Atlanta Design Associates was first introduced to Dr. Scot t Merrit t in May of 2000 when they were given the opportunity to design his first of fice in Canton, Ga. To bet ter accomodate his patients, Dr. Merrit t established Bridgemill Dentistry in 2002. When it came time to design the new of fice he naturally came back to Atlanta Design to create the 5,800 square foot building on Sixes Road in Canton, Ga. “My true desire is to serve the community where my family is located”.

The new of fice provides patients with a higher level of service that combines modern technology with sophisticated style. With state of the art digital technology, the spacious new facility features large exam rooms with TV monitors, nitrous oxide, & oral cancer screening equipment in each treatment room.

The large monitors allow his patients to view their digital x-rays from their chairs. “The digital technology not only helps reduce the patient’s exposure during x-rays but also helps the patient to bet ter understand their treatment. They can see every thing for themselves on the big TV screens,” he explains.

We recently had a chance to talk with Dr. Merrit t about his new of fice design and his overall experience with Atlanta Design.

What do your patients say about your new office? Overall has it had a positive impact on your practice?

It has been a very positive response! Our patients have always viewed us as a “quality first” practice, and they are able to feel that way when they first enter the parking lot. I was quite worried that people would be turned off by the size and scope of the building, but we kept such a warm homey feeling to it, that our patients have still felt right at home.

How has your new office helped with marketing your practice?

Our new location (right across the street from our old one) is much more visible to drive-by traffic. The structure is so unique to our area, that it really catches your eye when you drive by. Being a stand-alone building with plenty of parking provides easy access for everyone.

Has productivity increased since the design of your office?

I don’t think our productivity has necessarily increased, but we are definitely busier.

We are able to accommodate more flow into the office, as we now have plenty of space for them. The office design with peripherally located treatment rooms is basically a big square. This is broken up by columns and ceiling work, so there are no long scary-looking hallways.

What made you decide to bring on an associate?

I needed more space. I love to work hard, and I love to be busy (who doesn’t). Patients were leaving the practice because they couldn’t be seen in a timely manner. It was the perfect recipe for a larger, newer structure and the right second dentist. I interviewed several really wonderful people, but I think I was incredibly lucky to find Dr. Cox. He truly

understands how it takes time to build something longterm, and things are really taking off for him.

How was the overall process of working with Atlanta Design?

What benefits did they provide?

It was a remarkably easy process working through Atlanta Design. I know that I am supposed to say that, but it was really true.

They have a full service design team that is very well oiled. My practice is quite busy, so it was hard to find time to dedicate to the process. They would work late Fridays, or come to my office whenever I needed them. There are other outfits and companies that may have cost less, but as in my practice, there is no substitute for top quality.

From design to equipment to interior design, working with them was truly wonderful.

Feature Article What were some obstacles when planning your new office & how did you overcome them?

You hear it time and again when designing a dental office, but the flow of the office makes all the difference. We wanted to design a building that handled traffic well, while keeping the focal point on treatment room positions. We have 12 operatories, each with its own double sized window centered in the room. Our main difficulty with doing this was to maintain the external building symmetry as well. Atlanta Design used columns and fake door/ windows to achieve it. It was really remarkable what Gary Lowery and his team were able to do. Our old office was in the basement of a large building, so natural lighting was also a huge design motivator. The massive front windows really brighten the office…and the moods of the patients and staff.

Is there something you found out or had to deal with during construction or the design process that was a big eye opener… something you didn’t know before?

I never realized how much oversight each county has on construction. It took more time to get the property ready to build, than it did to actually build. There are SO MANY facets to getting a project like this underway, that I just had no idea where to start. My project manager attended all the hearings and council meetings. He met with the county commissioner multiple times, drafted documents, and handled the entire construction hiring process. I would have failed miserably had these two teams not worked so closely together.

Was there anything that surprised you about the space once you moved in? Anything you wish you had done differently?

