As we look at the dental market, one thing we can say for certain is that the environment is changing. We know that dental business owners are becoming more sophisticated, teams are being better trained, and systems are becoming more and more crucial.

It is critical to look at your business and decide whether it’s time to make changes so that you can reach the next level.

The chart to the right shows the lifecycle of a dental practice and what revenues look like during each phase of business.

The first thing you need to determine is where your practice is in its overall lifecycle.

As you’re reading this, stop for a moment and consider these questions.

Then determine where your business falls on this chart. Look at the last three years, and then plot that information on the chart.

Many doctors are selling their practices because they have waited until their business is in decline to make a change, even though decline is the hardest phase in which to make a change. Ideally, businesses should make changes in the growth phase, to prevent the business from leveling off. A business needs to constantly improve its overall look and feel, equipment, systems, talent, and mix of procedures/services.

Each distinct phase has its own set of unique challenges, which, if managed properly, can all be overcome.

STARTUP PHASE:

The startup phase of a dental practice is an exciting time. Usually, most if not all of the equipment is new, the office has updated décor, the staff is excited, and if the practice is marketed well, there tends to be an initial surge of patients.

This is also a time of experimentation, finding which systems work well and which ones do not. Typically, a dentist in the startup phase is excited to launch his

The growth phase is when you will see the most increase in activity, new patients, and overall excitement about your office.

This is the ideal time to make large reinvestments and changes, though it may seem like there is no good reason. If things are good, why risk changes?

One example is Netflix. The company revolutionized the at-home movie watching experience by sending DVDs straight to your home, removing the inconvenience of having to go to the store. The concept stole most of the market share away from Blockbuster, yet during this tremendous time of success, Netflix changed their platform to streaming content. It would have been easy to relax, get complacent, and feel like, “We’ve got it made.” This change, during a growth period, yielded an unprecedented level of success that transformed the industry.

During tremendous growth, it is easy to become comfortable. However, this is when you must make the biggest reinvestment and changes in your office, whether in the areas of equipment, team

At this point, ask yourself: Can I continue to expand in the current space? What will it take to continue to grow and make sure my long-term plan is in place?

During the maturity phase, it is easy to become complacent. Often, doctors in the maturity phase feel happy with their business and therefore do not have a desire to grow. Employees may have been on staff for some time, and they are accustomed to a “certain way of doing things.” This mindset makes changes uncomfortable. You may have a tremendous amount of pushback from staff, and forced changes can lead to turnover. Due to this difficulty,

many doctors decide not to make changes in their business during the maturity phase.

However, this is when change is needed the most.

The maturity period is a great time to bring new enthusiasm into the office environment. You can utilize the brand that you have built with your patients to increase referrals and community involvement. You will find that even with

pushback from certain staff members, others will be ready to jump in and assist.

There is a saying in business: “If you’re not growing, you’re dying.” In the maturity phase, you will see more patient attrition over time, and you must reinvest in your practice to prevent your business from entering the decline phase.

Many times, when things are going well, we tend to get complacent. By the time the business owner recognizes a problem, the amount of energy required to change the situation can be daunting.

Short of having a candidate take a psychological test administered by a clinical psychologist, it’s very difficult to determine the state of anyone’s personal life. With that said, I do think there are a few questions that can be sprinkled throughout an interview to help provide some insight.

1. What do you like about dentistry? How would you change what you don’t like?

This question does a couple of things.

First, it gives you insight into how the individual can verbalize something they don’t like. An answer that is only 5% about the problem and 95% about the solution is an indication that the individual does not get caught up with events of the past.

2. Tell me about a problem you were tasked with solving in your current position. How did you choose to fix it?

This exposes their problem solving and creative thinking ability. Did they have a thoughtful approach to solving the problem or did they blame someone else and pass it on to another for a solution? Individuals who take responsibility have more control over what happens to them during their interaction with others and less likely to be taken advantage of or becoming a victim.

If someone you’re interviewing says they work out three times a week, perform volunteer work, and is learning how to do something (cook or play piano for example) “just for fun”, then I think it’s fair to say they value achievement, goal setting and are always looking to improve themselves. I think this will also translate into their job.

3. Knowing that dentistry can be stressful; how do you balance your professional and personal life?

“Stable people strive to do well in most areas of their life including physical fitness, relationships, and learning.”

I have found that people who report they participate in team activities such as soccer, rowing, basketball, and volleyball tend to be driven, focused on achieving goals and physically fit, which keeps their mind sharp. Generally, they will also be better communicators, cope well under pressure and perform well while working with others.

We know balance is important for personal stability.

