple achieve a successful pregnancy.

WHAT ARE REASONS BEHIND NEEDING ASSISTED REPRODUCTIVE TECHNOLOGIES (ART)? There are various reasons for which ART is recommended for a couple, including a woman having endometriosis or blocked fallopian tubes, the husband/male partner having low semen analysis parameters, and unexplained infertility.

WHAT TREATMENTS ARE DONE TO ASSIST WITH ACHIEVING PREGNANCY IF PATIENTS CHOOSE TO UNDERGO IVF? Medications are used to assist the woman in growing multiple eggs within a cycle, after which they are ‘harvested,’ or retrieved, by the doctor. The eggs are then placed with the husband/male partner’s sperm in our state-ofthe-art IVF lab, and we can see how many fertilize and turn into embryos that begin to grow. The embryos that grow well and have the best chance of resulting in a successful pregnancy are then placed back into the uterus (embryo transfer).

ARE THERE DIFFERENT TYPES OF ART PROCEDURES A WOMAN MAY HAVE?  Yes, with conventional in vitro fertilization, the sperm and egg are placed next to one another in the lab and the sperm fertilizes the egg on its own. With intracytoplasmic sperm injection (ICSI), the sperm is actually placed into the egg by the embryologist. With either of these options, the embryo is then placed into the uterus and the woman takes estrogen and progesterone to help the embryo implant in the uterus and increase the chance of pregnancy. In some cases the embryologist will perform ‘assisted hatching’ prior to embryo transfer. This involves using a laser to open the covering of the embryo to increase the chance of a successful implantation leading to pregnancy.

“Motherhood is an experience no one has to miss out on, even if biological factors are interfering.” 3 4   Guide to Good Health 2 0 1 2

WHAT CAN I EXPECT TO FEEL DURING THE PROCEDURES? The egg retrieval procedure lasts roughly 15-20 minutes and is done in an office setting with IV conscious sedation. Patients will wake up with mild cramping. For embryo transfer, the patient is fully awake and can expect it to feel a lot like a pap smear exam, with some more mild cramping and some pressure during the process. Westlake IVF is unique since the IVF lab and procedure rooms are located within our office setting which was designed with patients’ comfort/familiar surroundings in mind. We take care of patients on a one-on-one level and can answer any questions for someone who is interested in having assisted reproductive technologies. We encourage anyone who thinks they are interested in an evaluation and treatment to visit www. austinfertility.com and set up a consultation.

CHIROPRACTIC By Ashley Erikson

When people think of visiting a chiropractor they usually just associate it with back and neck pain, when in reality it can help patients with much more. Katerina Sheffield DC RPh of Westlake Chiropractic shares how chiropractic can be good for your whole body. WHEN SHOULD PEOPLE THINK ABOUT SEEKING OUT CHIROPRACTIC CARE? For people with neck pain, back pain, shooting pain, numbness and tinglingchiropractic is an excellent choice, especially for those wanting a drug-free option or who want to leave surgery as a last resort. Structural imbalances can cause or contribute to other kinds of pain as well, including hip, knee, ankle and even jaw pain. Car wrecks, sport injuries, and even normal childhood trauma can cause damage and lead to spinal degeneration and osteoarthritis.

WHAT CAN PATIENTS EXPECT WHEN THEY VISIT YOUR OFFICE? I offer a no-charge initial consult to discover if a patient’s health concerns are a good fit with the care I provide. Next, a complete diagnostic work-up, including neurologic and orthopedic tests, as well as a postural assessment and possibly x-rays are completed. I utilize an upper-cervical specific approach to spinal care because it is one of the most effective and efficient forms of chiropractic care. Upper-cervical chiropractic prioritizes a gentle and extremely precise adjustment of the atlas vertebra, the top bone of the spine directly under your head. When this bone is misaligned, it can cause structural imbalances that affect your entire system, all the way down to your feet!

pharmacist trained in the Western medical mindset. Chronic pain lead me to chiropractic. When I was five years old I was run over by a truck, and I’ve sustained two whiplash accidents as an adult. The results I obtained from upper-cervical chiropractic impacted my life so much that I changed professions. My technique is gentle, specific and perfect for anyone who has hesitations about seeing a chiropractor.

ARE YOU ANTI-DRUG? WHAT IF PATIENTS ARE SCARED OR SKEPTICAL ABOUT SEEING A CHIROPRACTOR? I completely understand the hesitation to try chiropractic because I was once a skeptic myself. Before I became a chiropractor I was a

As both a chiropractor and a pharmacist I am not anti-drug, nor am I anti-Western medicine; I am pro-chiropractic. I am for the best solution for the problem, and drugs aren’t always the best solution. I think this may be why so many MD’s feel comfortable referring

to me. In fact, I have many patients who are MD’s.

HOW IS CHIROPRACTIC GOOD FOR YOUR WHOLE BODY? Chiropractic is not only about relieving back and neck pain. Virtually every nerve in your body runs through your spine, and chiropractic care relieves pressure on these nerves, allowing them to communicate freely. My patients often report relief from issues like asthma, constipation, acid-reflux and other complaints. One of the most significant studies on upper-cervical chiropractic found a decrease in blood pressure equal to double drug therapy. It’s no surprise that 90% of professional athletes use chiropractic!