From the beginning of the design process, Gary Lowery and the Atlanta Design team were very insightful and creative. One interesting thing in the design really impressed me shortly after we opened; beautiful thought was put into line-of-sight disruptions.

When walking down the various hallways, I never stare directly into a treatment room.

Everything is offset slightly to avoid patients and parents, etc. from watching me and making eye contact while I walk toward a room. This allows me time to study the digital chart on my tablet before saying hello.

The long hallways are broken up with columns and ceiling trays, making the whole place feel smaller and more homey.

These are just a couple of items, but the entire building received that kind of attention to detail. It is nice to work with an experienced team.

I really don’t think there is anything I would have done differently. That says a lot.

What advice would you give to anyone setting up a new office?

Advice going through the design process? Through the construction process?

As I stated earlier, I found it paramount to have someone who understands dental building construction from start to finish. Hiring a project manager at the very beginning was a huge stress relief.

Imagine if your patients were not triaged when the they walked through your front door.

You would be overwhelmed with problems and decisions constantly. Atlanta Design was very open to working with my project manager, and their line of communication involved me only when needed. This allowed me to focus on the practice during business hours.

- Dr. Scott Merritt Were you able to successfully relocate existing equipment, furniture, etc…

As it came time to move, the stress level jumped back up. We were closing for one week, and the entire old office was to be disconnected and moved into the new location.

We also were putting in a new stericenter and four new rooms of equipment. Two weeks out, the walls were not painted and the carpeting had not yet begun…and I was a mess! The interior was finally finished, but the move was looming. Atlanta Dental Supply coordinated things, and for the next eight straight days everything was unhooked, removed, re-hooked, and made operational. It was truly remarkable to see it all come together. We had some further new construction pains, but the team was always there to make it all right.

What was your favorite part about working with Atlanta Design?

This is an easy question to answer. I knew very little about what I was about to embark on. I just knew a basic design of what I wanted. Atlanta Design has a team dedicated to working together to make this happen. There is a stepby-step approach in which each member of their team takes their time and walks you through the bewildering process. I brought in my own project manager (which I highly recommend) and they went out of their way to work closely with him. If you are currently practicing dentistry, there is no way to have the time it takes to do this on your own. Atlanta Design worked nights and Fridays with me as much as I needed. They promptly retuned emails/calls and made excellent decisions whenever needed.

Embezzlement: It’s Easy To Do & Even Easier To Prevent!

Preventive Measures You & Your Team Can Take To Help Protect Your Practice.

Excerpt from Part 1 No one ever thinks it will happen to them. Believe me, it happens more often than you think, and often from the person you trusted the most. That’s right – embezzlement! Too often, doctors are shocked because they trusted their teams and did not feel it was necessary to have specific checks and balances in place.

The answer to overcoming fraud can be simply stated.

First, be aware of behavior trends of embezzlers.

Second, exercise prevention through daily attention, controlled authority, and systematic access.

“There’s a 75% chance that your practice will be embezzled during your dental career… don’t ever think it can’t happen to you!”

Part 2 In setting up an internal audit system, it’s common to divide financial responsibilities and functions so that no one employee controls all aspects of a transaction.

Preventive Measures:

• Require checks to be countersigned by two responsible parties. In addition, limit the endorsement of checks, by anyone other than the owner, to deposits for credit only.

• If you must delegate check-signing authority, limit the accessibility and value.

• Make it mandatory that all insurance claims are properly filed within fifteen days.

• Delegate the responsibility for receiving checks and cash to someone other than the person who records incoming funds.

• Require every patient to sign in and keep the sign-in sheets for your audit.

• Have each clinical person sign off on daily posting of codes and procedures.

• Review the day sheet at the end of every day.

• Mail statements to outside accounts directly at least monthly. Even better, pay your bills online using a secure service provided by your bank - but only if it is set up so that you are the only person authorized to make payments!

• Examine payroll records periodically to prevent padding.

