In the 40+ years that Capitol Lakes Health Center has been a part of the downtown Madison community, we’ve aligned with and adapted to many healthcare changes. With the projection of the 65+ population doubling by 2050, FALL 2024–WINTER 2025 SECOND ACT

according to the U.S. Bureau of Labor Statistics, patient-centered transitions and processes will continue to be a necessity, utilizing both innovative and creative ways to care for a growing senior population.

The healthcare industry has gone through many systemic changes in the past decade, and not just due to a global pandemic. Improvements to many robotic-assisted orthopedic procedures, such as total hip and total

Such developments have contributed toward a change in census and case mix for SNFs, like Capitol Lakes. Seniors still require similar levels of care, but the changes in insurance regulations, staffing models, and post-pandemic operations have prompted the need to rethink and reimagine healthcare for seniors moving forward.

With the increasing number of seniors enrolling in managed care insurance plans, Capitol Lakes and others have had to pivot to accommodate both the clinical and financial needs of in-house residents as well as new admissions from the community. Also known as Medicare Advantage or Medicare replacement plans, managed care insurances utilize clinical algorithms to drive SNF length of stay and dictate the timeframe in which a resident can graduate from receiving inpatient skilled nursing care to outpatient respite care. Some managed care insurances may not authorize inpatient skilled nursing following a hospital stay, so a

direct admission to respite is an option.

Currently, 37 percent of Dane County seniors are enrolled in such plans (as opposed to traditional Medicare), and the numbers are expected to continue to increase.

In other words, a respite stay involves additional oversight from caregivers and outpatient PT/OT staff but in a less clinically intensive environment as what the resident experienced at the skilled nursing level of care.

Respite programs offer an extended stay for residents who may be clinically designated as too high functioning for insurance to pay for an inpatient stay, but not quite ready to return home from a therapeutic standpoint. As an example, rather than staying at Capitol Lakes Health Center paying privately, a resident can transition to a respite stay at Capitol Lakes Terraces for a lessexpensive alternative. Not only does the resident save money, but the transition frees up a bed at the skilled nursing facility for a future rehab candidate.

Since respite is considered outpatient, the resident will also pay privately for their medications. Any physical or occupational therapy will be billed under the resident’s Part B benefit, which may include an additional co-pay depending on the managed care plan.

Therapy works with residents two to three times per week, as opposed to daily skilled nursing. A respite stay is a safe, therapeutic place for residents to be when they’re not safe to be at home by themselves just yet.

Respite services provide therapy on an outpatient basis and focus more on gaining strength, as when following a fracture; endurance building, as when following a stint with pneumonia; equipment teaching, as with a patient having a new ostomy; and participating in a trial period prior to returning home. The respite program is also an option for skilled nursing residents who may be clinically ready to go home but have new equipment needs (e.g. needing a ramp installed) or when a home health provider isn’t available immediately upon discharge.

ComForCare Home Care Services provides assistance with activities of daily living, such as light housekeeping, meal preparation, dressing, bathroom assistance, home maintenance, and other services. ComForCare is an effective solution for the elderly who prefer to remain at home.

Coalitions are a great source of information and a good place to start researching specific areas of interest (for example, senior housing options). Case managers are oftentimes on staff to help coordinate services and assistance when needed.

Enhancing Independence: Top Three Aging in Place Home Improvement Projects

As our population ages, the concept of aging in place has become increasingly important. Many seniors prefer to remain in their own homes rather than move to assisted-living facilities or nursing homes. To facilitate this, home improvements tailored to making living spaces safer and more accessible are essential. Focusing on the principles of universal design will help achieve optimum results and make your home accessible, welcoming, and safe for people of all ages.

Bathroom Accessibility Upgrades Bathrooms can be particularly hazardous for seniors due to slippery surfaces and the awkward movements required for daily tasks, like bathing and toileting. Upgrading bathrooms with accessibility features can greatly reduce the risk of falls and make daily self-care routines easier.

• Install grab bars near the toilet and in the shower/tub area for support.

• Replace traditional bathtubs with walk-in showers or tub-to-shower conversions to eliminate the need to step over high thresholds.

• Install linear drains and zero-barrier thresholds and waterproof shower enclosures and surrounding areas to allow for flexible space use and easier cleaning.

• Update lighting to facilitate grooming tasks and provide better

Entrance and Exterior Improvements Creating a safe and accessible entryway is crucial for seniors who wish to remain in their homes. Exterior improvements can enhance both safety and convenience for older adults.

• Widen doorways and install lever-style door handles for easier wheelchair or walker access.

• Add motion-sensor lighting along walkways and at entrances to improve visibility at night.

• Create a low-maintenance outdoor space with composite materials, raised garden beds, automatic irrigation systems, and similar features to facilitate gardening without excessive bending or kneeling.