HOLISTIC DENTISTRY

By Ashley Erikson

Usually when we think about dental care, we try to regularly brush and floss and then go to the dentist when scheduled. What we may not think about is how the health of our body may be affected by that of our mouth. Dr. Elizabeth Rayne, who helps patients remove metals safely from their mouths to keep them healthy, explains why the oral cavity is so important to our overall health. DEFINING HOLISTIC DENTISTRY With holistic dentistry, your dentist will focus on improving your whole body health through improving your dental health. The mouth is an important area as your teeth have nerve endings, also known as acupuncture meridians, which connect to other organs and tissue in your body. “Infections or diseases in the mouth can spread throughout the entire body,” shares Dr. Rayne. “As it spreads, this causes degenerations and other problems. This is why I focus with patients on keeping their mouths as healthy as possible, to help improve their overall health.” The products placed in your mouth, such as fillings, are just as critical to your well being as your food and dental habits. Different products in your teeth can possibly create reactions in your body. The focus should be using a product that doesn’t react with your body but will help keep it in a healthier state, which is what Dr. Rayne helps patients accomplish.

WHAT TO EXPECT When a patient first comes to see Dr. Rayne, a comprehensive examination is performed. The full assessment of the patient’s health includes a visual, radiological, and periodontal evaluation. Patients are educated about diseases of the mouth, the biocompatibility of certain products as well as saliva tests. These saliva tests look at genetics of periodontal disease, pathogens and viruses that are found in the mouth. These tests provide Dr. Rayne and the patient biological

information needed for treatment planning goals for the health of their teeth and mouth. Dr. Rayne looks at the products patients have in their mouths – such as in or underneath crowns, inlays, onlays, bridges. She educates the patient how this is affecting their health as some of these have dental amalgam (mercury) in them. If either is the case in the patient, Dr. Rayne runs tests for choosing new products and works with other practitioners to guide the overall health of the patient. Patients see these practitioners, such as naturopaths, homeopaths, chiropractors and herbalists in conjunction to their visits with Dr. Rayne, so that other aspects of their health can be examined and improved at the same time as their dental health. “While I focus on removing metals in the mouth, the other practitioners help remove it throughout the rest of the body,” shares Dr. Rayne.

REMOVING METALS SAFELY Once the metals in the mouth have been identified and a new healthier product has been chosen, Dr. Rayne safely removes the metals from the patient’s mouth. This involves isolating the tooth with a dental dam and using a suction device to not only remove the

metal but also the vapors that it releases. The patient is placed on oxygen, the doctors wear special filtration masks during this process, and the room is well ventilated to make sure that no one is exposed to the mercury or other harmful components of the metals as they are removed. Dr. Rayne will then give the patient the new product that has been chosen. “It is recommended that patients have fillings evaluated on an ongoing basis,” stresses Dr. Rayne. “This is to ensure the products within the mouth are in good condition and to be certain no other treatment is necessary to improve the patient’s oral care – and overall – health.”

BARIATRIC SURGERY By Ashley Erikson

Dr. Nicole Basa is a bariatric surgeon who performs sleeve gastrectomy and gastric bypass surgery. After training at the University of California, Los Angeles where she did a fellowship in bariatric and minimally invasive surgery, she moved to Austin five years ago and was the first doctor to perform a fully robotic gastric bypass in central Texas. Now, she shares what you should know if you are considering surgery for weight loss. THE TYPES OF BARIATRIC SURGERY There are two main types of bariatric surgery, restrictive and malabsorptive. Restrictive surgery comprises of physically making the stomach smaller, which is known as a gastrectomy. In malabsorptive surgeries, the intestine is bypassed so fewer nutrients are absorbed from foods. Both the sleeve gastrectomy and lap band surgery are restrictive procedures. In the sleeve, part of the stomach is removed whereas in lap band surgery, the top of the stomach is cinched by a band. Gastric bypass surgery is seen as both types as it involves bypassing a portion of the small intestine and making the stomach smaller.

WHAT TO KNOW BEFORE SURGERY Bariatric surgery is an elective surgery which can be life saving. However, it is important to have proper education about your options. Dr. Basa does free informational seminars, which explain the different types of bariatric surgeries and what to expect should you choose to move forward to have a consultation and see if you are a candidate for the procedure. Dr. Basa recommends that before choosing a surgeon, you should make sure to have the procedure done locally as opposed to travelling out of town or out of state. “It is important to look at the program to make sure you get proper education and treatment,” she states. “A multi-disciplinary

program includes a bariatric nurse coordinator, dietitian, support group, psychologist and exercise guidance. All of these components are an important part of the procedure as well as having access to your surgeon which is why you need to have local access to this type of care.”

THE BENEFITS Surgery, especially one that is elective, can sometimes be a scary thing. The good news is, in laparoscopic gastric bypass surgery, the mortality rate is less than 0.25 percent, similar to that of a laparoscopic gallbladder removal. It is important to know that though there are risks involved in surgery, there are also be risks without it. “In my seminar, I often refer to the following study performed by Christou,” continues Dr. Basa. “It shows if someone is morbidly obese and chooses not to have surgery, the risk of dying is nine times higher than those who have bariatric surgery.”

Gastric Bypass

Along with lowering complications from obesity, all of the bariatric procedures resolve diabetes. Dr. Basa shares, “With gastric bypass specifically, the benefits are its help to decrease blood sugars, which is why it lowers diabetes almost instantaneously – sometimes days after surgery.” Though all types of bariatric surgery help decrease diabetes through weight loss, gastric bypass rids the body of diabetes through an actual physiologic change. The resolution of diabetes is around eighty-five percent in patients, along with resolution of issues such as high blood pressure, sleep apnea, high cholesterol and joint problems. If you are interested in learning more about bariatric surgery, you can visit www. cedarparkbariatrics.com to attend an information session and schedule a consultation with Dr. Basa.