• Make sure that employees responsible for ordering goods and supplies are not the same ones responsible for receiving them or paying for them.

• Require backup documents to be attached to all payable checks for signing.

• Do not give the person who has the authority to write off bad debts the authority to authorize credit or loan money to an employee.

• Post and track write-offs individually as opposed to one large amount. I.E. senior courtesy, family discount, PPO reduction, etc.

• If someone else does payroll, make sure you have access to payroll data on the computer and that no one changes the passwords or access requirements for that data. Consider an outside service for this and your quarterly reports.

• Use pre-numbered receipts, checks and purchase orders. Use duplicate deposit slips and checks printed on special safety paper.

• Review revenue, receivables, payables, bad debts, account balances, statement reconciliations & payroll monthly. This can be done in less than 30-minutes (during lunch) in your office with the door closed.

Lead By Example: • Anything that you do, your staff will assume is okay for them also.

Don’t use office supplies/equipment for personal use.

Don’t bend tax rules Don’t dip into petty cash for non-business use

• Be involved with your front office. The more employees believe they will be caught, the less likely they are to steal.

• Understand how to run reports from the dental software. Print a monthly transactional report that shows all changes made to your data with additions and deletions.

Complaints: • Personally look into patient complaints.

If they have not received credit for payments, or have had other billing mistakes.

• Listen to employees.

If they comment that there are mistakes being made, or something isn’t being done properly, investigate it.

• Never use a signature stamp for any reason.

• Review lab bills. Does each charge match up

They should not be in the office. with a lab case invoice? Do you know all the patient names on it? Do the billed amounts look right?

#2 Open the monthly statement yourself. Check: • Who the checks are written to; do you know all of them?

• Duplicate payments; are any vendors or other people being paid more than once a month?

•

Review phone bill details. Look at every call. Look at call lengths. Look for phone numbers that show up multiple times.

• Payroll checks; are there the correct number of checks?

No employee has received any extra checks?

• Do not have any staff open your own mail.

• Check endorsements on back. Were they endorsed by the same personor vendor that the check is written to?

• Always look at monthly credit card statements.

• Bank charges; any fees for overdrafts, returned checks, late payments,or anything else you don’t recognize?

• Debits to your accounts. Are there any electronic debits or transfers that you don’t recognize?

• Automatic monthly payments; do you recognize every payment that islisted?

Checking account:

• The dentist should personally review and sign all checks.

• Outgoing checks being presented for signature should have attached invoice, statement, receipt, or other supporting documentation.

• Make sure you recognize each vendor, and know why you are writing them a check.

• Use checks that are professionally printed, and include multiple security features.

• Inspect checkbook, looking at numbers, to make sure no checks have been removed from the back of the stack.

Insist that it’s delivered to your desk unopened.

Make sure you recognize each & every charge.

• If you give your credit card information to a trusted employee, make sure that they understand that it is still to be used only with your permission every time.

• When payment checks are received in the mail, they should immediately be stamped with the bank deposit stamp.

• Check the “For Deposit Only” bank stamps.

Verify account numbers are correct on every stamp.

Regular Audits: • Have your accountant do some investigation for embezzlement and fraud. This is NOT a part of their regular services; don’t assume that they are keeping an eye on this for you.

• Keep all voided checks so all check numbers can be accounted for.

• Ask your accountant how you could improve your particular office to make it more embezzlement-resistant.

• Do random chart audits. Pull charts of patients who were

• Make sure your employees see you actually reviewing your office records. When they actually see you analyzing the books, they will know that you are involved, and keeping an eye on things.

• Do not get directly involved with an employee’s personal finances. Do not give an employee a loan. Do not co-sign on any loans or legal papers.

• Check your inventory. Are supplies disappearing faster than they should during normal patient care?

• Do random audits of your petty cash box, & your change box. Make sure all the money is accounted for.

Policies & Procedures: Have a comprehensive written Office Policy Manual.