These home improvement projects can significantly improve the quality of life for seniors who wish to age in place by promoting safety, accessibility, and independence within their own homes.

While the initial investment may seem daunting, the long-term benefits in terms of comfort and peace of mind are invaluable.

Look for design professionals who have training and certifications in universal design to help you implement sensible and stylish solutions. It’s important to look ahead and plan for improvements now so that they can be designed and implemented before you require them.

By proactively addressing potential hazards and barriers, seniors can enjoy their home for years to come while maintaining autonomy and dignity.

PROTECTING YOUR Digital Legacy

We prepare for so many possibilities, from severe weather to medical emergencies. While we insure ourselves against many potential risks, one piece is often overlooked: protecting our digital presence.

If you or a loved one unexpectedly passes away or is unable to use technology anymore, will your digital legacy be safe? What assets or accounts could be accessed online without your consent?

What subscriptions or memberships would continue to be charged?

Thinking through these risks can be overwhelming. From your favorite online shopping sites to your utilities, FALL 2024–WINTER 2025 SECOND ACT

you probably have accounts and credit card information saved in dozens of places online. We all do. The best way to protect your digital legacy is to be proactive. Taking these steps today will help you and your loved ones have peace of mind looking forward.

Update Your Beneficiary Contacts Make sure your bank, insurance, and retirement fund holder have your beneficiaries’ current contact information. This should be done after any major life change—marriage, divorce, death, new child, etc.—and after getting a new phone number or moving. Check with your major institutions periodically to stay current.

Designate a Legacy Contact Many major tech companies, such as Apple, Google, and Meta (Facebook and Instagram), allow you to designate a Legacy Contact. This documents permission for your username and password to be shared with a designee should you pass away or become incapacitated. It gives your loved one easy access to your accounts in an emergency.

Share Login Information with a Trusted Designee Give your designee all relevant usernames and passwords they may need to take care of your affairs. One of the easiest ways to keep this information

GENERAL | 27 accessible for you and your designee is to use a password management system, like LastPass or 1Password.

These systems require you to remember just one master password to open a digital vault that stores the usernames and passwords for all accounts you access on your device. By giving your master username and password to your designee, you give them the keys to all your accounts.

A password manager will automatically ask you to save accounts as you surf the internet, ensuring that you’ve tracked all your online accounts. Alternatively, you can keep a notebook with account information in a secure place at home.

No matter how you track it, be sure your designee knows where to find this information.

GERIATRIC Psychiatry

by Stoughton Health We’ve all heard it—good mental health is as important as good physical health, if not more so. With each new study, medical communities are giving patients better individualized care by approaching common diagnoses with more nuance. Part of that differentiation has to do with age.

Depression and anxiety might present similarly across age groups, but the underlying factors for those 55+ are often quite different than people in their 20s and 30s. When you consider these factors alongside the increased risk for dementias, it’s clear why the field of FALL 2024–WINTER 2025 SECOND ACT

geriatric psychiatry is so important in our communities.

Jacob Dunn, Geriatric Psychiatry Inpatient Program social worker with Stoughton Hospital, says, “Typically, we’re seeing people with depression, anxiety, suicidal thoughts or attempts, anxiety disorders, psychotic symptoms, bipolar disorders, schizophrenia. The majority of people we admit are those with some form of dementia and in various stages. In the later stages, what we’re seeing is more agitation; aggression; paranoia; suspiciousness

of staff or family member or caregiver; actually experiencing hallucinations, whether you’re seeing or hearing things that aren’t present.”

Stoughton Hospital’s geriatric psychiatry unit consists of 10 beds, but it’s important to emphasize that they’re not just there to treat extreme cases. As serious as dementia is for the individuals and families going through those hardships, the mental health of a recently retired person struggling to acclimate is just as important for the individual to live a fulfilling life.

HEALTH | 37 “We just admitted someone who had to place their spouse in memory care, and they’ve never had to cook. They’ve never had to clean. All those things were taken care of. That partner of theirs, after 50 some years, is now not at home with them. It’s just an adjustment at time for folks. It’s trying to get them through or find meaningful activities to do. Everybody wants to have that sense of self-worth and contribution to the community.”

Nonpharmacological solutions and behavior modifications are key components to helping people cope with their mental health. A combination of medications and psychotherapy has shown to be the most effective way to treat a lot of depressions and anxieties for people of all ages. Jacob says, “Even with dementia, we can try to find some activities that are meaningful and keep those people socializing and occupied throughout the day.”

To get into the program, patients need to sign in voluntarily, even if someone has guardianship or is an activated power of attorney. That said, the importance of family involvement can’t be understated. “We’re keeping the family updated,” says Jacob. “Multiple times a week, they’re visiting; they’re calling our nurse’s station. I call them at least three days a week. ... We’re constantly keeping everybody involved.

