Vous pouvez soumettre par email comme pièces scannées et jointes. Nous signerons la lettre et nous vous l’enverrons le plus tôt que possible. Veuillez noter que dû à des contraintes légales, la lettre d’accord est en anglais, mais vous pouvez remplir la section 2 en anglais, en français, en espagnol ou en portugais, si une de ces langues vous convient le mieux. Veuillez envoyer les documents aussitôt que possible; nous ne pouvons pas vous verser les fonds avant d’avoir reçu les documents complétés. Votre organisation sera requise de soumettre un rapport de progrès avant le 15 Decembre 2016 et un rapport final avant le 15 Juin 2017. Le rapport final doit inclure un rapport financier sur vos dépenses de la subvention accordée par GFC. Nous vous enverrons les formulaires et directives des rapports par email environ un mois avant la date de remise. LETTER OF AGREEMENT This agreement is made by and between the Global Fund for Children (GFC) and ONG Dedome Amis du Père Franck (Grantee) on May 16, 2016. This agreement shall have a term of 12 months, from May 16, 2016 to May 15, 2017. 1. Amount and Terms of Payment. The Global Fund for Children makes a total grant of 5,000.00 USD to Grantee for the purposes stated in Paragraph 2 of this agreement. The full amount of the grant will be paid following receipt by GFC of this letter of agreement, signed by an officer of the Grantee. 2. Purpose of Grant: The primary grant of 5,000.00 USD will be used solely for support of the program as outlined in Grantee’s proposal. Any significant change in the purpose for which grant funds are spent must receive prior approval from GFC.
3. Reports. Grantee will submit a progress report by December 15, 2016 and a final report by June 15, 2017. The final report must include a financial report on how grant funds were spent. Reporting guidelines will be sent to Grantee approximately one month before each report is due. Grantee is not required to send receipts or other documentation beyond these reports. 4. Acknowledgement and Communications. Grantee will acknowledge GFC support in its organizational publications, such as annual report and website, as appropriate. GFC also requests acknowledgement in other publications, reports, articles, and media references for activities undertaken under this Agreement. Grantee grants GFC full and free use of any photographs, films, tapes, and other communications-related products resulting from the grant, with consent of grantee. 5. Prohibited Activities. Grantee does not knowingly conduct or support violent or terrorist activities nor transact business with persons or organizations that do so. Grantee shall not use any part of the grant funds, directly or indirectly, to (a) support or oppose candidates for public office, (b) cause any improper benefit to any private person or organization, (c) induce or encourage any violation of law, or (d) conduct or support any activity not specifically authorized in this agreement. 6. Remedies; Return of Grant Funds. If GFC determines, in its sole discretion, that Grantee has violated or failed to carry out any provision of this agreement, GFC may–in addition to any other legal remedies it might have–demand the return of all or part of the unexpended grant funds, which Grantee shall repay
to GFC within thirty (30) days. If Grantee is dissolved or is otherwise unable to use the grant for the purpose approved in this agreement, Grantee shall promptly return any unexpended grant funds to GFC. On behalf of ONG Dedome Amis du Père Franck, I confirm that I have read and understood this Letter of Agreement; that ONG Dedome Amis du Père Franck understands its obligations under the Letter of Agreement; and that I am authorized by ONG Dedome Amis du Père Franck to sign this Letter of Agreement.