Include sections on: • How to track work hours: If time clock is used, no employee is allowed to clock in or out for another employee.

• Passwords: must never be revealed to other employees.

• Forms that new employees must complete:

include Consent for Identity and Background Screening.

• Office Property: Supplies and equipment are not for personal use.

• Computers: Are for business use only.

• Petty cash, change fund: Never to be used for personal purposes.

• Keys: are not to be duplicated or given to others; must be returned upon employment termination before final paycheck is given.

• Zero tolerance for theft of any kind.

• After-hours and overtime policy.

• Have limits on staff coming in to work after hours.

• Insist all business office staff take some vacation time; examine the records yourself during this time. Have their employees take over the duties rather than allowing the bookkeeping work to wait until the employee returns.

• Cross-train employees. Don’t have bookkeeping, financial, accounting, or insurance duties that only one person can do, and nobody else knows how to do.

• Have a written routine for processing cash payments.

A statement should be provided for every single cash payment, even if the patient doesn’t want a receipt.

• Employment Termination: List of examples which may result in immediate termination. Include false information or misrepresentation; breach of confidentiality; theft or dishonesty; conviction of a felony; misuse of office funds, equipment, computers, or telephone; punching time clock for another employee; drug or alcohol abuse.

• When employment is terminated, immediately change the person’s passwords in the practice management software, time clock, and office alarm system. Arrange for a locksmith to be at the office before you leave that day, and change the locks.

Hiring New Employees: • Run a background check, including criminal and credit check, on all potential employees before hiring them. Be sure to have them sign an authorization for this.

• When considering a new employee for hire, always check previous employers. Speak to them personally. Confirm facts on the application. Ask about reason for termination, ability to work with others, absent days, job titles and duties, performance level. Ask them the question “If you had the opportunity, would you hire them back?”. Their response can be very revealing.

• Check references.

Embezzlement prevention and detection is not an exact science. All your best efforts may not prevent it, and if it occurs, it might not be detected for years!

Nevertheless, I think it would be a huge mistake to merely rely on your instincts or hope that it will never happen to you. And, although you may not have the resources of a large business, there are certain important, and easy steps, that you can implement to reduce your risk.

OSHA is authorized under the law to inspect your facility to determine whether employers are complying with current regulations. OSHA enforces what is called the General Duty Clause which requires that every working person must be provided with a safe and healthful workplace. Will you be ready?

Inspections may be conducted with or without advanced notice and are of ten conducted based upon a current or former employees complaint.

OSHA has adopted a “worst first” system of priorities for conducting inspections that works as follows:

#1. Imminent Danger. These situations are given top priority. Imminent danger means that a condition is thought to exist that can be expected to cause death or serious physical harm. If an imminent danger situation exists, the Compliance Of ficer will ask the employer to voluntarily abate the hazard and remove endangered employees. #2. Catastrophes and Fatal Accidents. OSHA’s second priority is investigating accidents that involve fatalities and/or hospitalization of five or more employees. These catastrophes must be reported to OSHA within 48 hours of the incident.

#3. Employee Complaints. The third priority of OSHA is the inspection of formal employee complaints of alleged violation of the OSHA Standards. The OSH Act gives each employee the right to request an OSHA inspection when the employee believes they are in imminent danger from a hazard. OSHA will maintain confidentiality when an employee requests such an inspection and will inform the employee of any action regarding their complaint.

4. Programmed Inspections. These inspections are usually aimed at high hazard industries and occupations. For this reason dental of fices are unlikely to be the target of such an inspection although special emphasis programs or random selection may also prompt an inspection.

Inspector Credentials and Opening Conference The actual inspection first involves the inspector presenting his/her credentials which can be verified by calling the nearest OSHA of fice. Remember that a Compliance Of ficer may never collect any violation penalty or promote the sale of any product or service. Af ter credential presentation, the Compliance Of ficer will request an opening conference to explain the purpose of the visit, the

scope of the inspection and the standards that apply. The employer will be given a copy of the safety and health standards that apply as well as a copy of the employee complaint, if applicable.