We’re also providing that support for family members because this is a really difficult time for them as well.

An individual with dementia may not benefit from therapeutic interventions, but that’s part of my role: providing that education and support. Right away, I’m working on what services and things they’ll need if the person comes home or when looking at referrals to facilities.”

Over the 26 years of operation, possibly one of the longest-running geriatric psychiatry units in the state, Jacob and

everyone involved with the program bring a lot of experience to the table.

Some of the nurses have even been a part of the program since its inception.

What this means for patients and their families is working with someone who can pivot quicker and recognize potential treatments or issues.

“It’s just a really good treatment team,” says Jacob. “Everybody strives for an individualized approach for every patient who comes in. We’re working closely with families and community partners and any supports that individuals have to set them up for success when they’re discharged from our facility.”

Stoughton Hospital works with several different insurances, even those not in network. They’ll also work to get prior authorization so families don’t have to worry about what’s going to happen after or during care from a financial perspective. Whether with Medicare or Medicare and a secondary insurance, the experience is pretty much the same as a regular hospitalization.

“I think we need to start looking at behavioral health issues just as much as we do the physical health issues because those things coincide with each other,” says Jacob. “Mental health and physical health issues can impact an older adult, especially mental health. To focus on a holistic approach with that is something we want to spotlight a little bit more than what’s out there.”

The Benefits of ACTIVE AGING

We all know the importance of eating well and staying physically active as we age, but active aging involves much more than exercise and a healthy diet. The World Health Organization defines active aging as “the process of optimizing opportunities for health, participation, and security in order to enhance quality of life as people age.” It’s a mindset where people of any age, especially older adults, open themselves up to trying new things and FALL 2024–WINTER 2025 SECOND ACT

experiences, meeting new people, and embracing what’s possible.

While many older adults are determined to remain in their homes forever, studies show that this can often result in feelings of loneliness, isolation, and anxiety that can contribute to health issues, such as heart disease, depression, and cognitive decline. Active senior living communities can be a great solution, where resident lifestyle programming

provides seniors with regular activities and opportunities to socialize, exercise, and try new things. This increased activity can help both physically and mentally, resulting in higher levels of happiness and increased quality of life.

WITH PREVENTING A FALL

“One minute I’m rinsing string beans in the sink, the next I’m on the floor with my dog staring down at me. I slipped on the wet spot I made moving the dripping colander over to the countertop, tripped over the dog trying to keep my balance, and landed hard on my hip and elbow.

No major injury—this time. Next time, I might not be so lucky.”

The above is a common story, especially in Wisconsin. According to the Centers FALL 2024–WINTER 2025 SECOND ACT

for Disease Control and Prevention (CDC), one in four people age 65+ has a fall each year, with most happening in and around the home. While some result in bumps and bruises, others are more serious, causing head injuries or broken bones. In fact, 95 percent of hip fractures are due to a fall. Additionally, Wisconsin has the dubious distinction of having the highest rate of deadly falls among older adults in the country.

The good news is there are steps you

can take and resources you can use to reduce your fall risk.

Determine your risk. While falls are more common as people grow older, they’re not inevitable. There are varying degrees of risk based on your habits, health, and environment. To determine your risk, consider taking the National Council on Aging’s Falls Free® CheckUp. This free 13-question assessment determines your risk level

SAFE & HEALTHY AT HOME | 57 and provides suggestions based on your results. Have a conversation with your healthcare provider to discuss risks and concerns and get recommendations.

While a fall can happen in a number of scenarios, many are caused by a combination of factors.

• Balance and strength. Being unsteady on your feet and lacking lower body strength increases your chances of a fall.

• Home hazards. From cluttered walkways to poor lighting to pets underfoot, these and other conditions in the home can cause slips and trips.

• Medications. Some prescription and over-the-counter medications can make you drowsy, affecting your balance and gait.

• Problems with vision. Low vision can make hazards harder to spot and avoid.

• Unsafe footwear. Slippery bottoms, unstable heels, improper fit, and more can cause a fall.

Reduce your fall risk. Communities statewide offer local fall prevention programs and events.

• Falls Free© Wisconsin. A Wisconsinfocused website and coalition supported by the Wisconsin Institute for Healthy Aging, Falls Free Wisconsin is a one stop shop for falls prevention information and resources. Get strategies for putting falls prevention into practice; learn about programs, like the evidence-based Stepping On workshop; and visit the interactive Home Safety Challenge, where you’ll move virtually through a real home, spot the falls hazards, and get tips on how to eliminate them. fallsfreewi.org.

• Safe At Home. Offered by AgeBetter, Inc. in collaboration with the University of Wisconsin– Madison Occupational Therapy program, the Madison College Occupational Therapy Assistant program, and Safe Communities