Along with asking good questions; listen really well too. Obviously the more the candidate talks, the more you’ll learn.

People who are emotionally stable exhibit certain qualities and continue to explore them throughout their life. Listen to determine if these qualities are present in how the candidate lives their life.

Viewing other people with compassion and treating them with kindness is a hallmark of your own well-being. It indicates you’re sensitive to the needs and feelings of other people, and think it’s important to help others. It’s basically the idea that you’ll lend a hand to someone in need, even if it’s as simple as picking up something that was dropped by another person, or smiling and making friendly conversation with the person standing next to you in line.

They like who they are.

When you’re personally stable, you generally feel pretty good about who you are. You know yourself – quirks and strengths – and you’re okay with how you are inside. You’re also congruent: congruency means that the person you show to the outside world is reflective of who you are in the inside. While there are situations where you naturally shift your attitude or behavior a bit depending on the social situation, (it’s normal to behave differently at a work function than at a weekend picnic with friends), congruence means your overall “When you’re personally stable, you generally feel pret ty good about who you are.”

Another sign of personal stability is their ability to embrace their emotions – sadness, anger, anxiety, joy, fear, excitement – as a natural and normal part of life.

They handle it and acknowledge difficult emotions without becoming overwhelmed by them or by denying that these emotions exist. They know it’s normal to have periods of stress, and they know how to manage and express themselves when they feel upset, and know who they can go to and get comfort or help. Their feelings of

sadness, anxiety and fears don’t get in the way of what they love to do. If they’re afraid of public speaking, they manage to get up in front of an audience anyway.

Personal stability comes from being able to acknowledge and accept tough emotions, but also move forward from them without getting stuck. This means they might get nervous before meeting someone new, however, they don’t let the nerves stop them from making the acquaintance.

They also savor their positive emotions – reveling in the moments of pleasure, happiness and joy when they come as this is also linked to personal stability.

Regardless of the cause, being part of something you connect with and care about is largely associated with well-being and personal stability.

What is the return on investment or how do I justify this amount of capital?

When you purchase a cone beam, I suggest that you look at the ability of the machine to generate images in different formats. You need to ask yourself, besides taking a threedimensional image, can this machine create panoramic images? If it can take panoramic images, ask yourself, “How many panoramic images do I take in a year?” Then ask yourself, “What do I charge for those images?”

For example, if I charge $100 and I take 400 panoramic images a year, then I just billed out $40,000. If this machine cost you $120,000, dollar for dollar, you recoup the expenditure in three years. Realistically, you pay for it over four years.

Remember, it’s just not about the X-rays. It’s about the amount of undiagnosed dentistry and implant planning we can do with this technology. It generates better comprehensive treatment planning and ultimately better dentistry for the patient.

How has communication improved with your patients using CBCT? Patients are blown away when they see a three-dimensional image of their jaw. When I can show patients where I can place an implant in their jaw using their own anatomy and a chairside model, patients can visualize and comprehend the procedure more clearly.

I would encourage the use of videos that all the major dental implant companies produce. Check with their marketing department and incorporate a video into your website. While the patient is in the chair, you or your team can pull it up on your website for the patient to view. This serves two purposes.

First, patients can visualize it more clearly. Second, when they go home to their significant other, they can pull up your website and explain the procedure to them.

every scan to a dental radiologist. I can’t stress enough the importance of reviewing all your scans even after the patient has left the office.

The more we review and educate ourselves on the normal anatomy, the better we become at identifying the obscure. I also encourage taking classes that are continually being offered throughout the country on how to review a cone beam.

Do I need an intraoral scanner and CBCT? Having an intraoral scanner and a cone beam is the perfect marriage.

Both are completely independent of each other. The scanner focuses on teeth and tissue and the cone beam

addresses the bony architecture.

When the STL files are joined in the lab, there is complete harmony. Like a great marriage, they make each other better.

From my understanding and speaking with different vendors, one can take a DICOM file obtained from the CBCT, convert it, make a virtual model, 3D printed model and fabricate a crown. So, in theory, you could prep a tooth and have an edentulous area. Plan the implant and make the crown or partial all at the same time. And then there are times we just need to take an intraoral scan and make a crown inlay or onlay.

What’s your opinion on the obsolescence of a machine you purchase today?

The most important question to ask the equipment sale rep is, “Is the hardware upgradeable?” Also, is there a fee for upgrading to a newer version of the software?

Many companies have software version updates so you should ask what is included with your purchase.

Like any other technology, there are advances in how it’s delivered. Going with a company that’s one and done is not the way to go. I want to do business with a company (and there are many out there) that want a longterm relationship with the buyer.