An employer representative must accompany the Compliance Of ficer during the inspection. An employee representative may also accompany the Compliance Of ficer and may be chosen by either the Compliance Of ficer or the employees themselves, but may not be chosen by the employer.

The Inspection The Compliance Of ficer will determine the route and duration of the inspection and will make every ef fort to minimize of fice interruptions. During the inspection the inspector may: • Hold private conversations with employees • Examine records • Take photographs • Review existing engineering controls • Monitor employee exposure to chemicals

Closing Conference Af ter the inspection has concluded, a closing conference will be held and shall include the Compliance Of ficer, the employer and the employee representative. The Compliance Of ficer will discuss all alleged violations observed but no penalties will be given at this time. The employer will be given a copy of “Employer Rights and Responsibilities”; will discuss the contents of the booklet and answer any questions.

Office Compliance Citations and Penalties A writ ten citation and notice of proposed penalties shall be sent to the employer by certified mail. A proposed time period will be set for abatement of any violations found. The employer must post a copy of each citation in a place visible to all employees, for at least three days or until the violation is corrected. The following violations may be cited:

• Other Than Serious Violation. This violation relates to job safety and health but is a violation unlikely to cause death or physical harm. A penalty of no more than $7,000 for each violation may be levied.

• Serious Violation. This type of violation means that there is substantial probability that death or serious physical harm could result. A mandatory penalty of up to $7,000 per violation may be levied.

• Willful Violation. This violation occurs when the employer is aware that a hazardous condition exists, knows the condition exists and violates a standard of the Osh Act and makes no reasonable ef fort to correct the hazard. Penalties of up to $70,000 may be levied for each violation.

• Repeat Violation. These may incur fines of up to $70,000 each.

• Failure to Correct a Violation. This may carry a civil penalty of $7,000 for each day the violation continues beyond the abatement period.

Appeals Process The employer may request an informal conference to discuss issues raised by the inspection citation and/or proposed penalty. The OSHA Area Director may revise citations and penalties to help avoid prolonged disputes.

If the inspection was initiated by an employee complaint, the employee may request to review any decision to not issue a citation. Employees may not contest citations, amendments, penalties or lack of penalties.

Notice of Contest To contest a citation or proposed penalty, a writ ten response must be sent to the OSHA Area Director within 15 working days from the date of the citation.

This writ ten response is called a “Notice of Contest”.

A copy of this notice must be given to the employees authorized representative, posted in a prominent location or given individually to af fected employees.

Af ter 15 days no further opportunity will be given for appeal–the citation and penalty will stand as is.

Review Procedure The OSHA Area Director will forward the case to the Occupational Safety and Health Review Commission (OSHRC) which is an independent agency not associated with OSHA. The Commission will assign the case to an Administrative Law Judge and a hearing will be scheduled near the employers’ workplace. Once the Administrative Law Judge has rendered judgment, either the employer or employees may request an additional review by OSHRC. Any Commission ruling may be appealed to the appropriate U.S. Court of Appeals. For states that have their own occupational safety and health program, the procedures are generally similar to Federal OSHA except that the state review board or other equivalent authorities hear the cases.

Prevention is the Key Do any of us want to go through the exhausting procedures described above? Of course not! For this reason, it is paramount that we get our dental of fices into compliance immediately. Begin by assigning a trusted employee to oversee/manage your OSHA compliance program. Allow them to work with a dental supplier who can provide many of the items needed to achieve compliance including compliance manuals, eyewash, spill kits, various safety products, personnel protective equipment, and annual required training.

The most important reason to achieve and maintain compliance is not to avoid an OSHA fine, but is instead to create a safe and healthful working environment for you and your dental team. Such an environment will gain the respect and admiration of your employees, who will understand and appreciate your concern for them.
